Abstract

Nowadays, in modern medicine, there is a great tendency to use non invasive or less invasive diagnostic and therapeutic methods with high precision and effectiveness. This is also true about diagnosis and treatment of non-melanoma skin cancers (NMSCs), that the prototypes are basal cell carcinoma (BCC) and squamous cell carcinoma (SCC) which also considered as the most common human cancers. Therefore, in this review, we present the value and diagnostic features of the dermoscopy (as an emerging and growing non-invasive dermatological evaluation tool) for approaching to NMSC. It is hoped that this study will be an infrastructure for the ever-increasing use of dermoscopy in evaluation of skin cancers, especially NMSCs.

Abstract

Background and Aim: Oral cancer as the most common cancer of head and neck region holds the eighth rank among all cancers in order of prevalence. More than 90% of oral cancers are of squamous cell carcinoma (SCC). The main objective of this study is the determination of the survival rate in patients with oral SCC and the factors influencing thereon.

Methods: In this study, 174 recorded cases of patients with oral SCC were evaluated. Required data on survival of patients were gathered by phone calls. Using software SPSS 20, the statistical analysis of  data was carried out. The survival rate of the patients and the corresponding influential factors were analyzed by the Kaplan-Meier curves and Cox regression tests, respectively.

Results: 57.6% of the patients were male. The age of the patients ranged between 22-89 years with the average onset age of 61.5 years. The most common site of oral SCC was tongue. The overall five-year survival rate of the patients was  49.4%±0.05. The survival rate had a statistically significant relation with age of the patients and their treatment methods, including surgery or chemotherapy.

Conclusion: The prevalence of oral SCC is higher in men and overall survival rate decreases with increasing age of the patients. Due to the low survival rate of oral cancer patients, more attentions should be paid to this condition.

Abstract

Diseases of the skin can cause harm and emotional distress  in patients. The desirable  drug is one with least complications and most effectiveness. By prolongation of target exposure, drug effectiveness could be improved. There are several suggested solutions, including the use of chemical enhancers, electroporation, iontophoresis, and the use of nanoparticles as carriers of pharmaceutical agents. The use of various nanostructures, including liposomes, dendrimers, autosomes, and many mineral nanoparticles, have been proposed to prevent the limitations  with conventional formulations. Topical drug delivery has many benefits, including  using high concentrations of the drug and reducing  systemic passage of medicament. Many skin products, such as Estrasorb, Diractin and Aczone are available in the market with new and different manufacturing techniques for more skin penetration.
This paper introduces new approaches to drug delivery, types of nanocomposites and methods for increasing the penetration of pharmaceutical agents in the skin. Various factors such as physicochemical properties and the size of nanoparticles, as well as the effects of manipulation on the surface of these particles, have been discussed.

Abstract

Background and Aim: Environmental and climatic conditions in different geographical areas provide the basis for certain diseases. Skin cancer is one of the most common types of cancer, with a different incidence rate in geographical areas. The aim of this study is to determine the effects of climate and environmental factors on skin cancer and to map the geographical distribution of skin cancer in Iran.

Methods: This study was performed using data of patients with skin cancer, population and data of climatic and environmental factors that affect skin cancer incidence. In this study, after calculating the incidence of skin cancer rate for the whole country, we used the Geographically Weighted Regression model to establish a regression relationship between climate and environmental data and the incidence of skin cancer. The coefficient of detection between the map of incidence of skin cancer and its model map was calculated.

Results: Correlation coefficients showed that sun UV and relative humidity had the highest positive and negative correlation with the incidence of skin cancer, respectively. The southern, eastern and central regions of Iran had the highest incidence of skin cancer rate and the northern and northwestern coasts of Iran had the lowest incidence rate. Validating of actual incidence rate map and the modeled incidence rate map indicated a coefficient of detection of 0.71.

Conclusion: All of the climate and environmental parameters in this study contributed to in the incidence of skin cancer.

Abstract

Background and Aim: Preoperative assessment of skin tumors extensions have a potential role to increase the efficiency of surgery and maximal preservation of function and cosmetics in the treatment of invasive types of basal cell carcinoma (BCC).
Methods: A total of 72 patients with BCCs on face were included in the study. Gross maximal width of tumors were documented, and then high frequency ultrasonic (HFUS) maximal width and depth of these lesions were determined. Then conventional surgical excision was done and these margins were determined in pathology lab. The comparison of ultrasonic and pathologic depth, and difference of HFUS and pathologic widths from gross clinical width were done with paired t-test.
Results: The mean depth of BCCs were 3.09 mm (SD= 1.02) in HFUS and 2.86 mm (SD=1.22) in pathology (P=0.1). The mean difference from gross width were 4.05 mm (SD=1.18) in HFUS and 4.26 mm (SD=1.70) in pathology (P=0.09).
Conclusions: High frequency sonography has a good accuracy in detection of width and depth of BCCs of face, especially in tumors smaller than 10 mm, and can help to determine safe margins non-invasively, especially in cosmetic and functional critical areas.

Abstract

Cancer is a genetic condition. Some cancers are inherited, but most are caused by mutations in somatic cells. The cause of these mutations is inherent error in the transcription of DNA or exposure to carcinogens. Malignant melanoma is the most serious type of skin cancer which develops from pigment-containing cells known as melanocytes. The most potent risk factors for melanoma is the presence of multiple benign moles or abnormal spots, and family history of melanoma. Metastasis is a complex multi-stage process that results in development of secondary malignant growths and the spread of cancer cells to other areas of the body leading to patient’s death. Melanoma metastases are mainly to the lymph nodes, liver, lungs, and central nervous system. The aim of this article is to review the mechanisms of development and metastasis of melanoma by searching database such as SID, Google Scholar, American Electronic Library,Springer and PubMed.

