Abstract

Frequently, diagnosis of lichen planus as a relative common chronic inflammatory mucocutaneous disease, is difficult, therefor, direct Immunofluorescence (DIF) examination is recommended for definite diagnosis. In this study both an statistical analysis of files of oral and maxillofacial pathology department of Dentistry school and direct immunofluorescence examination using paraffin blocks with new method, were performed. According to analysis of 8026 cases, 262 (3.26%) cases were diagnosed as lichen planus most patients were in fourth decade of their life and most common affected region was buccal mucosa, (69.68%).
In direct immunofluorescence study of 14 cases (using firth's method) fibrinogen deposits in 6 cases and immunostaining of colloid bodies with various immunoglobulins (IgA, IgG, IgM), C3 and also fibrinogen (few values) in most of patients, were detected. In spite of the fact that density of staining was lower than routine method (using fresh-frozen tissue), except for fibrinogen immunostaining, these findings are compatible with previous studies. In spile of limitation of samples, this study suggests that using this technique needs more evaluation and this can be used for diagnosis purposes when fresh-frozen tissue is unavailable.

Abstract

IgA selective deficiency is the most common immunodeficiency. The prevalence of it in different races varies from  to . Since secretary IgA has has a defensive role in the mucosal surfaces, supposing is thought that IgA deficiency will be accompanied by oral manifestations. The previous studies showed controversial results about that. The aim of this cohort study was to finding out oral manifestations in IgA- deficient individuals. As s result oral specialists can find the patients in early stages. 11 IgA- deficient patients (with IgA level < 10 mg/dl in serum) and 11 normal volunteers with the same age and sex were compared. The ages of the people were between 3 and 18 years old and 5 girls and 6 boys were in each group. Their oral examination included DMFT (Decayed, Missed and Filled Teeth), periodontal condition, Plaque accumulation and oral mucosal lesions. Saliva immunoglobulin and secretary component levels were detected by enzyme- linked immunosorbent assay (ELISA) and serum immunoglobulin levels were detected by single radial immunodiffusion (SRID) methods. All of the IgA- deficient patients had the serum IgA level < 10 mg/dl and their immunoglobulin levels were normal.  of these patients didn't have SIgA and the rest of them had a little SIgA in their saliva(<  SIgA levels in sex and age matched normal group). IgA deficient patients showed no statistical significant difference about oral manifestations in comparison with normal group. It may be related to the increase of compensatory SIgM or assistance of other non- immunological defense factors in saliva, phagocytosis and cellular immunity. Thus IgA- deficiency cannot produce any oral manifestations as a criteria to diagnose it.

Abstract

Dentinal dysplasia type 1 is a rare herediatary disease which is attributed to an automosal dominant trait. It's incidence is about 1: 100.000. Both dentition are affected with, although the involved teeth have a normal size, shape and consistency, and they are occasionally amber. The most common clinical feature, due to their extreme mobility, is malalignement and malpositioning of teeth. Such a mobility is resulted from abnormal development of root structure. In radiographs, the roots are sharp, biunt, and conic which can be absent in both dentition. Sometimes, multiple periapical radioiucences, without any carious lesion, are observed. There is no treatment for this anomaly and retaining teeth, as long as possible, is the main goal. In this article two cases dentinal dysplasia type 1 observed in one family, are reported.

Abstract

Statement of Problem: Radiography is the most common technique in working length determination, however, because of its limitations is not considered as an ideal technique. Its application, particularly for children due to radiation hazards, technical problems in young and unco-operative children and the superimposition of permanent teeth bud on primary teeth root, lead to numerous problems.
Purpose: The goal of this study was to evaluate the accuracy of an electronic apex locator (EAL) in working length determination of primary teeth.
Materials and Methods: In this in-vitro study 96 canals of the extracted primary teeth, with at least 2/3 of the root length remained, were investigated. There were no obstructed canal, previous root canal therapy and perforation of pulp chamber floor. All working lengths were also measured by radiography. The results of Raypex 4 and radiography were compared with actual root canal lengths determined by direct observation. The applied EAL, in this study was called Raypex 4, a new device belonged to the fourth generation (Ratio Type). The results were analyzed by Chi-Square and Pearson correlation statistical tests.
Results: The accuracy of Raypex and radiography were 61.5% and 63.5%, respectively. The differences between Reypex 4 root canal length measurements and those of direct observation were not significant (P=0.08), but such difference between radiography and direct observation was statistically significant (P=0.01). The diameter of the apical foramen (the site of canal opening) did not affect on Raypex 4 accuray (P>0.05).
Conclusion: Considering the acceptable safetyness, Painlessness, simple and rapid application and an accuracy comparable to that of radiography, the use of Raypex4 EAL for the measurement of primary teeth length is suggested.

