Abstract

Background and Aim: The use of leech extract in the preparation of cosmetic supplements and medicinal compounds has always been of interest. So far, the effect of this extract on the volume of the skin's epidermal tissue has not been scientifically confirmed, and no research has been done regarding the comparison of the effects of different types of base oils in this extract.
 

Methods: The purpose of this study was to investigate the effect of leech extract in the bases of olive oil, castor oil and bitter almond oil on the facial skin using the basic geometric morphometry software. For this purpose, 25 people from the age group of 45 to 55 years were selected. In order to compare morphological changes for forty days at a specific hour, pictures were taken using a digital camera. 12 landmark points were placed on the two-dimensional images using TpsDig2 software. The shape data obtained after superimposition analysis were analyzed using Procrust analysis with multivariate PCA and CVA statistical methods and cluster analysis (CV).
 

Results: The results showed that there is a significant difference between leech extract in volumizing and removing wrinkles with olive oil base (P>0.001), but the two groups of castor oil and bitter almond oil did not have a significant difference (P>0.001).
 

Conclusion: Leech extract has no effect on removing wrinkles and does not cause local swelling in the epidermal tissue in the time frame of the present study.

Abstract

Background and Aim: Considering the increase of dermatophytosis trough the Iran and world as well, especially ringworm of the body and groin due to terbinafine-resistant species and the high speed of disease transmission, in order to choose the appropriate treatment, considering the time required for the treatment of the disease and early detection of resistant cases treatment is required. The present study was conducted to determine the antifungal sensitivity of Trichophyton rubrum, Microsporum canis and Epidermophyton flucosum strains to common antifungal drugs.
 

Methods: Antifungal sensitivity test (broth microdilution) was performed for 20 specimens obtained from patients according to CLSI (Clinical and Laboratory Standard Institute) M38-3rd ED protocol, for terbinafine, itraconazole and voriconazole.
 

Results: According to the antifungal susceptibility test results, 5 patients were resistant to terbinafine and showed a minimum inhibitory concentration more than 2, 3 patients were tolerant to terbinafine and showed a minimum inhibitory concentration between 0.5 and 1. Based on the results of the antifungal susceptibility test, 7 patients were tolerant to itraconazole and showed the minimum inhibitory concentration between 0.5 and 1. All isolates responded well to voriconazole.
 

Conclusion: The results of the antifungal sensitivity test highlight the importance of mycological tests on dermatophytic skin lesions to prevent the spread of drug-resistant species.

Abstract

Background and Aim: Side effects associated with antimicrobial drugs, as well as their high cost, have stimulated the search for inexpensive herbal medicinals with fewer side effects. These substances can be used as a medicinal supplement or to increase the antimicrobial and antifungal effects of drugs. The aim of this study is to investigate the inhibitory effect of oleuropein on fungal and bacterial pathogens isolated from nail infection in laboratory condition.
 

Methods: Antifungal and antibacterial properties of oleuropein by determining the Minimum Inhibitory Concentration (MIC), Minimum Bactericidal Concentration (MBC) and Minimum Fungicidal Concentration (MFC) of this substance on yeasts Candida albicans, Candida glabrata and Escherichia coli by microbroth dilution method using CLSI protocols were evaluated.
 

Results: The results showed that the Minimum Inhibitory Concentration (MIC) against all three studied microorganisms was equal to 65 mg/ml and the Minimum Bactericidal Concentration (MBC) and Minimum Fungicidal Concentration (MFC) of oleuropein was equal to 130 mg/ml.
 

Conclusion: due to the high antifungal and antibacterial activity of oleuropein, the present study introduces oleuropein as a natural antimicrobial drug. This substance can also be used to increase the antimicrobial power of existing antibiotics.

