Abstract

Background and aim: Oral lichen planus, is a chronic inflammatory mucocutanous lesion which is defined as a premalignant lesion. Matrix metaloprotenase 2 (MMP-2) and MMP-9 are protenases which facilitates invasion of tumoral cells to connective tissue and are over-expressed in premalignant and malignant lesions. The aim of this study was to determine the expression of MMP-2 in erosive and non-erosive oral lichen planus and inflammatory hyperplasia by immunohistochemistry.
Methods: in this cross sectional study 39 specimens of non-erosive oral lichen planus (with and without dysplasia), 36 specimens of erosive ones (11 dysplastic and 25 non-dysplastic, 32 inflammatory hyperplasias and 6 samples of normal oral mucosa were chosen form pathology archive of Faculty of Dentistry, Mashhad university of Medical Sciences and were stained immunohistochemically by MMP-2 and MMP-9 markers. Kruskal-Wallis, Mann-Whitney and Spearman correlation coefficient were used.
Results: The highest mean expression of MMP-2 and MMP-9 (in all layers) were observed in erosive oral lichen planus (68.18±31.62 and 94.5±3.5, respectively). MMP-9 expression in erosive oral lichen planus in comparison to non-erosive one showed significant difference in all layers (P<0.001) and the expression of MMP-2 showed a significant difference in basal –para basal and prickle layers (respectively: P=0.008, P<0.001).
Conclusion: the linear increase seen in the expression of markers from normal mucosa to erosive oral lichen planus indicates the difference of biological behavior between erosive and non-erosive oral lichen planus. Hence, it is concluded that erosive oral lichen planus is of the higher premalignan potential compared with its non-erosive correspondent.

Abstract

Background and Aim: Sarcoidosis is a non-caseous granulomatous disease that can involve several organs such as lung, kidney, liver, heart and skin. In systemic sarcoidosis, skin lesions occur in 20-35% of patients. Cutaneous sarcoidosis with no systemic involvement was found in about 25% of patients. Mutation within Butyrophilin-like 2 (BTNL2) gene, rs2076530 was reported in systemic sarcoidosis. However, there is no report of evaluation of mutation in BTNL2 gene with the diagnosis of skin sarcoidosis.
Methods: In this study ten patients with skin sarcoidosis were evaluated for the mutation of rs2076530 allele in exon 5 of BTNL2 gene. This assessment was performed by the single strand conformation polymerase chain reaction (SSCP-PCR) in which the existing mutations with positive shift were deteced using directl sequence analysis. Data from sequence analysis were evaluated and blusted by means of Choroms computer software.
Results: Our results showed the BTNL2 G→A transition of rs2076530 in seven patients and three patients were normal.
Conclusion: This pilot study concludes the presence of a mutation at rs2076530 in exon 5 of BTNL2 gene in patients with skin sarcoidosis. Larger studies are needed to evaluate the role of this finding.

Abstract

Background and Aim: Pemphigus vulgaris (PV) is an autoimmune blistering disease which is specific for skin and mucosal membranes. Its association with connective tissue diseases has already been reported. Considering this association, presence of Anti Nuclear Antibodies (ANAs) in PV patients will not be surprising. The aim of this study was to compare the frequency of ANA positive cases in patients suffering PV with a control group.
Methods: In this case-control study, the cases were selected from the patients with PV whom were hospitalized at Razi Hospital, Tehran. The controls were chosen from patients who did not have PV. The data were collected using a questionnaire, which was designed for gathering information on participants' age, sex, PV phenotypes, and the result of ANA test. ANA positivity was assessed using indirect immunofluorescence, HEP2.
Results: In 8 (26.7%) of 30 PV patients and 3 (10.0%) of 30 controls ANA was positive (P=0.095). The most common ANA positive patterns among cases and controls were homogeneous and speckled patterns, respectively (P=0.26).
Conclusion: Although in this study the frequency of positive ANA result among controls was similar to what were reported in previous studies, the frequency of this finding among PV patients was different from the previous reports. Differences in the sensitivity of the laboratory kits used in different studies as well as in the threshold for ANA positivity, and differences in the patients' eligibility criteria in different studies may explain the observed discrepancies. Clinical follow up of the PV and requesting an ANA test in the case of appearance of the signs of connective tissue diseases is recommended.

Abstract

Background and Aim: Consumption of the epidermis (COE) is defined as thinning of the epidermis with attenuation of the basal and suprabasal layers and loss of rete ridges in areas of direct contact with malignant melanocytes. The aim of this study was to investigate the importance of COE as an additional diagnostic criterion for malignant melanoma and to evaluate its relationship to clinicopathological findings.

