Abstract

Statement of Problem: Porcelain restorations are susceptible to fracture and a common method for repairing is the use of silane and composite on etched porcelain. Although HF is very effective in porcelain etching but has detrimental effects on tissues.

Purpose: In this study, the effect of APF and PHA was compared with HF in porcelain etching. Also the role of silane, unfilled resin and dentin bonding in bond strength of composite- porcelain was evaluated.

Methods and Materials: In this experimental in-vitro study, one-hundred twenty porcelain square blocks (552 mm) were prepared and bonding surfaces of each sandblasted. Samples were divided into three groups. The first group (n=40) were etched with buffered HF 9.5% (Ultradent) for 1 min., the second group (n=40) were etched with Iranian APF 1.23% (Kimia) for 10 minutes and the third group (n=40) were etched with Iranian PHA 37% (Kimia) for 1 min. Ultradent silane was applied on the surfaces of half of cases in each group. On the surfaces of half of silane-treated samples unfilled resin was applied and dentin bonding was used on the surfaces of the remaining. Samples without silane were treated in a similar manner. Composite cylinder with 4mm diameter and 2 mm height was bonded to porcelain. Specimens were stored in 37°C distilled water for 24 hours and subjected to 500 cycles. Shear bond strength was measured with an Instron machine and type of fracture was evaluated using a stereomicroscope. Results were analyzed using 3 way ANOVA, Kaplan- Maier and Tukey HSD tests.

Results: Findings showed that PHA and APF roughened the porcelain surface without creating retentive micro undercuts but HF etches porcelain and creates retentive microundercuts. Ultradent silane had no significant effect on bond strength of porcelain- composite. Unfilled resin with Ultradent silane compared with dentin bonding with the same silane is more effective in bond strength of composite- porcelain.

Conclusion: Based on present study, application of Ultradent silane on sandblasted and etched porcelain with PHA or APF cannot be used as an alternative to this silane on sandblasted and etched porcelain with HF.

Abstract

Background and Aim: Human and animal studies linking fluoride with diminished intelligence have been published. Although adverse effects of high intake of fluoride on intelligence and mental acuity continue to be reported, they are still controversial. The aim of this research was to investigate the relationship between fluoride in drinking water and children's intelligence.

Materials and Methods: In this cross sectional study, 41 children were selected from the high fluoride area with 2.5mg/l (ppm) fluoride in the drinking water and 85 children were selected from low fluoride area with 0.4mg/l (ppm) fluoride in the drinking water. The intelligence quotient (IQ) of each child was measured by the Raven's test. The history of illnesses affecting the nervous system, head trauma, birth weight (2.5kg or  2.5kg), residental history, age and sex of children were investigated by questionnaires completed by the children's parents. Data were analyzed by Chi-Square test with p<0.05 as the limit of significance.

Results: In the high fluoride area the mean IQ of children (87.911) was significantly lower than in the low fluoride area (98.912.9) (P=0.025).

Conclusion: Based on the findings of this study, exposure of children to high levels of fluoride may carry the risk of impaired development of intelligence.

Abstract

Background and Aims: High level of caries was reported in pre-school children at the national level. Due to importance of focusing on high-risk children, this study was carried out to evaluate an oral health promotion program designed for Ameneh orphan center.

Materials and Methods: Two centers were chosen and assigned to intervention (Ameneh orphan center) and comparison (Shobeir orphan center). Dental examination has been carried out using dental mirror and blunt probe under artificial light and dmfs index was recorded. Health promotion program including topical fluoride application after dental treatment as well as power point education session for caregivers was delivered in intervention group. Children in the other center were only received routine dental treatment. Data were analyzed using Mann-Whitney and regression tests.

Results: 79 children in the intervention group and 51 children in comparison group were evaluated in the study. The mean age of children in the intervention group was 4.7 (SD=1.6) and in the comparison group was 4.6 (SD=1.2). The dmfs index in the intervention and comparison groups was 6.86±10.23 and 8.75±12.42 (NS), respectively. The mean dmfs after intervention in the intervention group was 7.93±11.21 and in the control group was 9.88±12.51, which showed no significant difference (P>0.05). Comparison in subgroups showed that the mean dmfs in the group received fluoride varnish in addition to complete dental treatment was significantly less than the comparison group (P<0.05). Controlling the effect of baseline caries and age on the outcome variable with regression analysis showed that the effect of baseline caries should be considered when comparing two groups.

