Abstract

The aim of this retrospective statistical study was to determine the prevalcence and frequency of age and sex distributions of pemphigus disease. Pemphigus disease classified as autoimmune bullous dermatoses which is a chronic mucocutaneous disease.This study was performed in Razi Hospital and department of oral pathology of dental school, Tehran University of Medical Sciences. The most frequently effected area was buccal moucosa of oral cavity. The most rate of recurrence was found in oral cavity which being more common in middle age females (25-44 years).

Abstract

Peripheral giant cell granuloma (P.G.C.G) is an exophytic lesion with an approximate size of 0.5-1.5 mm. It usually occurs on gingival and alveolar ridge of mandible particularly in molar and premolar region. The relation of serum calcium (Ca), Phosphor (P), and alkaline phosphatase (Alk) levels to P.G.C.G is yet controversial. In this descriptive study, 33 patients with P.G.C.G were chosen and serum Ca, P, and Alk levels compared with the normal range. In all patients the level of Ca was in the normal range. Phosphor (P) was in the normal range in all patients over 17 years old and 80% under 17 years. The level of ALK in 75.8 percent of the patients over 17 years and 90% under 17 years was in the normal range. In conclusion, no relationship was found between serum changes and P.G.C.G. It means P.G.C.G can be compeletly independent lesions from serum changes.

Abstract

Statement of Problem: Parafunctional habits are the most important factor in the creation of non-carious cervical lesions and correcting of these habits may affect cervical lesions.

Purpose: The goal of this study was to survey the effects of one-year occlusal adjustment on clinical success of non- carious cervical restorations.

Materials and Methods: In this clinical study, sixty tooth-colored cervical restorations on canines and premolars in patients with parafunctional habits (bruxism and clenching) were studied. The teeth, according to the type of restorative material, were randomly divided into two groups: One group with a composite resin (Tetric Flow Group A: n=30) and the other group with a compomer (Compoglass Flow Group B: n=30). After restoration, each group was divided into two subgroups: (A1, B1) with occlusal adjustment (case group), (A2, B2) without occlusal adjustment (control group). The evaluation was perfumed single blind at 6 and 12 months after restorative operations according to Us Public Health Service (USPHS) classification. Assessment criteria included: marginal integrity, marginal discoloration, recurrent caries, postoperative sensitivity and loss of filling. Statistical analysis was completed using Fisher’s exact test.

Results: In 12 month period between two groups of compoglass Flow restorations, group B1 showed superior results than group B2, however, no significant difference was found between both groups of tetric flow restorations (A1 and A2). There was also no difference in clinical conditions between A2 and B2 groups.

Conclusion: Considering the results of this study, it can be concluded that occlusal adjustment puts a considerable effect on clinical success of compoglass non- carious cervical restorations. However, it should be mentioned that such and effect was not found among Tetric flow restoration.

Abstract

Statement of Problem: Giant cell granuloma (G.C.G) is a reactive lesion which affects the jaws and oral mucosa of gingiva.This lesion is classified to central and peripheral ones. The histopathologic aspects of central and peripheral G.C.G is the same. The central lesions are aggressive and cause osseous destruction. The peripheral G.C.G produces ulcerative swellings similar to pyogenic granuloma or peripheral ossifying fibroma. Since the peripheral and central G.C.G are common lesions, the awareness of dentists of different aspect of G.C.G is very important.

Purpose: The aim of this study was a statistical evaluation of variables such as age, gender and location of peripheral and central G.C.G in oral and maxillofacial pathology department of Dental Faculty of Tehran University of Medical Sciences.

Materials and Methods: This study was a cross-sectional, case series one. The medical records of oral and maxillofacial pathology department of Dental Faculty of Tehran University of Medical Sciences was assessed in two steps: In the first stage, the medical records of patients with pathologic report of peripheral and central G.C.G were selected and reviewed. In the next step, the informations such as age, gender and location of peripheral and central G.C.G were registered in data forms. The obtained data were analyzed by SPSS statistical software.

Results: The results showed that the age range in peripheral and central G.C.G were 2- 90 and 4-70 years, respectively. The peak incidence of peripheral and central G.C.G was in the third and second decades, respectively. The peripheral G.C.G was more common in men (52.70%) than women (47.30%). This finding in central G.C.G was 37.19% for men and 62.88% for women. 57.06% of peripheral G.C.G cases were in mandible and 42.94% in maxilla. The occurrence of central G.C.G was 67.07% in mandible and 32.93 % in maxilla.

Conclusion: Based on this study, the peripheral lesions were more frequent in men, third decade and mandible and the central lesions in women, second decade and mandible.

Abstract

Background and Aim: Oral lichen planus is a chronic mucocutaneous disease with various clinical expressions. The histopathologic features are not characteristic and may be seen in other diseases. The aim of this study was to determine the clinical and pathologic features of oral lichen planus in oral pathology department of dental school, Tehran university of medical sciences.

Materials and Methods: In this case series study, archive of oral pathology department from 1968 to 2002 was reviewed and cases diagnosed as lichen planus and related lesions selected. The diagnosis of oral lichen planus was confirmed by evaluation of microscopic slides. Clinical informations such as age, sex, site and duration of lesions and differential diagnosis as well as microscopic findings were recorded. Data were analyzed with SPSS software using t student, ANOVA, Chi-square and Post Hoc Tukey tests with p<0.05 as the limit of significance.

