Abstract

Background: Nowadays, researchers pay special attention to patients' satisfaction with emergency care services, as the first line of hospital health care services. However, few researches have been done about its related factors in our country. The aim of this study was assessment of patients' satisfaction with emergency care services in six military hospitals in our country and its related factors.

 Material and Methods: In this cross-sectional study, Satisfaction level of 360 patients of emergency care services in six military hospitals of Iran in 2007 was assessed. After discharge from emergency ward, a checklist containing basic information and a 12-item questionnaire about their satisfaction level with emergency care services was completed for each patient. A 5-level Likert scale was used for the response of each item. 20 to 100 scores were allocated to each response (completely dissatisfied to completely satisfied), respectively.

 Results: 3559/4220 responses (82.4%) were completely satisfied or satisfied. In respect to priority "Observation of moral points", "giving information" and "behavior of reception personnel" had the highest and on the other hand, "variety of medical specialists", "emergency ward facilities" and "speed in calling doctor" had the lowest satisfaction scores. The total satisfaction score which was reported by patients older than 35 yr(p=0.022), insurance coverage(p=0.002) and with history of previous referring to that emergency ward(p=0.017) were significantly higher than others. Sex, marital status and educational level had no statistical correlation with the total satisfaction score(p>0.05).

 Conclusion: The findings of this study revealed favorable satisfaction level of patients with emergency care services in military hospitals. However, it seems that using a variety of expert physicians and more facilities and also improvement in the process of calling doctor in emergency ward are the aspects which need the most amount of concern of health care managers in emergency centers.

Abstract

Background: Patients' safety is a critical component of health care quality. As health care organizations continually strive to improve, there is important growing recognize of establishing a culture of patients' safety. To establish a safety culture in a healthcare organization, the first step is measuring the current culture. The aim of the study was to measure physicians, nurses and Para clinical personnel perceptions in patient safety culture in Tehran's selected hospitals, and to compare findings with U.S. hospitals.
Materials & Methods: Physicians, nurses, and Para clinical personnel who worked in training hospitals affiliated with Tehran university of medical sciences were asked to complete a self-administrated patients' safety culture survey (n = 145). Data collection was carried by using the Persian version of HSOPS, developed by Agency for Healthcare Research and Quality (AHRQ). Cronbach's alpha and chi-square tests were employed in statistical analyses.
Results: Among the dimensions of patients' safety culture with the highest percentage of positive responses the teamwork within units (67%) was higher, whereas that with the lowest percentage of positive responses was non-punitive response to error (51%). Except to Handoffs and transitions dimension the entire dimension scores were lower than the benchmark scores. The study revealed that more than half of the participants were not reported the errors.
Conclusion: Improving patients' safety culture should be a priority among hospital administrators. Meanwhile, Healthcare staff should be encouraged to report errors without fear of punishment action.

Abstract

Background: Increasing the receivables collection period make problems in the payment of personal salaries, medicine fees & facilities under surveillance centers. It finally has negative effects on delivered services. At present economic circumstances, paying attention to liquidity and its present value is very important to doing economic activities and to decreasing receivables collection period. The aim of the study was calculating the receivables collection and turnover period in selected hospital of the Qom University of Medical Sciences.
Materials & Methods:
This is a descriptive research that conducted in the selected hospital of Qom university of medical sciences during the fourth national development program (2005-2009). Data was Collected from information based on the financial documents of the kamkar hospital and registered into information sheets and analyzed by excel software.
Results:
Results show that during the fourth national development program , Receivables turnover has gradually decreased and unlike thereceivables collection period increased. Both of these relations showed an undesirable circumstances and graving with the contracting insurance companies in the payment of their obligation with the hospital.
Conclusion:
Findings indicated that the receivables collection period of insurance companies takes Long and taken a long way from its normal .It needs a serious monitoring & intervention in the large scale decision making, except by using the power of law during the fourth development national program which it hasn 't a good impact in repayment approach of the policy makers in this issue that expected all of the general managers in the giving of services.

Abstract

Background: In order to achieve the goals of health system, trying to evaluate and improve of health system performance via policy and decision makers is necessary.
Materials & Methods: This is a descriptive and practical study .Information gathered through sheets through World Health Organization statistical reports. data analyzed by using DEAP software(version of 2) and Data Envelopment Analysis (DEA).
Results: Based on study technical efficiency in various scenarios were different. The results showed that health systems are high sensitive to human resources, and health sector's usually share the national expenditure difference scenarios during the period of study which show the managing efficiency and scale improvement s in Iranian health system.
Conclusion: This study showed the countries with the highest and lowest performance and efficient states (reference) for the inefficient countries among a few countries which increased their health resources to improve performance. However, with high efficient resource allocations in the most countries to achieve into these results are very significant to economic savings.

