Abstract
Background: Nowadays, health care section is one of the most important parts of sustained development in human societies because of having relationship with human health. Achieving this goal needs to have healthy and motivational staff, including nurses. We see nurses in all hospital wards who have been interested in their works when entering the hospital, but feel fatigue after several years and may give up their jobs. This survey has been conducted to study of effective factors on nurses' job motivation in Kerman University of Medical Sciences hospitals.
Material and Methods: This is a cross-sectional survey that studies 232 nurses occupied in Kerman university of medical sciences' hospital inpatient wards who were selected by stratified random sampling using a questionnaire which had 31 questions in 5 area: psychological, educational, financial, administrative and welfare factors. For determining the reliability of questionnaire, we conducted a pilot study and its _Krunbakh was 0.92. we used spss11.5 and Kruskal-Wallis and Mann-Whitney tests to analyze data.
Results: The results show the statistical relationships between welfare factor and hospital type (P=0.012), between psychological factor with sex, job experience and marital status (respectively, p=0/002, p=0/043 and p=0/011). But there weren't any statistical relationships between nurses' age and educational status and the studied area of motivation.
Conclusion: In this study, nurses who have following characteristics, have also better motivation status: occupying in hospital B, being married, being man, having master of science degree, having more than 10 years job experience and being between 30-40 years old.

Abstract
Background: Medical tariff is one of the most important tools for health policy making in a country
that influences equity, efficiency, quality and accountability of service deliveries. If relative values for
different services are not determined correctly, it may have negative consequences on availability and
sustainability of care. This study was designed to assess the challenges and respective solutions of the
tariff system in Iranian health sectors.
Materials & methods: This is a qualitative study, involving nine face to face interviews with health
managers, policy makers and researchers at the Ministry of Health and Medical Education and relevant
organizations, as well as public and private institutions and service providers who had experiences in
medical tariff. We used the thematic framework method for analyzing qualitative data.
Results: The problems and respective solutions were classified into stewardship power, policy
making and surveillance for tariff setting, structural organization of medical tariff system, methods and
principles of setting the tariffs, medical costs recording systems and other concepts related to the
medical tariff system in the health sector.
Conclusion: To improve medical tariffs system in the country, one need to have a deep
understanding of the current challenges and potential solutions in different levels. It is advisable to
follow a clear model based on focus on policy-making and stewardship of medical tariffs system as an
important cornerstone of any effort to rectify the current situation.

Abstract
Background: The existed difference statistical indicators is necessary to measure the efficiency of the health organization and as an exact strategy, could be removed the developed problems in heath services sectors.
Materials and methods: This is a descriptive research as a cross sectional study throughout obstetric and gynecology wards in the general hospital of RASOOL. The questionnaires and statistical Forms applied for data gathering. The methods were completed questionnaires, statistical Forms, face to face interviews and observation. The Delphi technique, standardized questionnaires and references’ text books used to determining the reliability and validity. Data analysis was done by using likret ranking.
results: The results showed that some indicators, such as inpatient bed occupied rate (63%), bed turnover interval(5) & mean length of stay(4), patient per nurse(19), meanwhile service education(10),staff per active bed(1for 12), midwife per delivery bed(0), staff(50.9%) and patient satisfaction(89%)
were lower than expected. Some indicators such as rooming in technique(100%), breast feeding(100%), patient information access from HIS system(100%), emergency and hostelling medicine access(100%), round and morning reports(100%), the mean admitted time in the ward(30 minutes), Infant and maternal mortality rate per total deliveries(0), and the percentage of trained patient(100%) were as expected. Some indicators such as expert physician per active bed (1 for 7) and C/S per total delivery ratio (69%) which was higher than expected, and the necessary data for calculating book & papers per assistant professor & non colonial infection rate were not found.
Conclusion: Hospital indicators showed the performance of hospitals in the various bases. Therefore,the broad attentions to these indicators are necessary. Meanwhile, it should be compared them and to distinct time periods.