Methodes: The age, gender, localization of the lesion and the histopathological parameters such as tumor type, Breslow thickness, ulceration, and Clark's level were recoeded in 90 malignant melanoma cases.

Results: In contrast to other studies, we found that COE was more common in tumors with an acral localization and in the acral lentiginous melanoma.

Conclusion: Although COE can be used as a pathological criterion in the diagnosis of malignant melanoma, but no correlation no of COE with ulceration and other prognostic factors were found.

Abstract

Background and Aim: Cicatricial alopecia is characterized by permanent destruction of the pilosebaceous unit, leading to irreversible hair loss, and has primary and secondary types. Primary type, based on predominant inflammatory infiltration in histology, is divided to neutrophilic, lymphocytic and mix types. Decalvans folliculitis, dissecting cellulitis and acne keloidalis are 3 subtypes of neutrophilic group. In this study frequency of each subtype and the distribution of age, gender and duration of disease have been reviewed.

Methods: This retrospective study was performed on patients with clinical diagnosis of cicatricial alopecia and histologic confirmation referred to Razi hospital in Tehran in 2005-2009.

Results: Of 135 patients with mean (mean±SD) age of 32±10.6 years, 71.9% were male and 28.1% were female. The diagnoses included decalvans folliulitis (72.6%) with mean age of 31.89 years, dissecting cellulitis (25.0%) with mean age of 32.4 years and acne keloidalis (1.5%) with mean age of 52 years. The mean duration of disease was 4.2±3.9 years (4.08 years in male and 4.7 years in female). The mean duration of disease in decalvans folliulitis was 4.7, in dissecting cellulitis was 1.9 and in acne keloidalis was 17.5 years.

Conclusion: Neutrophilic scarring alopecia is more common in men and in the third decade of age and the most common type is decalvans folliulitis.

Abstract

Background an Aim: parapsoriasis is a rare chronic idiopathic dermatosis which commonly manifests with round or oval scaling and itching erythematosus plaques on trunk and extremities. Considering the size of the lesions, parapsoriasis is divided into two groups: small plaque and large plaque. Study of demographic characteristics of this disease may help to identify the high risk groups who are more susceptible to cutaneous lymphoma with high mortality as well as proper allocation of health service resources.
Methods: In this retrospective cross-sectional study, affected parapsoriasis patients whom were referred to Razi Hospital in Tehran between April 2003 to March 2009 were studied. Three hundred and twenty patients were investigated for variables: age, sex, place of residence, location of the lesions and clinical subgroups according to the histopathology reports.
Results: 53.4% of patients were female. The disease was most commonly observed in age group of 20 to 50 years and the mean age of the patients was 43.3 years. Eighty-five percent of the patients were inhabitant of Tehran province. 88.1% of patients had large plaque lesions and 11.9% had small plaque lesions. The most common sites of lesions were on the trunk (53.4%) followed by the extremities (38.7%).
Conclusion: Based on these findings, it was determined that parapsoriasis most commonly affects middle-aged women. Involvement of trunks and extremities were most commonly observed. Large plaque type was the most common type.

Abstract

Great progress in optics, electronic engineering, and computer science provide an opportunity to diagnosis of skin diseases, estimating effectiveness of drugs and skin care products using non-invasive, non-destructive methods of analysis. Also many noninvasive methods using analytic devices are developed to assessment of the outcomes of dermatologic surgeries on malignant skin tumors. The aim of this article is to review advanced methods for skin analysis on human’s skin such as magnetic resonance imaging and time domain reflectometry. Many kind of interesting applications are defined for using these methods especially in cosmetic pressures.

Abstract

Ichthyosis is defined as a group of diseases with keratinization disorder and diffuse scaling with highly variable degree of involvement. According to our knowledge, coincidence of ichthyosis and dermatophytosis, which both are very common disorders, is a very rare event. We report a young man with congenital ichthyosis that histological analysis of his skin biopsies and direct smear revealed PAS positive fungi. He had used topical steroids for generalized scaling erythematous patches and plaques for long time.

Abstract

Background and Aim: Due to new popularity of cosmetic tattoos especially among women and its potential relationship with sarcoidosis, the present study was conducted to evaluate clinical presentation and some demographic data about tattoo granuloma, as part of non-infectious dermatologic granulomas. Methods: In this retrospective cross-sectional study, all patients with the histopathological diagnosis of non-infectious granulomatous lesions whom were admitted between at the dermatology clinics at Razi Hospital, Tehran, Iran between March 21, 2007 and March 20, 2012 were included. Results: A total of 1099 patients were included in the study. The commonest form of granuloma was foreign body granuloma in 511 (46.4%) of the patients and was followed by tattoo granuloma in 102 (9.2%) of the patients. All patients with tattoo granuloma were females whose ages were from 40 to 60 years. The mean±standard deviation age of the patients was 43±8.57 years. Twenty-six (25%) of patients with tattoo granuloma showed sarcoidal pattern, and one case (1%) showed caseouse pattern, after histopathological evaluation of biopsy specimens. Majority of the lesions were papular (32.3%) followed by nodular (24.5%) lesions. Conclusion: Tattoo granuloma is the second common form of non-infectious granulomas, and in one-quarter of cases has sarcoidal pattern. Considering this finding, it seems reasonable to evaluate these patients for sarcoidosis.

