Abstract

Statement of Problem: One of the problems associated with the treatment of periodontal diseases is caused through the extension of disease toward furcation area. Several techniques in Conservative, Resective and Regenerative categories have been suggested for the treatment of furcation involvement.
Purpose: The aim of this study was to compare the results of the treatment of grade II furcation involvement in mandibular molars using an allograft material named 'Dynagraft' (a type of demineralized bone matrix) and the coronally positioned flap.

Materials and Methods: In this randomized controlled clinical trial study, twelve patients (9 females and 3 males), aged 25 to 40, suffering from bilaterally grade II furcation involvement of mandibular molars who referred to dental faculty Tehran University of medical Sciences, were investigated. The molars of one side were treated by Dynagraft whereas those of the opposite side underwent the CPF method. Measurements of the probing pocket depth (PPD), clinical attachment level (CAL), keratinized gingiva (KG) and horizontal probing depth (HPD) were recorded at baseline, 3 and 6 months after surgery. In order to investigate the bone radiographic changes, radiovisiography at the mentioned periods in addition to clinical investigation, were performed. For statistical analysis, Paired West was used.
Results: The mean PPD reduction three months and six months after the operation were 1.75 mm and 2.25mm, respectively in the Dynagraft (test) group whereas 1.26mm and 1.27mm in the CPF (control) group (P<0.005). The mean attachment gain three months and six months after the operation were 1.1 mm and 1.5mm respectively in the test group, and 0.2mm and 0.3mm in the control group (P<0.005). The mean KG reduction three months and six months after the operation were 0.5mm and 0.6mm respectively in the test group and those of the control group were 1.1mm and 1.1mm. The mean HPD reduction three months and six months after the operation were 1.55mm and 2mm respectively in the test group (P<0.005) and 0.55mm and 0.55mm in the control group (PO.01). Radiovisiography of the mentioned areas three months and six months after the operation confirmed the changes obtained from clinical measurements, showing appreciable reconstructive results (Bone filling) in the test group as compared with the control group. Moreover, root resorption was not observed.
Conclusion: Based on the results of this study, Dynagraft can be used as an appropriate material in the treatment of grade II furcation involvement in mandibular molars. However, for a through evaluation of such regenerative techniques in furcation involvement, further studies with larger population and long term follow up in addition to histologic studies are suggested.

Abstract

Statement of Problem: There are different tooth brushing methods for removing dental plaque from tooth surfaces. The effectiveness of these methods, and the time needed for instruction, learning and using seem to be different. Also, it is possible that the effectiveness of each method reduces with time, based on its difficulty level and the reduction of the patient s attention in following the given instructions. Purpose: The aim of this research was to compare the effectiveness of six different accepted tooth brushing methods on total and specific teeth, as well as on special tooth surfaces. The research also compared the time needed for instruction, learning and using the methods. Materials and Methods: The study was designed as a single blind randomised controlled trial protocol to compare the six accepted tooth brushing methods: Roll, Bass, Charters (C), Modified Stillman (MS), and Modified Bass in two ways (MB1-MB2). 15 volunteers were selected from the basic science level dental students at Tehran University of Medical Sciences. All the six methods were instructed to each volunteer, using a randomly selected sequence. Each participant should, therefore, pass six consequent courses, by the same sequences as follow: 1st Visit: After polishing the teeth, the participant was asked to abstain any kind of tooth cleaning. - 2nd visit (48-72 hours later): O Leary Plaque Index (PI) was recorded and then the randomised selected method was instructed. Then, the participant brushed his/her teeth and PI was registered again. The time needed for instruction and using the method was registered too. The participant was then asked to use this newly learned tooth brushing method twice a day for 7±1 days. - 3rd visit (7±1 days later): PI was recorded. The teeth were polished. The participant was asked to abstain any kind of tooth cleaning for 48-72 hours, as washout period of the previous method and preparation period for the next method (2nd visit of the next course). The final stage was comparing the results, in which the effectiveness of each method was evaluated based on the mean percentage of plaque reduction after the first usage, and also the mean percentage of the plaque remained after using the method for 7±1 days. The time needed for instruction, learning and using the methods were also compared. The statistical analysis was done using the SPSS software and variance analysis (randomised block design).

