Abstract

Retention, resistance and marginal integrity mostly depend on tooth preparation. An appropriate convergence angle fulfil this purpose, to high extent. In this study, a new method was used to measure the convergence angle of the teeth prepared for single crowns in Genera! practitioners" offices in Tehran. In order to do this. 325 dyes, prepared by General dentists in Tehran, were collected from 10 laboratories. All dyes wore trimmed at the area below the finishing line and then were scanned (Genius Color page- FIR 6) buccoiingualiy and mesiodistaily. Convergence angle of dyes were also measured with Adobe Photoshop (5.0) software. Data were analyzed by variance analysis test and 1- student bv the help of SPSS software. Results showed that the average convergence angle ranged from 16.18+8.34 to 35.1 8~10.38 which belonged to maxillary canine and mandibular molars, respectively, and the measured convergence angle is more than the ideal value of 10-16 degrees. Dyes of the madibular molars were ma"illar" convergent. These conclusions are helpful for professors, dentistry students and dentists, and arc an indicative of the practice quality of General practitioners in Tehran. Due to the fact that a convergence angle, more than the allowed limitation, endangers retention, resistance and marginal integrity of the restoration, paying attention to the principles of tooth preparation and proper application of instruments and dental cements, can progress fwed restorations quality.

Abstract

Statement of Problem: Color matching and accurate shade selection are the challenging problems common to restorative dentistry. In ceramometal restorations, the type of substructure alloy affects the final color of bonded porcelain. Nickel- chromium alloy is the most commonly used one that its Iranian product, namely Minalux, is similar to Verabond2.
Purpose: The aim of this study was to assess and compare various color dimensions resulting from Minalux and Verabond2 alloys.
Materials and Methods: Nine disks, approximately 16 mm in diameter and 0.5 mm thickness, were cast from each alloy. Then, porcelain Vita VMK68 A2 was baked onto the entire test disks, following the manufacturer's instruction. Color samples, at the same time and under the same conditions, was measured by Data color spectrophotometer in CIE Lab System and Munsel system under four light sources (A, C, D65 and TL81). Then MATLAB TOOL BOX Statistic 5.2 was used to determine mean and bilateral variance analysis.

Results: It was indicated that the F value on hue, value and chroma was less than of the table value stated with 99% coefficient confidence, confirming Ho theory. In other words, there were not any significant differences between ceramometal disks made of Minalux and Verabond2 in the three dimensions of color.
Conclusion: Having desirable physical, mechanical and biological properties, Verabond2 can be replaced by Minalux alloy.

Abstract

Statement of Problem: In the treatment of edentulous patients with implant supported fixed partial dentures several factors such as implant numbers, implant position, superstructure pattern and cantilever length must be considered. Mandibular flexture in function exerts forces in peri-implant bone, however this phenomenon has received little attention.

Purpose: The goal of this finite element analysis (FEA) study was to evaluate the effect of mandibular dimensional changes on peri-implant bone stress in different prosthesis and implant treatment plans.

Materials and Methods: In this experimental study, three dimensional finite element computer model of mandible was simulated according to data from CT-Scan in 0.5 mm sections. The model of 4.110 mm ITI implant, measured by profile projector, was simulated in solid works 2003 software. Implant models were inserted, in two different patterns, on mandible and three different superstructures were placed on implants. Two clenching tasks were modeled (incisal clench and right molar clench).

Results: Analysis of Von Misses stress for peri-implant bone revealed the lowest stress values in three-piece superstructure.

Conclusion: According to this study, additional placement of implants in order to fabricate independent prostheses and to achieve the freedom of mandibular flexture are recommended.

Abstract

Background and Aim: Cementation is one of the most critical steps of the porcelain restoration technique. However, limited information is available concerning the bond strength of current ceramic bonding systems. The aim of this study was to evaluate the shear bond strength of three dual-cure resin cements to IPS-Empress2 ceramics.

Materials and Methods: In this experimental study, 30 pairs of IPS-Empress 2 ceramic discs were fabricated with 10 and 8 mm diameters and 2.5 mm thickness. After sandblasting and ultrasonic cleaning, the surfaces of all specimens were etched with 9% hydrofluoric acid for 60 seconds. Then, the three groups of 10 bonded specimens were prepared ceramic bonding resin systems including Panavia F2, Variolink II and Rely X ARC. After storage in 37±1c water for 24 hours and thermocycling in 5c and 55c water for 500 cycles with 1-minute dwell time, the shear bond strengths were determined using Instron machine at speed of 0.5mm/min. Data were analyzed by One Way ANOVA test. For multiple paired comparisons, the Tukey HSD method was used. The mode of failure was evaluated by scanning electro microscope (SEM). P<0.05 was considered as the limit of significance.

