Abstract

Background and Aim: Onychomycosis is the fungal infection of nails. Patient’s immune system has an important role in initiation of onychomycosis. One of the causes of the increased prevalence of onychomycosis is the increasing number of patients with local or general immune system disorders. In hormonal disorders such as diabetes mellitus, Cushing's syndrome and hypothyroidism superficial vascular disorders and individuals with immune deficiency, onychomycosis prevalent. The aim of this study was to investigate onychomycosis in individuals with underlying diseases who referred to Razi hospital in Tehran.
Methods: In this cross-sectional study, nail specimens of 250 patients who have nail dystrophy with an underlying disease whom were referred to mycological laboratory of Razi hospital in 2010-11 were evaluated with direct microscopy and culture on Sabouraud dextrose agar and Sabouraud dextrose agar containing chloramphenicol and cyclohexamide. For differential diagnosis of fungi, complementary tests were used.
Results: Eighty-nine (35.6%) of 250 individuals with underlying diseases and nail dystrophy, were affected with onychomycosis. Among these pateints, 52 (58.4%) cases had yeast infection, 19 (21.3%) had dermatophyte infection, and 18 (20.2%) had saprophyte infection. Diabetes mellitus (37.1%), cardiovascular diseases (14.7%), pemphigus (9%), psoriasis (7.9%), hypothyroidism (6.7%) and chronic eczema (5.6%) were the most common underlying diseases observed in patients with onychomycosis.
Conclusion: Onychomycosis is common in hormonal disorders like diabetes mellitus and hypo-thyroidism superficial vascular disorders and individual who suffer immunodeficiency.

Abstract

Background and Aim: Thalassemia is one of the most common single gene disorder that results from decrease or absence of globulin chain(s) synthesis. Repeated blood transfusion is required for prevention of severe anemia and related side effects, but it is associated with other complications such as cutaneous lesions and mucosal manifestations. This study conducted to assess the frequency of cutaneous and mucosal manifestations in patients with beta thalassemia.
Methods: This cross-sectional study was conducted on 300 patients with beta major thalassemia in Zahedan. Data were obtained from medical records and questionnaires as well as through physical examination of patients. The data were statistically described and tested.
Results: The mean age of patients was 17.9±6.3 years and 64.3% of patients were men and 35.7% were women. Among cutaneous lesions, freckles were the most common (70.7%) and the rarest one was peri orbital pigmentation (0.3%). Gingivitis (41.7%) and longitudinal lines on nails (72%) were the most common mucosal and nail manifestations.
Conclusion: Since cutaneous and mucosal manifestations are very common among patients with beta major thalassemia, regular dermatology follow up of these patients is recommended.

Abstract

Background and Aim: Cosmetic surgeries have been very common and popular in recent decades. Personality traits and mental health could affect people`s motivations of doing cosmetic surgery as a psychological pattern. The aim of this study was to compare  maladaptive personality traits and mental health between cosmetic surgery applicants and control group.

Methods: In this descriptive-comparative study, 45  applicants for rhinoplasty and filler injections, who were  selected by available sampling method, answered to DSM-5 Self-Rated Level 1 Cross-Cutting Symptom Measure and the personality inventory for DSM-5 (PID-5). Forty five nonapplicants matched with applicants with age, sex and education were also included as a control group . They answered to the same questionnaires too. The data were analyzed by "multivariate analysis of variance" (MANOVA).

Results: Rhinoplasty applicants were not significantly different from nonapplicants neither  in maladaptive personality traits nor mental health. But applicants for  gel injections were significantly different from control group in detachment (mean differences = - 0.36, P<0.05) , depression
(-0.91, P<0.01), Psychosis (-0.44, P<0.05) and substance abuse (-0.57, P<0.05).

Conclusion: Plastic surgeons and cosmetic professionals must reconsider applicant`s personality and mental health. Comprehensive psychological assessments must be done, specially for gel injection applicants before any action for surgery.

Abstract

Background and Aim: Schema modes include specific schemata and coping styles which are activated in emotional situations. They create formation of behavioral patterns compatible with their own schemata. The purpose of this study was to predict the quality of life in vitiligo patients according to their schema modes.

Methods: In this descriptive-correlational study,170 vitiligo patients were selected by simple random sampling method. They filled Schema Modes Inventory and brief form of the World Health Organization`s Quality of Life questionnaire. The data were analyzed using descriptive statistics, Pearson correlation coefficient, and stepwise regression.

Results: The results of the analysis of the Pearson correlation coefficient revealed that quality of life was related to all schema modes except detached self-soother mode. Happy child and healthy adult schema modes had a significant positive relationship with quality of life, while the other schema modes (except detached self-soother) had a significant negative relationship with quality of life in vitiligo patients. Also, the results of stepwise regression showed that vulnerable child, detached protector, compliant surrender and happy child schema modes had the strongest effect in predicting the quality of life, respectively.

Conclusion: Regarding the importance of the role of schema modes in quality of life in vitiligo patients, offering psychotherapies such as schema therapy seems necessary.

Abstract

Introduction: Fleas are small, blood-sucking insects that can transmit some pathogens to human and animals.

Case Report: This paper presents a 21-year-old woman with severe skin reactions in her right forearm. After initial examinations, the symptoms were recognized as skin reaction caused by the bite of some human blood-sucking insects. Finally, after 15 days of flea bites, the symptoms were treated with topical after bite cream and oral antihistamines. In order to prevent and control the infestation of this blood-sucking pest, public awareness should be increased specially in at risk populations.

Conclusion: Increasing awareness among physicians, about the symptoms of this health pest can help to accelerate treatment process.

Abstract

The current monkeypox outbreak reawakens the concern that poxviridae have a high potential for zoonotic spillover and causing a pandemic. Much fieldwork and research have been done by health care and public health workers in Africa during previous human outbreaks, and their knowledge should inform our global response to the current outbreak. However, unusual clinical presentations now have potential implications for recognizing the disease. In addition, infections from poxviridae, such as monkeypox, have common cutaneous signs that occur early, may be related to periods of transmissibility, and can leave scarring. Therefore, dermatologists will play a key role in recognizing and diagnosing infections and educating and preparing frontline health care workers for the early detection of new cases and clusters of monkeypox.