Results: Considering all the teeth and their total surfaces after 1st usage, the methods of C and MB1 were statistically more effective than the other methods. But the difference between these two methods was not significant. Also after 7±1 days, the differences between all methods were not statistically significant. On the posterior teeth, after 1st usage and also after 7±1 days, the method of C was significantly more effective than the others, with the exception of MB1 for which their differences were not significant. Also MB1 was significantly more effective than the methods of Bass and Roll, but only after the 1st usage. On the anterior teeth, after 1st usage, method of C was statistically more effective than the methods of Bass, MB2 and MS. Also MB1 was significantly more effective than MB2 and MS. But after 7±1 days, the differences proved insignificant. Considering the specific surfaces of the teeth on the proximal surfaces, after the 1st usage of the methods, the methods of C and MB1was significantly more effective than the others, except each other, which their difference was not significant. But after 7±1days the method of C was statistically more effective only than the methods of Bass, MB2 and MS, and the method of MB1was significantly more effective than MB2. On the buccal and lingual surfaces the differences were not statistically significant at both stage of evaluation. When statistically considering the time needed for instruction, learning and usage, the method of C needed more time than Bass and Roll methods. In the same way, MB1 needed more time than Roll method. The most time needed was related to MB2 which was more than the time needed for the methods of Bass, Roll and MS (P<0.001 and α=0.05).

Conclusion: The tooth brushing methods of Charters’ and a kind of Modified Bass method (in which Bass and Roll methods are used separately and consequently) were evaluated as the most effective methods.

Abstract

Statement of Problem: Hemodialytic patients may be exposed to periodontal diseases during treatment. No information of any kind is available regarding the periodontal treatment needs of such patients.

Purpose: The aim of the present study was to determine the periodontal treatment needs of hemodialytic patients, living in Tehran, to be presented to the public health system.

Materials and Methods: In this descriptive study using CPITN index 75 patients according to multi-stage sampling method and in proportion to the number of patients in Tehran were selected. Following necessary training, qualified subjects were chosen and a special questionnaire was completed for each using personal protection, CPITN index, by WHO prob, was examined and numbered according to (0-4) codes. Results: The findings showed that 10.6% of the patients had normal periodontal status (Code-0), 12% of the patients had bleeding upon probing (code-1), 21.4% had calculus (Code-2), 21% of the patients had shallow pockets (Code-3) and 24% had deep pockets. It was also shown that 89.4% of the patients needed oral hygiene instruction, 77.4% needed scaling and 24% required advanced periodontal treatments whereas only 10.6% did not need any treatment.

Conclusion: No statistically significant relationship was found between periodontal status and factors such as age, sex, the history of hemodialysis treatment and level of patients’ education.

Abstract

Statement of Problem: There are many risk factor for periodontal diseases and from this point the disorders of lipid metabolism also may be a risk factor for pcriodononlal diseases.
Purpose: The purpose of this study was to evaluate the relation of periodontal disease and disorder of lipids metabolism in cardio vascular patients.
Materials and Methods: In this sectional study, 45 patients who were hospitalized in shahid rajaee hospital divided and studied in two group, (included test group with 27 patents and control group with 18 patient). In both group the level of serum lipids including triglycerid, total cholesterol and LDL cholesterol were determined and periodontal status were estimated by CPITN index. Then statistical result processed by Mann-Whiney U test and spearman- correlation.
Results: The results showed thai CPITN index in high lipid group was more than low lipid group and spearman correlation between triglycerid and CPITN was 0.2S3 between cholesterol and CPITN was 0.372 and between LDL and CPITN was 0.230.
Conclusion: In this study we found that there is a specific and significant relation between periodontal disease and serum lipids in cardio vascular patients.

Abstract

Statement of Problem: Giant cell granuloma (G.C.G) is a reactive lesion which affects the jaws and oral mucosa of gingiva.This lesion is classified to central and peripheral ones. The histopathologic aspects of central and peripheral G.C.G is the same. The central lesions are aggressive and cause osseous destruction. The peripheral G.C.G produces ulcerative swellings similar to pyogenic granuloma or peripheral ossifying fibroma. Since the peripheral and central G.C.G are common lesions, the awareness of dentists of different aspect of G.C.G is very important.