Result: Significant differences were found between different cement types (P<0.05). Variolink II provided the highest bonding values with IPS-Empress2. A combination of different modes of failure was observed.

Conclusion: Based on the results of this study, according to the highest mode of cohesive failure, Variolink II seems to have the strongest bond with IPS-Empress2 ceramics.

Abstract

Background and Aim: Debonding is the most common failure type of fiber posts. This in vitro study aimed to evaluate the bond strength of 3 different fiber posts cemented with a resin luting agent in three regions of prepared post spaces after being subjected to termocycling.

Materials and Methods Thirty six mandibular first premolars were sectioned at the cemento-enamel junction and were treated endodontically. After one week the post spaces were prepared and roots were divided into 3 groups of 12 specimens according to the post types (D.T.Composipost, D.T.White, D.T.Light). Sealbond Cement Dual II was used in all groups. One day after cementation, specimens were subjected to 3500 thermal cycles (5oC/55oC). Each root was then embedded in acrylic resin and sectioned perpendicular to long axis. Three 1-mm tick segments from apical, middle and cervical thirds were prepared. Push out test was performed. Data were analyzed with 2-way ANOVA (P<0.05). All root segments and dislodged posts were observed with stereomicroscope to figure out their failure modes.

Results: The post type had no significant effect on bond strength (p=0.08). However bond strength at the coronl segment was higher than the other segments (p=0.005). Mixed mode was the mostly occurred failure mode with D.T.Composipost and D.T.Light but with D.T.White, the mostly occurred failure mode was adhesive between the post and cement.

Conclusion: Different fiber posts have relatively similar bond strength to root dentin, bond strength at the coronl segment was higher than the other segments.

Abstract

Background and Aims: The All-on-4 design with its significant advantages is an appropriate model in reconstruction of edentulous mandible. Evaluation of stress and strain distribution in this model is necessary for better judgment. The purpose of this FEA study was to measure stress and strain distribution on peri-implant bone in All-on-4 design in edentulous mandible.

Materials and Methods: Three dimensional finite element model of human mandible was simulated according to data from CT-Scan of a cadaver. The model of 4×13.5 mm Nobel Biocare implant was simulated. Posterior implants were inserted in 452 inclination and anterior implants were parallel and vertical. Implants were splinted with a titanium bar and an acrylic superstructure was then simulated around the bar. Vertical loads of 178 N and 300 N were applied at incisor and left first molar positions, respectively. After meshing, defining boundary conditions and materials properties, analysis was performed with the aid of ABAQUS.

Results: Maximum Von-Mises stress of 38.9 MPa during anterior loading was located in peri-implant bone of anterior implants but maximum strain was observed in peri-implant bone of posterior implants. In posterior loading, maximum stress (77.3 MPa) was in peri-implant bone of posterior implant which was next to the place of load insertion. Maximum strain was found in the same area.

Conclusion: During posterior loading, significant amount of strain was observed in peri-implant bone of posterior angulated implant. As a result, there was a possibility of resorption in this area. During anterior loading, detected stress and strain was absolutely favorable.

Abstract

Background and Aims: Since a large marginal opening allows more plaque accumulation, gingival sulcular fluid flow and bone loss, microleakage, recurrent caries and periodontal disease, marginal fit is of great importance in fixed restorations. The aim of this study was to compare the marginal fit of zirconium-based all ceramic (Cercon) frameworks, made by two different approaches (Direct and Wax-up scanning).

Materials and Methods: An abutment analog, with 5.5 mm width and height, was selected as an experimental model. Twenty all ceramic cores were made for this model (10 specimens per each group). In the first group (Direct), experimental models were scanned directly using Computer Aided Design/Computer Aided Manufacturing (CAD/CAM). The core was then designed with the aid of a computer and machined. In the second group (Wax-up or CAM) a wax-up pattern was made on the experimental model and then scanned and milled from Cercon blocks. The marginal discrepancy of each specimen was recorded using a stereomicroscope (SZX9, Olympus, Japan) and digital camera (Mode TK, C1380E, JVC, Japan) and then measured by Adobe Photoshop CS software. The data were analyzed using T-test.

Results: Mean marginal discrepancy of Direct group (85.2±3.95 µm) was significantly less than that of Wax-up group (120.2±6.91 µm).

Conclusion: Improved marginal fit of zirconia ceramics was found with the direct model scanning compared with that of Wax-up scanning using CAD/CAM.

Abstract

  Background and Aims: Thermal changes in the oral cavity can be transferred to the implants placed in bone and affect the prognosis of the treatments. Furthermore, some investigators proposed improvement of habitual consumption of hot or cold beverages. The aim of the present study was to measure the effect of temperature changes in the oral cavity following consumption of hot and cold drinks on the temperature of implant.