Abstract

Background and Aim: Bullous pemphigoid (BP) is an acquired autoimmune disease with subepidermal blisters commonly seen in the elderly over 60 years. Although the disease is usually manifested with tense blisters, but in some cases generalized pruritus is the only symptom of disease, which is less studied.

Methods: In a retrospective study, the medical records of 416 BP patients confirmed in pathology laboratory were evaluated for clinical findings, direct immunofluorescence (DIF) testing for BMZ-binding IgG antibodies and histological findings.

Results: Of the 416 confirmed BP patients, 213 were male and 203 were female. The mean age of patients was 64.36 years. Thirty four patients at the onset of the disease had generalized pruritus, which in the course of the disease, 29 patients also showed other symptoms of the disease. In 5 patients generalized pruritus was seen as the only symptom. Concerning to the results of DIF, 117 (90%) patients showed autoantibody deposit, and in 13 (10%) patients the test was negative. There was no significant relationship between DIF results and histological findings (p> 0.05).

Conclusion: According to the findings of this study, generalized pruritus is uncommon as the first symptom of the disease. There was not any association between generalized pruritus and histological observations including dermal edema, presence of eosinophils in tissue, subepidermal blister as well as DIF results. Further studies on the early detection of the disease are required to decrease complications of the disease.

Abstract

Background and Aim: Preoperative assessment of skin tumors extensions have a potential role to increase the efficiency of surgery and maximal preservation of function and cosmetics in the treatment of invasive types of basal cell carcinoma (BCC).
Methods: A total of 72 patients with BCCs on face were included in the study. Gross maximal width of tumors were documented, and then high frequency ultrasonic (HFUS) maximal width and depth of these lesions were determined. Then conventional surgical excision was done and these margins were determined in pathology lab. The comparison of ultrasonic and pathologic depth, and difference of HFUS and pathologic widths from gross clinical width were done with paired t-test.
Results: The mean depth of BCCs were 3.09 mm (SD= 1.02) in HFUS and 2.86 mm (SD=1.22) in pathology (P=0.1). The mean difference from gross width were 4.05 mm (SD=1.18) in HFUS and 4.26 mm (SD=1.70) in pathology (P=0.09).
Conclusions: High frequency sonography has a good accuracy in detection of width and depth of BCCs of face, especially in tumors smaller than 10 mm, and can help to determine safe margins non-invasively, especially in cosmetic and functional critical areas.

Abstract


Introduction: Idiopathic granulomatous mastitis (IGM) is a rare benign breast disease. The importance of this disease relies on its similar clinical manifestation to those of mammary carcinoma and a group of mastitis with other causes.

Case Report: The patient was a 32-year-old woman, that suddenly got fever, pain and swelling in left breast. In examination several masses were felt. Performed core needle biopsy reported IGM.

Conclusion: Similarity of clinical presentations and diagnostic tests to the other kinds of mastitis and malignancies may lead to misdiagnosis and mistreatment, which imposes socioeconomic and psychological burden to patients.

Abstract

ntroduction: Leprosy is an infectious disease which primarily affect skin and peripheral nerves. Mycobacterium leprae and Mycobacterium lepromatosis that are the acid-fast bacillus are known to be cause of leprosy. Genetic factors and immunological function have some roles in susceptibility of developing leprosy. There are some cases of familial leprosy.

Case Report: Here in, we report a case of familial leprosy which in one member it was symptomatic and others were asymptomatic with positive skin smear for mycobacterium leprosy bacilli.

Conclusion: We recommend in addition to approach and manage lepromatous patients, their families be also evaluated.

Abstract

Background and Aim: Direct immunofluorescence (DIF) is represented as a gold standard method in diagnosis of autoimmune blistering dermatoses. Normal saline, liquid nitrogen and michel’ solution are a widely accepted media for preserving biopsy samples of skin or mucosa before DIF examination. Occasionally clinicians put the biopsy specimen taken for DIF in formalin 10%, occasionally clinicians ordered DIF retrogradely and only a paraffin-embedded biopsy specimen exposed to formalin 10% is available. To determine the diagnostic value of DIF when it was performed on biopsy samples of skin or mucosa exposed to formalin 10% in comparison to the same biopsy samples exposed to normal saline.
 