Purpose: The aim of this study was a statistical evaluation of variables such as age, gender and location of peripheral and central G.C.G in oral and maxillofacial pathology department of Dental Faculty of Tehran University of Medical Sciences.

Materials and Methods: This study was a cross-sectional, case series one. The medical records of oral and maxillofacial pathology department of Dental Faculty of Tehran University of Medical Sciences was assessed in two steps: In the first stage, the medical records of patients with pathologic report of peripheral and central G.C.G were selected and reviewed. In the next step, the informations such as age, gender and location of peripheral and central G.C.G were registered in data forms. The obtained data were analyzed by SPSS statistical software.

Results: The results showed that the age range in peripheral and central G.C.G were 2- 90 and 4-70 years, respectively. The peak incidence of peripheral and central G.C.G was in the third and second decades, respectively. The peripheral G.C.G was more common in men (52.70%) than women (47.30%). This finding in central G.C.G was 37.19% for men and 62.88% for women. 57.06% of peripheral G.C.G cases were in mandible and 42.94% in maxilla. The occurrence of central G.C.G was 67.07% in mandible and 32.93 % in maxilla.

Conclusion: Based on this study, the peripheral lesions were more frequent in men, third decade and mandible and the central lesions in women, second decade and mandible.

Abstract

Statement of Problem: Odontogenic keratocyst is a developmental odontogenic cyst which arises from dental lamina. One of the important features of odontogenic keratocyst is strong tendency to recurrence.

Purpose: The purpose of this study was the statistical evaluation of age and gender of patient as well as area of involvement in odontogenic keratocysts in the oral and maxillofacial pathology department of dental faculty of Tehran University of Medical Sciences from 1971-2002. Materials and Methods: This study was a cross sectional, descriptive one. Medical records were reviewed and variables such as age, gender and site of involvement were recorded. The data were analyzed with SPSS software.

Results: The relative frequency of odontogenic keratocyst was 36%. 66% of cysts were in men and 34% in women. 68% of lesions affected the lower jaw and 32% the upper jaw. Regarding the site of involvement, 48% of lesions involved the molar region of mandible and 42%, the anterior part of maxilla. The occurrence of keratocysts was higher in this sites. Most of the cases were diagnosed in the third decade.

Conclusion: Based on the findings of this study, odontogenic keratocyst was more prevalent in men, mandible and the third decade. The posterior part of mandible and anterior region of maxilla were involved most frequently.

Abstract

Background and Aim: Along with higher usage of dental local anesthesia with risks to people health together with their positive role, the important goal of dentistry, patients’ health, has been stressed repeatedly nowadays. This study was conducted to compare O2 saturation, respiratory rate and heart rate of patients following injection of anesthetic containing vasoconstrictor (lidocaine 2%) and without vasoconstrictor (Mepivacaine).

Materials and Methods: This experimental study was conducted on 32 healthy humans (16 females and 16 males) with 25-50 years age range and no history of drug usage. Cases were classified into two matched groups. O2 saturation, heart and respiratory rate were recorded before extraction of a mandibular posterior tooth. In the first group, 3.6 ml lidocaine 2% with 1:80,000 epinephrine and in the second group Mepivacaine 3% was injected, using inferior alveolar dental nerve block with aspiration. Variables were measured and recorded. Tooth extraction was performed and the mentioned variables were recorded again. Data were analyzed with SPSS software using t and Paired t-test with P<0.05 as the limit of significance.

Results: After injection of lidocaine, the heart rate was increased (12.25±1.75 bpm), and the time to reach the maximum rate was 64.75±11.26 seconds. After extraction of tooth, heart rate in both groups was increased not considering the type of injection.

Conclusion: The study showed that the injection of lidocaine (containing epinephrine) in patients without contraindication has no risk. Also, possible increase in heart rate is not risky and is not associated with O2 saturation decrease and respiratory interruption.

Abstract

Oral health care workers may be exposed to a variety of infectious agents as they carry out their professional responsibilities. Dentists must recognize diseases that are most commonly transmitted in dental offices. Oral health care providers should assess their needs for immunization based on the characteristics of their clinical work setting and own risk-taking profile. Relevant articles in the medical literature were derived from searching Medline database (1966 to present) with key terms immunization, oral health care and dentist. On searching the database, a total of 70 studies were included in this review. Use of vaccines safeguards the health of workers in various type of oral health care delivery settings.

