Abstract

Normal 0 false false false EN-US X-NONE AR-SA Juvenile ossifying fibroma of the jaws is a relatively rare but important among fibro-osseos lesions that has controversial etiology, unclear origin and various histopathological views. Radiographic findings depending on extention rate, history and stage differs from a radiolucent cyst to ground glass view to a clearly defined borders lesion. In Histological view,  as well as general characteristics of  ossyifying fibroma, cellular stroma includes many osteoblasts and multinuclear ginat osteoclasts and psammoma-like bodies . due to its aggressive manner, ossifying fibroma is called locally invasive or low grade malignancy which is prone to recurrence. Differentiating JOF with fibros dysplasia is quite important. Two cases of aggressive and extensive mandibular JOF in 4 and 7 year old boys were mentioned in this article. Laboratory, radiographic and histopathologic confusions were cleared by diagnosis of JOF. Treatment includes hemmimandibulectomy without any post surgical complications.

Abstract

Tooth Transplantation from one individual to another is a method of replacing missing teeth. This method can be used as an applicable clinical treatment technique due to its numerous advantages .There are different methods of tooth transplantation. Among those, the safest and healthiest is transplantation of non-vital teeth or allostatic transplantation. In this method, the risk of antigenicity in recipient is low and also there is no risk of transmission of infectious diseases such as Hepatities or AIDS. The aim of this study was to evaluate the clinical results of tooth allotransplantation. Current study was conducted on 15 non-vital transplanted teeth within 18 months follow-up. Only one case of rejection was observed which was a Traumatic Rejection. Replacement resorption was found in all the cases while inflammatory resorption was recorded in only two cases. WBC Cross Match Test with pannel cell results were negative in all the cases.

Abstract

Normal 0 false false false EN-US X-NONE AR-SA Non-removable Keratotic lesions are relatively prevalent in various types in oral mucosa. The treatment modality is often problematic and is done by using  various specific regimens. Bleomycin was used in two primary studies with limited number of patients (N=9) . In current survey, more patients were recruited in a longer period of study. In fact, 50 leukoplakia and lichen planus patients were treated by local administration of belomycin  in a 21 months follow-up period. Statistical analysis was done by using Kaplan meier and cox Regression methods. The obtained results showed that belomycin can remove many of these lesions and can prevent both chronic pattern and malignant changes of the lesions. As In 9 cases of leukoplakia, 2 cases of recurrence were recorded. According to statistical analysis, in case of complete treatment up to 11 months , the recurrence  would not be possible. In 41 cases of lichen planus, recurrence rate was higher and after  a longer period (17 months ) following complete treatment, recurrence was totally eliminated. In fact, Recurrence can be due to psychological conditions. To conclude, local administration of Bleomycin is recommended to use to treat Oral  keratotic lesions and can act successfully based on current study's findings.

Abstract

Nucleolar organizer regions (NORs) are loops of rDNA, which in human, occur on the short arms of five acrocentric chromosomes, namely, 13, 14, 15, 21 and 22. NORs are usually demonstrated by binding If their associated proteins (NORAPsO to silver (Ag+) ions. The reaction is called AgNORs. It has been determined that, the number and the size of NORs is related to the proliferative activity and grade of malignancy. This has led to the application of AgNOR method to a wide range of diagnostic problem cases, including breast, salivary glands and lung malignant neoplasms.The purpose of this study was to determine the value of AgNOR method in assessment of the grade of malignancy in non- Hodgkin's lymphoma of head & neck, which often is difficult and essentially important in relation to the prognosis and treatment.41 cases of paraffin embedded specimens of NHL, which were classified by working formulation system,were selected and then AgNORs method was applied on them.AgNORs were enumerated for all 41 cases of NHL (Standard Crocker).The mean AgNORs in 11 cases of low grade 15 cases of intermediate grade and 15 cases of high grade NHL was 1.66 (SEM= 0.098), 2.25 (SEM= 0.084) and 5.19 (SEM= 0.11), respectively.In analysis of variance and multiple comparisons, a significant difference was found between the three groups of NHL (P<0.000I).It is concluded that the AgNORs, as a reliable method, could be used in differentiation of low,intermediate and high grade NHL in head and neck area.

