Abstract
This is a letter to editor criticizing the paper entitled “Efficacy of licorice extract in the treatment of melasma: A randomized, double-blind, placebo-controlled clinical trial”. We discuss some methodological points on external and internal validity of results. The paper suffers from lack of detailed information on sample size and power calculation to make sure generalisability of results to study population. The paper also did not report blinding procedure and placebo details. The results of the paper also suffer from multiplicity (multiple comparisons) and suitability of data analysis approach. It is recommended to apply either repeated measure ANOVA or a mixed model to consider correlation between repeated measure of outcomes and multiplicity problem.

Abstract
Mycetoma usually involves the skin, the subcutaneous tissues and deep structures such as the muscle and bone, but infection can also spread to more distant sites through the blood and lymph. If left untreated, the disease leads to deformity and loss of function, which may be fatal. The disease has a severe medical and socioeconomic impact on individuals, communities and health authorities. Elaborating a public health strategy for the prevention and control of mycetoma will require collection of epidemiological data on its burden as well as precise diagnosis and early treatment strategies.

Abstract
ackground and Aim: The purpose of this study was to determine the efficacy of group positive psychotherapy on social competence in women candidates for cosmetic rhinoplasty in Isfahan city.
Methods: The research design was a semi-experimental
pre-test-posttest, controlled study and available sampling method was used. From all women refered to relavant centers for cosmetic rhinoplasty in the third quarter (October, November and December) of 2018, 50 participants were selected. After filling social adequacy questionnaire, 30 individuals who obtained the lowest score, were selected and randomly divided into two experimental and control groups. Subsequently, positive group intervention was performed on the experimental group in 10 sessions of 2 hours based on the Rashid protocol (2015). Then the participants of both groups were asked to answer the questionnaire of social adequacy. Finally, the data were analyzed by SPSS-23 software in two descriptive and inferential sections.
Results: The results showed that group positive psychotherapy increased social competence on a general scale and its dimensions in women who were applicant for cosmetic surgery in the experimental group in post-test phase and these changes are statistically significant (P<0.01).
Conclusion: Positive psychotherapy can be used as an interventional and therapeutic method for women applying for rhinoplasty surgery to improve the social competence.

Abstract
Background and Aim: The aim of the present study is rapid and precise identification of yeast species isolated from nail of patients with clinical features of onychomycosis using PCR-RFLP technique.
Methods: 50 patients with involvement of 29 fingernails and 21 toenails were enrolled in the study. Different yeast species were identified by conventional mycological. The PCR-RFLP was tested on yeast isolated and the PCR-RFLP products were separated by electrophoresis in 2% agarose gel, with DNA stain.
Results: The main causative agents were yeasts in 12 cases (24%). Candida albicans was the most commonly isolated yeast species followed by Candida parapsilosis (6%), and Candida glabrata (4 %).
Conclusion: PCR-RFLP method using ITS1-ITS4 primers and MspI restriction enzymes is a rapid, accurate and cost-effective method for specific diagnosis of the most prevalent candida spp. Its ability to detect low amounts of fungal DNA in patient samples in 6-8 hours could be useful for clinical laboratories for optimal management of these infections.

Abstract
Introduction: Onycholemmal carcinoma is a rare slow-growing malignant epithelial neoplasm originating from nail bed epithelium. Clinical presentations are nonspecific and include warty mass, ulceration, and crust, onycholysis, periungual erythema, edema, and pain.
Case Report: Herein, we report a 65-year-old woman with a mass lesion in the second finger of right hand that lasted for 3 years, which finally managed by finger amputation. Histopathology of the tumor revealed solid islands of atypical keratinocytes with abrupt keratinization without granular layer formation. Immunohistochemistry for epithelial and cell proliferation markers was performed.
Conclusion: Malignant epithelial neoplasms of the nail should be considered in long-standing nail problems and appropriate nail biopsy should be performed.

Abstract
Introduction: Medial canthus area, as one of most common sites of facial skin cancers, is notified for reconstruction. There are numerous reconstructive options to cover resulted skin defects, which have their own advantages and disadvantages. Here, we decided to present our experience in medial canthal area with amazing result.
Case Report: A 47-year-old man with basal cell carcinoma (BCC) of right medial canthus, without extra involvement, was admitted. Following lesion excision with safe margin, the resulted skin defect in 30 mm diameter was reconstructed with full thickness skin graft from posterior auricular area. Several months later, it was really difficult to find scar or operated skin, as supported by complete patient`s satisfaction.
Conclusion: The space between eyes has unbelievable aesthetic and social importance. Any added scar or incision to this area is always challenging. It seems full thickness skin graft - if harvested from proper donor site - can be acceptable in long term aesthetic outcome compared to other reconstructive options.