Abstract

Background and Aim: Iranian temporary cement, Tem Band, has been manufactured by Golchai company, and introduced to market as a suitable material but its properties have not been tested yet. The aim of this study was to investigate the setting time, film thickness, compressive strength, disintegration and arsenic content of Tem Band and Temp bond temporary cements and compare them to ISO 3107. Materials and Methods: In this experimental study, 12 samples from each cement were obtained and setting time, film thickness, compressive strength, solubility and disintegration were tested according to ISO 3107. In addition, arsenic content was tested based on ASTM 6052. Data were analyzed with t-test and p<0.05 was considered as the limit of significance.

Results: The average setting time of Tem Band was 5 minutes and 47 seconds (4.17 minutes for Temp Bond), the average film thickness was 6.67 micrometer for Tem Band and 4.08 micrometer for Temp Bond. There was no difference between two cements regarding setting time and film thickness and for both was at the standard range. Compressive strength of Tem Band was 5.67 MPa and 8.5 MPa for Temp Bond. The difference was statistically significant but for both cements was less than standard (35 MPa). There was no significant difference regarding the solubility and disintegration tests. In both samples, the solubility and disintegration values were in standard ranges. Both cements did not contain arsenic but lead level was at the maximum standard range in Tem Band.

Conclusion: Considering these results, both cements are applicable to different clinical situations based on their physical properties. Studies on the biocompatibility of the Iranian cement is recommended.

Abstract

Background and Aim: Intrabony periodontal defects are important problems in periodontology and up to now several ways have been suggested for their treatment .Treatment with enamel matrix derivatives (EMD) has been shown to enhance periodontal regeneration. There is limited information available from clinical trials regarding the performance of EMD in the treatment of periodontal intrabony defects. This study was designed to compare the clinical and radiographic effects of EMD treatment to that of open flap debridment (OFD) for two and three walled intrabony defects.

Materials and Methods: 18 patients were included in this clinical trial which have 24 two and/ or three intrabony defects. Defects were randomly divided into two groups (test and control). Defects in test group were treated with flap surgery plus EMD and in control group with open flap debridment. At baseline and at 3 and 6 months follow up, clinical and radiographic measurements were performed. Data were analyzed using Greenhouse-Geisser test with p<0.05 as the limit of significance.

Results: At 3 and 6 months, mean probing pocket depth reduction was greater in the test group (EMD) (4.33 mm and 4.70 mm) compared to the OFD group (2.54 mm and 3.09 mm). Mean values for clinical attachment gain in the EMD group after 3 and 6 months were 4.29 mm and 4.98 mm, and in OFD group were 2.83 and 2.82 mm respectively. Radiographic bone gain measured by radiovisiography technique was greater in the EMD group compared to the OFD group (4.66 mm in EMD and 1.11 mm in OFD group after 3 months and 5.78 mm in EMD and 1.39 mm in OFD group after 6 months).

Conclusion: Based on the results of this study, treatment with flap surgery and EMD compared to open flap debridment, produced more favorable clinical improvements in two and three walled intrabony defects.

Abstract

Background and Aim: Microscopic differentiation between well differentiated squamous cell carcinoma and verrucous carcinoma is very difficult. As these lesions have different clinical course and treatment a reliable method to help distinguish between them is necessary. The basement membrane in oral verrucous carcinoma is generally intact whereas in oral squamous cell carcinoma is mostly discontinuous. This structure can be studied by different methods. The aim of this study was to differentiate these lesions with special staining of basement membrane.

Materials and Methods: In this tests evaluation study, a total of 26 cases consisting of 15 oral squamous cell carcinomas and 11 oral verrucous carcinomas were retrieved from Cancer Institute of Imam Khomeini hospital archive of pathology and their paraffin blocks were cut and stained with Periodic Acid Schiff method. Slides were studied with light microscope to evaluate the continuity of basement membrane. Sensitivity and specificity of this method were calculated.