Abstract

Statement of Problem: One of the dental lesions which is mainly diagnosed by periapical radiography is apical root resorption Diagnosis of the presence and extension of this lesion can put some effects on its treatment plan and prognosis.

Purpose: The aim of the present study was to evaluate the efficacy of periapical radiography on apical root resorption diagnosis. Materials and Methods: In this experimental study, sixty extracted teeth were examined radiographic and histological regarding the presence and extension of apical root resorption. It should be noted that periapical radiographs were taken before extraction of all mentioned teeth. The results were analyzed by Fisher’s exact test.

Results: The results of this study showed significant differences between radiological and histological examinations (P=0.0003). Significant correlation between these two observations was seen only in 53.33% of cases comprising 23% of teeth with root resouption. Non-correlation was found in 46% of samples. Moreover, there was no significant difference in the prevalence of apical root resorption between maxillary and mandibular teeth (P=0.233). Sensitivity of periapical radiographs in apical root resorption diagnosis was determined 57.3%. Conclusion: According to the findings of the present study, it seems that periapical radiography technique does not possess desirable efficacy in diagnosis of apical root resorption and its limitation should be considered by clinicians.

Abstract

Benign mesenchymoma is a soft tissue neoplasm that contains 2 or more mature mesenchymal tissues in addition to fibrous tissue. A rare case of gingival benign mesenchymoma in a 53- year- old man is presented and the clinicopathologic characteristics of this uncommon tumor in the head and neck region is reviewed and discussed.

Abstract

Statement of Problem: Epulides Fissurata (EF) are common proliferative and denture- induced lesions of the oral cavity with a predilection for female. This suggests a possible role for sex steroid hormones in the development and progression of these lesions.

Purpose: The objective of this study was the immunohistochemical evaluation of epulis fissuratum of the oral cavity for estrogen and progesterone receptors expression in epithelial, stromal, inflammatory and endothelial cells populations.

Materials and Methods: In this cross-sectional study, 15 samples of formalin- fixed, paraffin- embedded epulis fissuratums including marginal mucosal tissues in 4 cases as a control group, were immuno-histochemically evaluated for estrogen and progesterone receptors protein expression.

Result: In 10 cases, estrogen receptor positivity was found within the epithelium and progesterone receptor immunoreactivity was present in 7 cases. Stromal cells exhibited estrogen and progesterone receptor immunostaining in many cases but only few cases showed expression of these receptors in the inflammatory and endothelial cells. Estrogen and progesterone receptors were also detected in some cases containing salivary glands tissue.

Conclusion: Although chronic irritation may be the initiating factor for the occurence of epulis fissuratum, some of the cells in the lesion, could be potential targets for estrogen and progestrone hormones.

Abstract

Background and Aim: A strong causal relationship exists between cigarette smoking and development of oral squamous cell carcinoma, so oral screening using exfoliative cytology has been recommended to facilitate the early diagnosis of cellular alterations in oral mucosa and silver staining (AgNOR technique) has been proven to be of value in the detection of incipient cellular alterations. The purpose of this study was to compare the argyrophilic nucleolar regions (AgNORs) count of cells collected from normal mucosa of cigarette smokers with that obtained from non- smokers.

Materials and Methods: In this cross-sectional study, cytologic smears of normal tongue, buccal mucosa and floor of the mouth from 19 smokers and 19 non- smokers were stained for AgNORs. The AgNORs count was established on 100 cells. The count value of groups were compared and analyzed using the Levens, Paired T, Student and Factorial tests. Using P<0.05 as the limit of significance.

Results: The AgNORs were round and had a clustered distribution in both groups. The mean AgNORs count was statistically higher in cells of smokers than non- smokers (P<0.05). There was a significant difference between smears from the floor of the mouth and other anatomical sites in both groups. In this study, no correlation was found between AgNORs count and gender.

Conclusion: Analysis of AgNORs suggests that there might be a correlation between the smoking habit and an increased rate of cellular proliferation in the oral mucosal cells.

Abstract

Background and Aim: Radiography is the most commonly used technique in root canal length determination, but its application in pedodontics is difficult due to many problems such as radiation hazards, superimposition of permanent teeth buds and uncooperative children. The aim of this study was to investigate the accuracy of an electronic apex locator (Dentaport ZX) in measuring the root canal length of primary teeth in vivo.