Abstract

Background and Aim: Recently, many Iranian companies have started manufacturing hyaluronic acid based dermal fillers. Current review aimed to assess and compare the clinical safety and efficacy of 5 Iranian hyaluronic acid based dermal fillers.
Methods: The first part of the study reported, the overall safety and efficacy of 5 Iranian hyaluronic acid based dermal fillers for improvement of nasolabial folds in 150 participants (age 44.30±9.97 years) using clinical and objectives assessment methods. In the second part, we performed a comparative study among 5 tested dermal fillers.
Results: Generally, at weeks 2, 12, and 24, 80.6%, 63.3%, and 60.66% of the participants, showed one grade or more improvement in the severity of nasolabial folds respectively. In all visits, the nasolabial folds biophysical parameters as well as the echo density dermis improved significantly compared to baseline. In comparative study, two fillers with the hyaluronic acid concentration of 24 mg/ml, showed the best objective efficacy, while in clinical evaluations, the filler containing mannitol has the best performance in reducing the severity of nasolabial folds showed.
Conclusion: Iranian hyaluronic acid based dermal fillers showed different levels of safety and efficacy for improvement of moderate to severe nasolabial folds, with efficacy.

Abstract

Background and Aim: In line with the increase in cases of dermatophytosis of the body and dermatophytosis of the groin, the cases of species resistant to antifungal drugs, especially terbinafine, are increasing. The present study was designed to investigate the drug resistance pattern of dermatophyte species isolated from 27 patients referred to Razi Skin Hospital.
 

Methods: Skin samples of patients were collected from the groin, buttock and body from the active margin of the lesion. Some of the sample was examined under the microscope for the presence of arthroconidia and fungal hyphae. Cultivation on Saburo dextrose agar 2% special medium with chloramphenicol and cyclohexamide was used to determine the primary identity of dermatophyte species, morphological characteristics were used. To accurately identify trichophytons isolated from the skin of patients by (molecular) genotyping method, ITS1 and ITS4 primers were used. Universal primers) were used. Microdilution broth drug sensitivity test was performed according to M38-3rd ed - CLSI protocol for dermatophyte isolates to terbinafine, itraconazole, fluconazole and voriconazole.
 

Results: 25 patients were infected with Trichophyton indotinae. 2 patients were infected with Trichophyton interdigitale. In the present study, 4.27 (14.81%) of the patients had antifungal resistance (1 μg/ml≤the minimum concentration of fungal growth inhibition for terbinafine).
 

Conclusion: Failure to perform mycological examinations before initiating treatment can result in misdiagnosis.  Do not use the antifungal susceptibility test lead to  challenging to manage treatment-refractory cases. inadequate and inappropriate treatment combinations can increase the number of resistant species worldwide.

Abstract

Background and Aim: Marine seaweeds are a valuable source of bioactive ingeredients with the possibility of different effects in cosmeceutical industries such as skin health products. One of the main characteristics of the skin is moisture which plays an important role in maintaining metabolism, enzymatic activity, mechanical properties, appearance and protective barrier. The objective of the present study was to formulate the water extract of brown algae Sargassum tenerrimum with antioxidant and antibacterial properties as a moisturizing cream to evaluate physicochemical stability and microbial growth.
 

Methods: Algae were collected from the Persian Gulf intertidal zone and was washed with seawater, then transferd to Seafood Processing Laboratory in Gorgan University of Agricultural Sciences and Natural Resources for extraction process, after that formulation of 1% moisturizing cream was made at Center for Research and Training in Skin Diseases and LeprosyTehran University of Medical Scinces. Statistical analysis of antibacterial properties of water extract was performed by SPSS software version 22 and one way ANONA was used to analyse the data. The comparison between the means were investigated by Duncan’s test used for measuring significant differences at 5 percent level. Finally, the charts were created by using Excel software. The results of the study were reported as mean ± standard deviation.
 

Results: According to the results, the prepared 1% cream was completely stable and homogeneous in room temperature and oven for 6 months and successfully passed microbial tests.
 

Conclusion: In view of the above mentioned, the manufactured cream efficacy was well done and can be introduce as a skin moisturizing agent.

Abstract

Background and aim: As the use of botulinum toxin to treat glabellar lines increases, particularly in Iran, where foreign products (e.g. Dysport) are relatively expensive, it becomes important to compare the efficacy, side effects, and duration of action of two locally produced botulinum toxin brands, Masport® and Dyston®. This study seeks to assess and compare the treatment results of these two medications.
 