Abstract

The differential diagnosis between pleomorphic adenoma, the most common salivary gland neoplasm, microscopically, and two other common salivary gland neoplasms, meaning adenoid cystic carcinoma and mucoepidermoid carcinoma, is difficut.The purpose of this study was to determine the differences between pleomorphic adenoma, adenoid cystic carcinoma and mucoepidermoid carcinoma regarding the staining intensity with PTAH and H&E and also to investigate the relationship between staining intensity with PTAH and malignancy grade in mucoepidermoid carcinoma. 72 paraffin embeded samples including 24 pleomorphic adenoma, 24 adenoid cystic carcinoma and 24 mucoepidermoid carcinoma were selected and stained with PTAH. The staining intensity in total, in nucleus and cytoplasm of tumoral cells were evaluated and compared with H&E staining, statistically. The results were analyzed by Kruskal-Wallis, and Wilcoxon signed ranks tests (P<0.05).The logistic model was presented to predict the degree of malignancies through the assessment of nucleus and cytoplasm staining intensity in tumoral cells, and the patient's age. In this study, a statistically significant relationship was observed between staining intensity by PTAH and H& E in nucleus of tumoral cells in pleomorphic adenoma. Moreover, statistically significant relation between staining intensity by PTAH and H & E in cytoplasm of tumoral cells in adenoid cystic carcinoma was found. But there was no relation in other cases. On the other hand, a statistically significant relation between intensity of staining in total, in nucleus and cytoplasm of tumoral cells and the type of tumor was found. No relation was obtained between malignancy grade of mucoepidermoid carcinoma and staining intensity in total, in nucleus and cytoplasm of tumoral cells. The presented logistic model indicated a direct relation between tumor malignancy with patient's age and staining intensity in nucleus of tumoral cells, but a reverse relation was found between cytoplasm staining intensity in tumoral cells and tumor malignancy.Our findings show that PTAH and H & E staining methods, lead to similar results, however, PTAH staining is a reliable method in differential diagnosis of such salivary gland tumors.

Abstract

Statement of Problem: The correlation between the activity of aspartate aminotransferase (AST) and alkaline phosphatase (ALP) enzymes in gingival sulcular fluid (GCF) with inflammation and periodontal attachment loss has been proved, however there are not adequate studies about dental implants.

Purpose: The aim of present study was to investigate the presence and activity level of AST & ALP and their correlation with pocket depth (PD) and bleeding of peri-implant slcular fluid (PISF), and to evaluate the possibility of using these assessments as a diagnostic index in oral implantology.

Material and Methods: In this study, 41 implants as test group and 41 contralateral teeth as control group, in 21 patients were evaluated. At first visit, the general information about implants and the values of pocket probing depth (PPD), modified sulcus bleeding index (mSBl) and modified plaque index (mPI) were recorded. At the second visit, samples of GCF/PISF were collected. AST & ALP activity was determined spectrophotometrically and data were analyzed by "t", "Mann-Whitney" tests and Pearson Spearman correlation coefficient.
Results: The results showed that there was a significant difference in the activity of AST between two study groups (P<0.0001). The average activity of ALP in test group was more than control group but the difference was not significant. After elimination of the confounding variables, the average AST in test group was 54.6 (S£=2.3) and in control groups was 44.8 (SE=2.3) (P=0.004). The average ALP in test group (SE=2.2) and in control (SE=2.2) were 36.6 and 35.4, respectively. Values of AST and ALP were positively correlated with other clinical parameters such as PD and mSBI which was significant in test group.
Conclusion: The present study suggests that PISF analysis could be considered as a proper diagnostic strategy in the evaluation of dental implant success.

Abstract

Statement of Problem: Root canal cleaning and shaping are important steps in endodontic therapy. In order to develop better techniques, new instruments are constantly designed and delivered to the market.
Purpose: The aim of the present study was to compare the effectiveness of one manual with three rotary root canal instrumentations techniques.
Materials and Methods: In this in-vitro study, 80 mesial roots of freshly extracted human mandibular molars were divided in to four groups equally and prepared by one of the following instrumentation methods profile NiTi rotary system, Protaper NiTi rotary system, Flex Master NiTi rotary system and hand NiTi K-file instrumentation. In each root one of the canals was instrumented and the other was used as control.After decalcification, cross sections at 3mm and 5mm of apical portion were prepared and examined for remaining debris, predentin and smooth walls under light microscope. The data were analyzed by Kxuskal-Wallis test.
Results: Significantly less debris was found at 3mm cross- section using protaper and Flex Master techniques (P<0.05). No significant differences were found at 5mm cross section. Also no significant differences were observed for remaining predentin at all regions. Significantly more smooth walls were found at both regions using protaper technique (P<0.05).
Conclusion: The protaper technique employed in this study resulted in cleaner root cana! walls than the
other techniques.

Abstract

Statement of Problem: Total removal of tissues and remnant microorganisms as well as canal shaping are the essential objectives of endodontic therapy. A successful endodontic treatment is obtained through Shilder’s principals, however complete observation of this technique using stainless steel files manually is problematic and time-consuming. Modern technology, in order to eliminate such problems, has presented new facilities such as Nickel-Titanium (NiTi) files and engine driven instruments.

