Abstract

Background and aim: Saffron possesses a lot of medical applications in traditional medicine. One of its properties is ultraviolet (UV) light absorption. The aim of this study was preparation, determination of properties and investigating sun protevtive effects of nanoliposomes containing saffron extract and safranal, a substance existing in saffron.
Methods: Nanoliposomal hydroalcoholic extract of saffron stigma in concentrations 2%, 4%, 8% and nanoliposomal safranal in concentrations 0.25% , 0.5% and 1% were prepared. Standard lotion of homosalate 8% was used according to FDA standards for sun protection factor (SPF) determination and assessment of accuracy of the method. SPFs of substances were calculated in vitro by diluted solvent method using Mansur, et al equation and absorptiometry.

Results: SPf of Nanoliposomal saffron 8% extract was significantly higher than standard sunscreen homosalate with similar concentration. SPFs of safranal (1% and 0.5%) component of saffron were significantly higher than the standard sunscreen homosalate 8%.
Conclusion: Saffron can act as a sunscreen. Safranal existing in saffron is much more sun protective than standard substance, homosalate 8%. Thus , the safranal existing in saffron may have significant sun protective effect in low concentration if encapsulated in liposome.

Abstract

Backgound and Aim: Skin cancer is the most prevalent type of cancer in Yazd city. Childhood and adolescense is an important time for preventing the skin cancers. The aim of this study was to survey the role of appearance concern with skin cancer prevention behavior based on protection motivation theory.
Methods: Participants in this cross- sectional study were 360 female students from 4 high schools in Yazd city. Data were gathered by using self-reporting questionnaires and were analyzed using SPSS software applying ANOVA, Pearson's correlation coefficient and linear regression tests.
Results: Using Pearson's correlation test, a statistically significant correlation between the appearance concern with perceived susceptibility, perceived severity, self-efficacy, response efficacy, protection motivation and behaviors was found (P<0.01). The perceived susceptibility, perceived severity, self- efficacy, response efficacy, protection motivation, response costs and appearance concern could predict 0.56 of the behaviors and perceived susceptibility was the strongest predictor (β=0.120). Also, protection motivation could predict behavior (41 %).
Conclusion: Results of this study support the effectiveness of protection motivation theory variables and the role of appearance concern for prediction of skin cancer prevention behaviors in participants.

Abstract

Background and Aim: There are some reports regading the anti-inflammatory and antimicrobial effect of honey and Nigella sativa and their therapeutic effects in improvement of wound healing. This study was designed to evaluate the efficacy of adding honey and Nigella sativa extract compared to honey alone in patients with cutaneous leishmaniasis receiving intralesional Glucantime.
Methods: In this clinical trial, 150 patients with confirmed cutaneous leishmaniasis were randomly allocated into two groups. One group was treated with topical honey twice daily along with weekly intralesional injection of Glucantime. In the second group, patients were treated with topical honey and 60 % hydroalchoholic Nigella sativa extract instead of honey alone. The patients were treated until complete healing of the ulcer or for maximum of 12 weeks.
Results: At 12 weeks, complete cure was achieved in 61 (81.3%) of patients treated with topical honey and 60 % hydroalchoholic Nigella sativa extract versus 48 (64%) in the second group (P=0.002).
Conclusion: The efficacy of adjunct honey and Nigella sativa extract along with intralesional Glucantime is more than honey alone in increasing complete cure rate and decreasing the residual scar size and the required dose of Glucantime.

Abstract

Background and Aim: Androgenetic alopecia is an extremely common disorder affecting both men and women. The incidence is generally considered to be greater in males than females, although some evidence suggests that the apparent differences in incidence may be a reflection of different expression in males and females. This genetically determined disorder is progressive through the gradual conversion of terminal hairs into indeterminate hairs and finally to vellus hairs. Patients have a reduction in the terminal-to-vellus hair ratio, normally at least 2:1. Following miniaturization of the follicles, fibrous tracts remain. Patients with this disorder usually have a typical distribution of hair loss.
Methods: This is a cross sectional study that conducted in Isfahan health center in 2007-2008 .Using simple random sampling, we selected 1800 women in urban health center areas for hair loss. We evaluated the type of hair loss by Ludwig classification. The data was collected through administration of a specifically designed questionnaire and was analyzed using SPSS software. The chi-square and student tests were used for data analysis.

