Abstract
Topical corticosteroids (TCs) are very useful in the treatment of some skin disorders such as eczema and psoriasis. TCs exhibit potent anti-inflammatory and anti-proliferative effects responsible for their efficacy in the treatment of skin disorders. TCs can be classified into different groups according to their potency that depends on the intrinsic activity of the corticosteroid molecule, the characteristics of the vehicle (cream, ointment, lotion) that directly influence patient’s compliance and may modify penetration through the skin. British National Formulary classified them into four potency groups with Class I grouping the most potent molecules (e.g. clobetasol propionate) and Class IV the least potent (e.g. hydrocortisone). In contrast, the American classification considers seven potency groups, with super potent, potent, upper mid-strength, mid-strength, lower mid-strength, mild and least potent.

Abstract
Three decades have passed since the first time evidence-based medicine (EBM), in its current form, was introduced. Arguably, over the past two decades it has been the most significant movement in medicine. There is no need to emphasize any approach to clinical medicine practice, including EBM, has its proponents and opponents. Certainly, EBM has been subjected to many challenges and criticisms. Some of those challenges are basically related to misunderstanding and misinterpretations of EBM. However, there are some challenges that are real and are acknowledged by the most loyal EBM supporters. After hearing some criticisms posed by few knowledgeable and competent dermatologists during the recently held 7th Evidence-Based Dermatology (EBD) seminar in Tehran, the authors of this letter, two of the pioneers of EBD in Iran, decide to open a gate for sharing and discussing different viewpoints on EBM and EBD.

Abstract
Background and Aim: Androgenetic alopecia (AGA) is the most common form of alopecia in men, worldwide and topical minoxidil appears to be the most effective and safe treatment for AGA beside oral finasteride. Safety assessment of topical minoxidil formulation could be conducted using in vivo studies on human volunteers. Using skin biophysical parameters is a fast and noninvasive method for safety assessment of topical products.
Methods: A topical combination of Minoxidil 5% solution in combination with chamomile extract from Iran’s pharmaceutical market tested on the skin of the forearm of 15 healthy volunteers. Changes in transepidermal water loss (TEWL), stratum corneum hydration, pH, melanin and hemoglobin contents have been evaluated, using biometric assessments with MPA 580 Cutometer®.
Results: There was not any significant change in skin hydration (P=0.692), TEWL (P=0.191), stratum corneum pH (P=0.613), melanin (P=0.533), and hemoglobin (P=0.579) contents before and after topical minoxidil application.
Conclusion: No significant change detected in skin erythema and assessed parameters Application of minoxidil 5% + chamomile 7% topical solution on human volunteers has not shown any adverse effects and these formulations could be recommended to use.

Abstract
ackground and Aim: Psoriasis is a complex disease with lifelong social and emotional consequences. It also impairs quality of life (QoL) and needs prolonged management. So in addition to suitable treatment, self management strategies are required to improve patient health and QoL. On the other hand, mobile phone applications have changed the interaction of patients with health services. The objective of this study was recognition of informative and educational parameters for the development of an application for self management of psoriasis.
Methods: In this cross-sectional study, 100 psoriatic patients and 26 dermatologists were randomly selected. The investigation. Tool was a questionnaire developed by researchers including demographic and clinical data, education and management of life style, and capabilities of application for self management of psoriasis, which its validity was approved by a group of experts and its reliability was confirmed by test- retest.
Results: The mean scores for importance of demographic and clinical data, education and management of life style, and capabilities of self management application were 80.6%, 85.7%, 88.8% by the patients, and 83.7%, 71%, and 75% by dermatologists, respectively.
Conclusion: The recognition of self management parameters considered important by patients, as those involved directly with the disease, and by dermatologists, as experts in the management of disease, are essential in the efficacy and applicability of self management of psoriasis. Patients who know the benefits and risks of self management programs and have self-confidence are more compliant with treatment programs determined by their physicians, and manage their disease more seriously.