Purpose: The aim of this in-vitro study was to compare the canal debridement efficiency of three engine driven instruments: Rotary, Reciprocal and Vertical.

Materials and Methods: In this experimental study, 60 mesial roots of human first and second mandibular molars were divided into three groups randomly. In each sample, one canal was considered as case, the other one as control. Files used in Reciprocal and vertical groups were of handy Ni-Ti type and in rotary group, rotary Ni-Ti files were used. After debridement, the roots were sectioned at 3mm and 5mm from anatomic apex, stained and examined under light microscope. Comparison criteria between case and control groups were based on residual debris and predentin and the level of root canal preparation and shaping after debridement. Data were subjected to kruskal-Wallis non-parametric test.

Results: There was no significant difference between the efficiency of debridement at 3mm and 5mm sections between all groups. But difference in time consumption was significant ranked from the shortest to the longest as rotary, reciprocal and vertical.

Conclusion: The efficiency of debridement between the three automated instruments was approximately equal, however the instrumentation time was different between three groups. Rotary system was the fastest one, as compared with reciprocal (second) and vertical (last). It may be concluded that rotary system has a superiority over the other two groups in conventional root canal therapies.

Abstract

Statement of Problem: Today, cross infection control is an integral part of dentistry and many dental health care workers no longer question its necessity. All dental equipments and instruments could be potentially considered as a source of infection.

Purpose: The aim of this study was the evaluation of aerobic contaminations of four disposable materials used in routine dental practice.

Materials and Methods: In this cross-sectional study, random sampling was performed and tryptone soy broth transfer culture medium was used. After 24 hours of incubation, samples were transferred to specific culture media. (Mckangy, Chocolate agar and Blood agar). Cultured bacteria were stained and studied using gram staining method. The study was carried out in a 17 weeks period.

Results: All suction tips were infected with Bacillus cereus bacterium. Two cases of cotton roll samples showed contamination with Bacillus cereus, 2 cases with gram positive, coagulase negative cocci, 1 case with Streptococcus viridians and one case with Enterobacter class A. One case of anesthetic cartridges was contaminated with Staphylococcus epidermidis and two cases with Streptococcus viridians. Two cases of gutta-percha samples were contaminated with Bacillus cereus.

Conclusion: Based of the findings of this study, the highest level of contamination was observed in saliva ejector tips. Contamination with Bacillus cereus was seen more frequently.

Abstract

Background and Aims: One of the main disadvantages of light cured composite resin materials is polymerization shrinkage upon curing. This leads to the microleakage of restoration and finally failure of the restoration. The purpose of this study was to investigate the effect of two types of light curing devices with different modes on the microleakage of posterior composite filling in Cl II restorations.
Materials and Methods: In this experimental study, 30 extracted sound molar human teeth were collected. All specimens were randomly divided into 3 groups of ten: Standard LED irradiation, pulse curing LED irradiation and QTH irradiation. All samples were prepared with mesial and distal boxes and composite fillings were done with three different light curing patterns for 20 s. After thermocycling, the specimens were kept in 2% methylene Blue solution for 24 h for microleakage test. After sectioning, the samples were evaluated by a stereomicroscope. For data analysis one-way ANOVA and Kolmogorov-Smirnov were used. P<0.05 was considered statistically significant.
Results: The quantitative tests showed that there were no statistical difference between study groups (P>0.05).
Conclusion: The results showed that both light curing devices were effective and no significant difference between different modes of LED light curing device on microleakage of class II composite restorations was found.

Abstract

Background and Aims: Dental diagnosis may be affected by the occurrence of the errors in the intra-oral radiographic images and the patients’ treatment plan can be problematic in these cases. The repetition of these radiographs increases the risks of radiation exposure for the patients in turn. The present study assessed the incidences of common periapical radiographic errors taken by dental students in oral and maxillofacial radiology department of Mashhad dental school in 2017.
Materials and Methods: In this cross-sectional descriptive study, 1470 periapical radiographs prepared by student during a semester of 2017 were collected and evaluated. Radiographic observations were done and the incidence of conventional radiographic errors were determined. Finally, the frequency and percentage of radiographic errors were determined based on the type of radiography sex, the age of the patient, and the history of radiography. Then, results were analyzed using the Chi-Square test and SPSS19 (P=0.05).
Results: Film placement (27.2%), cone-cut (27.2%), elongation (13.2%) and horizontal angle (10.9%) were reported as the most common radiographic errors. The repetition rate of the radiographs was 2.9%; among them, the highest errors were Cone-cut errors in the upper jaw
Conclusion: Based on the results of this study, the most radiographic errors included film position and Cone-cut errors and these two errors were the most frequent cause of repetitive radiographies that required the provision of specific training for the prevention of these errors.