Results: According to results of this study the of 1800 women, 712 (39.6%) had hair loss. Mean age of females who had hair loss was 50.4±13 years.
Conclusion: Thyroid disorders, ferritin serum level and androgenic hormones may play role in hair loss and their existance should be assessed.

Abstract

Background and Aim: Hirsutism is defined as the presence of terminal hairs in androgen dependent areas in women. It is a common clinical condition and affects 5% to 10% of women of reproductive age. The aim of this study was to determine the clinical features and etiologic factors of hirsutism in premenopausal women.
Methods: In this cross-sectional study, 200 premenopausal women with hirsutism who were admitted at our dermatologic clinic during a 2-year period were nivestigated. After a careful history and physical examination, a transabdominal ultrasonography of the ovaries and laboratory evaluation including early morning total testosterone, prolactin, dehydroepiandrosterone sulfate (DHEA-S) and 17-hydroxyprogestron in all patients and thyroid stimulating hormone (TSH) in 10 patients were performed.
Results: Polycystic ovary syndrome (PCOS) was the most common cause of the hirsutism (54.5%) followed by idiopathic hirsutism (40%). Other causes of hirsutism were prolactinoma (2%), hypothyroidism (1%) and drugs intake (2.5%). Hirsutism was mild in 109 patients (54.5%), moderate in 85 patients (42.5%) and severe in 6 patients (3%). Family history of hirsutism was present in 105 (52.5%) patients. Clinical conditions associated with hirsutism were: acne (52.5%), menstrual irregularity (35%), androgenetic alopecia (15%), acanthosis negricans (7.5%) and obesity (37.5%).
Conclusion: PCOS and idiopathic hirsutism are the most common cause of the hirsutism in premenopausal women. A detailed history and physical exam often provide enough information to determine the etiology of hirsutism.

Abstract

Background and Aim: The heterogenous population of memory T lymphocytes is distinguished based on surface markers and effector functions such as cytokine secretion. Recently, two subsets of memory T cells are defined by expression of chemokine receptor CCR7 and CD45RA designating as "central memory" T cells (TCM) and "effector memory" T cells (TEM). The objective of this staudy was to evaluate the phenotype and function of these lymphocytes in healed cases of cutaneous leishmaniasis.
Methods: The phenotype of lymphocytes were determined in blood samples of 13 volunteers with history of self healing cutaneous leishmaniasis (HCL) and in 6 healthy controls.
Results: No significant difference was found in memory T cell subsets between HCL volunteers and healthy controls using flow cytometry. However, following sorting of different memory subsets, a significantly higher proliferation was seen in cells of HCL volunteers comparing to the control group. A significantly higher IFN-γ response in TEM and a significantly higher IL-2 response in TCM were observed in cell culture of HCL volunteers comparing controls.
Conclusion: The responses were elicited when the cells were stimulate with SLA in vitro, it is concluded Leishmania-specific TEM and Leishmania-specific TCM subsets exist in HCL volunteers and since the volunteers with history of CL presumed to be protected against reinfection, it seems that both TCM and TEM play role in the protection against Leishmania infection in these individuals.

Abstract

Background and Aim: Melasma is an acquired symmetrical brown hyperpigmentation of the face or neck. Melasma has been shown to have a significant emotional and psychologic distress on affected patients. Although this pigmentary disorder is more prevalent among women of reproductive age, the effect of melasma on quality of life in this population is unknown. The aim of this study was to evaluate health-related quality of life in patients with melasma attending to dermatology clinics in Kerman city, Iran.