Results: From a total of 402 cases recorded as lichen planus and related lesions, 251 cases were confirmed for final analysis. The mean age of patients was 42 years (5 to 83 years). 50.4% of cases were men and 49.6% women. The most prevalent clinical type was the ulcerative form and the mean duration of disease was 18.4 months. Buccal mucosa was most frequently involved followed by the tongue and gingiva. A white patch, Wickham’s striae and mucosal erythema were the most prevalent clinical appearance and parakeratosis , orthokeratosis and eosinophilic band the most frequent microscopic features.

Conclusion: Based on the results of this study, the correlation of clinical and pathological findings in the proper diagnosis of oral lichen planus is emphasized.

Abstract

  Background and Aims: Dental caries is one of the most common infectious diseases which could be prevented in various ways. The aim of the present study was to evaluate the effect of a toothpaste containing different concentrations of nano-hydroxyapatite on the remineralization of incipient caries.

  Materials and Methods: 9 sound first premolar teeth of maxilla and mandible were sectioned to four pieces. 6 specimens were stored in distilled water as control group. The remaining 30 specimens were demineralized for 72 hours and then their microhardness was measured. 0, 0.5, 1, 2 and 5% wt nano-hydroxyapatite were added to the solution of distilled water and toothpaste. Specimens were divided into 5 groups (N=6) and after demineralization, each group were randomly immersed in the above-mentioned solutions for 15 minutes. Next, specimens were kept in artificial saliva for 1 hour followed by immersion in the related remineralizing solution for another 15 minutes. This procedure was repeated for 5 days. Finally, the microhardness of the teeth was measured. Data were analyzed using repeated measures analysis of variance (ANOVA).

  Results: The microhardness of demineralized teeth was increased following exposure to different concentrations of nano-hydroxyapatite, but this increase was not statistically significant (P=0.62).

  Conclusion: Nano-hydroxy apatite can enhance remineralization and increase the tooth microhardness although this increase was not significant.

Abstract

Background and Aims: Pre-cancerous lesions of the oral mucosa as important malformation need acceptable knowledge of dentists. The purpose of this study was to determine the knowledge of Isfahan city general dentists about pre-cancerous lesions of oral cavity.
Materials and Methods: This descriptive cross-sectional study based on validated questioner form was performed in 115 general dentists in Isfahan City in 2019, associated with sex and year of graduation from general dentistry course. The mean score was in the range from 0 to14. Data were recorded in Microsoft Excel and then analyzed by SPSS. In order to compare quantitative variables, independent T–Test, One-Way ANOVA and Mann-Whitney was used. P≤0.05 was considered as significant.
Results: The mean age in the participants of: lower than 5 years, between 5-10 years, and more than 10 years of graduation were 28.6±4.9, 30.6±4.0 and 47.9±10 years old, respectively. In total population, the mean score of knowledge was 5.97±2.79. There was not any significant difference between the mean score of knowledge in women when compared to men (5.96±2.99 versus 5.98±2.6; P=0.58). The mean knowledge score in the participants of: lower than 5 years, between 5-10 years, and more than 10 years, of graduation were as follow: 7.8±2.2 versus 5.8±2.9 versus 4.4±2.1, correspondingly (P≤0.05).
Conclusion: For the early diagnosis of pre-cancerous lesions of the oral cavity, retraining program based on oral cavity pathology courses is recommended for the dentists in Isfahan city.

Abstract

Background and aims: Substance abuse is recognized as a serious public health issue. The aim of this study was to comprehensively assess the correlation between the substance use and oral health status  as well as its impact on the quality of life of addicts in a multi-center approach.
Materials and Methods: In this cross- sectional and analytical study in year 2023, individuals with a history of addiction who visited addiction treatment centers in Isfahan province were recruited conveniently and examined through questionnaires (about demographic information, methods of substance use, oral health behaviors, the use of dental services, and perceived oral health) and clinical examinations. The oral health-related quality of life (OHQOL) was assessed using the OHIP-14 questionnaire. Clinical examinations assessed DMFT, gingival index, mucosal lesions, temporomandibular joint disorder, and treatment needs of the individuals. Data were analyzed using ANOVA, t-tests, and linear regression models (α=0.05).
Results: 232 individuals with age of 45 ± 9.9, mostly male (94.8%) and married (68.5%) were recruited. About 50% of individuals never brushed their teeth. A total of 123 individuals (53%) used substances with moderate harm intensity (opium, hashish and cannabis). Pigmentation was the most common oral lesion. The mean DMFT was 19.27 ± 3 and the perceived oral health mean was 4.53 ±2.98 (out of 10). A total of 187 individuals (80.6%) required prosthetic treatment. The mean OHQOL was 15.93 ± 11.6. Functional limitation had the lowest (1.53), and psychological discomfort had the highest score (3.31) among the seven domains of OHQOL. The linear regression model (P<0.001 and R2=24%) found that the use of high-risk psychoactive substances, oral health behaviors, and age contributed to changes in DMFT. The OHIP linear regression model (P<0.001 and R2=36%) revealed that the duration of addiction, perceived oral health, age, self-employment, government jobs, and retirement contributed to changes in OHIP.
Conclusion: Compared to the general population, addicts have poorer oral health behaviors and more oral health problems, which significantly reduce their oral health-related quality of life.