Methods: In 74 patients (38 immunobullous and 23 chronic dermatitis), which the latter served as the normal controls, 2 perilesional punch biopsy of skin or mucosa were done, one put in formalin 10% and fixed in paraffin and one put in normal saline, and DIF was done on both samples. 
 

Results: DIF sensitivity and specificity was with IgG 31.5% and 100% in pemphigus and 15.36% and 93.44% in BP, with C3 39.47% and 100% in pemphigus and 7.69% and 91.80% in BP, respectively.
 

Conclusion: DIF on specimens exposed to formalin 10% in comparison to specimens exposed to normal saline is less sensitive but approximately as specific as it is in the diagnosis of pemphigus and BP patients and especially can be useful in pemphigus patients when only a formalin exposed samples is available.

Abstract

Background and Aim: One of the most important pathologies of tattooing is granulomatous reaction to the tattoo which is usually represented by symptoms like pain, inflation, redness and tenderness at the tattoo area. Granulomatous reaction to tattoo occurs at all ages and has been seen among both male and female. Due to the new rise in popularity of cosmetic tattoos and its pathologies, we intend to explore a descriptive study of the demographic and histopathologic characteristics of the granulomatous reactions due to cosmetic tattoos.
 

Methods: Razi hospital patients with proven tattoo granuloma pathology in a 3-year period were identified and studied and their demographic-histological characteristics were recorded. The study is of a cross-sectional type. 196 patients were enrolled in the study, a questionnaire was prepared for each patient and information was collected through them. 
 

Results: The mean ± standard deviation age of the patients was 45.46±8.54. The mean ± standard deviation tattoo time was 4.18±4.33. The mean ± standard deviation frequency of tattooing was 1.64±1.8. The most common tattoo area was eyebrow in 88.3% of the patients and least common was upper eyelid in 0.6%. The most common pathology result was tattoo granuloma in 41.7% of the patients and the least common was Lichenoid in 2.8%.
 

Conclusion: Considering the highest frequency of pathological diagnosis of tattoo-granuloma (Sarcoidal and foreign body) with 41.7% in our study and observing the relationship between tattoo-granuloma and sarcoidosis in previous studies, complete examination of patients, long-term follow-up of patients which will have sarcoidosis in the future and examining the clinical response to treatment seems logical and beneficial. It is also important to consider whether the treatment of current lesions will play a role in preventing sarcoidosis in the future or not.

Abstract

Introduction: Darrier disease is a genodermatoses evolving more in adolescence and appears mostly in seborrheic areas. Because of the absence of typical lesions and other diseases appearing in these areas, diagnosis of darrier may be delayed
 

Case report: A 46 year-old woman came with inguinal, asymptomatic lesions lesions since 3 years ago. The report of biopsy which was taken, showed findings compatible with Darrier disease. Oral isotretinoin initiated with good response after 2 months.
 

Conclusion: In intertriginous areas, some less common diseases may be mis diagnosed as other more common and similar diseases.

Abstract

Background and Aim: The most prevalent form of leishmaniasis is cutaneous leishmaniasis (CL), which manifests as a single ulcerative or nodular lesion that usually heals on its own. The aims of this study were to investigate the histopathologic and clinical features of CL patients.
 

Methods: This cross-sectional study was accomplished on 70 CL patients that referred to Razi dermatology hospital in Iran, over a five-year period from 2016 to 2020. Included patients were those who were diagnosed clinically suspected patients whom their disease was confirmed through histopathology examination.
 

Results: The age of patients ranged from 2 to 76 years with a mean of 42.26 ± 20.86 years. The majority of patients were men (54.3%). The plaque form lesions were the most frequent type (64.3%). Respecting lesion site, face, and upper limbs were more frequent with the abundances of 47.1% and 34.3%, respectively. All of the patients had evidence of cellulitis on ultrasound. The most prevalent change of epidermis was hyperkeratosis (42.8%). Regarding dermal histopathologic changes, infiltration of lymphocytes (61.4%), plasma cells (58.6%), and histiocytes (48.6%), as well as granuloma formation (54.3%) and leishman body presence (54.3%) were the dominant changes. Some patients had atypical forms of CL as six of them had lupoid cutaneous leishmaniasis (LCL) and five had sporotrichoid cutaneous leishmaniasis (SCL).
 

Conclusion: In this study, men and those who were in middle age were more susceptible to be infected, so it suggests considering them particularly in the preventive programs. Moreover, histopathological findings of CL, including LCL and SCL, are presented. These findings may be beneficial for dermatologists and dermatopathologists to differentiate them from other cutaneous diseases.