Methods: In this cross-sectional, descriptive, retrospective study, the medical records of 81 patients who visited Razi Dermatology Hospital for the treatment of moderate to severe glabellar lines were reviewed. Data related to patient satisfaction, duration of action, and side effects of Masport® and Dyston® botulinum toxin were collected and analyzed via a questionnaire.
 

Results: The mean satisfaction score was 7 for the Dyston® group and 7.7 for the Masport® group, with no statistically significant difference between the two groups (P=0.142). Additionally, the mean duration of action was 3.5 months for Dyston® and 3.8 months for Masport®, with no significant difference observed. Headache was the most common side effect reported, with no other major adverse events noted. Furthermore, there was no statistically significant difference in the incidence of side effects between the two groups.
 

Conclusion: The findings of this study indicate that both Dyston® and Masport® have similar efficacy and safety profiles. The choice between them can be primarily based on cost and patient or physician preference rather than any significant therapeutic differences. Larger studies are recommended to further validate these findings.

Abstract

Background and Aim: The most prevalent form of leishmaniasis is cutaneous leishmaniasis (CL), which manifests as a single ulcerative or nodular lesion that usually heals on its own. The aims of this study were to investigate the histopathologic and clinical features of CL patients.
 

Methods: This cross-sectional study was accomplished on 70 CL patients that referred to Razi dermatology hospital in Iran, over a five-year period from 2016 to 2020. Included patients were those who were diagnosed clinically suspected patients whom their disease was confirmed through histopathology examination.
 

Results: The age of patients ranged from 2 to 76 years with a mean of 42.26 ± 20.86 years. The majority of patients were men (54.3%). The plaque form lesions were the most frequent type (64.3%). Respecting lesion site, face, and upper limbs were more frequent with the abundances of 47.1% and 34.3%, respectively. All of the patients had evidence of cellulitis on ultrasound. The most prevalent change of epidermis was hyperkeratosis (42.8%). Regarding dermal histopathologic changes, infiltration of lymphocytes (61.4%), plasma cells (58.6%), and histiocytes (48.6%), as well as granuloma formation (54.3%) and leishman body presence (54.3%) were the dominant changes. Some patients had atypical forms of CL as six of them had lupoid cutaneous leishmaniasis (LCL) and five had sporotrichoid cutaneous leishmaniasis (SCL).
 

Conclusion: In this study, men and those who were in middle age were more susceptible to be infected, so it suggests considering them particularly in the preventive programs. Moreover, histopathological findings of CL, including LCL and SCL, are presented. These findings may be beneficial for dermatologists and dermatopathologists to differentiate them from other cutaneous diseases.

Abstract

Background and Aim: Cancer is the second leading cause of death in the world, as well as skin cancer is the most common cancer among human cancers and causes great costs and economic and social harm to society. The increase in skin cancer in our country and also in Ardabil province led us to study the prevalence and epidemiological trend of 10 years of skin cancer in patients referred to the skin clinic in Ardabil 2011-2021.
 

Methods: In this study, 331 patients had been referred to a dermatology clinic with a diagnosis of skin cancer during 2011 to 2021. The checklists were completed based on the information in their files.
 

Results: Of all patients, 208 (62.8%) were male, 60.4% from Ardabil, 14.2% were in year 2016 and 46.2% were in the age group of 50-70 years. The mean age of patients was 62.5±14.7 in range 25-96 years. The most common type of skin cancer in the studied samples during ten years was related to BCC with 184 cases (55.6%) and the most studied patients were in grade 3 with 231 patients (69.8%). The most common site of skin cancer with 93 cases (28.1%) was in the nose.
 

Conclusion: Results showed that the trend of skin cancer in the studied samples similar to the provincial statistics did not have a constant decrease or increase trend and had a fluctuating trend. The incidence of this disease in male clients was about twice that of female clients and most cases were in high grades and BCC type.

Abstract

ackground and Aim: Vitiligo, as one of the most common skin pigment production disorders has significant psycho-social consequences for patients, so it is important to discover the variables affecting mental health in these patients. present study aims to consider mediating role of internalized shame in the relationship between cognitive avoidance and interpersonal difficulties in predicting the quality of life of vitiligo patients.