Abstract


Introduction: Idiopathic granulomatous mastitis (IGM) is a rare benign breast disease. The importance of this disease relies on its similar clinical manifestation to those of mammary carcinoma and a group of mastitis with other causes.

Case Report: The patient was a 32-year-old woman, that suddenly got fever, pain and swelling in left breast. In examination several masses were felt. Performed core needle biopsy reported IGM.

Conclusion: Similarity of clinical presentations and diagnostic tests to the other kinds of mastitis and malignancies may lead to misdiagnosis and mistreatment, which imposes socioeconomic and psychological burden to patients.

Abstract

Urticaria is a common and challenging skin disorder. Diagnosis and treatment of urticaria is not limited to the field of dermatology. General physicians, allergologists and clinical immunologists are also commonly involved in the management of patients with urticaria.

Iranian Society of Dermatology developed a clinical practice guideline concerning diagnosis and treatment of urticaria that was published in 2015. The current guideline, with respect to scientific papers that got published from 2015 to August 2018, is the update it.

Abstract

Background and Aim: Skin cancer is the most common malignancy in the world with high morbidity. The aim of this study was to determine the effect of stress inoculation training on quality of life and life expectancy in patients with skin cancer.

Methods: This research was a semi experimental study with pre test - post test - follow up assessment in experimental and control group. The study population consisted of all male patients with skin cancer at Negah Clinic Center in Saveh during the year 2017. Thirty patients were recruited by randomization followed by random allocation dividing them into two groups of 15 subjects. The training included 6 weekly sessions and subjects were asked to fill quality of life and life expectancy questionnaires both at the beginning and end of the coarse. Data was analyzed using ANOVA with repeated measure.

Results: There was a significant difference in life expectancy between the experimental group and control group (P<%001). Furthermore, similar significant difference was found between two groups regarding quality of life.

Conclusion: Stress inoculation training can be used as a valuable and practical intervention in patients with skin cancer,which leads to a significant improvement both in quality of life and life expectancy.

Abstract

Notalgia paresthetica (NP) is a neuropathic complication which occurs within the innervating field of T2-T6 spinal nerves. The patient complains of burning and itching sensation in the lower part of the inner margin of the scapula, and because of frequent itching,signs of skinscratching and hyperpigmentation are observed in the affected area. The definite cause of this pathology is unclear. Available resources have suggested the followings as the possible causes of the disorder: entrapment of the sensory branches within surrounding muscles, postural abnormalities such as scoliosis and kyphosis and degenerative changes of the spine. NP is usually represented as a rare pathology, however, the relatively common clinical incidence suggests a lack of attention to appropriateidentification, diagnosis and treatment ofthe patients. Diagnosis is done through history taking and physical examination, and there is often no need to
additional studiesfor confirmation. A wide spectrum of treatment options have been recommended. Although there is often a tendency to prescribe topical and oral medications for treatment of NP, but application of non-pharmacological therapies such as therapeutic exercises to correct abnormal postures, physical agent modalities such as electrical stimulation and also acupuncture can be effective in accelerating the treatment process and increasing the duration of recovery. Given the undeniable importance of the disorder and its impact on patient's quality of life, a comprehensive approach to existing treatment options with an emphasis on the role of physical medicine and rehabilitation specialists will be discussed in this paper.

Abstract

Rosacea is an inflammatory, chronic and cosmetically annoying common dermatologic disorder which presents with cutaneous telangiectasia, sustained erythema with courses of flares and flushings, papulopustular lesions, ocular involvement or disfigurating phymas. Although there are many relative effective medical therapies, however there are not any promissable treatment. Due to chronic nature of disease and exhausting usage of topical or systemic therapies , we decided to review Pubmed’s literature for non-medical physical therapies, looking for more effective modalities and minimal side effects, particularly in intractable cases.

Abstract

Fungi are eukaryotic organisms that potentially have the ability to cause disease in humans and animals. Fungal infections are called mycosis, which are divided into four types of superficial, cutaneous, subcutaneous and systemic, depending on the area of ​​the body involved. Though cutaneous mycoses are rarely life-threatening infections, they can isolate the patient socially due to the disfigurement of the tissues they caused, as a result of which, treatment has always been an important issue. On the other hand the similarity of fungi to mammalian cells led to difficulties in the development of novel antifungal drugs. Consequently, in recent years, extensive efforts have been made to design and develop novel drug delivery systems with better efficacy for drug delivery of conventional anti-fungal drugs. In this study, we compared conventional forms and novel drug delivery systems of a number of antifungal drugs. All studies confirm the relative priority of novel drug delivery systems to conventional forms in terms of penetration, release, and antifungal effects.

Abstract

Introduction: Beckers’s nevus is a cutaneous hamartoma which usually appears as a circumscribed hyperpigmentation with hypertrichosis. It usually presents unilaterally and the usual site is shoulder and scapula. It is rarely congenital and it is usually noticed first during adolescence.

Case Report: Herein, we report a congenital bilateral large Becker’s nevus with positive familial history of Becker’s nevus. There were no associated anomalies in our patients.

Conclusion: Familial and bilateral cases of Becker’s nevus can present with no associated anomalies as “Becker’s syndrome”.
