Abstract

Abstract Background: Conducting further research on medical tourism as a lucrative business is required. Very little research has been conducted into medical tourism barriers and a large amount of them just note structural barriers. This study is aimed to identify the most important communicational barriers of medical tourism and provide and examine a model of such barriers considering medical tourism`s intercultural nature. Materials & Methods: This study is applied by purpose and survey by method. The three stages in completion of this study are : 1) Identifying the most important communicational barriers of medical tourism in Iran,2) designing a model of identified barriers by using interpretive structural modeling approach 3) examining the designed model in a sample of 300 individuals. Results: Correlation test indicates a positive and significant relationship between variables in less than 0.01 significant level. Regression analysis confirms that all independent variables are significant predictor of the dependent variables .Therefore all hypotheses are confirmed. Conclusion: This survey identified main medical tourism communicational barriers in Iran as: Lack of understanding, Perceived linguistic difference, Lack of trust, Perceived cultural difference, Prejudice, Stereotyping. Overcoming to Lack of understanding as the major obstacle has a dramatic effect on operating medical tourism. Keywords: Communicational Barriers, Medical Tourism

Abstract

Background:An organization`s  effectiveness and efficiency is strongly dependent on its staff`s   commitment.  Therefore, identification of organizational commitment and its related variables seems necessary. This study is aimed to examine the relationship between emotional intelligence and organizational commitment.
Materials & Methods: This is a  descriptive correlation survey. Our study statistical population included all administrative staff of in southern Khorasan province` hospitals. 110 samples were chosen by a stratified random sampling. The data were collected by standard questionnaires of emotional intelligence and organizational commitment and analyzed by using descriptive and inferential statistics. 
Results: The results showed that organizational commitment is in the moderate level among administrative staff. There is a statistically significant relationship between emotional intelligence and organizational commitment. There is a statistically significant relationship between emotional intelligence and commitment subcomponents(normative and emotional commitment). By performing  multiple regression analysis ,the outcome showed among the all  emotional intelligence subcomponent ,solely “self-awareness” can predict organizational  , emotional  and normative commitment .
Conclusion: Improving individual's emotional intelligence should be taken into consideration due to the  strong bond between  emotional intelligence and organizational commitment.

Abstract

Background: Industry growing and work load of financial turnovers are one of the medical tourism’s reason. Because of additional capacity building in health domain especially in medical levels, medical tourism can be solved some problems similar to extra capacityby means of an essential strategy. The aim of this study was to Segment external clients based on their benefits expectations about health services in four hospitals in Iran.
Materials andMethods:In current study, library reviewing and health care experts’ viewpoints were used in order to determine the characteristics of healthcare services.After determining the variables, factor analysis was performed to brief variables.Then, cluster analysis was performed on extracted factors using K MEANS algorithm and characteristics of each cluster was determined.
Results: Four factors were extracted by factor analysis .Then, Cluster analysis was done on the factors andPatients were divided into four sections whilethe first section respect with health care services efficacy, the second and fourthparts tend to get care with respect and the third parthave a tendency to have individual ability
of hospitals.
Conclusion: Individuals obtained the most important percent to care providing with respects among four extracted clusters. Thus, hospitals should have the most focus on communication quality regarding their marketing efforts with their clients.

Abstract

Background: This study is aimed at investigating the effect of work-life balance in hospitals of Sanandaj. Work-life balance includes different aspects such as : family, work,leisure and personal growth. Moreover,the quality of life had been assessed by physical, mental, social, and environmental dimensions of health.

Materials and Methods: The statistical population includes personnel of hospitals of Sanandaj including doctors, nurses, office employees, technicians and guards. 150 individuals had been selected among mentioned population by using cluster sampling and Cochran formula as sampels. The data were analyzed using structural equations and AMOS18 and SPSS20 softwares.