Abstract
Background and Aim: Cicatricial (scarring) alopecia is a group of disorders in which permanent hair loss results from replacement of hair follicles by fibrosis or hyalinized collagen, which includes two types: primary such as discoid lupus erythematosus (DLE), lichen plano pilaris (LPP), frontal fibrosing alopecia (FFA), Folliculitis decalvans, and secondary like kerion and burn. The aim of this study was to evaluate the frequency of different types of scarring alopecia in patients referred to Imam Reza Dermatology Clinic in Ardebil in 1397.
Methods: In this descriptive cross-sectional study, 96 patients referred to the dermatology clinic of Imam Reza Hospital, Ardebil, Iran, were evaluated. Epidemiologic and pathologic causes of primary and secondary cicatricial alopecia were analyzed using descriptive statistics in SPSS version 16 software.
Results: The most common causes among patients with primary cicatricial alopecia were DLE (51.16%), LPP (6.25%), acne keloidalis (4.16%), folliculitis decalvans (2.08%), FFA (2.08%) and pseudopelade of Brocq 1.04%). In patients with secondary cicatricial alopecia the most common causes were kerion (55.2%), followed by morphea cutaneous lymphoma and burn (1.04%).
Conclusion: Considering that the majority of patients in rural area had kerion (92.45%), we can prevent the spread of the disease by improving hygiene standards as an important factor in controlling this disease in order to early diagnosis and treatment.

Abstract
Background and Aim: Trichoscopy is a non-invasive diagnostic tool for hair diseases. The aim of this study was to evaluate the trichoscopic findings in different types of alopecia.
Methods: This cross-sectional study was conducted on 155 alopecia patients referring to Hamadan’s Sina hospital from March 2019 to July 2020. The patients were recruited after giving written informed consent. Scalp biopsy was performed to confirm the diagnosis and trichoscopic evaluation was then carried out. Trichoscopic findings along with demographic data were recorded. Data was analyzed using SPSS 16.0 software and appropriate statistical methods.
Results: A total of 155 patients including 79 males and 76 females were included in the study. The most common trichoscopic finding in alopecia areata was vellus hair (95.5%). The most common finding in androgenetic alopecia (AGA) was hair shaft thickness heterogeneity which was observed in all AGA patients. The most common findings in telogen effluvium (TE) were single hair pilosebaceous units, peripilar sign, thin terminal hairs and upright regrowing hairs which were observed in all TE patients. The most common finding in lichen planopilaris was perifollicular scaling (81.8%). The most common findings in discoid lupus erythematosus were large yellow dots (100%) and fine scaling (100%). Black dot was the most prevalent finding in tinea capitis (85.7%).
Conclusion: Trichoscopy ,as a non-invasive tool, could be helpful in differentiating different types of alopecia.

Abstract
Introduction: Tinea capitis is the most common skin infection on the scalp and an uncommon dermatophytosis in adults. Its clinical appearance varies greatly depending on the causative organism, the type of hair involvement and the extent of the patient inflammatory response. Its common features are fragmented hair loss with varying degrees of scaling and erythema. However, if the clinical symptoms are mild, the diagnosis can be challenging.
Case report: We report a case of tinea capitis in a 60-year-old woman who initially had few clinical symptoms and according to the biopsy result and the presence of fibrosis, lichenpilanopilaris (LPP) were diagnosed and she received related treatment with methotrexate and cyclosporine, but after 4 months, no response to treatment and exacerbation of symptoms were observed. Tinea capitis was diagnosed and the patient was given appropriate treatment.
Conclusion: Tinea capitis can imitate clinical features of lichenplanopilaris and mistreatment with corticosteroids can lead to extension of the disease.

Abstract
The fifth paper of reviews on COVID-19 addresses COVID-19 vaccines, as vaccination has started in the world and Iran. In this paper, we review different types of available vaccines and also various platforms for production of COVID-19 vaccines as well as the vaccines produced in each platform in the world and Iran. Then the considerations about vaccination in patients with skin diseases, specially those using immunomodulator drugs will be presented. Finally, the cutaneous adverse effects of COVID-19 vaccines will be reviewed.

Abstract
Alopecia areata (AA) is a sudden non-scarring hair loss that can be seen in any hairy area in the form of round or oval patches. It is an autoimmune disorder with an immunological response against hair follicles that is caused by genetic and environmental factors. The prevalence of AA is about 0.1% to 0.2% worldwide. Depending on the severity of the disease and sites of involvement, it can be divided into: Alopecia with Patchy pattern, alopecia reticularis, alopecia totalis, alopecia universalis, alopecia ophiasis, alopecia sisiapho, diffuse type and perinevoid alopecia areata. There are various treatment options for AA, but once terminated, they have a high recurrence rate. AA is known for the infiltration of T lymphocytes around the hair follicle bulb, and corticosteroids are its main treatment. Another treatment is topical immunotherapy, the most important of which is Diphenylcyclopropenone (DPCP). It is used in cases that have more than 50% hair loss or are resistant to treatment. The exact DPCP mechanism of action is not well understood, but it may have some kind of antigenic competition and reduce the production of anti-hair follicle antibodies. It can alter the immune response in the skin and alter the ratio of CD4 to CD8 cells around the hair follicle in a way that enhances healing of the disease. It is stated that the rate of hair regrowth in DPCP treatment is about 50% on average. In this article, we review the latest findings of about AA and discuss its therapeutic aspects, especially with DPCP.