Methods: The current research was of a descriptive correlation type. The statistical population of the research consisted of all the people referred to Razi Hospital in Tehran who were suffering from vitiligo. 170 people purposefully selected based on the inclusion criteria. In this research, Sexton and Dagas cognitive avoidance, Barkham's interpersonal problems, World Health Organization's quality of life, and Cook's internalized shame questionnaires were used. Data analysis was done by Pearson correlation coefficient test, regression analysis and path analysis using SPSS 26 and PLS software.

Results: The results showed that the internal shame in the relationship between interpersonal problems and quality of life was recognized as a relative mediating variable. The path of cognitive avoidance with quality of life and internal shame as a mediating variable was not significant with a significance level of less than 0.05.

Conclusion: Mental health professionals can play an important role by considering the role of internal shame in helping vitiligo patients to overcome challenges, improve their mental health and well-being.

Abstract

Background and Aim: Skin cancer is one of the most common cancers. Numerous studies have shown the positive effect of using sunscreen creams in preventing skin cancer. The purpose of this study was to determine the inequality in the use of sunscreen cream.
 

Methods: This cross-sectional study was conducted among adults in Kermanshah city. Data was collected using questionnaire by interview. Concentration index and curve were used to quantify and analyze inequality in sunscreen use. The results of adjusted odds ratio and decomposition analysis for sunscreen use were also estimated.
 

Results: 14.39% used sunscreen. The ratio of use in men was 5.65% and in women was 21.72%. The concentration index for sunscreen use (P<0.001) was 0.344, which indicates the greater concentration of sunscreen use among the rich. The rich groups are 3.72 times more likely to use sunscreen than the poor, and people in the middle class are also 1.48 times more likely to use sunscreen. The highest share of participation in the creation of inequality in sunscreen consumption was achieved by the socio-economic status variable with 61.26%, education level with 13.23% and age with 13.02%.

Conclusion: The focus of health promotion interventions should target groups with low socio-economic status. Men and less educated people should be prioritized in the development and implementation of interventions.

Abstract

Background and aim: Pityriasis lichenoides (PL) is a rare papulosquamous disorder with an unknown etiology. Due to limited recent studies on this subject, the present research was conducted to describe the clinical manifestations and treatment course of these patients.
Methods: This retrospective cross-sectional study (2019-2023) was conducted on patients diagnosed with PL at Razi Dermatology Hospital, Tehran University of Medical Sciences, Iran confirmed by biopsy of skin lesions. Basic information was obtained from hospital records, and treatment follow-up was gathered from medical files and phone interviews with patients. Data were analyzed using R software, with a P<0.05 considered statistically significant.
Results: A total of 147 patients were evaluated. No significant difference was found between genders (P=0.358). The predominant subtype was PLC (91.8%), with a mean age of onset of 29.79 ± 18.73 years. The most common type of skin lesion was papules (48.4%), and generalized body involvement was the most frequent manifestation, observed in 58 patients (32.4%). The most commonly used treatment was topical corticosteroids, administered to 86 patients (52.4%). The average duration from onset to full recovery was 160.80±221.40 days. A total of 45 patients (30.6%) reported recurrent episodes of the disease. No significant association was found between treatment efficacy and gender (P=0.620), underlying conditions (P=0.293), or age (P=0.876).
Conclusion: Given the limited clinical data on patients with pityriasis lichenoides, establishing an electronic database and a national registry system for this condition is recommended.

Abstract

Background and Aim: Telogen effluvium (TE) is a type of diffuse hair loss that occurs 2 to 3 months after a stressful event, including a covid-19 infection. The aim of this study was to determine the frequency of TE in women with covid-19.
Methods: This descriptive study was conducted on 270 women who referred to the skin and hair clinic of Imam Reza Ardabil Hospital with complaints of hair loss. Information related to the history of covid-19 infection, hospitalization, medications, vaccination and demographic characteristics were collected. The severity of alopecia was checked by clinical examination and traction test, and the data were analyzed with SPSS software.
Results: The average age of the patients was 41 years. 61.1% had a history of hospitalization, 43.7% used antibiotics, 61.8% used corticosteroids, and 51.8% used NSAID. The severity of Covid-19 had a significant relationship with the earlier occurrence of TE (P=0.001) and the increase in the number of pulled hairs in the traction test (P=0.001). Hospitalization due to covid-19 and corticosteroid use reduced the time between illness and onset of TE symptoms. The injection of AstraZeneca vaccine was associated with an increase in the number of plucked hairs (P=0.02).

