Abstract

Background and Aim: The efficiency of disinfecting materials and procedures in removal of contamination from dental surfaces and equipments is essential. In authors' previous study, daily use of 2:100 dilution of 5% sodium hypochlorite in water and disposable covers were recommended since HBV contamination was found on semi-critical parts of the operative dentistry department. The aim of this study was to evaluate the HBV contamination following application of the recommended procedures.

Materials and Methods: The study was conducted in two parts. In the first cross-sectional part, samples were collected from 17 sites of dental surfaces. In the second interventional part samples were collected from 10 sites of 9 dental and 3 sites of 2 light cure units, before and after disinfection with 5% sodium hypochlorite solution diluted to 2:100. Sterile cotton swabs moistened with sterile BSAS (Bovine Serum Albumin in Sodium Chloride) solution were used for sampling. Samples were tested by PCR technique in Pasteur Institute, Iran.

Results: None of the samples collected in the first part of the study showed contamination. In the second part of the study, from 96 samples taken from various parts of dental and light cure units, before and after disinfection, there was only one HBV contaminated site before disinfection which showed no contamination after disinfection.

Conclusion: Based on the results of this study, disinfecting procedure with 5% sodium hypochlorite solution diluted to 2:100 along with using disposable covers is effective in preventing HBV contamination.

Abstract

Background and Aim: Sodium hypochlorite can remove the organic phase of the demineralized dentin and it produces direct resin bonding with hydroxyapatite crystals. Therefore, the hydrolytic degradation of collagen fibrils which might affect the bonding durability is removed. The aim of this study was to evaluate the effect of collagen fibrils removal by 10% NaOCl on dentin shear bond strength of two total etch and self etch adhesive systems.
Materials and Methods: Sixty extracted human premolar teeth were used in this study. Buccal surface of teeth were grounded until dentin was exposed. Then teeth were divided into four groups. According to dentin surface treatment, experimental groups were as follows: Group I: Single Bond (3M) according to manufacture instruction, Group II: 10% NaOCl+Single bond (3M), Group III: Clearfil SE Bond (Kuraray) according to manufacture instruction, and Group IV: Clearfil SE Bond primer. After that, the specimens were immersed in 50% acetone solution for removing extra monomer. Then the specimens were rinsed and dried. 10% NaOCl was applied and finally adhesive was used. Then composite was bonded to the treated surfaces using a 4 2 mm cylindrical plastic mold. Specimens were thermocycled for 500 cycles (5-55ºC). A shear load was employed by a universal testing machine with a cross head speed of 1mm/min. The data were analyzed for statistical significance with One-way ANOVA, Two-way ANOVA and Tukey HSD post-hoc tests.
Results: The mean shear bond strengths of groups were as follows: Single Bond=16.8±4.2, Clearfil SE Bond=23.7±4.07, Single Bond+NaOCl=10.5±4.34, Clearfil SE Bond+NaOCl=23.3±3.65 MPa. Statistical analysis revealed that using 10% NaOCl significantly decreased the shear bond strength in Single Bond group (P=0.00), but caused no significant difference in the shear bond strength in Clearfil SE Bond group (P=0.99).
Conclusion: Based on the results of this study, NaOCl treatment did not improve the bond strength in self etch adhesive and caused a significant decrease in the bond strength of total etch adhesive system.

Abstract

Background and Aims: Formocresol is one of the most common pulpotomy medicaments for primary teeth. Because of its systemic and local side effects, it may be essential to use another material instead of formocresol. The aim of this study was to evaluate the histopothalogy of pulp after pulpotomy teeth with sodium hypochlorite and formocresol.

Materials and Methods: In this randomized clinical trial, 22 canine teeth that must be extracted because of orthodontic treatment were selected. The teeth were randomly divided into 2 groups (n=11) and pulpotomized with formocresol or sodium hypochlorite. These teeth were extracted after 2 months and pulpal response was evaluated according to the degree of inflammation and extent of pulpal involvement. Dentinal bridge formation was also evaluated. The data were analyzed by Mann-Whitney test.