Abstract

Background: This study aimed to survey the relationship between job stress, self- efficiency and coping ability among staff of therapeutic- educational hospitals of Tabriz University of Medical sciences.
Materials & Methods: This cross-sectional study was conducted in 2009 among therapeuticeducational hospitals of based on a two stage random sampling. Initially seven hospitals were selected using a simple random method and then 288 persons from the therapeutic and administrative staff were chosen as the sample to be studied using a classified random method. Data were collected using 4 questionnaires titled personal information, self-efficiency, Tores Theorell's job stress scale and Schwarzer's self efficacy and Coping Inventory for Stressful Situation CISS-21 ( Dutch's coping). Data were analyzed Using SPSS software version 11.5 and also Chi-Squared test, T -test, One-Way Anova , Regression analysis and Pearson correlation coefficient.
Results: All the demographic variables including age, gender, marital status, working hours and work experience had a significant relationship with job stress. Pearson correlation test indicated a negative correlation between job stress and self- efficiency, working hours, age and work experience (P<0.001). Additionally there was a positive correlation between self- efficiency and working hours and age and coping ability with work experience ( p< 0.05).
Conclusion: Considering the importance of staff`s mental health issue it is suggested that background factors such as high work load ,dignity level and social value and social supportive programs are taken into close consideration staff`s needs related to these issues are fully satisfied so that better health services are provided.

Abstract

Background:Hospital is the most important health care services provider center.Undoubtedly, properand effective management performance can be very important. This study aimed at assessing professional and non-professional managers’ performance among selected hospitals in Tehran. Materials and Methods: In thisdescriptive study the population was staffs of ten selected hospitals in Tehran. With the intention of performing the study, a 41-items questionnaire was used which confirmed its validity by experts and its reliability by Cronbach alpha(=90).After data entry in SPSS software,frequency distribution tables, diagrams and central tendency and dispersion were used. With the intention of measure probable correlation Contingency Correlation Coefficient - C was utilized. Results:The comparison betweenprofessional managers’ responses with non-professional ones revealed an incremental trendfrom “seldom” to “always” in all domains of management functions (including planning, organizing, coordinating and control)and Contingency Correlation Coefficient - C indicated statisticallysignificant relation between domains and types of managers (professional and non-professional)P = 0.003, P = 0.006, P = 0.001, P0.0001 respectively. Conclusion:Managers’ professional education is inevitable for hospital managing duties.

Abstract

Background: Strategic plan in hospitals will not be run spontaneously and managers encountered with some barriers in order to have a successful strategic planning implementation. This study aimed at recognizing the barriers of strategic plan implementation from hospital managers' perspective.

Materials and Methods: This cross-sectional study was performed in all Kerman hospitals in 2014. Fifty three individuals including chairmen, internal administrators, quality improvement managers, matrons and strategic plan executers participated by census sampling method. Data was collected using a questionnaire including demographic information and human, processes, structures and institutions factors affecting the strategies implementation. Data analysis was performed by SPSS software version21.

Results:  Study results revealed that the barriers to strategic plan implementation were structural factors (27.71±6.68), process agents (19.54±10.06), human factors (19.09±5.37) and institutional factors (15.73±4.52) respectively. There was no significant association between management background and barriers of strategic plan implementation in hospitals (P < 0.05).

Conclusions: In order to achieve desired results, chairmen and hospitals managers should be entirely conscious about barriers /failure factors of strategic decisions concerning foresight and appropriate managing or approaches change during strategic management process implementation.

Abstract

Background: Today, due to the diversity of organizational structure and culture, the diversity is one of the basic principle in crisis management.Crisis management consists of six components including flexibility, inclusion, trust, risk perception, adaptability and equity.This study aimed at investigating the managers' perception about components of crisis management in hospitals affiliated to Tehran and Iran University of Medical Sciences.

Materials and Methods: This study was a cross-sectional one. A questionnaire was used to collect data using five likert scale. Validity and reliability of questionnaire assessed by content validity and Cronbach's alpha coefficient. Sixty six questionnaire were collected. Data analysis was performed by SPSS software using ANOVA and independent t-test.

Results: The highest and the lowest level of managers' perception were inclusion and equity(3.19 ± 0.51) and adaptability(2.73 ± 0.75), respectively. There was a statistical significant difference between crisis management components with gender and job type. The highest level of importance was regarded to inclusion and mutual trust between personnel and managers, and  the lowest level was related to flexibility in hospital crisis management .

Conclusion: Employees in diverse positions have different definitions of adaptability in crisis. It is necessary to perform further research and educate hospital manager’s duties.

Abstract

Background: Physical and verbal violence against hospital personnel is very critical problems that could influence the quality of clinical and nursing cares. Today, problem prevention and identification of related factors with domestic violence against women have been considered as a health priority in many countries. Therefore, due to the importance of this issue, this study aimed to determine related factors with domestic violence against women in educational hospitals affiliated to Isfahan University of Medical Sciences.
 
Materials and Methods: This cross-sectional study was a descriptive-analytical one which conducted on 836 female employees using census method. Data collection performed using a researcher-made questionnaire. This questionnaire consisted of 34 items in four dimensions (physical, verbal and mental, sexual) and violence experienced by the husband. Data were analyzed using SPSS version 16 with descriptive statistics and analytic statistics such as Kolmogorov–Smirnov, ANOVA, independent t test, and Pearson correlation coefficient and multivariate regression.
 