Methods: A cross-sectional study was carried out by clinical examination and questionnaire on 200 patients with melasma. Melasma area and severity index (MASI), Dermatology Life Quality Index (DLQI) and Melasma Quality of Life Scale (MELASQOL) were used to assess melasma severity and quality of life.
Results: There were 200 women with mean age 31.86±6.42 years old (range: 18-55 years). The mean score of DLQI and MELASQOL were 6.90±4.48 and 40.24±14.95, respectively. In 113 (56.5%) patients, melasma had very large effect on their quality of life. There was a significant correlation between the scores of quality of life questionnaires and MASI (P<0.001). In DLQI scale the most frequent impairment was related to emotional well-being and based on MELASQOL questionnaire, embarrassment due to skin appearance had an important impact on quality of life.
Conclusion: This study revealed that physicians must consider the devastating psychosocial impact of pigmentary imperfections in addition to pharmacological and physical treatment.

Abstract

Eruptive syringoma is a rare benign tumor of eccrine sweat gland. Clinically, it presents as successive crops of symmetric, small yellow-brown to erythematous or skin-colored papules on the anterior body surfaces. It usually occurs in young women. Definitive diagnosis can be made on histopathological findings. The pathogenesis of eruptive syringoma is unclear but there are few reports about its association with antiepileptic drugs, radiation, trisomy 21, depilation and hyperthyroidism. The presented case was a 25-year-old woman who had referred with papules in forehead, periorbital regions, cheek, chin, chest, abdomen and forearm. Eruptive syringoma was confirmed based on its distinctive pathological findings. She thought her skin lesions were related to consumption of oral contraceptives. The importance of this disease is its cosmetic problem.

Abstract

Background and Aim: Cutaneous leishmaniasis (CL) is endemic in Iran, where it is one of the most important health problems. Both anthroponotic CL (ACL) caused by L. tropica and zoonotic CL (ZCL) caused by L. major are reported. Antimoniate derivatives as the standard therapy for CL need multiple injections and are not easy to tolerate for the patients. This study was conducted in Mashhad to compare the efficacy of weekly versus twice a week intralesional injections of meglumine antimoniate (MA) in the treatment of ACL.
Methods: This randomised controlled trial was performed during 2006 to 2008 in Mashhad, Iran. Using computerized sequence of random numbers, participants were randomly allocated in the two arms of the study: one receiving weekly and the other receiving twice-a-week intralesional injections of MA. The lesion size, induration and healing rate were assessed, recorded and compared. Healing was defined as complete re-epithelialisation and disappearance of induration.
Results: A total of 252 suspected CL patients with 372 lesions were screened. 82 parasitologically proven cases with 121 lesions caused by L. tropica were included and 74 patients with 113 lesions completed the study. At 12th week after initiation of treatment, complete healing was observed in 38 out of 44 lesions (86.4%) in the group which received weekly intralesional MA injection. The median time-to-heal in this group was 36 days (95% confidence interval [CI]: 32.0-39.9). Complete healing was recorded in 60 out of 69 lesions (86.9%) in the group which received twice a week intralesional injections of MA with a median time-to-heal of 25 days (95% CI: 20.9-29.1). While no significant difference was observed between the two groups in terms of complete healing rate (P=0.999), time-to-heal was significantly different between the 2 groups (P=0.003).
Conclusion: It seems that the effectiveness of twice-weekly intralesional injections of MA is similar to once-weekly regimen while the former regimen causes more rapid healing of lesions.