Results: In formocresol group, mild inflammation was seen in 4 and moderate inflammation in 3 and severe inflammation in 4 cases. In sodium hypochlorite group mild inflammation was seen in 6 cases and moderate inflammation in 4 cases and severe inflammation in 1 case. Mann-Whitney test revealed that this difference was not statistically significant (P>0.05). In formocresol group, necrosis was seen in 5 cases, but abscess and internal resorption were not seen in any cases. In sodium hypochlorite group, internal resorption was seen in 3 cases but necrosis and abscess were not seen in any cases.  Mann-Whitney test showed that the difference between two groups was significant in terms of necrosis (P=0.02). In sodium hypochlorite group, dentinal bridge was formed in 3 cases however, no dentinal bridge formation was seen in formocresol group. Mann-Whitney test showed that this difference was not significant statistically (P>0.05).

Conclusion: Based on the results of this study, sodium hypochlorite can be used as a pulpotomy agent in primary teeth.

Abstract

  Background and Aims : The aim of this study was to evaluate the effect of chlorhexidin 2%, sodium hypochlorite 2.5% and hydrogen peroxide 3% as three effective and regular disinfectants on the microleakage of 7th generation bonding agents in vitro.

  Materials and Methods: 45 extracted molar teeth without carries were collected and disinfected. On buccal and lingual aspects of these teeth conventional class V cavity preparation were done (90 cavities), then randomly divided to 4 groups, three of them had 10 teeth (20 cavities) and one of them had 5 teeth (10 cavities) as control group. Cavities in each experimental group prepared with one of the disinfectants and then 7th generation bonding (Optibond all in one, kerr) was used as noted by manufacturer, then cavities filled with composite and polished. Bonding agent was used without our previous disinfectants manipulation in control group. Specimens were thermocycled with 1000 thermal cycles between 5 and 55 0 C each for 30 seconds and then immersed in the methylene blue 5%, then sectioned mesiodistally and investigated for microleakage under stereomicroscope (Olympus, Japan). Data were analyzed using Kruskal-Wallis and Wilcoxon mean rank tests.

  Results: Despite the lower mean rank values for the untreated group at both occlusal and gingival aspects, the Kruskal-Wallis procedure (α=0.05) showed that the treatment factor did not significantly affect the mean rank values neither in occlusal (P=0.12) nor in gingival (P=0.39) part of cavities.

  Conclusion: According to the results of this study, antimicrobial agents such as chlorhexidine 2%, sodium hypochloride 2.5% and hydrogen peroxide 3% can be used perior to 7th generation dentin bonding agent
(Optibond all in one, kerr) without much concern.

Abstract

  Background and Aims: Considering the complications of root canal system, debridement would not be effective only by mechanical instrumentation. Therefore, root canal irrigants with the antimicrobial effect that can eliminate more microorganisms during treatment, becomes important. The aim of this study was to compare the antimicrobial effects of 2% thyme essence, 2.5% sodium hypochlorite (NaClO), and normal saline as root canal irrigants for primary molar teeth.

  Materials and Methods: This study was performed on 34 primary molars. Subjects were randomly divided into three groups (N=10). The root canals of the teeth were prepared up to the file #35 and all of the specimens were sterilized, and were then contaminated with entrococccus faecalis suspension (Enterococcus Faecalis ATTC 29212). After 48 hours of incubation, the specimens of each group were irrigated with corresponding irrigants. Then, the microbial specimens were collected with sterile paper points. After incubation, the numbers of Colony Forming Units (CFU) were counted using colony counter. Data were analyzed using Smirnov-Kolmogorov, Kruskal-Wallis and Mann-Whitney tests.

  Results: Sodium hypochlorite showed the highest antimicrobial effect and showed significant differences compared with normal saline (P<0.001). However, the differences were not significant with thyme essence 2% (P=0.3).

  Conclusion: Thyme essence could be considered as a natural irrigator with a comparable antiseptic effect to sodium hypochlorite.

Abstract

Background and Aims: Bacteria and their products have a major role in pulp and periapical diseases. The purpose of this study was to compare the antimicrobial activity of sodium hypochlorite, iodide potassium iodide and chlorhexidine on the enterococcus faecalis after root canal filling.