Materials and Methods: In this test evaluation study, pulp tissues of 52 primary teeth planned to extraction were removed and the canal length were measured by Dentaport ZX. After extraction, real lengths of canals were measured by using# 15 K-Type file with 0. 1 mm accuracy and both measurements were compared. Data were analyzed by Smirnov Kolmogorov, chi-square, ANOVA and intraclass correlation and p<0.05 was considered as the limit of significance.

Results: The difference between the canal lengths measured by eye and Dentaport ZX was–0.17±0.384 mm. Maximum deviation of the real length of canals was between-0.2 to+0.2 mm from apical foramen. Accuracy of Dentaport ZX in the range of-0.2 to+0.2 from apical foramen was 66.96% and in the range of-0.5 to+0.5 from apical foramen was 92.17%.

Conclusion: Based on the results of this study, the use of Dentaport ZX in endodontic treatment for primary teeth could be advisable.

Abstract

Background and Aim: Human and animal studies linking fluoride with diminished intelligence have been published. Although adverse effects of high intake of fluoride on intelligence and mental acuity continue to be reported, they are still controversial. The aim of this research was to investigate the relationship between fluoride in drinking water and children's intelligence.

Materials and Methods: In this cross sectional study, 41 children were selected from the high fluoride area with 2.5mg/l (ppm) fluoride in the drinking water and 85 children were selected from low fluoride area with 0.4mg/l (ppm) fluoride in the drinking water. The intelligence quotient (IQ) of each child was measured by the Raven's test. The history of illnesses affecting the nervous system, head trauma, birth weight (2.5kg or  2.5kg), residental history, age and sex of children were investigated by questionnaires completed by the children's parents. Data were analyzed by Chi-Square test with p<0.05 as the limit of significance.

Results: In the high fluoride area the mean IQ of children (87.911) was significantly lower than in the low fluoride area (98.912.9) (P=0.025).

Conclusion: Based on the findings of this study, exposure of children to high levels of fluoride may carry the risk of impaired development of intelligence.

Abstract

In this paper, an unusual case of pigmented dentigerous cyst in a 37-year-old Iranian man with multiple dentigerous cysts of maxilla and mandible is reported. In addition to the characteristic histopathologic features of dentigerous cyst, numerous melanin granules which were distributed throughout the epithelial lining of one of these cysts were also noted. The histochemical study using Fontana’s staining indicated that the pigment was melanin and many of the cells containing melanin were immunohistochemically positive for S-100 protein. Although no firm conclusion could be drawn from the present case regarding melanin origin in dentigerous cyst, the possible histogenesis of melanocytes in odontogenic lesions is discussed.

Abstract

Background and Aim: Extensive studies on the epidemiology of teeth and oral diseases are an important part of health care programs specially for hearing impaired groups. For adequate programming in this field, proper situation analysis is mandatory. The aim of this study was to assess the DMFT (decayed missed filled teeth) of 12 years old students attending hearing impaired schools in Tehran and exploring the relation between sex, hygiene and hearing threshold with the index.

Materials and Methods: This cross sectional descriptive study was based on examining 12 years old (± 6 month) students (117 cases) attending hearing impaired schools in Tehran. A questionnaire was filled for each case. T, Chi-square and Kruskall-Wallis tests were used to analyze the results with p<0.05 as the limit of significance.

Results: The mean DMFT in these students was 3.07. Mean DMFT in students with very severe hearing loss was 2.99. Mean ranked DMFT in girls and boys was similar (56.09 in boys and 61.96 in girls). Mean ranked DMFT in students who didn’t use floss (66.40) was higher than those who used floss (46.71). Mean ranked DMFT in students who seldom brushed, was the highest (72.82) and in students who brushed once a day was the lowest (51.26).

Conclusion: The DMFT index in hearing disabled children was 3.07. Regular brushing and flossing reduced the index.

Abstract

Background and Aim: One of the complications following major oral surgeries is mucosal defects and delayed healing process. Up to now, various mucocutaneous grafts have been used in this field and recently, amniotic membrane has been proposed as a biological dressing in dermatologic, ophthalmologic and otolaryngologic practices. The purpose of this pilot study was to evaluate the healing process following human amniotic membrane graft on oral keratinized mucosa of rabbit.

Materials and Methods: In this experimental animal study, two surgical mucosal defects with the same size were made in palatal mucosa of 10 rabbits with the same weight, gender and race and a graft of human amniotic membrane was used on one of the defects. On the 7th, 14th and 28th postoperative days, surgical biopsies were randomly obtained from grafted and ungrafted regions of 3, 4 and 3 rabbits, respectively and submitted for microscopic study.

