Abstract
Background and Aim: Treatment of cutaneous leishmaniasis, especially when caused by L. tropica, is challenging. Meglumine antimoniate (Glucantime®) is used as the standard treatment, but multiple injectiond are necessary. The objective of this study was to compare the efficacy of weekly intralesional injections with twice weekly injections of Glucantime for the treatment of anthroponotic cutaneous leishmaniasis (ACL).
Methods: This randomized open clinical trial was conducted, in Bam, Kerman province, Iran. 96 eligible patients according to inclusion and exclusion criteria who were willing to participate were included. The included patients were randomly assigned into two groups, one group treated with weekly intralesional injections of Glucantime® and the other group treated with intralesional Glucantime® twice a week. Type and size of each lesion (induration, ulcer and scar) were recorded weekly. Complete healing was defined as complete re-epithelialization and absence of induration in all lesions and was considered as the primary outcome measure.
Results: A total of 48 patients completed the study complete cure was seen in 24 of 27 (89%) patients who received weekly intralesional MA with a mean duration of healing equals to 70±10 days. Complete cure was seen in 24 of 31 (77%) patients who received intralesional MA twice a week, the mean duration of healing in the latter group was 58±5 days. There was no significant difference between the two groups (P=0.23).
Conclusion: It seems that the efficacy of intralesional injections of Glucantime® once a week is similar to efficacy of twice a week Glucantime® injections.

Abstract
Background and Aim: Cutaneous leishmaniasis (CL) is endemic in Iran, where it is one of the most important health problems. Both anthroponotic CL (ACL) caused by L. tropica and zoonotic CL (ZCL) caused by L. major are reported. Antimoniate derivatives as the standard therapy for CL need multiple injections and are not easy to tolerate for the patients. This study was conducted in Mashhad to compare the efficacy of weekly versus twice a week intralesional injections of meglumine antimoniate (MA) in the treatment of ACL.
Methods: This randomised controlled trial was performed during 2006 to 2008 in Mashhad, Iran. Using computerized sequence of random numbers, participants were randomly allocated in the two arms of the study: one receiving weekly and the other receiving twice-a-week intralesional injections of MA. The lesion size, induration and healing rate were assessed, recorded and compared. Healing was defined as complete re-epithelialisation and disappearance of induration.
Results: A total of 252 suspected CL patients with 372 lesions were screened. 82 parasitologically proven cases with 121 lesions caused by L. tropica were included and 74 patients with 113 lesions completed the study. At 12th week after initiation of treatment, complete healing was observed in 38 out of 44 lesions (86.4%) in the group which received weekly intralesional MA injection. The median time-to-heal in this group was 36 days (95% confidence interval [CI]: 32.0-39.9). Complete healing was recorded in 60 out of 69 lesions (86.9%) in the group which received twice a week intralesional injections of MA with a median time-to-heal of 25 days (95% CI: 20.9-29.1). While no significant difference was observed between the two groups in terms of complete healing rate (P=0.999), time-to-heal was significantly different between the 2 groups (P=0.003).
Conclusion: It seems that the effectiveness of twice-weekly intralesional injections of MA is similar to once-weekly regimen while the former regimen causes more rapid healing of lesions.

Abstract
Background and Aim: Malassezia is a lipophilic and dimorphic fungus which has different species. Some of them can be found as natural flora on skin and in some conditions may cause pityriasis versicolor. The aim of this study was to identify Malassezia species associated with pityriasis versicolor in Iranian patients, using PCR-RFLP.
Methods: In this study out of 65 patients with pityriasis versicolor to have pityriasis versicolor,isolates of 60 patients were positive. Malassezia species. using by PCR-RFLP. The Internal Transcribed Spacer 2 (ITS2) region was amplified by PCR employing the ITS3 and ITS4 primers and The restriction endonucleases AluI, BanI and MspAI were selected for producing distinct RFLP patterns.
Results: M. furfur (36.7%), M. globosa (30.0%), M. sympodialis (20.0%), M. slooffiae (8.3%), M. restricta (3.3%) and M. obtusa (1.7%) were the microorganisms responsible for the infection among participants. The M. sympodialis infection was strongly correlated with the female gender (P=0.02).
Conclusion: Our findings suggest that, the most common Malassezia species associated with pityriasis versicolor was M. furfur, followed by M. globosa.

