Abstract

Background and Aim: Proper shaping and cleaning of the root canal system is one of the most important aspects of endodontic treatment. To estimate the canal length before instrumentation in endodontic treatment, traditionally, conventional radiographic techniques and recently Direct Digital Radiography (DDR) are applied. The application of computer technology to radiography has allowed less exposure time, image acquisition, manipulation, storage, retrieval, and transmission to remote sites in a digital format, elimination of wet processing and considerable reduction in the time lapse between image acquisition and display. The purpose of this study was to compare the accuracy of DDR versus conventional radiography in estimating endodontic file lengths of curved canals in first mandibular molars.

Materials and Methods: In this test evaluation study, fourty extracted human first mandibular molars with root curvature were selected.Samples were divided into two groups: With root curvature less than 25 and more than 25. Samples were mounted in plaster blocks and canal lengths were estimated by using DDR and conventional radiographs. Regression analysis and correlation coefficient were used to calculate statistical differences between the groups with P<0.05 as the limit of significance.

Results: Conventional radiography was more precise in canals with less than 25 degrees curvature (P=0.160). While, DDR was more precise for canals with curvature more than 25 degrees (P=0.605). However, these differences were not statistically significant.

Conclusion: The image quality of DDR system has improved to the point that it can now be used for estimating canal lengths, even for curved canals, with accuracy comparable to that of conventional radiography.

Abstract

Background and Aims: Fissure sealant therapy is a national program in elementary schools in Iran. Therefore, evaluation of the parents' knowledge and attitude is necessary to improve the program.
Materials and Methods: In this cross-sectional and descriptive-analytic study, Iranian students were chosen in a randomized cluster manner. The questionnaires were filled-out by students' parents to evaluate their general information, knowledge, and attitude. Moreover, students were examined for having any fissure-sealed teeth. Data were collected and analyzed by multiple logistic regression tests.
Results: 414 boy students and their parents were participated in this study. 57.6% of parents knew that fissure sealant prevents the caries and 44.5% believed that fissure sealant is important to prevent caries. However, only 8.9% of children had fissure sealant. Parents' knowledge was significantly related to the source of oral health information and oral health education. Regular dental visits and high level education was effective in increasing the knowledge about sealant therapy. There was a significant relation between parents' attitude toward fissure sealant and level of education, oral health education and numbers and times of dental visit (P<0.001). There were a significant relation between tooth sealing and parents' oral health information.
Conclusion: Regular dental visits and oral health education have positive role in parent's knowledge and attitude and practice. So parents must be encouraged to have regular dental visit and dentists must be emphasized to give them necessary education. School is the best place for giving proper information to parents.

Abstract

Background and Aims: Optimization drilling parameters in order to temperature decrease during creation of hole in the bone is an interested issue. The aim of this study was to achieve optimum values of drilling parameters based on the creation of minimum temperature during jawbone drilling.

Materials and Methods: In this study two models of mandible and maxilla was created and teeth 2, 5 and 8 from maxilla and teeth 25, 28 and 31 from mandible were removed. The drilling operation was performed under different conditions on jawbone models using finite element analysis and the maximum temperatures were measured in adjacent of holes.

Results: Drill bit head angle of 70 degrees was created the lowest maximum temperature during drilling operation. The lowest maximum temperatures were observed in the drill bit rotational speed, drill bit feed rate and the force exerted on the drill bit equal to 200 rpm, 120 mm/min and 60 N, respectively. The use of irrigation can decrease the maximum bone temperature about 7ºC. The maximum temperature differences in various regions of mandible and maxilla were approximately about 1ºC.

Conclusion: Sharpness of drill bit head angle, reduction of drill bit rotational speed, increasing drill bit feed rate and exerted force on drill bit and also the use of irrigation played effective roles in temperature decrease during jawbone drilling. Drilling site did not have important effect on the temperature changes during jawbone drilling.