Conclusion: Oral health promotion programs including topical fluoride application can be used in high-risk groups of children.

Abstract

  U ntreated caries are common findings in low-income communities. Treatment of dental caries in young children is challenging and requires a high skill and high cost of dental anesthesia for control ling patients and thus, chemical methods for arresting dental caries have been proposed. Silver Diamine Fluoride (SDF) is a material that is used to control and to arrest dental caries , especially in primary teeth . It is noninvasive, safe , easy to use , and cost-effective. The SDF can be used as a therapeutic agent in oral health projects in low-income communities . In this article we review ed the silver diamine fluoride and its application in dentistry.

Abstract

  Background and Aims: Since recurrent caries are one of the major causes of failure in resin restorations, the production of antibacterial resin composites was always under investigation. The aim of this study was to evaluate the efficacy of fissure sealants containing nanosilver particles against the Streptococcus mutans.

  Materials and Methods: In this experimental study, the antibacterial properties of two sealants (with fluoride (Clinpro 3M) and without fluoride (Concise 3M) was investigated with 0, 0.01, 0.02, 0.03, 0.04, 0.05% w/w after adding nano-silver using direct contact test. Sealants formed on the walls of 500ml micro tube and after curing, they left in contact with bacterial suspension. In periods of 3, 24, 48h, a 10 µl volume of liquid medium was placed in blood agar culture and after 24 h incubation at 37ºC, the number of S.mutans colony was counted by colony counter. Data were analyzed using ANOVA and
T-test.

  Results: Results reported sealants with fluoride comparing to non fluoride ones had significant effect on inhibition of S.mutans growth (P<0.001). The direct contact test demonstrated that by increasing the amount of nano particles, the bacterial growth was significantly diminished (P<0.001).

  Conclusion: While sealants with fluoride demonstrated antibacterial effect, sealants with incorporation of higher weight percentage of nanosilver particles, had stronger and more significant antibacterial effect in direct contact test.

Abstract

Background and Aims: Despite significant decline in dental caries in recent decades, the disease remains a public health problem especially in developing countries. In recent years combined application of laser and fluoride has been introduced as a new caries-preventive measure. The purpose of the present study was to review the literature on the preventive effect of combined application of laser and fluoride on the dental caries. Materials and Methods: PubMed, Google Scholar and EMBASE databases were searched for articles published from 1990 to November 30, 2013. Various combinations of the following keywords were used: Dental caries, Laser, Fluoride, Fluoride therapy, Prevention and Enamel. The inclusion criteria included all levels of available evidence. Articles published only in english language were evaluated, and unpublished data were not sought. Results: Several studies evaluating the effect of laser alone or combined with topical fluoride to prevent dental enamel demineralization were found. Studies assessing the combined effect were more frequently done on permanent teeth and just a few studies were done on primary teeth. In total, most of the studies confirmed that combined application of laser and topical fluoride has a remarkable effect in prevention of enamel demineralization compared to application of each of them alone. Conclusion: Laser irradiation combined with topical fluoride application seems to provide the best prevention effect on the dental caries initiation and progression in enamel of permanent teeth. However, more experimental and clinical studies are necessary to be done on the primary teeth in this area.

Abstract

Background and Aims: Dental caries is an infectious disease transmitted by multiple factors in the mouth and one of the most common chronic diseases in the world that requires a lot of treatment expenses. The aim of this study was to evaluate the effect of fluoride 0.2% and Fuchs mouthwash (a combination of xylitol and fluoride 920 ppm) against two bacterial growths (Streptococcus mutans and Lactobacillus acidophilus).

Materials and Methods: In this in vitro study, first the well diffusion method and then MIC method (minimum inhibitory concentration) were used. Data were analyzed with Excell (2016).

Results: The results of the MIC method showed inhibitory effect of both mouthwashes in pure concentration (128 μg/ml). However, fluoride was effective in concentration higher than 32 μg/ml for Streptococcus mutans and higher than 2 μg/ml for lactobacillus acidophilus. Fuchs was effective for both bacteria at concentration higher than 64 μg/ml and in lower concentration it was ineffective. The results of the well diffusion test showed the inhibition of fluoride mouthwash in pure concentration on the Streptococcus mutans was (17 mm) and on the lactobacillus acidophilus 24 mm was obtained. In Fuchs mouthwash was zero and it represented effectiveness of fluoride on bacteria and ineffectiveness of Fuchs.