Results: The sensitivity of this method for differentiating oral squamous cell carcinoma from oral verrucous carcinoma was 100% whereas the specificity was 90.9%. Positive predictive value was 93.75% and negative predictive value 100%.

Conclusion: Based on the results of this study, the sensitivity of this test for diagnosis of oral squamous cell carcinoma was high (100%). Further studies are needed to evaluate the use this method for distinguishing oral verrucous carcinoma from oral squamous cell carcinoma.

Abstract

Background and Aim: Qigong is a type of Chinese psychosomatic exercise that integrates meditation, slow physical movements, and breathing. Numerous physical and mental benefits have been classically ascribed to qigong. On the other hand, unstimulated saliva is thought to play an important role in oral immunity, enamel stability and moisturizing the oral mucous membrane. Impaired salivary secretion leads to dental caries, mucosal deterioration and mouth dryness. The aim of the present study was to analyze the effect of a qigong program on various parameters of saliva such as quantity of unstimulated saliva, pH and SIgA.

Materials and Methods: Twenty three subjects participated in this clinical trial study. The experimental subjects underwent a qigong training program, conducted by a qualified instructor. The program consisted of half an hour daily practice for 6 months (spring and winter). Saliva was collected in two periods: once during the spring before the experiment commencement and the second, in the following spring. During each period saliva collection was done on tuesday of each week. pH and quantity of salvia measurements were taken simultaneously. SIgA measurements were also taken based on the values obtained in the last phase of the experiment at the end of each spring. The results were analyzed using paired sample T test, one way repeated measure and Bon Ferroni multiple comparison. P<0.05 was the level of significance. Results: Based on our findings, the change in the amount of unstimulated salvia as well as SIgA was statistically significant (P<0.001) however, there was no significant difference in pH values before and after experiment.

Conclusion: These findings demonstrate that after 6 months of practicing qigong, significant changes in amount of unstimulated saliva and SIgA occurred in participants. The authors suggest that qigong may be a beneficial adjunctive treatment that enhances amount of unstimulated saliva and SIgA.

Abstract

Background and Aim: Low birth weight is a significant public health issue. Recent studies have suggested periodontal diseases as risk factor for preterm labor. The aim of this study was to assess the relation between gingival health and preterm labor in a sample of Iranian female population.
Materials and Methods: In this cross sectional study, 201 pregnant women without any systemic diseases or other risk factors like psychotic conditions were selected. Ninety-nine of them had term labor (infant≥37 weeks) and 102 had preterm labor (infant<37 weeks). Bleeding index, pocket depth and debris index were measured. Data were analyzed by Chi-square and Mann-Whitney tests with p<0.05 as the level of significance.
Results: Bleeding index, probing depth and debris index showed statistically significant differences between preterm and term labor mothers (P<0.001).
Conclusion: According to the findings of this study, there is a significant relationship between gingival health and duration of pregnancy. Consequently, periodontal diseases could be risk factor for preterm labor. Oral hygiene is strongly recommended to be included in pregnancy health programs.

Abstract

Background and Aim: The efficiency of disinfecting materials and procedures in removal of contamination from dental surfaces and equipments is essential. In authors' previous study, daily use of 2:100 dilution of 5% sodium hypochlorite in water and disposable covers were recommended since HBV contamination was found on semi-critical parts of the operative dentistry department. The aim of this study was to evaluate the HBV contamination following application of the recommended procedures.

Materials and Methods: The study was conducted in two parts. In the first cross-sectional part, samples were collected from 17 sites of dental surfaces. In the second interventional part samples were collected from 10 sites of 9 dental and 3 sites of 2 light cure units, before and after disinfection with 5% sodium hypochlorite solution diluted to 2:100. Sterile cotton swabs moistened with sterile BSAS (Bovine Serum Albumin in Sodium Chloride) solution were used for sampling. Samples were tested by PCR technique in Pasteur Institute, Iran.

Results: None of the samples collected in the first part of the study showed contamination. In the second part of the study, from 96 samples taken from various parts of dental and light cure units, before and after disinfection, there was only one HBV contaminated site before disinfection which showed no contamination after disinfection.

Conclusion: Based on the results of this study, disinfecting procedure with 5% sodium hypochlorite solution diluted to 2:100 along with using disposable covers is effective in preventing HBV contamination.