M. pachydermatis, M. japonica, M. dermatis, M. restricta, M. obtuse, M. nana and M. yamatoensis were not isolated from any samples.
Conclusion: Our findings suggest that the most common Malassezia species associated with seborrheic dermatitis was M. globosa, followed by M. furfur.

Abstract
Background and Aim: Onychomycosis is the fungal infection of nails. Patient’s immune system has an important role in initiation of onychomycosis. One of the causes of the increased prevalence of onychomycosis is the increasing number of patients with local or general immune system disorders. In hormonal disorders such as diabetes mellitus, Cushing's syndrome and hypothyroidism superficial vascular disorders and individuals with immune deficiency, onychomycosis prevalent. The aim of this study was to investigate onychomycosis in individuals with underlying diseases who referred to Razi hospital in Tehran.
Methods: In this cross-sectional study, nail specimens of 250 patients who have nail dystrophy with an underlying disease whom were referred to mycological laboratory of Razi hospital in 2010-11 were evaluated with direct microscopy and culture on Sabouraud dextrose agar and Sabouraud dextrose agar containing chloramphenicol and cyclohexamide. For differential diagnosis of fungi, complementary tests were used.
Results: Eighty-nine (35.6%) of 250 individuals with underlying diseases and nail dystrophy, were affected with onychomycosis. Among these pateints, 52 (58.4%) cases had yeast infection, 19 (21.3%) had dermatophyte infection, and 18 (20.2%) had saprophyte infection. Diabetes mellitus (37.1%), cardiovascular diseases (14.7%), pemphigus (9%), psoriasis (7.9%), hypothyroidism (6.7%) and chronic eczema (5.6%) were the most common underlying diseases observed in patients with onychomycosis.
Conclusion: Onychomycosis is common in hormonal disorders like diabetes mellitus and hypo-thyroidism superficial vascular disorders and individual who suffer immunodeficiency.

Abstract
Background and Aim: Leishmanization (LZ) is an effective tool to prevent
cutaneous leishmaniasis. Standardization of Leishmania is
the main drawback of LZ. The aim of this study was to assess the effect of
various preservatives on the infectivity of Leishmania.Methods: L.major harvested
at different stages of growth logarithmic, early and late stationary phases
were frozen using various preservatives of saccharose, glycerol, trehalose,
glucose, sorbitol, and dimethyl sulfoxide (DMSO). The harvested parasites were
inoculated into BALB/c mice before and after freezing. The infectivity of the
parasites was checked. IFA test was used to assess the rate of metacyclic
parasite.
Results: The ratio of
live Leishmania in different growth stages and various
preservatives were 89.0% to 98.2%. The lesion development in groups of mice
which received Leishmania in sacarose + glycerol or DMSO was started from 3rd
week and at 5th week all the mice showed lesion. The group of mice
which were inoculated with early or late stationary phases in saccharose +
glucose, saccharose + glycerol, glycerol 15% or DMSO showed lesion from 4th
to 5th week and in 100% showed lesions at 8th week. The
rate of metacyclic parasites increases from log phase to early and late
stationary phases.
Conclusion: There was a
correlation between percent of live parasite and the rate of lesion development
in BALB/c mice. Saccharose 22.5% + Glyserol 22.5% were the most appropriate preservative to freeze L. major. IFA
test is used to detect metacyclic Leishmania. A correlation was seen between the rate of
lesion development in BALB/c mice and IFA positivity.

Abstract
Background and Aim: Skin cancer is one of the most common human cancers. Its incidence rate trend is increasing in Iran. The aim of this study was to investigate the prevalence, incidence rate and geographical distribution of skin cancer to identify the high risk areas in Kurdistan province, Iran.
Methods: In this descriptive cross-sectional study, the collected data were analyzed using version 16 of SPSS software, and t-test and analysis of variance (ANOVA) were used for comparison of the means. The distribution map of skin cancer in the province was prepared using Geographic Information System (GIS) software, version 9.3 between the beginning of 2006 and the end of 2010. In order to investigate the reason for high rates of skin cancer in some cities, ultraviolet (UV) radiation distribution map was also prepared.
Results: Sanandaj with 17.55/100000 had the highest and SarvAbad with 3.73/100000 had the lowest incidence rates. The number of male patients (542) was nearly two times of female cases (324), (P<0.0001). In addition, skin cancer was about two times more prevalent in urban (%68.01) than rural (%31.99) residents (P=0.008).
Conclusion: Life style of men and their sun exposure because of their job seemed to be a major risk factor in increasing the incidence of skin cancer in Kurdistan province, Iran. There was no correlation between the skin cancer incidence at the district level and UV index. Further research is recommended to explain this finding.