Conclusion: The fluoride mouthrinse in different concentrations, because of having a good inhibitory effect in both methods on Streptococcus mutans and Lactobacillus acidophilus was more effective compared with that of Fuchs mouthrinse.

Abstract

Background and Aims: Increased tooth sensitivity is a common dental problem. One of the mechanisms of dentin hypersensitivity treatment is blocking dentinal tubules. The aim of this study was to compare the average closing dentinal tubules affected by carbonated Nano-hydroxyapatite and fluoride toothpaste using scanning electron microscope (SEM).
Materials and Methods: The enamel of gingival buccal crown surfaces of 42 sound maxillary premolars teeth were removed until the dentin surfaces were exposed. The dentine specimens of 3×3 mm were prepared. The specimens were washed by distilled water after etching and randomly divided into 6 groups. 3 groups were interfered with the carbonated nano-hydroxyapatite toothpaste and the other three groups interfered with Colgate toothpaste in 3, 7 and 14 days. The samples were examined by SEM to check the status of dentinal tubules. Then, they were evaluated by two researchers according to the Hulsmann index. Data were analyzed by using SPSS23 software and Kruskal-Wallis statistical test. The significance level of 0.05 was considered.
Results: The amount of dentinal tubules closure by the two toothpastes was significantly different. This difference was higher in the hydroxyapatite toothpaste than fluoride which were (P=0.024, 0.004 and 0.000), respectively. There were no significant differences among the groups in 3,7 and 14 days (P=0.230, 0.069, 0.537).
Conclusion: The toothpaste containing carbonated nano structural hydroxyapatite was more successful in closing dentinal tubules than Colgate toothpaste in short period of time.

Abstract

Background and Aims: The systemic and topical preventive effect of fluoride on tooth decay has been reported. The aim of the present study was to determine the fluoride level in imported and national fluoridated dentifrices in the Iranian market.
Materials and Methods: Sampling was randomly performed from the main market centers and pharmacies in Tehran. The total fluoride (TF) and total soluble fluoride (TSF) concentrations were determined by ion specific electrode potentiometer method. The samples were coded and one trained researcher performed the analysis. Descriptive statistics as percentage and analytical statistics reported as independent t-test were reported.
Results: In adults’ toothpastes, the total fluoride ranged from 191 to 1601.9 ppm with the mean of 911.2 (SD=331.4) ppm. The total soluble fluoride ranged from 322.8 to 1518.8 ppm with the mean of 1021 (SD=279.8) ppm. In children’s toothpaste total fluoride ranged from 236.3 to 1163.7 ppm and the total fluoride of 594 (SD=375). Total soluble fluoride in children’s toothpaste ranged 655.6-1102.6 with the mean of 879.1 (SD=316.07). Analysis of the total fluoride (TF) and total soluble fluoride (TSF) showed that there was no significant difference between the local and imported toothpastes (P>0.05).
Conclusion: The fluoride level of Iranian toothpaste was similar to that of the imported toothpastes, however there was variation in the level of fluoride in products.

Abstract

Background and Aims: Using fissure sealant is one of the most effective methods of preventing pit and fissure decay. Fluoride has been added to various materials as a known anti-caries agent. Fluorinated graphene (FG) has gained attention due to its unique properties. The specific structural characteristics of graphene fillers, besides having antibacterial properties by increasing the rate of fluoride release and charge and neutralizing the acidic pH of the environment, is a suitable option in many treatments. This study investigated the effect of addition of fluorinated graphene nanoparticles on the fluoride release in Fissurit®, a commercial fissure sealant.
Materials and Methods: In this in vitro study, fluorinated graphene oxide (FGO) with bright white color was prepared. After synthesis, 0, 1, 2 and 4 by weight percent were added to the fissure sealant (Fissurit®). Then, the release of fluoride in this material was measured and compared with the commercial fissure sealant containing fluoride (Fissurit®).
Results: According to the obtained results, the amount of fluoride released from groups with different percentages of FGO had a direct relationship with the percentage of FGO addition. Also, at different times, there was a significant difference between the groups with FGO and the control group without FGO (P<0.001).
Conclusion: Addition of FGO to the fluorinated fissure sealant caused the release of fluoride and the possibility of recharging it. The power of releasing fluoride and its recharging in fissure sealant with FGO was higher than the fluorinated fissure sealant, but its fluoride was discharged at a faster rate.
