Abstract

Field of study and Goals: The provision of medical services and the distribution of agonist drugs in substance abuse treatment centers has led to a lack of attention to patients' treatment plans and a greater focus on drug distribution. In order to separate health and pharmaceutical services that are provided for opioid addicts in drug addiction treatment centers, a study has been designed with the aim of locating the selected pharmacy for the distribution of agonist drugs in District 21 of Tehran Municipality with a spatial approach.
Materials and Methods: This applied research is a descriptive-analytical method. The spatial and descriptive data were collected from the municipality organization and treatment and food and medicine departments of Iran University of Medical Sciences and Health Services and they were analyzed using ArcGIS software. Distribution of pharmacies and drug addiction treatment centers in the study area, locating the appropriate areas, and locating the selected pharmacy were studied by using the kernel density estimation functions, hierarchical analysis, and location model - allocation in two regional and district modes with maximum coverage of drug addiction treatment centers, respectively and analyzed using SPSS software and t-test. 
Results: This study shows that the distribution of pharmacies and population in different regions of the study area is commensurate. Based on the results of locating using hierarchical analysis method, all pharmacies in the study area are appropriate to be the pharmacy for distributing the agonist drugs, and the results of the location model – allocation shows that the average time, and distance to access to the selected district pharmacy is 17 minutes and 5.9 km, respectively and for the regional pharmacy is 2 minutes and 0.7 km, respectively. 
Conclusion: Separation of medical services and distribution of agonist drugs is essential. The findings of this study indicate that the distribution of agonist drugs through selected regional pharmacies increases access to medicine and the satisfaction of patients.

Abstract

Introduction: In the present era, the implementation of open innovation process is necessary for any organization, and hospitals as the main medical centers are no exception. In fact, hospitals, as health operational units, are always directly exposed to changes in the field of health services. Therefore, the present study has investigated the factors affecting open innovation in public hospitals in Guilan province.
Methods: From the point of view of purpose, this research is an applied study and in terms of data collection, it is in the category of descriptive research, which was conducted cross-sectionally in 1400. The statistical population of the study was 1600 senior managers and staff of public hospitals in Guilan province. For sampling, a non-randomized judgmental sampling method was used to access community members (senior managers and employees with at least a bachelor's degree). 250 questionnaires were used to perform the test. Data collection tools are standard questionnaires. The reliability of the questionnaire was confirmed through Cronbach's alpha, and the validity of the questionnaires was confirmed through the face and content validity, convergent and divergent validity. The structural equation modeling method has been used to test the research hypotheses. The software used in this research is SPSS 26 and Smart PLS3.
Results: The results of the structural equation modeling test showed that among the external factors, cooperation with partners and the user, among the internal organizational factors, organizational structure, exploratory learning, and organizational culture, and finally among the individual internal factors, organizational motivation affects the open innovation of public hospitals in Guilan province. The impact of trust between partners, technology, personality traits, and knowledge on hospital open innovation has been rejected.
Conclusion: In collaboration with other health care services, universities, and users, hospitals should develop appropriate policies to transition from a closed innovation system to open innovation, and support effective measures in this regard.

Abstract

Introduction: Socio-economic inequalities in the use of dental services are a major challenge to health policies in many countries. This cross-sectional study was conducted to investigate socio-economic inequalities in utilization of dental services in households of Lorestan province and comparing it with neighboring provinces.
Methods: Data required for the study were collected from the household expenditure and income survey (HIES). The study population included 5547 households in the western provinces of the country. Logistic regression method was used to determine the most important factors affecting the use of dental services, and the focus index was used to measure inequality between income groups. Data analysis was performed using Stata software version 14.
Results: The highest and lowest percentages of dental services among households were related to Hamedan (7.02%) and Lorestan (3.19%) provinces, respectively. The results of the focus index showed that the benefits and costs of dental services were concentrated among households with higher socio-economic status. In other words, inequality benefits the rich. Being a female head of household and increased size of the household leads to a decrease in the use of these services. The higher education level of the head of the household and the increase in the number of employed and literate household members showed a positive relationship with the utilization of dental services.
Conclusion: Benefiting from dental services was significantly higher among higher socio-economic groups. These inequalities can be avoided by introducing and implementing targeted policies. Low-income households, lower education levels, and the unemployed should be the focus of health policies to address unmet dental care needs.