Abstract
Background and Aim: Tinea unguium is a common disease with worldwide distribution most commonly seen in adult patients. Trichophyton rubrum and T. interdigital are the most common causes. The aim of this study was to investigate the frequency of tinea unguium causative agents in a referral dermatology hospital in Tehran, Iran.
Methods: This cross sectional study was conducted in 2010 and 2011 on clinically suspicious patients for tinea unguium referred to the Mycology Laboratory, Razi Hospital, Tehran, Iran. Samples from 700 patients were examined using direct smear microscopy and culture. Direct microscopic examination of the specimens was carried out using 20% potassium hydroxide solution. The specimens were cultured on Sabourad dextrose agar culture media containing chloramphenicol and cyclohexamid (Scc). For identifying the species of dermatophytes, complementary tests were used. Frequencies and relative frequencies were demonstrated in tables and chi-square and Fisher's exact tests were used to investigate any association between the categorical variables.
Results: Of 700 dystrophic nail samples, 53 samples (7.6%) were positive according to both direct examination and culture. Thirty-eight patients were males. The most common clinical type was distal subungual onychomycosis which was observed in 79.2% of cases. The most frequent detected dermatophyte species. was T. interdigital (39.6%) followed by T. rubrum (37.7%). Forty-seven patients had tinea unguium on their toe nails, 4 patients on their finger nails, and 2 patients had it on both finger and toe nails. Nineteen patients had underlying diseases, and the most common underlying disease was cardiovascular disease (26.3%).
Conclusion: Tinea unguium is a disease with worldwide distribution and identifying the causative agents and predisposing factors are necessary for better management of the patients.

Abstract
Background and Aim: Bacteria are one of the causes of nail infections. Although nail infections are not always associated with a major health problem, their treatment, particularly in immunosuppressed patients could be chalenging. This study was conducted to investigate the prevalence of bacterial agents in patients with dystrophic nails referred to mycology laboratory of Razi Hospital, Tehran.
Methods: This was a cross-sectional study on patients with dystrophic nail. Samples were examined by both direct examination and culture. Blood agar and eosin methylene blue (EMB) agar media were used to culture and differentiate bacteria.
Results: Of 700 dystrophic nail samples, 183 (26.1%) samples were diagnosed as onychomycosis. One hundred and four (56.8%) of the patients were female. One hundred and eighty-one patients had bacterial nail infection (25.9%), of whom 109 (60.2%) were female. Forty-six (6.6%) patients had both bacterial and fungul infections, of whom 32 (69.6%) were female. The most common detected agent was Staphylococci.
Conclusion: Bacterial infections can result in dystrophioc nails. If these infections remain untreated they can lead to nail loss. In most cases, treatment of bacterial nail infections is easier than fungul nail infections.

Abstract
Background and Aim: Staphylococcus aureus is the second cause of hospital acquired infections, and responsible for 80% of purulent infections, and majority of skin infections. About 30 to 50 percent of normal people carry staph in their nose or groin and armpits. VanA, VanB, VanC1, VanC2/C3, VanG, VanL, and VanX are genes responsible for encoding resistance to vancomycin, TychoPlanyn and Avoparcyn, among them vanBand vanAare the most common cause of resistance that could be located on a plasmid or a chromosome and can be transferred via conjugation. The aim of this study was to The aim of this study was to investigatethe role of VanA and VanB genes in Cefoxitin resistant Staphylococci aureuscausing skin infections in patients admitted to Razi Hospital in Tehran using real time PCR method.
Methods: The samples were collected from Khordad 1394 for one year in the Microbiology Department of the Clinical Laboratory of Razi Hospital. Exudative skin lesions were sampled by sterile swab and cultured on the blood agar and EMB medium. Then catalase, oxidase and coagulase tests were performed on the gram-positive cocci and the sensitivity to vancomycinin Cefoxitin-resistant Staphylococcus aureus was determined using the E-test method. The presence of vanAand vanBgenes were investigated by Real Time PCR.
Results: Out of 978 patients with infected skin lesions, 733 samples of Staphylococcus aureus were isolated. Of these, 124 were Cefoxitin resistant, among them 8 samples had a high response rate of 3, and 5had high response above 16. But VanA and VanB genes were not responsible for resistance in any of them.
Conclusion: Due to the development of resistant strains of Staphylococcus in skin and hospital infections, identification of its encoding genes are necessary in order to use appropriate antibiotics to reduce the course of treatment and the side effects of taking antibiotics.

