Abstract

Background and Aim: The utilization of information and communication technologies in medical librarianship would put librarians in new created conditions. Therefore, the aim of this study was to evaluate the effects of external factors on medical librarians' attitudes toward their future professional situation.

Materials and Methods: This was a cross-sectional study. All libraries and affiliated with ministry of health, and medical education were included in the study. The research consisted of all medical librarians working in the mentioned centers. Data was collected through questionnaires including two major parts and an additional section pertaining to personal information. From total of 1368 cases, 1001 answered to the questions. Statistical measurements consisted of frequency distribution, mean, percentage, measures of dispersion, correlation coefficient, and two tailed tests analyzed by SPSS software. P<0.05 was considered as the limit of significance.

Results: The results  showed that the mean score of external factors and attitude, were 3.25 and 4.18 respectively, at the scale of 5. There was a positive and significant relationship between external factors and attitude. Medical librarians did not consider the presence informaticians as a threat to their job security, and they have been able to adapt themselves to modern situations. Also, they were aware of the fact that if they can not succeed to match themselves with technological revolution, they will loose their jobs.

Conclusion: Structural changes in staff and educational establishments are necessary to satisfy future needs and is a priority to improve the influence of external factors on medical librarians' attitude toward the future of their profession.

Abstract

Background and Aim: According to controversies in the prevalence of hypersensitivity to dental local anesthetic drugs and patients who claim hypersensitivity to these drugs, the aim of this study was to determine the prevalence of hypersensitivity to dental amide local anesthetic drugs in patients referred to Tehran Allergy Clinic in 2005-2007.

Materials and Methods: In this Study (Review of existing data), records of 130 patients who were referred to "Tehran allergy Clinic" (2005-2007) were studied.

Results: The average age of patients was 29.5±18.8 years. 34% of cases showed positive skin reactions to at least one of the tested Lidocain concentrations and 10% of cases showed positive skin reactions to at least one of the tested Prilocain concentrations. There was a statistically significant difference in hypersensitivity to Lidocain 0.01 and 0.001 (p=0.017) and also between Lidocain 0.001 and 0.0001 (p<0.01). There was no statistically significant difference between other tested drug concentrations (p>0.05).

Conclusion: Many patients with history of hypersensitivity, show positive reaction to local dental anesthetic drugs. Prilocain hypersensitivity reactions are less than Lidocain. So application of Prilocain accompanies with less risk but its application should not be considered completely safe.

Abstract

Background and Aim: The aim of this investigation was to evaluate the osteopromotion property of homogenous demineralized dentin matrix (HDDM) on experimental surgical bone defects in parietal bone of rabbits using the guided bone regeneration (G.B.R.) technique incorporating Paroguide collagen membrane.

Materials and Methods: Surgical bone defects were created in 6 Newzland white rabbits (2 defects in each rabbit). The defects were protected by Paroguide membrane alone (control group) or filled with HDDM and protected by Paroguide membrane (experimental group). The HDDM had been obtained from the central incisors of rabbits. The rabbits were sacrificed after 15, 30, 45, 60, 75 and 90 days and the defects examined histologically. Data were analyzed using pair-t test. The level of significance was set at p=0.03.

Results: Histologically, the volume of newly formed bone matrix was significantly greater in the experimental group. No inflammatory reaction was seen in either experimental or control groups.

Conclusion: Bone regeneration was accelerated in the bone defects filled with HDDM in comparison to the control group.

Abstract

Background and Aims: The present study was designed for evaluation of bovine demineralized bone matrix (DBM) in healing process of bone defects and comparison of bovine DBM (xenograft) and human DBM (allograft) which is used clinically.

Materials and Methods: Seven male white New Zealand rabbits were used in this study. The incision was made directly over the midsagital suture of the parietal bone. Then 3 bicortical defects were created with trephine bur No.8 (8mm diameter). The defects were randomly filled with graft materials. One of the defects was left without any graft in all samples (as a control defect). The amount of bone formation was evaluated 3 months after surgery histopathologically. The data were analyzed using Friedman test, and when P-value was less than 0.05, the pair wise group comparison were performed by Wilcoxon (Boneferroni adjusted) test.