Results: According to the results, grafted regions showed more surface epithelialization and thicker newly formed epithelium. Also inflammatory cells infiltration was less in these areas. In all cases, there was a remarkable cartilage formation in the connective tissue of the recipient sites.

Conclusion: The results of this study suggest that the use of amniotic membrane graft in oral surgery could be effective in healing process. Additional studies should be done using animal and human models with more samples. Furthermore, the formation of cartilage in the grafted sites and its possible potential in reconstruction of bone defects, needs to be studied.

Abstract

Background and Aim: The efficiency of disinfecting materials and procedures in removal of contamination from dental surfaces and equipments is essential. In authors' previous study, daily use of 2:100 dilution of 5% sodium hypochlorite in water and disposable covers were recommended since HBV contamination was found on semi-critical parts of the operative dentistry department. The aim of this study was to evaluate the HBV contamination following application of the recommended procedures.

Materials and Methods: The study was conducted in two parts. In the first cross-sectional part, samples were collected from 17 sites of dental surfaces. In the second interventional part samples were collected from 10 sites of 9 dental and 3 sites of 2 light cure units, before and after disinfection with 5% sodium hypochlorite solution diluted to 2:100. Sterile cotton swabs moistened with sterile BSAS (Bovine Serum Albumin in Sodium Chloride) solution were used for sampling. Samples were tested by PCR technique in Pasteur Institute, Iran.

Results: None of the samples collected in the first part of the study showed contamination. In the second part of the study, from 96 samples taken from various parts of dental and light cure units, before and after disinfection, there was only one HBV contaminated site before disinfection which showed no contamination after disinfection.

Conclusion: Based on the results of this study, disinfecting procedure with 5% sodium hypochlorite solution diluted to 2:100 along with using disposable covers is effective in preventing HBV contamination.

Abstract

Background and Aims: Using the conservative adhesive resin restoration (CAR) in uncooperative children lead to numerous problems because of being time consuming. The purpose of this study was to compare the microleakage of conservative adhesive resin restoration under separate curing and co-curing.

Materials and Methods: In this experimental study, 120 intact premolar teeth were collected and 120 vertical grooves were prepared on them. Then the teeth were divided into four groups: group 1, separated curing of bonding agent, flowable composite and sealant group 2, co-curing of all materials for 60 seconds group 3,
co-curing of all materials for 40 seconds and group 4, co-curing of all materials for 20 seconds. Then the specimens were thermocycled and immersed in basic fuchsin solution. The teeth were sectioned horizontally and dye penetration was evaluated with stereomicroscope. Date were analyzed using one-way ANOVA and Scheffe test.

Results: Mean value of dye penetration in groups 1, 2, 3, and 4 was 1.53±0.6, 2.06±0.6, 2.5±0.7 and 3.53±0.6, respectively. There was a statistically significant difference between group 1 and the other groups (P=0.0001).

Conclusion: Considering the problems caused by microleakage in conservative resin adhesive restorations, co-curing method should not be used. In the case of using co-curing method, 60 second curing time is suggested for sufficient polymerization.

Abstract

Background and Aims: Selecting the appropriate education evaluation method and the effects of this choice according to different fields of medical sciences, is one of the current priorities of medical education. Therefore, the aim of this study was to design a checklist for evaluating the practical skills of dental students and to get compared with the global ranking method.
Materials and Methods: A checklist was developed for the assessment of clinical skills of the practical pediatric course according to an action research method. 32 students were evaluated by the checklist method and 32 students were evaluated by the conventional global rating method (sampling was based on available sample).  Faculty staff and students’ satisfaction regarding the two methods were reported by a 6-scaled measure and statistically analyzed by Wilcoxon Signed Ranks test. The students' scores by the two methods were subjected to paired t test for comparison.
Results: The students were more satisfied with the checklist method (P=0.003). The faculty staff considered a greater ability of the checklist method to differentiate between the students (P=0.03). However, in terms of ease of use and suitability, most faculty staff preferred the traditional method (P=0.02). In other cases, there were no significant differences in the views of students and faculty staff towards the two methods.
Conclusion: In the present study, both Checklist and global rating methods were valuable for evaluation, although due to the greater satisfaction of students and faculty staff with the developed checklist method, the possibility of identifying the weak points leading to decreased scores and better assessment of capabilities, this checklist can be used for evaluation instead.