Abstract

Background and Aim: Improving esthetics is a major goal in periodontal treatments and specially reconstruction of dental papilla is of great importance. Different techniques have been developed, however the results are not completely predictable. The aim of the present study was to compare the possibility of papilla reconstruction using connective tissue graft with and without Emdogain.

Materials and Methods: In this clinical trial, 40 class II lost interdental papilla (Nordland- Tarnow classification) in 18 patients (4 male and 14 female) were studied. Samples were randomly devided into two groups. Case group was treated by connective tissue graft and emdogain (Straumann-USA) and controls were treated using connective tissue graft only. Periodontal parameters including papilla height (PH), periodontal pocket depth (PPD) and clinical attachment level (CAL) were measured before surgery, and 3 and 6 months after treatment. At the tenth days and 1 month after treatment only the papilla height was measured. Repeated measures analysis was used for data analysis with p<0.05 as the level of significance.

Results: In the control group the mean papilla height changed from 3.38 0.89 to 2.85 0.93 on the 3rd month and 2.90 0.95 on the 6th month. The mean PPD changed from 2.8  0.68 to 2.55 0.70 on the 3rd month and 3.06 1.25 on the 6th month. The mean CAL changed from 3.88 1.29 to 3.79 1.25 on the 3rd month and 4.11 1.44 on the 6th month. In test group the mean papilla height changed from 2.98 0.91 to 2.45 1.55 on the 3rd month and 2.55 1.48 on the 6th month. The mean PPD changed from 2.75 1.49 to 2.63 1.38 on the 3rd month and 2.88 1.39 on the 6th month. The mean CAL changed from 3.75 1.24 to 4.01 1.45 on the 3rd month and 4.28 1.63 on the 6th month. These differences were not significant between the studied groups (P>0.05).

Conclusion: Based on the results of this study, use of connective tissue with or without emdogain is not effective in interdental papilla reconstruction.

Abstract

Background and Aim: Because of polymerization shrinkage and high viscosity of posterior composites, there are some difficulties in using them in posterior restorations. Several methods have been represented to reduce the effect of shrinkage. The aim of this study was to investigate the effect of curing flowable composites under condensable ones in adaptation and microleakage reduction of posterior composite restorations.

Materials and Methods: In this experimental in vitro study, forty class II MO cavities were prepared on extracted intact molar and premolar human teeth. Gingival margins were placed 1 mm apical to CEJ. The teeth were divided into two groups. In group 1, flowable composite (Filek Flow, 3M, ESPE, USA) with 0.5-1 mm thickness was applied and cured following application of bonding agent (Single Bond, 3M, ESPE, USA). The rest of the cavity was filled by condensable composite (p60, 3M, ESPE, USA). In group 2 the flowable composite was not cured, and the condensable composite was applied in two increments. After light curing of composites, all the specimens were thermocycled and then immersed in 0.3% basic fuschin. Specimens were sectioned and evaluated for degree of dye penetration under a stereomicroscope. Data were analyzed by Mann-Whitney test with p<0.05 as the level of significance.

Results:. There was no significant difference between the two studied groups regarding microleakage.

Conclusion: Based on the results of this study, neither cured nor uncured flowable composite under condensable composite can omit microleakage in posterior composite restorations.

Abstract

Background and Aim: Several materials have been introduced for retrograde fillings, pulp capping and sealing root perforations, but their biological effect on vital tissues and cells is not clear. The purpose of this study was to evaluate the reaction of human periodontal ligament fibroblasts to four root canal filling materials: Pro Root MTA, Root MTA, Portland cement and amalgam.

Materials and Methods: In this experimental study, impacted or semi impacted third molar teeth were extracted in aseptic conditions and tissues around the roots were used to obtain fibroblast cell line. After proliferation, cells were cultured in chamber slides and extracts of materials were added to wells. Fibronectin, type I collagen and TGF-  expression were measured by immunocytochemistry method. Data were analyzed by SPSS 11.0 using one way ANOVA and Tukey test. P<0.05 was considered as the limit of significance.