Abstract

 Background and Aim: Expenditure in the health sector requires appropriate resources management. This study was conducted to determine the amount of moral hazards associated with the service providers in the medication prescriptions paid by an insurance organization.
Methods: Study population included paid pharmaceutical documents in Tehran province in 2019. 2000 prescriptions for 500 insured people were randomly selected from the database of the organization. Two indicators (the number of suspicious prescriptions based on the incompatibility of a drug with the history of the disease or the drug spectrum of an insured and the number of prescriptions with a financial burden higher than the average usual burden of a normal patient) were matched with transcript data and analyzed with SPSS version 24. An experienced pharmacist was consulted to examine the drug spectrum of all prescriptions.
Results: 5.4% of total prescriptions were incompatible drugs and 6.4% had created extra financial burden. The highest incidence of risks by speciality was related to general practitioners (16 % with a surplus financial burden of 87/500/000 Rials). The highest number and amount of risks in Rials per insured group was related to supervisors (63 % with a surplus financial burden of 26/000/000 Rials). The risk in the military centres’ prescriptions were more than private and governmental centres and it was relatively equal in contracted (48%) and non-contracted (52%) centres.
Conclusion: The abuse in the health service provision is a high risk for  health care managers  in countries that have health insurance programs which their intelligent tracking reduces many of the insurers’ costs and prevent the shortage of vital medicines.

Abstract

Background and Objectives: the  purpose of this study was to assess the validity and reliability of the Yas Job Satisfaction Questionnaire in order to develop a useful tool for measuring and evaluating job satisfaction among Yas Hospital Complex staff.
Materials and Methods: Face validity, content validity ratio, and content validity index were used to evaluate the questionnaire's validity and reliability. Cronbach's alpha and the halving technique were used among 150 medical personnel in this cross-sectional study The key elements of the final questionnaire were four dimensions of leadership style, job nature, reward system, performance appraisal, and organizational climate.To report the mean of quantitative variables at different levels, qualitative factors were evaluated and analyzed using analysis of variance (ANOVA).
Results: There were 111 females and 39 males among the 150 participants. The content validity ratio and content validity index were both acceptable.Cronbach's alpha method reported a reliability of 0.92 for the questionnaire, indicating that the internal reliability is acceptable.The lowest level of satisfaction was in the leadership style dimension, with an average of 10.53 4.91, while the highest level of satisfaction was in the job nature dimension, with an average of 21.24 7.05.

Conclusion: The Yas Job Satisfaction Questionnaire has acceptable validity and reliability to assess the level of job satisfaction among medical staff, according to the findings of this study. Furthermore, job satisfaction was moderate in most of the dimentions surveyed.

Abstract

Background and Aim: As difficulties increase, the level of uncertainty and risk in the supply chain increases. Medicine is a strategic product and is directly related to community health. The aim of this study is to evaluate the risk factors of pharmaceutical supply chain with artificial intelligence methods.
Materials and Methods: By reviewing the texts and interviewed 6 adept experts who had a Master’s degree and Ph.D. and had experience between 7 and 15 years in the field of risk and pharmaceutical supply chain, risk factors were identified. Finally, using multilayered perceptron neural networks and support vector machines with polynomial linear kernel functions and radial base in two low-risk and high-risk classes were classified in Python software.
Results: 22 factors were identified and classified using neural networks in 5 categories: assets, network and transportation, government and market, strategy and supplier. Shift in interest and inflation, Changes in exchange rates, Inflexibility in production and disruption of customer service are the most important risks in the pharmaceutical supply chain, respectively. The results of evaluation criteria showed that the multilayer perceptron model had better performance than the support vector machines with linear, polynomial and radial basis functions.
Conclusion: The results showed that artificial neural networks are able to classify pharmaceutical supply chain risk factors with acceptable accuracy. As a result, classification of risk factors with an accuracy of 97/07% indicates the high ability of multilayer perceptron network in risk assessment of pharmaceutical supply chain.