Abstract

Background and Aims: Oral health is a necessity and part of general health and its importance in childhood is more than ever. The aim of this study was to investigate the knowledge, attitude, and practice of parents in Delfan city-Iran about oral health of their 6-12-year-old children in 2021.
Materials and Methods: The present study was a descriptive-analytical study that was performed on 365 parents of 6-12-year-old children in Delfan city. Convenience sampling was used. The questionnaires included four sections: demographic information, knowledge, attitude and practice about oral health. Data were entered in SPSS23. Independent t-test, analysis of variance and Pearson correlation were used to analyze the data. Significance level was considered 0.05.
Results: The mean age of the subjects was 36.5±7.1. 201 of parents were mothers. Parents' knowledge and attitude were good and their performance was fair. There was a statistically significant relationship between all three variables of knowledge, attitude, and practice (P<0.00). Also, there was a statistically significant relationship between the knowledge and education (P<0.00), mother job (P<0.04), father job (P<0.00), and economic status (P<0.00). There was a statistically significant relationship between the attitude with education (P<0.00), mother job (P<0.04), father job (P<0.01), and economic status (P<0.01), There was a statistically significant relationship between practice and education (P<0.00), mother job (P<0.01), father job (P<0.00), and economic status (P<0.00) of parents.
Conclusion: The results of this study showed that there was a significant relationship between the knowledge, attitude, and practice with education level, job and economic status. Therefore, oral health education for parents with low educational and economic level should be highly considered.

Abstract

Background and Aims: Multiple sclerosis (MS) is an autoimmune disease. Oral manifestations in these patients include paralysis or spasm of facial muscles, temporomandibular joint disorders (TMJ), trigeminal neuralgia, dysphagia, or dysarthria. The aim of the study was to investigate the characteristics of orofacial pain in patients with MS.
Materials and Methods: The current cross-sectional study was conducted on 250 patients with a definite diagnosis of MS referred to Iran MS Association and Iran MS Research Center. The patients were in the age group of 17 to 57 years and were selected completely randomly. Data were obtained based on two checklists and clinical examination. Finally, DC/TMD criterion was used to check TMD problems. Then the data was analyzed by SPSS version 26 software and by Fisher's Test.
Results: The prevalence of MS disease was higher in women and the average age of patients was 37 years and the average duration of MS disease among patients was 8 years. The most common pain was related to shooting and shock-like pain with a frequency of 40.4% and the lowest quality was related to dull and diffuse pain with a frequency of 5.3%. In total, 24.5% of patients with buccal-oral pain had TMD, and in this case, the DC/TMD criterion was used.
Conclusion:  Temporomandibular disorders and shooting and shock-like pains are usually observed in a number of patients with MS.  As a result, dentists should be aware of the presence of these pains in the patient, the pain trigger, and the drugs used by the patients, in order to provide medical services and being aware of drug interactions.

Abstract

Background and Aims: Recently, the use of the palatal process as a source of autogenous bone graft has been considered. Given the anatomical limitations of this space, the assessment of available bone in this area can play an important role in the correct treatment plan. The aim of this study was to assess the amount and density of available bone for the preparation of autogenous graft from the maxillary palatal process using CBCT (Cone Beam Computed Tomography).
Materials and Methods: This cross-sectional analytical study was conducted by collecting CBCT images from 78 patients referred to one of the maxillofacial radiology clinics in Kermanshah in 2022 and 2023. All the patients had no signs of trauma, impacted teeth, pathological lesions, cleft palate, severe periodontitis, or extensive surgery, and also had all the teeth in the area of interest (right first premolar to left first premolar) and their bone growth was completed (age over 20 years). The CBCT images were collected and analyzed using the ondemand program. Different indices such as the area of each cross-sectional slice, bone volume (main variable) were measured with the help of mathematical formula, bone density, and incisive canal diameter. In order to test the hypotheses, the Smirnov-Kolmogorov test was used to check the normality of data, and since none of our variables were normal, we used Spearman and Mann-Whitney correlation tests.
Results: This study was conducted on 78 subjects with a mean age of 43 years, 39 of whom were female and 39 of whom were male. The mean bone volume obtained was 1.84 cm3. The highest bone density was observed in the lateral incisor region and the mean incisal canal diameter was reported to be 2.68 mm. According to the results of the Spearman test, none of the variables had a significant relationship with the age variable. According to the results of the Mann-Whitney test, there was no significant difference in bone density in the lateral, canine, and premolar tooth areas between men and women. However, available bone volume was associated with gender, and this amount was calculated to be greater in men than in women.
Conclusion: According to the findings of this study, the average bone volume in the anterior region of maxilla was 1.51 cm3 and the highest bone density was in the lateral tooth region. The average incisal canal diameter was 2.68 cm3 and the available bone volume was related to gender and this value was higher in men than that of women. However, there was no difference in the bone density between men and women.