Abstract

Background and Aim: Adolescents with polycystic ovary syndrome concern about weight gain, menstrual irregularities, acne and hirsutism. The aim of this study was to determine the frequency of facial and trunkal lesions of acne in 14- to 18-year-old high school students with polycystic ovary syndrome in Shiraz in 2009.
Methods: In this cross-sectional study, 3189 14- to 18-year-old female students were enrolled.  Using questionnaires, data on personal information, clinical features and laboratory findings of hyperandrogenism and results of ultrasound examination were collected. Severity of acne was classified as mild, moderate and severe, based on the type and number of the lesions. Chi-squared and independent t tests were used for comparing proportions and means, respectively.
Results: The prevalence of severe acne was 5%. Mean total testosterone in individuals with severe acne was higher in comparison with those without acne. Severity of facial and trunkal acne were associated with polycystic ovaries (P<0.05). Mean level of luteinizing hormone (LH) was higher in those without acne than in those with severe acne (P >0.05).
Conclusion: Associations between severity of facial and trunkal acne lesions and polycystic ovaries were observed. Considering the undesirbale effects of acne on quality of life, hormonal screening and ultrasound examination in girls with moderate acne is recommended.

Abstract

Hyaluronic acid (HA), the main and most important constituent of extracellular matrix, is a glycosaminologycan with water-absorbing capacity found in large amount in growing and repairing tissues. One of the main causes of skin problems, particulary in aging skin, is HA deficiency. More than half of the body HA is in the skin and is necessary for the maintenance of internal matrix and several cellular functions. Filler gels containing HA are used to repair skin defects. As these substances are derived from animals and bacteria, not the human, may cause skin reactions and have short half-life. So efforts to maintain and/or increase HA secretion from skin fibroblasts are important in the prevention and treatment of skin aging.

Abstract

Skin injury caused by burns, surgery and other traumas may result in unpleasant psychological experiences and be reflected in behaviors. Extracellular matrix (ECM) is the largest component of natural skin which is gel-like and is produced by skin cells. ECM synthesis is a key factor for filling up skin wounds such as burns, leishmaniasis, chicken pox, acne, etc. ECM is composed of a variety of polysaccharides, water, and collagen proteins. Considering its weight, natural skin strength and its expandability are like steel, while it has high elasticity and compaction capacities. These characteristics are due to dual effects of main ECM molecules, which are secreted by fibroblasts and epidermal cells: 1) structural fiber proteins like: elastin, fibronectin and laminin which give strength and flexibility to ECM, and 2) proteoglycans such as dermatan sulfate and hyaluronic acid which are consisted of few glycosaminoglycan chains that branch out from a linear protein core. Proteoglycans are large and hydrated molecules which are resistant to external forces and protect underneath cells. In general, understanding the skin structure and wound healing phases can help us to design useful experiments and to conduct proper researches in this area.

Abstract

Background and Aim: Androgenic alopecia is one of the most common dermatological disorders. Hormonal and genetic factors determine the incidence of alopecia. Androgenic alopecia is the most common form of hair loss. It is progressive and symmetric in both sexes. The aim of this study was to determine the prevalence of androgenic alopecia in 14- to 18-year-old female high school students in Shiraz.

Methods: This cross-sectional study was conducted on 3190 14- to 18-year-old female high school students in Shiraz. A questionnaire was designed to collect personal information and record clinical features of androgenic alopecia.  Moderate and severe hair losses were defined as alopcia. For evaluation of alopecia associated with polycystic ovary syndrome, total and free testosterone blood levels were measured and abdominal ultrasound was performed. The collected data were described and analyzed through performing chi-square, Fisher's exact and and t tests using SPSS.

Results: 135 (4.2%) of participants had moderate to severe alopecia according to Ludwig's classification. No statistically significant correlation was observed in the mean of eather total or free testosterone levels between patients with androgenic alopecia, and those without (P>0.05). There was no statistical significant association between alpocia and polycystic ovary existence (P=0.2).

Conclusion: This study did not show a statistically significant relationship between patients with alopecia and androgenic hormones and polycystic ovary syndrome.