Abstract

Background: The aim of this study was to evaluate the effect of health sector evolution plan (HSEP) on the prevalence of cesarean section and vaginal delivery in public hospitals in Kermanshah province.

Methods: In the present study, cesarean section (C-section) and vaginal delivery data were collected and evaluated in 17 public hospitals from 2009 to 2019. The main variables of this study included the prevalence of C-section and the prevalence of vaginal delivery for 121 months, both were obtained by dividing the number of deliveries by C-section and vaginal delivery by the total number. Interrupted time series analysis was used to examine the effect of HSEP on the prevalence of cesarean and vaginal delivery rates.

Results: The prevalence of C-section before and after the HSEP was 42.5% and 43.2%, respectively. Months before the intervention, the rate of C-section was increasing by 0.13%, which was statistically significant. In the first month after the intervention, the prevalence of     C-section decreased by 3.6% that was statistically significant. The share of C-section in the months after the implementation of the HSEP showed a monthly increase of 0.12% compared to before the intervention, which was statistically significant.

Conclusions: The results of the study showed that the HSEP has led to a significant reduction in C-section in short term, but in the months after the implementation of the plan, the prevalence of C-section has increased again.

Abstract

Background: The use of simulation models can help elucidate the intuitive behavior of complex health care problems. Today, the complexity of problems goes far beyond our capacity to solve them manually. System simulation is considered as a wise option by approaching structural problems and understanding the complex interactions within the problems and their changes. The purpose of this study is to review the most advanced applications of system dynamics in healthcare.
Materials and methods: This study is a systematic selection of articles that explored the applications of system dynamics in health care in 11 databases between 1999 and 2019. The focus of this research was on articles with the keywords including system dynamics in health care, dynamic health care analysis, continuous health care simulation or dynamic health care simulation.
Results: 79 articles related to system dynamics applications in healthcare were identified in databases. Eighty-five percent of the articles focused on finance, politics, government, regulation, public health, and health care planning. Among the articles, the patient's treatment path, obesity and demand for healthcare workforce were examined.
Conclusion: The tendency to use continuous simulation in healthcare has increased. However, the power of hybrid simulation can take advantage of the inherent strength of the system dynamics overview perspective, and in a hybrid model it can utilize a holistic perspective for managing health care systems. Potential areas with the application of systems dynamics in future research include planning and policy-making in public health decision-making, purchasing quality management, care and risk.

Abstract

Purpose: Considering the problems of nursing profession, it is very important to find the best method to improve the resilience of nurses. The purpose the present study was to compare the effectiveness and durability of treatment based on acceptance and commitment and paradoxical time schedule of resilience of emergency department nurses.
Methodology: To perform this quasi-experimental research, which was carried out with a pre-test-post-test design with a control group and a follow-up stage, 45 nurses working in the 5 emergency departments of Isfahan hospitals in the second half of 2019 were selected using convenience sampling method. They were randomly divided into three groups of 15 subjects: treatment based on acceptance and commitment, paradoxical schedule treatment and control. Subjects in the acceptance and commitment treatment experimental group participated in 8 sessions and the paradoxical schedule treatment group in 6 90-minute sessions but for the control group, there was no intervention. Participants completed the resilience scale by Connor and Davidson (2003) in three steps. The research data were analyzed using the variance analysis method with repeated measurements in SPSS-26 software.
Results:The results showed that there is a significant difference between the resilience score in three stages of pre-test, post-test and follow-up measurements. It was also found that there was a significant difference between scores of two experimental groups and control group, but there was no significant difference between the scores of the two experimental groups.
 Conclusion: According to the results, it is recommended that the authorities use these two types of treatment programs under expert supervision to improve the resilience of nurses.