Results: Statistical analysis showed that there was a significant difference between bovine DBM group with control group (P=0.03). Furthermore, human DBM group was significantly different from control group (P=0.02). However, the difference between bovine DBM group and human DBM group was not statistically significant (P=0.87).

Conclusion: The results of this study showed the satisfactory bone healing in rabbit parietal bone defects filled with bovine DBM. The amount of healing in these defects was similar to bone defects which were filled with human DBM that is used clinically.

Abstract

Background and Aims: Candida species are opportunistic pathogens which live as benign commensal organisms in the oral cavities. The infants are more susceptible to opportunistic microorganism colonization. This study was design to evaluate the presence and quantity of Candida colonies in infants' oral cavities and its relation with age, gender, and type of diet and mothers' education level.
Materials and Methods: This historical cohort study was performed on two groups of infants 1) infants predominantly on breastfeeding (n=45) and 2) infants on bottle feeding (n=45). Oral samples were collected with swab technique and Candida species were identified using germ-tube test. The data were analyzed by using SPSS software, chi-square, logistic regression and Mann-Whitney tests.
Result: The Candida colony was seen in 53.3% of infants. The difference in oral Candida carriage and mean numbers of Candida colonies, according to the mothers' education level, was statistically significant (P=0.0001, P=0.000, P=0.0001) however, the mentioned two groups were not significantly different in terms of the presence of Candida and non Candida species (P=0.45). Candida colonization was not statistically related to the age and sex (P=0.48, P=0.4). By logistic regression test, it was found that type of diet and mother's education level affected oral Candida colonization (P<0.05).
Conclusion: The oral Candida colonization in breast-feeding infants is less than the bottle-feeding ones that might be attributed to the antifungal effects of mother's milk and the role of bottles in transferring the Candida species. Also increase in mother's education level leads to reduction in oral Candida colonization, which may be the result of improved hygiene. But no relation was seen between oral Candida carriage and age or gender of infants.

Abstract

Background and Aims: While there are a lot of information about an illness risk factors, there are still some gaps in understanding factors influencing the progression of disease. Given the important role of psychological factors in physical conditions, the purpose of this study was to compare the cognitive emotion regulation, stress, depression, anxiety, and stress related to life events in people with and without periodontal disease.
Materials and Methods: The type of this study was causal comparative. Research society was all clients referred to dental clinics in Lamerd and Mehr cities, then 50 people with and 50 people without periodontal disease (male and female, 15-49 years old) were selected using purposive sampling. The group without periodontal disease was matched with patient group in terms of demographic characteristics. Both groups completed cognitive emotion regulation questionnaire (CERQ), Depression Anxiety Stress Scale (DASS), and life events questionnaire. Data analysis was conducted with SPSS version 17 using multivariate analysis of variance (MANOVA).
Resultes: Data analysis showed that people with periodontal disease had more stress, depression, anxiety, and stress related to life events than matched control group. The groups had significant differences in the components of self-blame, rumination, positive attention to evaluation and catastrophizing (P<0.05). People with periodontal disease had higher scores in self-blame, rumination and catastrophizing and lower scores in positive attention to evaluating compared with matched control group.
Conclusion: The results showed people who were vulnerable to stressfull situations, depression, and anxiety during their lives, and also use maladaptive emotion regulation strategies, were more susceptible to periodontal disease.