Conclusion: The severity of TE was related to the severity of covid-19, hospitalization, corticosteroid use and injection of AstraZeneca vaccine.

Abstract

Background and Aim: Angiokeratomas are rare vascular lesions, typically presenting as small red to purple macules or papules on the skin. These lesions are usually benign, though they can undergo changes in thickness and color over time. Certain types of angiokeratomas may also signal underlying metabolic disorders, necessitating careful clinical attention. This study aims to analyze the clinical and demographic features of angiokeratoma in Iranian patients at Razi Dermatology Hospital.
Methods: This descriptive, retrospective cross-sectional study examined 199 patients diagnosed with angiokeratoma between 2013 and 2023. Data on demographics, lesion characteristics (location and size), family history, and treatment methods were collected from medical records and analyzed using SPSS. Both parametric and non-parametric statistical tests were applied to assess associations, with a significance level set at 0.05.
Results: The mean age of patients was 24.75 years, with the second and third decades showing the highest prevalence. Among the patients, 51.3% were male and 48.7% female. Lesions most commonly appeared on the lower extremities (40.7%) and genital area (18.6%). Treatment methods included cryotherapy (68.14%), laser (27.14%), and electrodessication (4.44%), with cryotherapy proving the most effective among them.
Conclusion: This study provides valuable insights into the prevalence, clinical characteristics, and preferred treatment approaches for angiokeratoma in Iranian patients. Broader studies across various centers, along with long-term follow-up, are recommended to further investigate the clinical course, therapeutic outcomes, and potential complications associated with different treatment methods.

Abstract

Background and Aim: Skin cancer is the most preventable type of cancer. Farmers, due to their work under the sun's ultraviolet radiation, are at a higher risk of developing skin cancer. The purpose of this study was to determine the determinants of skin cancer prevention behaviors in farmers.
Methods: This cross-sectional study was conducted among farmers in the rural of Javanroud city in the west of Iran. Information was collected by questionnaire and interview. Data were analyzed in SPSS version 16 statistical software. Crude and adjusted linear regression was used to measure the determinants of skin cancer prevention behaviors.
Results: The mean age of the participants was 36.47 years. Perceived self-efficacy (ß=0.193, P=0.004) and response costs (ß=0.145, P=0.025) were the most important influencing determinants on skin cancer prevention behaviors.
Conclusion: It seems that if the development of interventions to promote skin cancer prevention behaviors among farmers focuses on promoting perceived self-efficacy and reducing response costs, it can seek useful findings in promoting skin cancer prevention behaviors.

Abstract

Background and aim: In the severe cases psoriasis, there is a need for biologic treatments which target the immunological responses contributing to the disease pathophysiology. There is no study in definite indications for the treatment choice and the time to start the biologic treatment in Iranian psoriasis patients.
Methods: We designed a cross-sectional study and evaluated all psoriasis patients who had received biologic treatment. After determining the patients who were eligible to participate in the study, their records were studied and analyzed.
Results: 50 patients with the mean age of 42.86 (SD=14.33) participated in the study. Twenty-five patients had received phototherapy for the treatment of psoriasis. All patients had received topical agents and 47 (94%) had received systemic treatment. Adalimumab was the most common biologic treatment used for the treatment of psoriasis patients (74%). 24% of patients had received Infliximab and 4% had received Etanercept.