Abstract

Introduction: The present study qualitatively investigated the main components that should be considered for the implementation of the referral system in Bojnord city.
Methods: In this study, data were collected through doing interviews with experts, and finally, the importance of all the extracted components was evaluated using a researcher-made questionnaire. Also, the exploratory factor analysis method was used to classify the components in main sets. Kolmogorov-Smirnov, one-sample t-test, performed in Excel and SPSS software, was used for statistical analysis.
Results: The most frequent noted components by the experts included "empowerment (knowledge, skill, psychological) of human resources involved in the referral system", "paying attention to the motivation of human resources involved in the referral system", and "developing suitable processes for the referral system and re-engineering the processes to meet the conditions". Finally, the main requirements in the referral system of Bojnord city were divided into three categories included organizational and management, manpower, and technology-based requirements.
Conclusion: Briefly, some components may lead to improvement of the referral system, which can be categorized into organizational and management, manpower, and technology-based requirements. Detailed planning to improve the referral system can lead to the improvement of the health system.

Abstract

Management is a key pillar in order to achieve the goals of health systems. An efficient structure for identifying, recruiting, training, promoting, monitoring and evaluating the performance of managers is highly likely to help the specialization of management, prevent non-specialized selection processes and possible deviations. Given the managerial level in which they play a role, health managers ought to have the required competencies and characteristics. The leading countries generally thrive to have structures where proper candidates are systematically placed in the position of management and promoted to the higher positions. Therefore, the experiences of these countries should be solicited in line with the local social, political, economic and cultural factors in order to achieve the health system goals.

Abstract

Background: Corona virus is a phenomenon that has faced the world with a crisis due to its rapid spread and uncontrollable nature. Although this crisis is only related to the medicine and health system, it is a multidimensional phenomenon whose effects and consequences can be observed and tracked in various economic, social, and political fields. The purpose of this article is to investigate the results of the Corona Virus on the environment.
Materials & Methods: The current study is a scoping review article that was performed based on the collection of information from articles published in the databases ScienceDirect, Web of Science, Google Scholar, and PubMed in the period from 2019 to 2022 with the aim of investigating the consequences of the Corona crisis on the environment.
Results: The findings of this research illustrate that the corona virus crisis and the quarantine conditions have brought many consequences for the environment. On the one hand, as a result of the cessation of many human activities, there is a significant reduction in the emission of pollutants, including greenhouse gases, and on the other hand, waste, especially medical and plastic waste, has increased.
Conclusion: Despite the positive effects of Covid-19 on environmental factors, its short-term and long-term negative effects are obvious. In addition, it seems that the economic activity of country will intensify after Corona, and therefore, with a comprehensive analysis of the effects of Covid-19 on several sectors, significant proceedings should be taken in this field.

Abstract

Background and Purpose: The purpose of this study is to validate the model of organizational evolution in hospitals affiliated with the Bushehr University of Medical Sciences.
Materials and Methods: This study was applied in terms of its purpose and quantitative in terms of its implementation method, employing a structural modeling approach. The statistical population in this part of the research included all managers, deputies, and experts in the field of the subject studied in Bushehr hospitals, totaling 130 individuals. A sample size of 100 was determined using Morgan's table and was selected via simple stratified random sampling. The research tool was a researcher-developed questionnaire. The questions and items of the questionnaire were compiled using research literature, theoretical foundations, and the results of a qualitative study. The face and content validity was confirmed by experts, and its reliability was obtained with a Cronbach's alpha method result of 0.976. Data were analyzed using the structural equation modeling method in SMART PLS software. Results: The findings of this research demonstrated that the structural model of organizational evolution has 43 sub-components across six components: causal conditions, background, mediating, central phenomenon, strategies, and outcomes. All variables obtained can be considered in the design of the final model. In conclusion, the effectiveness of each variable was discussed and analyzed from an existential perspective. The results indicated that the causal conditions, background conditions, intervening factors, and outcomes in the model of organizational evolution in hospitals affiliated with Bushehr University of Medical Sciences were significant.
Conclusion: The results showed that the model of organizational evolution in hospitals affiliated with Bushehr University of Medical Sciences is valid.