Results: Collagen I expression was higher in Pro Root MTA group after 24 hours (p<0.05) and in Portland cement group and positive controls after 48  hours. Portland cement group showed the highest expression of collagen after 1 week. There was no significant difference in fibronectin expression after 24 hours. After 1 week the highest expression of fibronectin was seen in Portland cement, Root MTA and Pro Root MTA groups. TGF-  expression was higher in amalgam, Root MTA and Pro Root MTA specimens after 24 hours and was the highest in Pro Root MTA group after 48 hours.

Conclusion: Based on the results of this study, Portland cement and Root MTA are comparable with Pro Root MTA and better than amalgam regarding their effects on human periodontal ligament fibroblasts.

Abstract

Background and Aim: The role of applying rotary instruments in reducing preparation time and proper root canal preparation have been approved in permanent teeth. The aim of this study was to compare the rotary instruments (Hero642) and K-files in root canal preparation of primary molars. Time and canal transportation were compared.

Materials and Methods: In this experimental, invitro study, 30 human extracted primary molars with 8 mm length were selected and divided into two equal groups. After preparing access cavity, group 1was prepared by K-files and group 2 by Hero642 systems. Before canal preparation, with # 15 K-file and after canal preparation, with master apical file, canal curvature was evaluated by Weine technique using Radiovisiography. Apical transportation was calculated. Time taken for preparation was recorded by chronometer. The results were analyzed with T-test and Mann-Whitney. P<0.05 was considered as the level of significance.

Results: The average canal transportation angle was 3.42 for Hero642 group and 7.33 for K-file group (p<0.04). The mean instrumentation time was 4.28 for Hero642 group and 8.76 minutes for K-file group (p<0.001). There was a significant difference between two groups.

Conclusion: Based on the findings of this study, it is suggested to apply Hero642 rotary instrument for primary molar canal preparation due to less preparation time and less canal transposition.

Abstract

Background and Aim: Polymerization shrinkage of resin cements during setting leads to stress on teeth receiving indirect esthetic restorations. Some methods like blocking out undercuts during cavity preparation with glass Ionomer can reduce the mass of resin cement and therefore the amount of stress. The aim of this study was to evaluate the fracture resistance of molar teeth, receiving class I ceramic inlays with two different methods of blocking out undercut.

Materials and Methods: In this experimental study, thirty intact molar teeth were randomly divided into three groups of 10 each. One group was maintained intact as the control group. Similar class I cavities with identical undercuts were prepared on the other groups. In group 2 the undercut was filled with light cured glass Ionomer (vitremer 3M) before making impression, and in group 3 they were blocked out in the lab before preparation of the inlay. All the teeth in test groups were restored with indirect ceramic inlays.(vitadur - Alpha/ single bond / Rely X ARC). After thermocycling load causing fracture was assessed by universal testing machine. All the specimens were examined under a steriomicroscope (X40) to determine type of fracture. The results were analyzed by one way ANOVA and Tukey HSD tests. P< 0.05 was considered as the level of significance.

Results: The restored groups showed significantly less fracture resistance than the control group. The difference between two test groups was not significant. The fracture in group2 had less destruction.

Conclusion: According to the findings of this study, blocking out the undercut with two different methods (using glass ionomer/ or blocking it out on the die) made no diferrence in fracture resistance.

Abstract

Background and Aim: Dental pulp calcification may have the same pathogenesis as vascular calcifications. Pulp stones are calcified mass, which are commonly observed in usual dental radiographs. The purpose of this study was to evaluate the relationship between pulp stones and ischemic CVD.

Materials and Methods: This was a cross-sectional descriptive-analytic study on 20-25-year-old patients who had at least 8 intact teeth and were referred to Afshar hospital for cardiovascular examinations. Relationship between vascular stenosis and dental pulp stone was explored.

Results: Sixty one patients who had undergone angiography were studied. They included 32 male (52.5%) and 29 female (47.5%). 38 patients had at least one stenotic vessel (from one to three vessels) and the others (37.7%) showed normal angiographic findings. 73.8% (31 cases) of the patients with dental pulp stones suffered from coronary narrowing where as only 36.8% (7 cases) of the patients without dental pulp stones showed coronary vessels narrowing. Pulp stones were mostly seen in first and second Molar teeth.

Conclusion: According to the results, we can state that oral and maxillofacial radiology is helpful in screening of cardiovascular diseases.