Abstract

Background and Aims: The emergence of social web and impact on the communication of the individuals and the scientific activities of various communities has created serious research links among researchers. This issue that how Iranian researchers keep up to date themselves with new technologies and use them to engage in scientific issues has been a challenge that is constantly being addressed in the digital world. Therefore, the purpose of this study was to evaluate the scientific production of dentistry in Tehran University of Medical Sciences in the networks of ResearchGate and Mendeley using the Scopus citation database between 2015 and 2016.
Materials and Methods: The study population included all scientific articles in the field of dentistry of Tehran University of Medical Sciences which were indexed in Scopus database between 2013-2016. The results were presented using descriptive statistics in the form of charts and tables.
Results: In this study, it was determined that the scientific outputs of the researchers from Tehran University of Medical Sciences increased in 2015 and 2016. It was also found that the coverage of articles in the field of dentistry published by researchers at Tehran Medical Sciences Universities in the Scopus citation database in the mentioned years, were in a good position in both the Mendeley and ResearchGate networks. In addition, the number of references to the articles by researchers at Tehran University of Medical Sciences on the ResearchGate network in the field of dentistry was more than the Mendeley network.
Conclusion: Based on the results of this study, it can be concluded that the performance of the researchers of the Tehran University of Medical Sciences in the field of oral and dental care and the extent of their coverage in the social networks of ResearchGate and Mendeley was high and desirable.

Abstract

Background and Aims: Activity-based costing enumerate every input consumed in treatment. The Covid-19 pandemic changed the dental treatment protocols and the ordinary circumstances of patients. The aim of this study was to calculate and compare the costs of three common dental services in Iran from the viewpoint of the providers and recipients of the services before the Covid-19 pandemic (2019) and during the pandemic (2021).
Materials and Methods: Direct medical and non-medical costs were considered from the service provider's perspective. Direct medical costs, indirect costs, direct non-medical costs, and intangible costs were considered from the recipient's perspective. To calculate the equivalent costs, the base year was considered 2021, and the costs of 2019 were calculated with a discount rate of 16%.
Results: From the service provider's perspective before the pandemic, the cost of dental amalgam restoration was 2,851,235 Rials, root canal treatment was 6,351,580 Rials, and tooth extraction was 1,887,295 Rials. The cost of these services in 2021 was 5,562,150 Rials, 8,070,591 Rials, and 4,865,563 Rials, respectively. From the recipient's perspective, before the pandemic, the cost of tooth restoration was 715,466 Rials, root canal treatment was 2,883,001 Rials, and tooth extraction was 3,267,359 Rials. The cost of the mentioned services in the year 2021 was 1,212,528 Rials, 3,971,640, and 3,680,880 Rials, respectively.
Conclusion: The Covid-19 pandemic has had an effect on increasing the cost of dental services from both the service provider and recipient perspective, but economic volatility has a vital role in increasing the estimated costs in the country. Probably, a part of the increase in costs was due to the role of brokers in providing dental materials and equipment.

Abstract

Background and Aims: To prevent dental caries, investigations have focused on finding new antibacterial and anti-biofilm agents without the drawbacks of the currently used synthetic agents. This study aimed to assess the effect of methanolic extract of Zingiber officinale (Z. officinale) on expression of virulence genes of Streptococcus mutans (S. mutans) using real-time polymerase chain reaction (PCR).
Materials and Methods: In this in vitro study, which was conducted in the year 1402, at the Microbiology Department of the Faculty of Medicine and the Faculty of Dentistry at Shahid Beheshti University of Medical Sciences, the methanolic extract of Z. officinale was obtained by the maceration technique. 10 clinical isolates of S. mutans were obtained from the patients with dental infection. The minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) of the extract against S. mutans were determined by the microtiter plate technique. The effect of extract on the expression of relA, comDE, brpA, gtfC, and spaP virulence genes by the clinical isolates was evaluated by the real-time polymerase chain reaction (PCR) technique. Data were analyzed by the Wilcoxon and Mann-Whitney tests (alpha=0.05).
Results: The mean MIC and MBC of the methanolic extract of Z. officinale against S. mutans were 32±11.8 and 64±26.12 mg/mL, respectively. The extract caused down-regulation of relA, comDE, brpA, and gtfC by 50%, 40%, 70%, and 70%, respectively. It also caused 4 times reduction in expression of spaP gene.
Conclusion: The methanolic extract of Z. officinale caused significant down-regulation of gtfC, brpA, relA, comDE, and spaP genes, indicating its optimal efficacy to control the virulence of S. mutans.