Results: Study results revealed that verbal and mental (55.33%), physical (36.16%), and sexual (6.66%) violence were the most common type of violence against women, respectively. There was a significant statistically association between age gap, forced marriage, husband’s addiction, income and precedent violence Experience and violence against women.
 
Conclusion: This study presented prevalence and related factors of demotic violence among women. Therefore, it is imperative that health authorities, especially hospital managers, pay attention to this phenomenon assessing further investigation of factors related to violence against women in order to take the necessary steps to mitigate and reduce related factors using life skills workshop, training and counseling for couples.

Abstract

Background: In Iran, the accreditation system is mainly focused on hospitals and has not yet succeeded in providing standards for independent medical centers such as independent clinics. The present study reports the development of an accreditation framework for independent clinics affiliated with the country's armed forces.
Materials & Methods: This is a mixed (qualitative-quantitative) study. Experts' agreement (30 people) on the results of a systematic review (201 items) was obtained using a questionnaire and through holding three expert panels. The necessity of each item was calculated using the method of content validity ratio and the weight of standards and headings based on the average.
Results: The existing 201 items were converted into 75 standards in 13 headings as final standards with the weight of each item.
Conclusion: This framework can be used as a comparison tool between independent medical centers and it is necessary to train evaluators, and prepare legal and organizational requirements in order to implement it.

Abstract

Background and Aim: Clinical governance is one of the important frameworks for continuous quality improvement and safety in health care systems. Identifying the axes of this approach according to local conditions is one of the important priorities of the health system. The aim of this study was to identify the views of stakeholders on the axes of clinical governance in primary health care based on family physicians in Mazandaran province.
Methods: The present qualitative study was conducted using the conceptual framework analysis method in 2018-2019. The study population were key policy makers of Ministry of Health, Health deputy of the University, the county health network, family physicians association, family physicians, and the parliament research center. Participants were selected using purposeful and snowball sampling methods. Data were collected through semi-structured interviews and were analyzed and coded using MAXQDA 11 software.
Findings: According to the interviewees' views, the research findings were classified into 4 main dimensions: dimensions of clinical governance, requirements and structures, decision-makers and dimensions of quality and safety assessment. 17 sub-themes including community participation, clinical audit, clinical effectiveness, personnel management, training, information use, risk management, guidelines and procedures, promotion of health indicators, equipment and facilities, referral system, financing, policy makers, effectiveness, efficiency, human aspects of services and justice were identified and extracted.
Conclusion: According to the research findings, to facilitate the implementation of clinical governance, solutions such as the existence of appropriate infrastructure, commitment of managers, supportive culture, sufficient knowledge, monitoring and evaluation, appropriate culture building, facilities and equipment and sufficient financial resources are suggested.

Abstract

Background: Due to the legal necessity of changing the approach of universities from traditional budgeting into the operational budgeting system, it should be noted that this method of budgeting is associated with challenges. The aim of this study was to evaluate the feasibility and prioritization of operational budgeting dimensions in a military medical university.
Materials & Methods: This descriptive-analytical and cross-sectional study was conducted in 2020 in two stages. In the first stage, 68 managers, staff and experts of the deputy for Planning and Finance of a Military Medical Sciences university were enrolled by census method. In the second stage, 10 experts were purposefully included in the study. Data collection tool was two valid and reliable questionnaires. Data were analyzed using t-test and SPSS software in the first stage and using AHP approach and Expert Choice software in the second stage.
Results: The average overall score and the score for each of the dimensions of ability, authority and acceptance of operational budgeting was 2.3, 2.39, 2.38 and 2.31, respectively which was at an undesirable level (P< 0.05). Also, dimensions of the ability with a weight of 0.547, acceptance with a weight of 0.244 and authority with a weight of 0.209 had the highest weight and importance in the implementation of the operational budgeting system, respectively.
Conclusion: Based on the results, it is necessary to pay attention to the three main dimensions of ability, authority and acceptance in implementing operational budgeting in the study population.

Abstract

Introduction : The Internet of Things (IoT) enables the connectivity of all devices in our daily lives, and it has had a positive impact on healthcare, specifically in disease diagnosis and prevention, especially during times of crisis. The objective of this research is to identify the factors that influence the use of IoT in combating COVID-19 in hospitals.

Materials and Methods: This qualitative study was conducted in 2023, using semi-structured face-to-face and telephone interviews with ten experts in health information technology and the Internet of Things. Data analysis was conducted using directed content analysis with MAXQDA software, version 2018.

Results: The data analysis resulted in 124 codes, which were then divided into 32 subcategories. These subcategories were further classified into six categories according to the model of the International Telecommunication Union. The categories include Network (five subcategories), Application (ten subcategories), Equipment (four subcategories), Support (three subcategories), Management (six subcategories), and Security (four subcategories). The Application category had the highest frequency, while the Support category had the lowest.

Conclusion: Infrastructure and legal aspects are among the most significant factors in the implementation of the Internet of Things in healthcare, particularly in the fight against COVID-19.