Materials and Methods: In this experimental study, 95 single canal human teeth were collected. Then after washing and cleaning and filing, the sterilization done using an autoclave. The teeth were smeared with enterococcus faecalis except five which selected as controls and then divided them into 3 groups which A randomly represented the irrigation by sodium hypochlorite, iodide potassium iodide (IKI) and chlorhexidine solution were done. The teeth were filled with gutta-percha and then incubated in a incubator for 90 days at a temperature of 37° C. The specimens were analyzed for Colony Count. Data were analyzed using Kruskal-Wallis test and Chi-Square.

Results: According to the results of this study, the correlation between the 3 groups of cultured teeth were not significant (P=0.812).The specimens which were washed by IKI had the most positive amount of cultures (23.3%) and the specimens which washed by chlorhexidine had the lowest (16.7%).

Conclusion: The results of this study showed that after the use of different wash solutions, no statistically significant difference exist in their antimicrobial activity after root canal therapy.

Abstract

Background and Aims: The presence of microbes inside the canal is the main reason for post-treatment infection. Therefore, the maintenance of the disinfection obtained during the treatment is imperative. The aim of this study was to compare the antimicrobial effect of Tricresol formalin and 5.25% Sodium hypochlorite on the Enterococcus faecalis.

Materials and Methods: In this study 66 human single-rooted extracted teeth were used. After access cavity and root canal preparation, the teeth were sterilized in autoclave, and then contaminated with Enterococcus faecalis suspension and incubated at 37°C for 7 days. Then, they were randomly divided into three groups of 20: In group 1, Tricresol formalin was used as intracanal medicament. In group 2, 5.25% Sodium hypochlorite and in group 3 (control group), normal saline were used as irrigants. 7 days after incubation at 37°C, the microbiological sampling was performed. For this purpose, dentinal shaves were collected from the root canals and cultured in Tryptic Soy Broth, and the number of Colony-Forming Unit (CFU) was counted. Data were analyzed using Kruskal-Wallis and Mann-Whitney tests.

Results: A significant reduction of CFU was observed in the Tricresol formalin and Sodium hypochlorite groups compared to the control group (P<0.001). No significant difference was reported between Tricresol formalin and Sodium hypochlorite groups (P=0.69).

Conclusion: The present study showed that the antimicrobial effect of Tricresol formalin was comparable with 5.25% Sodium hypochlorite on the Enterococcus faecalis.

Abstract

Background and Aims: Root canal treatment failures are often attributed to incomplete removal of bacteria, particularly Enterococcus faecalis. While, chlorhexidine and sodium hypochlorite serve as conventional treatments. herbal medicine has been today considered as an alternative to synthetic medicine due to its safety. Therefore, the aim of the present study was to compare the antibactrial effect of Scrophularia Striata extract with chlorhexidine and sodium hypochlorite on Enterococcus Faecalis in the tooth root canal.
Materials and Methods: In this study, sixty extracted teeth were examined. Following the infection of dental canals with Enterococcus faecalis, materials were tested in 3 groups: 1) chlorhexidine 2% (18 teeth), 2) sodium hypochlorite 2.5% (18 teeth), and 3) Scrophularia striata (at the concentration obtained from MIC, 20%; 18 teeth). Additionally, the positive control and negative control were included (3 teeth in each group). Samples were then cultured on bile esculin agar. After 72 hours, black colonies were eaxamined. Chi-squared test was used to analyze the results.
Results: Enterococcus faecalis bacteria grew in 6, 8, and 17 out of the 18 teeth within the sodium hypochlorite, chlorhexidine, and Scrophularia striata extract groups, respectively. Consequently, the bactericidal effects for sodium hypochlorite, chlorhexidine, and Scrophularia striata extract were 66.6%, 55.5%, and 5.55%, respectively.
Conclusion: The antibacterial effect of the Scrophularia striata extract was significantly lower than chlorhexidine and sodium hypochlorite on Enterococcus faecalis in dental canals viewed as a suitable replacement to the already in use synthetic therapeutic irrigants.
