Abstract
Background and Aim: Psoriasis is a chronic skin disorder involving many parts of the body including skin, nails and joints with severe negative effects on patient's quality of life. Nail involvement is one of the most difficult involvements of this disease to treat. Psoriatic arthritis categorized in sero-negative spondilo-arthropaties with some deforming characteristics. The aim of the present study was to evaluate the relationship between psoriatic arthritis and nail involvement based on standard psoriatic nail involvement score (NAPSI).
Methods: Psoriatic patients visiting dermatology clinic of Razi hospital were selected. Patients with arthritis enrolled as the case group while patients that had no joint involvement participated in control group. After matching, demographic data of both groups as well as NAPSI score and nail involvement characteristics collected through special questionnaires and analyzed with SPSS version 14.
Results: A total of 30 patients with psoriatic arthritis and 75 patients without arthritis were enrolled the study. Analysis of collected data demonstrated that nail involvement among patients with psoriatic arthritis (mean NAPSI score=46) was more common and severe than patients without joint involvement (mean NAPSI score=9.23) (P<0.05).
Conclusion: It seems that there is a significant difference between psoriatic patients presenting with or without arthritis and presence of arthritis probably can determine occurrence and extension of nail involvement.

Abstract
Background and Aim: Chronic renal failure is a clinical state which results in declining the kidney function. Chronic renal failure has numerous cutaneous manifestations, when the process of hemodialysis begins, new changes may appear as a result. The aim of this study was to assess the prevalence of cutaneous problems among patients with chronic renal failure undergoing hemodialysis in Bandar Abbas, Iran.
Methods: In this cross-sectional study, 60 patients with chronic renal failure under hemodialysis in Shahid Mohammadi Hospital Dialysis Center were examined by a dermatologist. The data were gathered and analyzed by SPSS software (version 16) using descriptive statistics, t and chi2 tests. P less than 0.05 was considered significant.
Results: The most common skin findings were dry skin (78.3%), followed by pallor (71.7%) and pruritus (58.3%). Other common cutaneous manifestations were half-half nail (35%), idiopathic guttate hypomelanosis (33.3%), yellowish skin color change (31.7%), subungal hyperkeratosis (30%), onycholysis (20%), beau's line (15%), cherry angioma (15%), skin hyperpigmentation (15%) and purpura (13.3%).
Conclusion: Chronic renal failure is associated with various cutaneous manifestations caused by hemodiaysis or the disease itself. The most common ones are dry skin and pallor. The early diagnosis of cutaneous changes may decrease the morbidity and eventually lead to quality of life improvement in these patients.

Abstract
Background and Aim: Onychomycosis is the fungal infection of nails. Patient’s immune system has an important role in initiation of onychomycosis. One of the causes of the increased prevalence of onychomycosis is the increasing number of patients with local or general immune system disorders. In hormonal disorders such as diabetes mellitus, Cushing's syndrome and hypothyroidism superficial vascular disorders and individuals with immune deficiency, onychomycosis prevalent. The aim of this study was to investigate onychomycosis in individuals with underlying diseases who referred to Razi hospital in Tehran.
Methods: In this cross-sectional study, nail specimens of 250 patients who have nail dystrophy with an underlying disease whom were referred to mycological laboratory of Razi hospital in 2010-11 were evaluated with direct microscopy and culture on Sabouraud dextrose agar and Sabouraud dextrose agar containing chloramphenicol and cyclohexamide. For differential diagnosis of fungi, complementary tests were used.
Results: Eighty-nine (35.6%) of 250 individuals with underlying diseases and nail dystrophy, were affected with onychomycosis. Among these pateints, 52 (58.4%) cases had yeast infection, 19 (21.3%) had dermatophyte infection, and 18 (20.2%) had saprophyte infection. Diabetes mellitus (37.1%), cardiovascular diseases (14.7%), pemphigus (9%), psoriasis (7.9%), hypothyroidism (6.7%) and chronic eczema (5.6%) were the most common underlying diseases observed in patients with onychomycosis.
Conclusion: Onychomycosis is common in hormonal disorders like diabetes mellitus and hypo-thyroidism superficial vascular disorders and individual who suffer immunodeficiency.