Results:The results of this study showed that the main hypothesis of  work-life balance  among personnel of hospitals had an effect of 0.70 on the quality of their working life. Nevertheless , second  hypothesis titled as "The effect of holding the balance in family on quality of life" was rejected because its critical value was in the range of rejecting the hypotheses.

Conclusion: Each three non-working dimension had an effect on the quality of life personnel which might be a result of the current economic downturn in Sanandaj.

Abstract

Background: Health care systems especially hospitals often encounter several risks which has weaken efficiency and hospitals performance entirely. Resilience engineering (RE) enables to remain high risk systems stable faced to sudden changes or minimizes negative effects of changes.

              

Materials and Methods: This study considered a new hybrid framework concept regarding the non-value added waiting time and resilience engineering indices as efficiency indices to evaluate private and public hospitals performance. With the intention of evaluating the hospitals performance, a hybrid framework including simulation and MADM methods utilized. Output-oriented Data Envelopment Analysis (DEA) approach used as MADM method.

Results: The results revealed that RE factors play a significant role in hospitals performance's promotion; also the private hospitals had better performance compared to the public ones.

Conclusions: Study results indicated that teamwork efficiency index compare to other efficiency indices had more effect on hospitals performance. Health system managers should be considered improved performance and efficiency policies and focus more on other RE indices parallel team work index.

Abstract

Background: Although the role of Spirituality has been acknowledged in the social entrepreneurship literature, we know of no research that has empirically investigated relationships between spiritual intelligence and health and care benefactors. Thus, this paper aims to propose and empirically test a theoretical model positing relationships among spiritual intelligence and intentions of hospital-makers benefactors. Materials & Methods: This study is descriptive which is conducted with total number of 164 hospital-makers benefactors, and is tested by path analysis. For measuring spiritual intelligence, three questionnaires distributed between every subject and two of his/her acquaintances. Results: The results suggest that spiritual intelligence and other related variables explained 41 per cent of the variance in the rate of hospital-makers benefactors intentions (R2=0/41) that is a sizable amount in behavioral studies. Results provide strong support for the proposition that social mission and social responsibility fully mediate the positive effect of spiritual intelligence on intentions of hospital-makers benefactor. Conclusion: With existence of opportunities for merely finical investments, hospital-makers benefactors should have high levels of spiritual intelligence to gain motivation for social wealth creation. Policy-makers of health system, to increase charitable activities, should focus on heightening spiritual intelligence in health and care sector.

Abstract

Background: Recently, the medical tourism is developing more and more over the world. Due to the potential of medical tourism in Iran, this study aimed at identifying medical tourism characteristics in terms of tourist destination and therapeutic centers in Iran.

Materials and Methods: According to literature reviews and experts interviews, components of medical tourists attraction were investigated ana a questionnaire provided. After baseline, items which were not valid and reliable omitted and final questionnaire distributed and completed among experts Such as physicions, academic members, managers and medical staff in eight public and private hospitals in Tehran city.

Results:  Regarding the study results based on the respondance view, the more and the less important components of medical tourist's attraction in Iran included hospital validity, costs, quality of care, hospital staff training, position and political environment, regulatory policy, infrastructure and location of hospital and economic situation. The main problem of medical tourism represented as lack of obvious treatment process and support system. In similar studies, low tretmant costs in destination country was the important motivation of medical travel and factors such as political, economic, regulations and standards and medical centers validity have been addressed to make decisions about the origin country.

Conclusion: This study based on current and priorities assessed the developing dimensions of the medical tourism in macro and micro levels from perspective of medical tourism activists. Medical tourism development requires comprehensive and integrated approach which has been achived using coordinate cooperation, planning and implementation of micro and macro strategies among relevant organizations.

