Abstract
Background: vulnerable events could damage structural, non structural and functional components of hospitals that might lead to community crisis, accordingly. Risk assessment is the first step to develop of a hospital disaster plan. In this study, a primary phase of developing a local tool for Iran's hospital disaster risk assessment entitled "Hospital Safety Index (HSI)" was conducted by World Health Organization originally.
Materials and Methods: First, the original index was translated into Farsi in five forms and 145 items. Then a group of four experts from the fields of medicine and engineering assessed the items one-by-one for their relevance and applicability. Revised edition tested three times at Shariati hospital in Tehran. An expert panel also evaluated the feature and content validity of the index. They also weighted the items as well.
Results: Test-retest of the index by two independent research groups found 93% of agreement. Furthermore, 244 points were revised or added to the original edition. The most important changes included: Combining instruction parts with related items for increasing educational properties, completing the list of hazards, developing a plan to analysis, and to present a quantitative and graphic of the model and finally to prepare an educational package as well.
Conclusion: Persian version of HSI, in response to needs of Iranian's health system, represents a rapid no-expensive tool for screening disaster risks at hospitals based on an international template that was tested in several countries. Based on this study, the Farsi index would be evaluated in more hospitals around the country. The assessment results will provide Iranian's health system with evidence-based information for more effective allocated resources and interventions evaluating.

Abstract
Background: aging is a very sensitive period of life and paying attention to the problems and requirements of this period seems to be a social necessity. Improving medication for old patients is challenging and sometimes medication`s benefits are less than its disadvantages for the patient.Because of the numerous evidences of problems caused by medication in the elderly in the recent decade, many specialists and physicians have introduced tools and techniques for identifying these problems which are caused by medication and prescription of risky medicines.
Materials and Methods: Several methods, tools and criteria for assessment of quality of medication and medicine prescription in elderly have emerged since 1990.. We found out two categories of criteria reviewing the literature which are generally named as distinct criteria (criterion based) and implicit criteria (judgement based).
Result:The distinct category including Beers, Zhan, McLeod, Laroch and STOP/START criteria are all developed by a panel of experts and consensus in several stages. The second category of criteria concentrates on evaluation of clinical information.
Conclusion: All the tools and methods described in this study are used in Europe and in the USA which have limited market similarities with our country thereforedevelopment of tools for assessing prescription quality for the elderly in Iran is absolutely necessary.

Abstract
Objective: This study was aimed to assess Service Quality (SQ) of maternity care from the perception of pregnant women.
Methods and materials: A cross-sectional study was conducted using a sample of 185 pregnant women at the 9th month of pregnancy were selected randomly from 40 health posts and urban health centers in Tabriz, Iran. Service Quality was calculated using: SQ = 10 – (Importance × Performance) based on importance and performance of non-health aspects from the customer’ perspective. Data collection used a researcher-developed questionnaire whose validity and reliability was reviewed and confirmed. Data analyzed using SPSS-17 software. Independent sample T-test and ANOVA were used to investigate relationship between service quality dimensions and categorical variables.
Results: From the customers’ perspective the average service quality score was 7.59 of 10. Service quality aspects of “confidentiality” achieved scores at the level of good quality (≥9) and “support group” (3.48) reached low service quality scores. Also, result indicate housewife assess SQ better than worker (p=0.047) and mother who's have planned pregnancy has had greater SQ score (p=0.022). Although, in the linear regression analysis, job status and planned pregnancy were significantly and independently related to SQ score.
Conclusion: Findings revealed a significant room for quality improvement in most aspects of provided care, particularly support group and safety from the perception of people who received maternity care.

Abstract
Background: A method of performance assessment is required to improve Productivity and reduce hospital costs . Pabon Lasso diagram is known as a tool for performance assessment. The current study is aimed to assess and compare Kerman province`s hospitals performance by using Pabon Lasso diagram.
Materials & Methods: This descriptive and cross-sectional study was conducted retrospectively between 2008 and 2010. All hospitals of Kerman province in three groups of public, social security and private were selected by census method (N=23). The data were gathered from statistics unit and entered into SPSS 18.0. Three indicators include bed occupancy rate, bed turnover and average of length of stay were calculated and then hospitals` performance were analyzed by Pabon Lasso diagram.
Results: In first two years, seven and eight hospitals out of twenty-three and in third year nine hospitals out of twenty-two located in third zone of the model (desired productivity) respectively. Among all hospitals, regarding number and ownership type, social security hospitals had better performance in 2008. Additionally Public hospitals had better performance than others in 2009 and 2010,.
Conclusion: Public hospitals had better performance.Although studied hospitals` performance experienced growing trend, only one third of the them had desired performance. It can be caused by changing the provided services type from inpatient to outpatient services.It seems that Considering type of ownership and assessing past trend of 3 studied indicators concurrently is necessary for Planning for development of a broader range of inpatient services and diversified medical services based on province`s need.