Abstract

Background and purpose: Supplementary health insurance plays a pivotal role in the health economy and individual payments, enhancing access to health services and improving individual quality of life. This study aims to explore the challenges and strategies for developing supplementary health insurance.
Methods: This systematic review examined research related to the challenges and strategies for the development of supplementary health insurance in various countries, focusing on articles published in the last five years in both domestic and international databases. For each article, a data extraction form was completed, and the data were subsequently classified, summarized, and analyzed.
Results: Seventeen articles met the study's inclusion and exclusion criteria and were analyzed. The main challenges and strategies for developing supplementary health insurance were identified across seven primary themes: premium and financing, electronic infrastructure, quality improvement, moral hazards and adverse selection, cost-effectiveness and efficiency, evidence-based decision-making, and effective awareness and advertising.
Conclusion: Considering the numerous identified challenges and barriers in the development of supplementary health insurance, it is recommended to focus on creating electronic infrastructures and necessary platforms to enhance and develop policies in supplementary health insurance, particularly in the dimensions of service packages, cost-effectiveness, and efficiency

Abstract

Background and Purpose: Experiential marketing is a type of marketing strategy that attracts and engages customers by creating real-life experiences, allowing them to form lasting positive memories. Creating an excellent customer experience in the healthcare sector is challenging. Therefore, the aim of this study is to investigate the effect of experiential marketing on the intention to revisit social security clinics in Qom province.
Methods: This quantitative and applied research utilized a survey approach for data collection. The statistical population comprised clients of the social security clinics in Qom province. A total of 522 individuals were invited to participate in the study using stratified random sampling. Data analysis was conducted using covariance-oriented structural equation modeling with the help of Amos V24 software.
Results: The effect of experiential marketing (with dimensions of sensory experience, physical experience, emotional experience, cognitive experience, and relational experience) on the intention to return to the clinic was confirmed, with a path coefficient value of +0.540. Additionally, the structural indicators of the model and the validity of the constructs demonstrated a good fit for the model.
Conclusion: Experiential marketing, along with the enhancement of patient-doctor relationships, is crucial. Patient-centered facilities can increase the likelihood of patients returning to clinics by focusing on experiential marketing, providing amenities, and utilizing technologies to improve service delivery.

Abstract

Background and Purpose: Artificial intelligence (AI) plays a crucial role in the optimal management of hospital waste, particularly in predicting the volume and type of waste generated. This study aims to identify and rank the risks associated with the use of AI systems in hospital waste management by employing a multi-criteria decision-making approach.
Methods: This descriptive, cross-sectional study was conducted in 2023 (1402 in the Iranian calendar) at two hospitals, Ali Ibn Abitaleb and Khatam al-Anbiah, in Zahedan. Ten hospital staff members were selected as expert participants for the Delphi panel. The Shannon entropy method was utilized for risk weighting, and the TOPSIS method was applied to rank the identified risks.
Results: Kendall's coordination coefficient was used to assess the level of consensus among the Delphi panel members, with the coefficient values for the first, second, and third Delphi rounds being 6.3, 7.1, and 7.3, respectively. The indicators were weighted using the Shannon entropy method, based on three criteria: impact intensity (0.3), probability of occurrence (0.4), and detection probability (0.32). The TOPSIS method was then employed to rank the identified risks, with the most significant risks being the need for necessary infrastructure (0.847), the requirement for accurate and complete data (0.751), and budget constraints (0.749).
Conclusion: By applying multi-criteria decision-making methods, healthcare managers can effectively identify and prioritize the risks associated with using AI systems in hospital waste management, enabling them to focus on strengthening waste management practices based on these priorities.