Abstract
Background and Aim: Tinea unguium is a common disease with worldwide distribution most commonly seen in adult patients. Trichophyton rubrum and T. interdigital are the most common causes. The aim of this study was to investigate the frequency of tinea unguium causative agents in a referral dermatology hospital in Tehran, Iran.
Methods: This cross sectional study was conducted in 2010 and 2011 on clinically suspicious patients for tinea unguium referred to the Mycology Laboratory, Razi Hospital, Tehran, Iran. Samples from 700 patients were examined using direct smear microscopy and culture. Direct microscopic examination of the specimens was carried out using 20% potassium hydroxide solution. The specimens were cultured on Sabourad dextrose agar culture media containing chloramphenicol and cyclohexamid (Scc). For identifying the species of dermatophytes, complementary tests were used. Frequencies and relative frequencies were demonstrated in tables and chi-square and Fisher's exact tests were used to investigate any association between the categorical variables.
Results: Of 700 dystrophic nail samples, 53 samples (7.6%) were positive according to both direct examination and culture. Thirty-eight patients were males. The most common clinical type was distal subungual onychomycosis which was observed in 79.2% of cases. The most frequent detected dermatophyte species. was T. interdigital (39.6%) followed by T. rubrum (37.7%). Forty-seven patients had tinea unguium on their toe nails, 4 patients on their finger nails, and 2 patients had it on both finger and toe nails. Nineteen patients had underlying diseases, and the most common underlying disease was cardiovascular disease (26.3%).
Conclusion: Tinea unguium is a disease with worldwide distribution and identifying the causative agents and predisposing factors are necessary for better management of the patients.

Abstract
Background and Aim: Hand, foot, and mouth disease (HFMD) is a common contagious infectious disease. Because of the current limited knowledge about the etiology of this disease and its associated complications, especially nails lesions, we conducted this study to determine the prevalence of the nail disorders in children with HFMD.
Methods: In this cross-sectional study, 84 children whom were referred to Bo Ali Sina Hospital in Sari, in 2013 and 2014, and were diagnosed as HFMD, were recruited using a census sampling method. Diagnosis of HFMD was made by a dermatologist or a pediatrician based on the clinical findings. Then, the skin lesions were recorded and the nails of all HFMD patients were examined to detect any nail disorder (onychomadesis, transverse ridging, etc.). Nail examinations were repeated fortnightly up to 8 weeks after the first visit. During this follow-up period, through using a questionnaire, the type of the nail lesions and their relationship with HFMD were recorded.
Results: The mean±standard deviation (SD) age of the patients was 38.2±19.5 months, and 58.3% of the patients were male. At the time of HFMD diagnosis, 30 (35.7%) patients had hand and nail involvement. Thirty-one cases (36.9%) had toenails involvement. Onychomadesis frequency was higher 4 weeks after initiation of the disease in comparison with 2 weeks after the onset of illness (15.5% vs. 1.2%, P<0.001) and 6 weeks after the beginning of the disease compared to 4 weeks after the disease onset (29.8% vs. 15.5%, P=0.001) the frequency of nail involvement at 8 weeks after the disease onset compared to 6 weeks after the beginning of HFMD was decreased (17.9% vs. 29.8%, P=0.007).
Conclusion: It is necessary to take measures for early detection and treatment of involved nails in HFMD. Periodic follow-up visits is recommended too.

Abstract
Onychomycosis is a common nail disorder. If left untreated, it may cause nail deformity and nail plate dystrophy. Since nails have aesthetic importance, onychomycosis may have some impact on quality of life by causing a change in the appearance of nail and/or nail dystrophy and by interfering with fine activities carried out by hand. The aim of this review is to evaluate the quality of life in patients with onychomycosis.

Abstract
Introduction: Onycholemmal carcinoma is a rare slow-growing malignant epithelial neoplasm originating from nail bed epithelium. Clinical presentations are nonspecific and include warty mass, ulceration, and crust, onycholysis, periungual erythema, edema, and pain.
Case Report: Herein, we report a 65-year-old woman with a mass lesion in the second finger of right hand that lasted for 3 years, which finally managed by finger amputation. Histopathology of the tumor revealed solid islands of atypical keratinocytes with abrupt keratinization without granular layer formation. Immunohistochemistry for epithelial and cell proliferation markers was performed.
Conclusion: Malignant epithelial neoplasms of the nail should be considered in long-standing nail problems and appropriate nail biopsy should be performed.

Abstract
Although skin manifestations of COVID-19 were reported later than major organs affected, but it seems that they have gained relatively defined and definite described patterns. Regardless of strength and quantity of published papers and reports of rare cases, generally reported cutaneous manifestations are similar. However affection of vairous nail lesions have been mentioned recently. Even though no skin finding is recognized as specific, this does not mean not to look for and describe skin and nail changes. Common skin findings could be observed in many cutaneous diseases. In this issue, we decided to review literature about nail apparatus affected by COVID-19.

Abstract
Onychomycosis is a nail infection caused by dermatophyte and non-dermatophyte molds and yeasts. The nail involvement is a feature of psoriasis, although skin manifestations are the most principal findings in psoriasis. Onychomycosis and psoriasis are both common disorders in communities. It is possible that in some patients these two diseases are present at the same time. A review was performed using the keywords including nail psoriasis, onychomycoses, dermatophytes, non-dermatophyte molds and yeasts in valid medical databases, such as PubMed, Google scholar, Scopus and Science direct. Articles that were most relevant to the purpose of the study were selected and studied. We investigated the incidence and pathogenic function of fungal agents in causing fungal infections in psoriasis nails.