Abstract
Background: There is now a global concern about significant numbers of patients who are harmed or faced financial problems.Therefor, interving in organized approach to provide safe services seems necessary. This study is aimed to assess the basic indicators of patient safety in hospitals affiliated with Tehran University of Medical Science.
Materials and Methods: This descriptive–analytical study was conducted in the 5 selected hospitals affiliated with Tehran University of Medical Sciences. The data were collected using patient safety assessment protocol published by WHO(WORLD HEALTH ORGNAZIATION) tool.The questionnaires were filled using variety of different ways such as : interview, observation and documents investigation. The data were analyzed using SPSS software and also with descriptive and analytical tests.
Results: The results showed total average of maintaining the standards was 51.1% which categorized as weak level based on the protocol . The highest and lowest rates of the domains studied were safe environment (68.8%) and continuing education (24.8%) respectively. Farabi and Rasul Akram hospitals had the best and the worst levels of maintaining indicators of patient safety standards .It is worth mentioning that the status of all the hospitals was in the same level. Statistical tests indicate that maintaining the standards does not differ significantly between participating hospitals.
Conclusion: All participating hospitals has a poor performance through developing basic patient safety indicators .Managers` support and staff participation can be helpful to solve the mentioned issues. Paying attention to patient safety in managerial plans and putting equal value to various aspects of patient safety are pivotal

Abstract
Background: In recent years, Hashemi Nejad Hospital was outsourced or insourced some of their services to private sector or will have decision to do it. Selection and assessment of suppliers in outsourcing of hospital services is a critical issue. In this study, selecting and evaluating suppliers for outsourcing services in hospitals was evaluated.
Materials and Methods: In order to achieve the goal, evaluating and selecting outsourcing service providers with studies and using opinion of the experts and medical experts, consisting of hospital manager, quality manager, HR managers, officials outsourced parts and other experts in this respect which includes 14 criteria. Identified criteria were clustered in three areas of service features, characteristics and criteria for communications suppliers in the supply chain; supplier selection problem is the problem multi-criteria decision. So, criteria were ranked and weighted using the Expert choice 11 software and AHP.
Results: Based on the study results, sub-criteria of the quality of service, management systems, customer care, and information security had greatest impact on the selection of suppliers and sub-criteria, geographic location, flexibility and problem solving had the lowest priority.
Conclusion: C supplier had the highest priority according to the communication criteria and A supplier had the highest priority according to two other criteria. In total, the supplier A had the first priority, supplier B had the second priority and supplier C had the third priority.

Abstract
Introduction and purpose: Risk assessment is a necessity in high-risk work environments like hospitals. During epidemics, the need to maintain the health of healthcare staff increases as they are effective people in controlling the spread of the disease. The purpose of this study was to assess the occupational safety of healthcare staff against coronavirus using FMEA in
infectious diseases ward of Bu
-Ali Hospital in Zahedan.
Methodology: Failure modes were identified using brainstorming technique. After scoring them with S, O and D, they were prioritized by calculated RPN. To improve the traditional FMEA, failure modes were prioritized with weighted FMEA and MCDM techniques. After identifying the critical failure modes, the root causes of them were identified and categorized. Finally, corrective solutions were provided to handle them.
Results: Three processes including emergency admission, patient visit, and sampling were identified as priority processes. 58 failure modes and their effects were identified in 6 categories. 13 critical failures modes (RPN above 100) equivalent to 22% were identified. Then 42 root causes of them were identified by brainstorming technique and their classifications were done by Eindhoven. Finally, 49 corrective strategies were presented to handle critical risks.
Conclusion: Identifying 58 risks and their effects, identifying and classifying root causes and providing corrective solutions indicate the capability of the FMEA to assess the risk of critical departments such as hospitals. As a result, the FMEA is able to detect risks, reduce their consequences and improve quality. Risk assessment techniques along with the commitment of managers and the renewal of organizational policies can ensure the effectiveness of these activities

Abstract
Background: It is expected that graduates of healthcare management have the necessary skills to perform various managerial and administrative positions. This study was carried out with the aim of designing and compiling an evaluation framework for students based on educational standards in the internship period.
Materials &Methods: The current study was conducted in four phases: In the first phase, in order to identify the problem and assess the needs, the existing curriculum and topics were examined, as well as the studies conducted in this field. In the second phase, the perspective of the stakeholders was used to complete and modify the initial framework. In the third phase, worksheets for registering internship learning activities were prepared for students and a checklist of educational and learning objectives, personal and management skills, and skills for developing professional qualifications was compiled. Finally, a specialized panel was used to determine the type of formative and final exams.
Results: The problems and challenges of internships can be investigated in 4 areas: inappropriate planning of the course, faculty, group - professors and hospital. In designing the evaluation tool, 9 criteria were taken into consideration. Also, tests designed in a compilation framework were used as formative tests and reasoning tests were used as final tests.
Conclusion: The compiled framework can be used as a valid and comprehensive tool to evaluate the practical competence and capabilities of healthcare management students during the internship period.
