Abstract

Background: Hospitals are best places to provide preventive and health promotion services along with healthcare services. These studies aimed at determine the effect of establishing health promoting hospitals’ standards on hospitals indicators in Fatemieh hospital.

Materials and Methods: This is a comparative study which was conducted in Fatemieh (case) and Khatamolanbia (control) hospitals in 2013. The standards of health promoting hospitals as an independent variable were established in Fatemieh hospital. Data related to indicators of leaving hospital with personal satisfaction, Cardio pulmonary Resuscitation (CPR) and neonatal mortality were collected in both hospitals in second half of 2012 and first half of 2013. SPSS version 16 used for data analysis and Chi-square and Fisher exact tests were utilized. The results were shown by comparative tables.

Results: The neonatal mortality rate before and after the intervention was 7.28 and 14.03 in case hospital, while this rate was 5.26 and 10.83 in control hospital respectively. The percentage of leaving hospital with personal satisfaction before and after interventions was 3.43% and 2.93% respectively in case hospital and success of cardiopulmonary resuscitation in this group was 56.1% and 61.3% correspondingly. There was no significant relation between establishing standards of health promoting hospitals and indicators of leaving hospital with personal satisfaction and neonatal mortality in Fatemieh hospital, while significant relation was obtained between promoting of cardiopulmonary resuscitation indicator and standards of health promoting hospital in mentioned hospital(P=0.001).

Conclusion: Initial results revealed that establishment of health promoting hospitals’ standards had positive effects in Fatemieh hospital and lead to improve a number of indicators.

Abstract

Background: Chronic renal failure disease is considered as a health problem . Because of their high prevalence and economic burden on the society, health system managers consider to the disease as a public health concern.  This study aimed at comparing the cost-effectiveness of long-term dialysis treatments and kidney transplant using DALY index.

Materials and Methods:This cross-sectional study was designed to compare the cost-effectiveness of two methods such as long-term hemodialysis and kidney transplant using Disability Adjusted Life Years (DALY) in Afzalipour Hospital of Kerman in 2012. The decision tree model and TreeAge software were used for data analysis. In this research, costs and effects were studied from the patient perspective.

Results: The cost of a kidney transplant was 165 million Rials, effectiveness of a kidney transplant was 4.40 DALY, chronic dialysis cost was 1.283 billion Rials and its effectiveness was 6.52 DALY. Cost-effectiveness ratio for dialysis was 196 million Rials per DALY and this ratio is 37 million Rials per DALY for a kidney transplant respectively. According to the findings, kidney transplant is more cost effective than dialysis.

Conclusion: This study suggests kidney transplant over dialysis due to its better quality of life and more life expectancy. The results can help out the government to decide about the more resource allocation to kidney transplant programs. The method in this research can be applied to measure the cost-effectiveness of other organs transplantation.

Abstract

Background: Recent years, many efforts provided to design hospitals ;as the most stressful environments; to  reduce stress , improve health , patients' remedy  and satisfaction based on patients 'points of view. This study aimed to identify the physical and mental factors effective on patient view in hospitals of Yazd city.
 
Materials and Methods:  Regarding the purpose, this study was an applied one which used survey and correlation methods. In order to determine the normality of data distribution, Kolmogorov – Smirnove, the type, amount and direction of relation between the variables Persons correlation coefficient and linear regression analysis were used, respectively. Additionally, in order to clarify the multi regression model, variance analyze method of ANOVA has been utilized. According to data gathering literature and instrument review have been utilized.
 
Results: Study results revealed that there was a significant relation between physical factors and individuals' satisfaction (P<0.05). There was no significant association between mental factors and individuals' satisfaction. 
 
Conclusion: It seems it is necessary that conditions and facilities need to be provided regarding physical and subjecting factors improving in wards which can be result in stress reduction and satisfaction increasing in the hospital environment, as well. Also, in patients' point of view, using appropriate colors, providing positive entertainment (films and comic books), proper ventilation and fresh air are some of the most important factors in patients ' satisfaction in wards.