Abstract

Background and Aim: Synthesis of extracellular matrix (ECM) is a key factor in wound healing. The predominant component of ECM is hyaluronic acid (HA). Recent studies have shown that adipose derived stem cell-conditioned medium (ADSC-CM) can increase the expression and synthesis of collagen I, III and fibronectin which are the components of ECM. The purpose of this study was to investigate the effects of ADSC-CM on the expression of HA synthase and degrading genes in human dermal fibroblasts (HDF).
Methods: ADSCs were isolated from three liposuction samples and their conditioned medium was collected. Skin biopsies were isolated from six patients and their fibroblasts were isolated. These cells were cultured in 100% of ADSC-CM. Expression of three HA synthase and two hyaluronidase genes were assessed with the use of real time PCR, with ELISA and final amounts of secreted HA were measured in the treated and control groups.
Results: Gene expression of two HA synthase genes HAS1 and HAS2 were upregulated in treated samples compared to the control group. The expression of one hyaluronidase gene, HYAL2, was downregulated in them. Final amount of HA was increased in the treated samples.
Conclusion: The expression and synthesis of HA can be increased by the stimulatory effect of growth factors present in ADSC-CM. Thus, secretomes of ADSCs could play a crucial role in wound healing by up-regulating the production of ECM’s components in HDFs.

Abstract

Background and Aim: C-reactive protein (CRP) is an inflammatory biomarker and its level increases in serum of psoriatic patients. Its level is also associated with Psoriasis Area and Severity Index (PASI) score.
Methods: CRP serum levels in patients with psoriasis were measured before and after treatment with narrow-band ultraviolet B (NB-UVB) and the data were analyzed in relation the PASI score improvement.
Results: Baseline CRP levels among psoriatic patients  were higher than normal. These levels decreased significantly after treatment. At the beginning of the study, patients with higher levels of CRP also had more extensive and severe skin involvement. Highest decrease in CRP was observed in patients who responded better to the treatment and acheived higher PASI 75%. There was an association between baseline PASI scores and CRP levels.
Conclusion: Patients with moderate to severe plaque-type psoriasis had systemic inflammation too, which was demonstrated by having higher levels of CRP. Skin disease activity also was associated with higher levels of CRP. Phototherapy healed the psoriatic skin lesions and also reduced inflammation and decreased CRP levels.

Abstract

Background and Aim: Dermatophytes are keratinophilic fungi that cause infection in the skin, hair and nails. Dermatophytic infections are one of the important health problems in the world that related to social and economic life of communities.
Methods: This cross-sectional study was conducted on all suspected patients in the medical mycology laboratory of Afzalipoor Faculty in Kerman from March 2007 to March 2011. For each patient after obtaining the detailed history, direct smear, staining and culture on specific media were performed.
Results: Out of 1800 referred cases, 166 patients were positive for dermatophytosis. The highest prevalence of infection was in the under 10 years old (28.31%), men and women were also involved equally. The most common agents of the cutaneous infection were Trichophyton mentagrophytes (45.78%) and T. verrucosum (18.1%). The most common infections were tinea manuum (35.54%), tinea capitis (16.87), and the least common one was tinea faciei (4.82%).
Conclusion: It is necessary to increase the level of community health in order to prevent this dermatophytosis.

Abstract

Background and Aim: Basal cell carcinoma (BCC) is the most common type of malignancy that rarely causes death but results in cosmetic problems and is associated with high treatment cost. Clinical characteristics of this cancer are related to the host and environmental factors. Theses factors are specific for each area and may change over the time. The aim of this study was to find clinical characteristics of BCC in Kermanshah city.
Methods: This cross-sectional study was done on 119 histopathologically documented BCC cases in Haj Dayi skin clinic in Kermanshah in 2009. Demographic data of patients and characteristics of the lesions including their number, location, size and clinical type were recorded. Data were analyzed using ANOVA and chi squared tests.
Results: The patients had a total number of 171 BCC lesions. Nodular type encompassed 158 (92.4%) lesions and was the most common clinical form. Pigmented subtype included one third of lesions and was significantly more common in male patients with moderate brown skin and following radiotherapy (P<0.05). The nose was the most common area of involvement and actinic keratosis was the most common associated skin lesion.
Conclusion: In this study, the most common clinical and pathological type of BCC was nodular type. It was observed in old patients, those with outdoor jobs and was associated with actinic keratosis. Educational and screening programs for prevention, early detection and diagnosis, and appropriate treatment for high-risk individuals are suggested.

