Abstract

Background and Aims: Cl II malocclusion is one of the most common abnormalities in human societies and using a simple, affordable and accessible treatment that can be provided by general practitioners or specialists, to prevent future malocclusion complications is vital. Cl II malocclusion treatment using functional appliances such as twin block and anterior inclined bite plan have less cost, side effects and complications compared to fixed orthodontic or orthosurgery. Until now, the dentoskeletal changes resulting from the application of twin block and anterior inclined bite plan in patients with Class II malocclusion has not been evaluated. The objective of the present study was to assess the dentoskeletal changes following the use of twin-block functional appliance and inclined anterior bite plan in the patients with class II malocclusion div 1 during mixed dentition.

Materials and Methods: In this retrospective cohort trial, 60 patients with the definitive diagnosis of Class II div. 1 malocclusion having ANB>2 and FMA angel between 20 and 30 and without any previous treatment or syndrome who were treated with twin-block or inclined anterior bite plan appliance were selected and their lateral cephalometries were traced before and after treatment. Selected distance and angular landmarks were measured on the cephalograms with the good reliability (ICC=0.953) and the changes occurred in the landmarks were statistically analyzed using Student t test.

Results: Due to the increased mandibular growth, most of the landmarks experienced significant changes following the treatment with twin-block and inclined bite plan (P<0.05). The appliances corrected Class II malocclusion through improvement of mandibular length and position, maxillary and mandibular skeletal and dental relationships (decreased ANB angle and overjet) and stability of mandibular plan inclination. No significant differences were found between the devices regarding most landmarks changes (P>0.05), however, SNA (P=0.04), overjet (P=0.007) and wits appraisal (P=0.004) changed differently after using the appliances.

Conclusion: Despite with most similarities of both twin-block and anterior inclined bite plan to correct class II div. 1 malocclusion, Due to the advantages of anterior inclined bite plan such as less size, good patient cooperation and simple laboratory preparations the appliance can be used as substitute for complex functional appliances to correct class II div. 1 malocclusion before the growth spurt.

Abstract

Background and Aims: Cervical tooth hypersensitivity (CTH) is defined as a sharp pain of short term that occurs in dentin exposed to the oral environment. The aim of this study was to evaluate the efficacy of a desensitizing toothpaste containing potassium nitrate (Sensodyne®) for relieving of CTH, when used locally or as tooth paste.

Materials and Methods: 36 patients were enrolled in this study. They were randomly divided into three groups I) Tooth brushing without toothpaste, II) Local application of toothpaste containing potassium nitrate 5%, III) Tooth brushing with toothpaste containing potassium nitrate 5%. Assessment of CTH was done by evaporative (using air and water spray) and cold stimuli at baseline, 1, 3, and six weeks after the treatment. To compare pain changes over time, due to the interaction between the variables of the studied groups and time, one-way ANOVA was used at any time. Due to the difference of variance between groups to compare pairs of groups, Dunnett T3 test was used.

Results: During the study period, no adverse event observed. In all three groups, pain decreased gradually. Local application of toothpaste had the most favorable effect on CTH. Therefore, when compared with that of the first group, the percentage reduction in pain were statistically significant (P=0.03).

Conclusion: Local application of toothpaste containing potassium nitrate as a desensitizing ingredient is a better choice for treatment of CTH than that of application with toothbrush.

Abstract

Background and Aims: Although dental implants have become a successful and predictable ong-term treatment for patients, not all implant treatments are necessarily successful and pre-implant diseases are spreading. The aim of this study was to determine the prevalence of peri-implantitis in patients one year after prosthetic loading in Khorramabad in 2021.
Materials and Methods: In this cross-sectional study, 53 patients who had implants and undergone prosthetic loading for one year were studied. In each of the 53 patients, the probing depth (PD) and bleeding during probing (BOP) around the implant and the characteristics of the surrounding soft tissue, including gingival redness and tissue sensitivity were evaluated and recorded. To assess the bone level or possible bone resorption, a P.A. graph was taken from the patient using a parallel technique. Information such as demographic characteristics and specialized information about the disease were collected through a checklist. Data were collected using SPSS22 software. Frequency tables and chi-square and t-tests were used to analyze the data.
Results: The mean age of patients participating in the present study was 46.5±10.63 years. The number of women was 33 (62.3%). The prevalence of peri-implantitis was 17% at the patient level and 9.3% at the implant unit level. There was a significant relationship between the age (P<0.03) and systemic disease (P<0.03) with the prevalence of peri-implantitis. However, there was no statistically significant relationship between the prevalence of peri-implantitis and other demographic variables (P<0.05).
Conclusion: The results of the present study showed that the prevalence of peri-implantitis was higher in people with a higher mean age. In addition, systemic diseases are a protective factor for peri-implantitis.