Abstract

Background: Today, one of the most important issues in hospital site selection is to consider the equity. This study aimed to provide a multi-objective model for fair Hospital site selection considering cost and use for hospitals construction in the future.
Materials and Methods: This study was an analytical-descriptive one conducted to determine the best locations for hospitals construction with respect to criteria of equity in Rasht city. For this purpose, after literature review, a three-objective model consist of p-median, p-envy and equitable loads objects was proposed; and the Rasht city was analyzed by model using the real data. In order to achieve the results and comparisons, the total weighting technique from the multi-objective decision-making techniques was used and solved with GAMS 24.1.1 software.
Results: This research can assist the decision-makers to consider the equity criteria to Hospital site selection in addition to the costs. Citizenship equity establishment and increasing in hospitals efficiency along with cost considerations can be regarded as the major achievements of this research for decision-makers in the health field.
Conclusion: Lack of proper attention to equity criteria hospital site selection, the results can be very unfair and can lead to irreparable consequences. This case study indicated the desirable function of the proposed model in hospital site selection with respect to the citizenship equity criteria in Rasht city. This study results can be given to managers as the proper basis for decision-making.

Abstract

Background: The Family Physician Program, one of the most important efforts of the Iran health system to establish a referral system, was developed and implemented in several provinces, but it faced challenges due to several reasons that prevented the program's progress. So This study was conducted to analyze the family physician program to identify the causes and challenges of the program failure.
Materials & Methods: This retrospective study of policy analysis is a qualitative study with Purposive sampling. Semi-open interviews and document analysis were used for data collection. Data analysis was performed through thematic analysis in the policy triangle framework using MAXQDA software.
Results: According to the study framework, the challenges of the Family Physician Program in the context are conflict of interest, dependency of plans to oneself, instability in management and plans, insufficient attention to culture, resources, and infrastructure. In terms of content include disproportionate executive approach, Lack of localization, selection of inappropriate tools, insufficient transparency, and ambiguity in goals, tasks, and responsibilities; The challenges of the process were examined based on the policy cycle.
Conclusion: The implementation of the family physician program is influenced by cultural, social, political, managerial, and economic factors. As the Ministry of Health officials has re-introduced this crucial program, reviewing the content and methods of program implementation seems necessary.

Abstract

Background: Social media is one of the low cost tools used by many organizations. The purpose of this qualitative research is to present a public accountability model based on the use of social media in public universities of medical sciences in Tehran.
Materials and Methods: The present study was performed using Clark's position data analysis method. In-depth interviews were conducted with 21 experts in 1400 to extract the main components of this model. The subjects of this study were managers and experts of medical universities in Tehran (Tehran, Shahid Beheshti, Iran) that participants were selected using purposive sampling method of theoretical sampling.
Results: The analysis of the accountability situation in medical universities showed that the "government" is the main and determining factor. Within this factor is the "Ministry of Health". The third factor is the "universities of medical sciences" and the social worlds active in it. The fourth factor is "public relations units", but in the corner of these overlapping factors is the "target community" factor.
Conclusion: The basis of accountability is reporting and information. Universities should give their staff the necessary leeway to report directly. According to the research findings, it is suggested that by forming a specialized department of public relations in relation to public accountability, in addition to identifying capacities, to remove existing barriers.

Abstract

Background: The performance of nurses as the largest human resources element of health care organizations has an important impact on the quality and results of patients' treatment. The main purpose of this research is to investigate the impact of internal marketing orientation on nurses’ performance by considering the mediating role of nurses’ engagement.
Materials & Methods: In this regard, while reviewing the implications of internal marketing orientation, engagement with the job and performance of employees, using path analysis and structural equation modeling, we examined the effect of internal marketing orientation on the performance of nurses by examining the role of mediation in job engagement. The study population included nurses of Sasan and Parsian hospitals in Tehran. Stratified random sampling and a questionnaire were used to collect the data. In order to analyze the data, Kolmogorov-Smirnov tests were used for the normal test and Path Analysis and Structural Equation Modeling for confirmation of hypotheses.
Results: Results showed that internal marketing orientation, through the engagement of nurses, influences their performance. Moreover, it was proved that internal marketing orientation has a direct relationship with engagement, but it does not directly influence nurses’ performance.
Conclusion: The most significant achievement of this research is that the managers and executives of healthcare centers can provide a more suitable environment for nurses caring patients, through improving working conditions. Implementing internal marketing orientation is one of the practices that can enhance nurses’ performance through affecting work attitudes, especially work engagement.