Abstract

Background and Purpose: Health status is undeniably one of the most critical indicators of social development and progress. Providing healthcare poses a significant challenge for human life, and managing the healthcare supply chain is of strategic importance. The aim of this research is to analyze and compare the results of meta-synthesis with thematic analysis in identifying the risks of the pharmaceutical industry's supply chain.
Methods: This research follows a qualitative approach, utilizing both meta-synthesis and thematic analysis to identify supply chain risks in the pharmaceutical industry. In the first step, a meta-synthesis and systematic review of related studies over the past twenty-three years were conducted, identifying one hundred articles, which were refined to twenty-six key articles for the research. In the next step, risks specific to Iran's pharmaceutical supply chain were identified through thematic analysis and semi-structured interviews with experts, using targeted sampling. Finally, the results from these two approaches were compared and analyzed.
Results: The meta-synthesis approach identified ten general supply chain risks in the global pharmaceutical industry. Similarly, the thematic analysis approach identified ten specific supply chain risks in Iran's pharmaceutical industry. Six risks were common to both approaches: low quality of raw materials, complexity and incompatibility of information systems, supply of foreign currency and financial payments, transportation and insurance issues, increase in the price of raw materials, and unavailability of medicines. These common risks are critical for both the global and Iranian pharmaceutical supply chains.
Conclusion: Stakeholders in Iran's pharmaceutical supply chain (including hospitals) should prioritize managing these six common risks to improve supply chain performance. Additionally, they should focus on the four unique risks identified through thematic analysis specific to Iran's pharmaceutical supply chain, applying appropriate control measures and activities.

Abstract

Background and purpose:As people age, physical dysfunction often increases, leading to various challenges and a heightened need for urgent medical services. Elderly individuals are among the primary users of healthcare and pre-hospital emergency services. Their subjective experiences during emergencies are influenced by their need for timely and relevant information regarding emergency procedures and their clinical condition—a need that is also shared by their caregivers. This study aims to elucidate the experiences of elderly individuals who have used pre-hospital emergency services in Shiraz.
Methods:
This qualitative study was conducted in two phases. In Phase 1, semi-structured interviews were conducted with 21 elderly participants using purposeful and snowball sampling until data saturation was achieved. The interview transcripts were analyzed using Colaizzi’s method to extract key themes. In Phase 2, a panel of experts was convened to further discuss the findings and provide recommendations.
Results:
Analysis of the interviews revealed several themes related to the elderlys experiences with pre-hospital emergency services. These themes were categorized into sub-themes including ambulance condition, performance of technicians, timeliness of emergency response, and issues related to emergency rules and regulations. In Phase 2, the expert panel offered practical solutions to address the identified challenges.
Conclusion:The findings indicate that elderly patients face significant challenges when accessing pre-hospital emergency services, such as poor ambulance conditions, suboptimal technician performance, delayed emergency response times, and restrictive emergency protocols. The recommendations provided by the expert panel could inform policy development and lead to improvements in the quality of pre-hospital care for the elderly.

Abstract

Background and purpose: Political intelligence is critical for enhancing managerial performance and organizational productivity, while effective communication skills are essential for successful leadership. This study aims to examine the relationship between political intelligence and communication skills among managers and officials in teaching hospitals in Yazd City.
Methods: This descriptive, cross-sectional, and applied study was conducted on 65 hospital managers, selected via census. Two standardized questionnaires were employed to measure political intelligence and communication skills. Data were analyzed using descriptive statistics and analytical tests in SPSS version 26.
Results: The mean (±SD) score for political intelligence was (107.15±11.96), and for communication skills, it was (59.46±7.46)—both above average. No significant relationship was found between political intelligence scores and any demographic variables. However, a significant association was observed between overall communication skills and the gender variable (p=0.029). Spearman’s correlation analysis revealed a positive correlation between political intelligence and communication skills (r=0.527).
Conclusion: The findings underscore the importance of enhancing both political intelligence and communication skills among hospital managers. In particular, the lower scores in the sub-dimensions of social games (within political intelligence) and feedback skills (within communication skills) indicate a need for targeted improvement programs. These enhancements could contribute to better managerial performance and overall organizational effectiveness.