  Materials and Methods: In this clinical trial, 3 eligible patients were selected. Two months after implant insertion, temperature of the implants was measured following the drinking of hot and cold beverages using a sensor connected to a thermometer. The thermal changes of the implants through the different time intervals were analyzed using Friedman non-parametric test.

  Results: The implant temperatures were significantly increased in the time following drinking of the hot beverages (P=0.009). Furthermore, the implant temperature were statistically decreased in the time after drinking cold beverage (P=0.004). When drinking hot beverages, the maximum and minimum temperatures were 41.57 0 C and 36.77 0 C, respectively. The maximum and minimum temperatures of the implants were also 34.13 0 C and 26.83 0 C , respectively, when drinking cold beverages.

  Conclusion: It seems that the temperatures noted in the implants in the current study cannot weaken the prognosis of implant restorations however, habitual consumption of hot and cold beverages can lead to debilitating implant prognosis and therefore, should be given the necessary warnings to patients.

Abstract

  Background and Aims: Nowadays, the application of all ceramic restorations are being raised, because of their physical characteristics, such as translucency and good appearance. Numerous researchers investigated the impact of surface treatments on the bond strength of zirconia ceramic with resin cements. The aim of this study was to compare the microshear bond strength of Cercon and Zirkonzahn (two kind of zirconia) ceramics, to two types of resin cements after thermocycling.

  Materials and Methods: In this study, 24 rectangular specimens were made from each group of Cercon and Zirkonzahn ceramics. After sandblasting, these specimens were connected to 3×1 mm2 composite cylinders by two resin cements (Panavia F2 and Rely X Unicem2). After performing a thermocycling regime for 5000 cycles (5-55 ◦ C), the microshear bond strengths were measured by a universal testing machine. The mode of failures were determined by a stereomicroscope. Data were analyzed using two-way ANOVA.

  Results: Type of ceramics had no significant impact on the microshear bond strength (P=0.317). The highest bond strengths in both ceramics were obtained with Reply X Unicem (P=0.035). The predominant failure mode was adhesive between the cement and ceramic.

  Conclusion: Type of resin cement had a significant effect on their bond strengths to zirconia ceramics.

Abstract


Background and Aims: Diferences whithin shape and size of hand wrenches in diferent systems could affect the torque applied. Therefore, evaluation of wrenches has been emphasized. The aim of the present study was to compare the torque values of the long and short torque wrenches in different implant systems including ITI, Implantium and Nobel Biocare.
Materials and Methods: In this experimental trial, 5 dentists with at least 5 years of professional experience closed the abutment screw in different implants systems using the long and short manual wrenches while the wrench-induced torque values were calculated by digital torque-meter. Torque application was done for 3 times with 10-minutes resting time between each wrench uses in the habitual closing torque manner. Data were analyzed using two-way analysis of variance test in different implant systems, while the paired comparisons were done using Tukey post hoc test.
Results: The maximum torque values of the long manual wrenches were 27.73±4.57 Ncm, 34.6±8.09 Ncm and 30.6±6.94 Ncm for the ITI, Implantium, and Nobel Biocare systems, respectively. While the values were reported to be 26.4±5.3 Ncm, 35.27±7.94 Ncm and 30.13±5.26 Ncm in the short manual wrenches for the ITI, Implantium and Nobel Biocare systems, respectively. Significant differences were found between ITI and Implantium (P<0.0001), and between Implantium and Nobel Biocare systems (P<0.021). However, no significant differences were observed between ITI and Nobel Biocare systems regarding wrench-induced torque values (P>0.05).
Conclusion: From the results, the torque values obtained by different long and short manual wrenches were specific to different implant systems. These values were less in ITI and Nobel Biocare systems and higher in implantun than those range provided for the implant systems by the companies.

Abstract

Background and Aims: Endocrown is a type of monoblock restoration using the pulp chamber and remaining coronal tooth structure for retention. Advancements in adhesive techniques have breathed fresh spirit into endocrown. Nowadays, there are two approaches for endocrown: conventional approach and  biomimetic approach. This study aimed to review data on the steps and clinical and philosophical points of using these approaches to help dentists’ selection and performance.
Materials and Methods: The review was conducted by reviewing the articles available in PubMed, Scopus, and Google Scholar databases from 1995 to 2022. The keywords used were; Crown, Endodontically-treated teeth, Light-curing of dental adhesive, Onlay, and Post-Core.  
Results: After removing duplicates, titles/abstracts were reviewed. Animal studies, studies without available full text or non-English language studies were excluded from the study. Then, the full texts of the remaining 59 articles were evaluated and 49 articles were eligible for the present study. The articles included clinical trials, in vitro, systematic reviews, and case reports.
Conclusion: Endocrown is a successful alternative to crown, especially in molar teeth. Regardless of the conventional or biomimetic approach, this treatment has a more favorable fracture pattern while preserving the remaining tooth tissue as much as possible.