Abstract

Background and Aim: Melasma is an acquired, chronic, and recurrent pigmentation disorder that usually affects sun-exposed skin areas. SAHA syndrome is also an acronym for four words: seborrhea, acne, hirsutism, and androgenic alopecia. Given the association of melasma with some components of SAHA syndrome, such as acne and hirsutism, this study was conducted to determine the relationship between melasma and SAHA syndrome.
Methods: This case-control study was conducted on 75 women with melasma as the case group and 75 women without melasma as the control group. Data were collected in the form of a questionnaire consisting of 28 questions and analyzed.
Results: In the case group, 32% had hirsutism and 70.7% had acne. In the control group, 10.7% had hirsutism and 26.7% had acne. There was a significant association between factors (history of melasma in first-degree relatives, history of hirsutism in first-degree relatives, acne, alopecia) and melasma. Among the case group, 5.3% had SAHA syndrome.
Conclusion: Considering that androgenic disorders are implicated in the etiology of SAHA syndrome and melasma, it seems that melasma lesions are more common in patients with SAHA syndrome.

Abstract

Background and Aim: Leishmaniasis is difficult to control and is not cost-effective in endemic areas. Antimony toxic compounds is the drug of choice, which drug resistance is increasing. Animal studies showed that interferon-gamma deficiency impairs the recovery of leishmaniasis. In this study, the therapeutic effects of subcutaneous injection of recombinant gamma interferon on leishmaniasis lesions in Balb/ C mice were evaluated.
Methods: 40 Balb/ C mice were infected with Leishmania major. After developing lesions, the therapeutic effects of interferon gamma alone or in combination with glucantime were investigated. Also, the standard treatment of glucantime and the control group without treatment were evaluated for comparison.
Results: The treatment groups (glucantime, gamma interferon, and gamma interferon-glucantime) had a significant difference (P<0.01) in terms of reduction in the footpad thickness compared to the control group. There was no significant difference in the mortality rate in the groups receiving gamma interferon and the group receiving standard glucantime treatment.
Conclusion: It seems that gamma interferon was effective in treating cutaneous leishmaniasis lesions in Balb/c mice alone and in combination with standard treatment and can be considered as an alternative treatment.

Abstract

Background and Aim: Image brightness heterogeneity is one of the major challenges in computer image processing that can lead to inaccurate results in image segmentation. Despite the existence of numerous segmentation methods, few studies have been conducted on the effect of brightness heterogeneity and the selection of the best color channels in segmentation. In this paper, different color spaces have been used for automatic detection of skin lesions.
Methods: In this study, the LSE (Level Set Evolution) segmentation method along with intensity smoothing has been used for computer recognition of skin lesions. First, the brightness heterogeneity is reduced and a more uniform image is created. Then, the proposed segmentation divides the image domain into distinct regions. This method results in more accurate recognition of skin lesions.
Results: The proposed method has been tested on 200 dermoscopic images from the known PH2 dataset using different color channels. The results show that this method performs better than other methods. Accuracy of 97%, sensitivity of 98%, specificity of 99% and Dice coefficient of 92% have been obtained.

Conclusion: This method has the ability to accurately isolate and diagnose lesions and can help doctors in the treatment process of skin lesions.

Abstract

Background and aim: Macular amyloidosis (MA) is a form of primary localized cutaneous amyloidosis (PLCA), characterized by amyloid deposits in the papillary dermis. This study aimed to compare the biophysical properties of MA lesions with those of adjacent uninvolved skin.
 

Methods: We measured several parameters in active MA lesions of 22 patients, including stratum corneum (SC) hydration, transepidermal water loss (TEWL), surface friction, pH, sebum, melanin, erythema, temperature, elasticity parameters (R0, R2, and R5), as well as epidermal and dermal thickness and echo-density using ultrasonography. Measurements from the lesions were compared with those from healthy skin adjacent to the lesions, serving as controls. Statistical analysis was performed using the paired t-test, with P-values less than 0.05 considered significant.
 

Results: Compared to adjacent normal skin, MA lesions demonstrated significantly lower SC hydration and surface friction, while TEWL, pH, erythema index, melanin content, and epidermal thickness were significantly increased. No significant differences were found in sebum levels, temperature, elasticity parameters, or dermal thickness and echo-density.
 

Conclusion: MA lesions exhibit distinct biophysical and ultrasonographic changes consistent with their histopathological features. These characteristic alterations may serve as useful, non-invasive markers for the early diagnosis of cutaneous macular amyloidosis.