Abstract
Background and Aim: Undesirable appearance of the scar tissue after aesthetic surgery can affect the outcomes and patient satisfaction. In recent years, early laser therapy had been encouraged to reduce the scar tissue formation. However, the available evidence regarding the outcomes of laser therapy utilizing combined lasers are limited. In the current study, the effects of combined laser therapy using pulsed dye laser (PDL) and fractional CO2 laser on the appearance of the scar tissue after aesthetic surgeries were investigated.
Methods: There were 15 patients enrolled in the current study including 13 mammoplasty and 2 abdominoplasty patients. In mammoplasty, each breast was randomly assigned to the laser therapy or control group. In abdominoplasty, split scar design was used. In laser therapy group, at the end of the third postoperative week, PDL 585 nm was radiated. Furthermore, at the end of the sixth week, fractional CO2 laser was radiated. The patients were followed for 6 months after the last laser therapy session. Visual analogue scale was used to rate the appearance of the scar tissues by 3 surgeons blinded to the treatment of scars. The appearance of the scars was determined as poor, fair, good or excellent.
Results: VAS averaged 5.5±0.8 in laser therapy group and 4.8±0.8 in the control group. The difference was not statistically significant (P=0.057). There was no scar with excellent appearance in the current study. All of the scars were fair and good in the laser therapy group. In the control group, 13 scars were fair. The difference was not significant (P=0.227).
Conclusion: Early combined laser therapy using PDL and fractional CO2 laser was associated with improved appearance of the scar tissue in short-term follow up, however, the effects were not statistically significant.

Abstract
Today, the valuable metabolites of microalgae are widely used in the production of many cosmetic products, as the biomass of algae contains pigments, proteins, essential fatty acids, polysaccharides, vitamins and minerals, all of which are present in both natural foods and cosmetics are used. In fact, these secondary metabolites with anti-blemish and anti-microbial, anti-aging, sunscreen and skin whitening effects can repair and heal the skin and protect the skin from UV damage and prevent inflammation. Existing cosmetics can in many cases be replaced with microalgae-derived cosmetics that are environmentally friendly as well as safe. The effects of microalgae and their derivatives have been tested in various nutritional studies around the world. However, there are very few studies on their cosmetic applications, so in this review article we tried to study several species of microalgae, cultivation methods, use of biomass and bioactive molecules derived from microalgae in the production of anti-aging products, sunscreen and skin whitening in the formulation of cosmetic compounds.

Abstract
Background and Aim: Psoriasis is a chronic inflammatory skin disease, influenced by genetic and immune-based factors that is associated with skin inflammation. Aloe vera gel is traditionally used to treat skin diseases including psoriasis.
Methods: In this study, ethanolic extract of aloe vera gel was evaluated for its anti-inflammatory and antioxidant activity in skin-derived A431 cells. First, after preparing the extract and then the green synthesis of gold nanoparticles using the extract, their effect on the expression of inflammatory genes as well as their antioxidant properties were investigated.
Results: The use of different concentrations of aloe vera extract caused a relative increase in EGFR and Nrf2 expression levels and decreased expression of NF-κB, TNF-α and IL-6 inflammatory genes in A431 skin cells. But the effect of synthesized nanoparticles was greater than aloe vera extract in altering the expression of signaling and inflammatory genes.
Conclusion: The synthesized extracts and nanoparticles had antioxidant properties and also reduced the expression of inflammatory genes in cells. However, the effect of nanoparticles was much greater than that of extracts in reducing inflammatory genes.

Abstract
Background and Aim: Direct immunofluorescence (DIF) is represented as a gold standard method in diagnosis of autoimmune blistering dermatoses. Normal saline, liquid nitrogen and michel’ solution are a widely accepted media for preserving biopsy samples of skin or mucosa before DIF examination. Occasionally clinicians put the biopsy specimen taken for DIF in formalin 10%, occasionally clinicians ordered DIF retrogradely and only a paraffin-embedded biopsy specimen exposed to formalin 10% is available. To determine the diagnostic value of DIF when it was performed on biopsy samples of skin or mucosa exposed to formalin 10% in comparison to the same biopsy samples exposed to normal saline.
Methods: In 74 patients (38 immunobullous and 23 chronic dermatitis), which the latter served as the normal controls, 2 perilesional punch biopsy of skin or mucosa were done, one put in formalin 10% and fixed in paraffin and one put in normal saline, and DIF was done on both samples.
Results: DIF sensitivity and specificity was with IgG 31.5% and 100% in pemphigus and 15.36% and 93.44% in BP, with C3 39.47% and 100% in pemphigus and 7.69% and 91.80% in BP, respectively.
Conclusion: DIF on specimens exposed to formalin 10% in comparison to specimens exposed to normal saline is less sensitive but approximately as specific as it is in the diagnosis of pemphigus and BP patients and especially can be useful in pemphigus patients when only a formalin exposed samples is available.