Abstract
Background and Aim: Dandruff and seborrheic dermatitis (SD) are common chronic skin diseases. There are many chemical drugs for treating dandruff and SD. Prolonged use of them may cause side effects and drug resistance. The aim of this study was to compare the efficacy of a herbal extract lotion with clotrimazole 1% lotion in the treatment of SD.
Methods: Twenty volunteers (12 men and 8 women) aged 20 to 40 years, with seborrheic dermatitis were enrolled in this trial and were randomly allocated into two groups. One group was treated with a herbal extract lotion and the other group with clotrimazole lotion. The herbal extract contained common sage, thyme vulgaris, tea tree oil, lavender, willow, rosemary, and aloe vera. The patients were instructed to use the lotions three times a week for 4 weeks. We evaluated the patients by clinical assessment and measured the scalp sebum, pH, and dandruff number before two weeks, and four weeks after completed a treatment. At the end of the study, each participant satisfaction form.
Results: At the end of the study, the participants whom received the herbal extract lotion had a significantly greater reduction in scalp scaling, erythema, itching, and number of dandruff compared with those who received clotrimazole (P<0.05). The patient satisfaction score was also higher in the herbal extract group (60% versus 10%).
Conclusion: This study showed that herbal extracts lotion was effective in the treatment of dandruff and seborrheic dermatitiand can be an appropriate alternative to chemical agents.

Abstract
Application of sunscreens is one of the most important ways for skin protection against sunburn, early skin aging and skin cancer.
Due to variety of sunscreen products, it is necessary to assure their protection ability against the sun’s damages. Performance assessment of sunscreens against UVB expressed by sun protection factor. Due to great cost of determination methods of this factor in foreign countries, developing a comprehensive protocol for in vivo SPF determination in Iran would have distinct benefits.
In this study four international standards have been reviewed and compared for Sun Protection Factor (SPF) determination including ISO24444, FDA2011, AS/NZS 2604:2012 and CEN 2006. The general process of in vivo SPF determination is similar in these protocols but there are some obvious differences in details that leading to advantages and disadvantages for each of them.
In this study a complete document were collected, then regarding to available facilities and with cooperation of Iran food and drug organization a comprehensive in vivo SPF determination protocol will be developed.

Abstract
Background and Aim: Preoperative assessment of skin tumors extensions have a potential role to increase the efficiency of surgery and maximal preservation of function and cosmetics in the treatment of invasive types of basal cell carcinoma (BCC).
Methods: A total of 72 patients with BCCs on face were included in the study. Gross maximal width of tumors were documented, and then high frequency ultrasonic (HFUS) maximal width and depth of these lesions were determined. Then conventional surgical excision was done and these margins were determined in pathology lab. The comparison of ultrasonic and pathologic depth, and difference of HFUS and pathologic widths from gross clinical width were done with paired t-test.
Results: The mean depth of BCCs were 3.09 mm (SD= 1.02) in HFUS and 2.86 mm (SD=1.22) in pathology (P=0.1). The mean difference from gross width were 4.05 mm (SD=1.18) in HFUS and 4.26 mm (SD=1.70) in pathology (P=0.09).
Conclusions: High frequency sonography has a good accuracy in detection of width and depth of BCCs of face, especially in tumors smaller than 10 mm, and can help to determine safe margins non-invasively, especially in cosmetic and functional critical areas.

Abstract
Urticaria is a common and challenging skin disorder. Diagnosis and treatment of urticaria is not limited to the field of dermatology. General physicians, allergologists and clinical immunologists are also commonly involved in the management of patients with urticaria.
Iranian Society of Dermatology developed a clinical practice guideline concerning diagnosis and treatment of urticaria that was published in 2015. The current guideline, with respect to scientific papers that got published from 2015 to August 2018, is the update it.

Abstract
Onychomycosis is a common nail disorder. If left untreated, it may cause nail deformity and nail plate dystrophy. Since nails have aesthetic importance, onychomycosis may have some impact on quality of life by causing a change in the appearance of nail and/or nail dystrophy and by interfering with fine activities carried out by hand. The aim of this review is to evaluate the quality of life in patients with onychomycosis.