Abstract

Background: Identification of competencies is necessary base for a succession plan and efficient human resource management. Studies show that this important issue has been neglected by using the competency model at the middle-level nurse managers.
Since, the methods based on fuzzy logic provide the possibility of mathematical formulation of ambiguous concepts, such as competence. The aim of this study was to design a competency model for middle-level nurse managers using FDM in the second half of 1399.
Materials & Methods: This study was conducted according to health system management studies. The population included all managers of nursing services (matrons) of hospitals affiliated to Tehran University of Medical Sciences. Census sampling used and research samples included 14 nursing service managers (as experts). After reviewing literatures, an initial conceptual model was designed. In the following, two next steps were conducted as a survey form was used to receive the opinions of experts confirming the model leveling. In the second step, the indicators were screened using the FDM.
Results: All participants were female and 9 people had a master degree. The experts' agreement with the model leveling was 90%, which indicated that it was approved. In FDM, the indicators with crisp values higher than 0/7 were confirmed by experts. The design model consisted of 3 dimensions technical, relational, perceptual, 6 components, 14 indicators and 28 sub-indicators.
Conclusion: This study showed that the criteria for selecting middle-level nurse managers include various dimensions of competence including technical, relational and perceptual. Senior managers can use the results of this research to achieve an appropriate planning for succession plan.

Abstract

Background: Brand citizenship behavior is one of the new concepts in branding with emphasis on the role of employees. This study aims to provide a model for development of brand citizenship behavior in a way that is compatible with the local environment of medical tourism in Iran.
Materials & Methods: The present qualitative research is based on the systematic database method. The required data were collected through semi-structured interviews with 16 experts in medical tourism and academic experts using judgmental, snowball, and accessible sampling methods, following the saturation rule. Data analysis was done using the Strauss and Corbin model and in three stages of open, central and selective coding.
Results: Brand citizenship behavior is created as a result of causal conditions, including factors related to employees, organizations, and tourists, and through human resource support strategies and comprehensive marketing and branding planning with the influence of intervening factors, including organizational structure, management performance, characteristics of the treatment industry, Environmental factors, career history and personal quality of life are implemented in the framework of the social capital of the organization, organizational justice, quality of work life, competitive atmosphere of the work environment and social responsibility of the organization and lead to consequences related to tourists, organizations, employees and extra-organizations.
Conclusion: The model obtained from this research shows the influence of several factors in the formation of brand citizenship behavior of medical staff, which, if paid attention to by managers, will lead to the satisfaction and attraction of tourists and gain a competitive advantage.

Abstract

Background and purpose: The globalization of medical tourism has intensified competition among destinations, making it crucial to identify key success factors. While research emphasizes the role of host communities in tourism development, non-medical aspects of medical tourism remain underexplored. This study examines healthcare staff perspectives to identify the drivers and barriers affecting medical tourism in public hospitals.
Methods: This qualitative study employed structured interviews with 16 healthcare staff at Masih Daneshvari Hospital, selected through purposive sampling. Interviews were conducted in person during the summer of 2024 until data saturation was reached. Data analysis followed an inductive content analysis approach using MAXQDA software for coding, grouping, and categorization of themes into drivers and barriers.
Results: Key drivers of medical tourism development include skilled human resources, strong medical potential, affordable healthcare and associated costs, and the presence of complementary attractions. Conversely, major barriers include inadequate welfare services for medical tourists, managerial and institutional inefficiencies, hospital infrastructure deficiencies, and political and cultural challenges.
Conclusion: Developing a successful medical tourism sector requires a holistic approach. Identifying, prioritizing, and implementing strategic plans to strengthen facilitators and address obstacles are essential steps toward sustainable growth in this field.