Abstract
Onychomycosis is a common nail disorder. If left untreated, it may cause nail deformity and nail plate dystrophy. Since nails have aesthetic importance, onychomycosis may have some impact on quality of life by causing a change in the appearance of nail and/or nail dystrophy and by interfering with fine activities carried out by hand. The aim of this review is to evaluate the quality of life in patients with onychomycosis.

Abstract
Leishmaniasis is a neglected disease caused by different Leishmania species and transmitted by various sand flies. Iran is endemic to both visceral and cutaneous form of the diseases. The aim of this study was to investigate the epidemiological factors of leishmaniasis in patients referred to the Center for Research and Training Skin Disease and Leprosy from 1399-1400. The present study is a cross-sectional study in which every patient with suspected cutaneous leishmaniasis lesion(s) referred to the center during 1399-1400 were included. The results showed a higher number of patients with leishmaniasis are in the age group of 10 to 19 years. Among the mentioned jobs, a higher percentage was assigned to housewives in the affected women and workers in the affected men. According to the results, autumn is the most common season for cutaneous leishmaniasis. The average time from beginning to the diagnosis of leishmaniasis was 1.5-3 months and most patients showed a history of traveling to rural areas. The number of wet type lesions were higher than dry type and most of the lesions were in the hands. Most cases were reported from Isfahan province.

Abstract
Background and Aim: Leishmaniasis is a parasitic disease transmitted by sandflies and is one of the endemic diseases of Iran. With the spread of the Covid-19 epidemic worldwide, the virus will interface with endemic infectious diseases in different regions. In this study, coronavirus disease was evaluated in patients with a history of leishmaniasis referred to the Center of Research and Training in Skin Diseases and Leprosy.
Methods: A total of 91 patients with a history of leishmaniasis were studied for possible SARS-CoV-2 infection. The number of patients referred to the Center of Research and Training in Skin Diseases and Leprosy during the coronavirus epidemic was also compared with the same period before the epidemic
Results: A total of 60% of patients with leishmaniasis
did not report coronavirus disease. Twenty-one percent of cases had a mild corona disease, 13% had a moderate and 3% had severe corona disease. Only a single death was reported in corona patients with a history of cutaneous leishmaniasis. The number of referrals before the beginning of the corona epidemic was twice as high as during the epidemic.
Conclusion: In these participants, mortality and side effects of COVID-19 were significantly less reported. However, further studies are needed to evaluate the immune system and cutaneous leishmaniasis protection against COVID-19. Various factors can be effective in reducing patients during the epidemic, including not going to medical centers, reducing the number of trips, and observance of health issues.

Abstract
Background and Aim: Cutaneous leishmaniasis is one of the most important health problems in Iran. There is still no effective vaccine available against human leishmaniasis and current treatment is based on chemotherapy. Conventional drugs are toxic and expensive, and drug resistance has caused the search for new treatments. Tofacitinib is a Janus kinase inhibitor that disturbs the signaling pathway of several cytokines in the immune system. The aim of this study was to evalute the effect of topical tofacitinib 2% ointment in healing cutaneous leishmaniasis in Balb/C mice.
Methods: This experimental study was performed using tofacitinib and glucantim drugs in 40 female Balb/C mice. The metacyclic form of promastigote of Leishmania major standard strain (MRHO/IR/75/ER) was injected into the footpad of the mice, the mice were divided into 4 treatment groups: tofacitinib, glucantim, tofacitinib-glucantim and control without treatment. The treatment period was 28 days. The footpad thickness of all groups was measured at the end of each week.
Results: After the end of the treatment in the groups treated with tofacitinib, a significant decrease in the size of lesion and the footpad thickness was observed compared to the control group without treatment (P<0.01).
Conclusion: It seems that tofacitinib ointment accelerates lesion healing in Balb/C mice and has worked similarly to standard glucantim treatment. Ointment can be a better option for leishmaniasis treatment and due to its ease of use and no need for painful injections it can be considered as an option for the treatment of patients with cutaneous leishmaniasis.