Abstract

Background and Aim: Onychomycosis is the fungal infection of nails. Patient’s immune system has an important role in initiation of onychomycosis. One of the causes of the increased prevalence of onychomycosis is the increasing number of patients with local or general immune system disorders. In hormonal disorders such as diabetes mellitus, Cushing's syndrome and hypothyroidism superficial vascular disorders and individuals with immune deficiency, onychomycosis prevalent. The aim of this study was to investigate onychomycosis in individuals with underlying diseases who referred to Razi hospital in Tehran.
Methods: In this cross-sectional study, nail specimens of 250 patients who have nail dystrophy with an underlying disease whom were referred to mycological laboratory of Razi hospital in 2010-11 were evaluated with direct microscopy and culture on Sabouraud dextrose agar and Sabouraud dextrose agar containing chloramphenicol and cyclohexamide. For differential diagnosis of fungi, complementary tests were used.
Results: Eighty-nine (35.6%) of 250 individuals with underlying diseases and nail dystrophy, were affected with onychomycosis. Among these pateints, 52 (58.4%) cases had yeast infection, 19 (21.3%) had dermatophyte infection, and 18 (20.2%) had saprophyte infection. Diabetes mellitus (37.1%), cardiovascular diseases (14.7%), pemphigus (9%), psoriasis (7.9%), hypothyroidism (6.7%) and chronic eczema (5.6%) were the most common underlying diseases observed in patients with onychomycosis.
Conclusion: Onychomycosis is common in hormonal disorders like diabetes mellitus and hypo-thyroidism superficial vascular disorders and individual who suffer immunodeficiency.

Abstract

Background and Aim: Pruritus is a major symptom of dermatologic diseases and a common symptom of systemic diseases. Severity of itch cannot be measured objectively. It’s usually determined to using a Visual Analogue Scale (VAS). But none of them can determine the effect of pruritus on patients' quality of life. So in the present study we sought to translate and develop a validated reliable Farsi version of pruritus specific quality of life instrument, ItchyQoL.

Methods: ItchyQoL questionnaire was translated to Farsi by two bilingual people, and then it was back-translated to English by two other bilinguals. These two questionnaires were matched and revised and the final Farsi questionnaire was established. Forty four patients with dermatologic pruritus filled the questionnaire at the baseline and 72 hours later, and the structural reliability and validity of it was analyzed.

Results: The questionnaire had internal consistency and reproducibility. The questionnaire reliability was also analyzed by test-retest which was significant. Construct validity was analyzed by examining the relationship between intensity and frequency of itching with the total score in the three main axis symptoms, which was significant in all except the dysfunction axis.

Conclusion: The Farsi version of ItchyQoL questionnaire has favorable reliability and validity.

Abstract

Background and Aim: Psoriasis is a chronic inflammatory dermatosis that affects 0.6% to 4.8% of the general population. Our aim was to compare the effect of narrow band UVB (NB-UVB) on normal flora of cutaneous plaques and normal skin in psoriatic patients.

Methods: Twenty psoriatic patients without any contraindication of phototherapy were enrolled in this study. Samples from psoriatic lesions and adjacent normal skin, before and after phototherapy with NB-UVB were taken. Patients were treated with NB-UVB three times weekly until complete remission. Finally, quantity of normal cutaneous flora on normal and involved skin, before and after treatment was compared.

Results: The mean±SD of the NB-UVB sessions and the mean cumulative dose were 26.35±4.95 and 2.05±0.57 J/cm2, respectively. The number of Staphylococcus epidermidis, S. aureus and diphtheroids on psoriatic plaques reduced after phototherapy.

Conclusuion: The quantity of normal cutaneous flora on normal skin is less than psoriatic plaques, and it reduced on normal and involved skin after phototherapy.