Abstract
Background: Patient`s self-discharge from the hospital is a process in which a patient leaves the hospital before completing his or her determined treatment period by his own will. This study is aimed to survey the factors affecting patient`s self-discharge from the hospital in emergency ward of a referred hospital. Materials and Methods: This is a case-control study. The 280 selected samples were divided to two groups, each with 140 members. The data were gathered using a questionnaire evaluating the scale of patients and their companions` satisfaction of the hospital services. Chi-square test was used for analyzing the relationship between independent variables and the patient discharge type. Moreover logistic regression was used in order to identify the effect of significant independent variables on own will-discharge possibility. Results: Results indicate that there is a meaningful relationship between variables such as age, having supplementary insurance, residency, income rate, reference shift, reference day, satisfaction of the physicians, satisfaction of the physical environment and welfare facilities, the patient`s satisfaction, the waiting time, satisfaction of the instructions given by nurses and physicians and total satisfaction with discharge type(p<0.05). Conclusion: The matter of patient`s self-discharge is a multi-dimensional phenomenon and depends on patient’s characteristics and structural and medical factors. Therefore predicting its scale and determining factors might play an effective role in controlling this phenomenon.

Abstract
Abstract
Background and Aim: Especially in the case of Hospital services, quality assesment is an complicated and vague issue. For achieving this goal Multiple Criteria Decision Making methods help us. These studies aims to knowledge and prioritize quality indicators and rank selected inpatient wards in an elected hospital of Tehran university of medical science by using Analytical Hierarchy Process and TOPSIS methods.
Materials and Methods: This is a descriptive-analytical and cross-sectional research. At the first, with an overview of the research literature and expert opinion, indicators of quality assessment were selected. Then we did prioritization of these indicators with using of expert opinion and AHP method. In the final stage, selected inpatient wards the Hospital were ranked in terms of quality of service using TOPSIS by a sample consist of 300 of inpatients.
Results: Eight main domains (Responsiveness, Safety and risk management, Tangibles, attention to patient, Health communications, Competency and professionalism, accessibility, Credit and assurance) were selected for quality assessment. Among these indicators, competency had the highest priority and attention to patient had the lowest priority. Also the department of Heart had the highest quality of service and the department of Neurology had the lowest quality.
Conclusion: According to crucial role of the service quality on patients’ satisfaction, the use of appropriate indicators for quality of services, prioritizing them and also proper assessment of the quality of hospital services can play a major role in health policies.

Abstract
Background: Reward system program is an investment tools in order to improve quality of services. Accordingly, this study was conducted aimed to identify the challenges of the reward system and the solutions for improving that based on the views of stakeholders in Tehran University of Medical Sciences hospitals.
Materials and Methods: This directional content analysis qualitative study was performed in Tehran University of Medical Sciences during 2014and 2015 using content analysis method. Targeted sampling was done and in-depth semi structured and face to face interviews were performed with a wide variety of experts and professionals including managers, faculties and hospital staffs. After word by word transcripting and extracting codes (meaningful phrases), these codes was reviewed again in
order to achieve subcategories. Based on subcategories, information was classified to assossiated categories. For validity and reliability checking, stakeholders and faculties member check and peer debriefing were used.Results: Eventually, a total of 29 interviews were held. The comments were classified within 6 main categories, 19 subcategories and 1100 codes. The main categories included existing arrangements in hospitals in order to improve staff motivation; reward system challenges; derived opportunities from rewarding experiences; rewarding systematic actions; types of appropriate rewards and making
infrastructure for better rewarding system implementation.
Conclusion: Regarding multiple aspects of a reward system which could have an effective role in increasing staff motivation or a contradictory effect when it is inefficient, implementation of an effective reward system in hospitals by designing an appropriate model based on identified key factors in this study, is recommended.

Abstract
Background: Necessity of development, quantity and quality improvement needs of medical tourists had been unavoidable based on medical tourism growth with its competitive function. This study aimed to determine satisfaction rate of medical tourists referee to Tabriz hospitals to get treatment services.
Materials and Methods: This descriptive and analytical study was conducted on 82 medical tourists referee to all wards of selected hospitals using convenient sampling method in Tabriz city. Data was collected using a-28 items valid and reliable researcher made questionnaire. Cronbach’s Alpha was calculated 0.708. In order to data analysis, SPSS Software version 23 was utilized. Also, Spearman’s correlation coefficient was used to assess the association between different aspects of patients’ satisfaction.
Results: Satisfaction rate was obtained in five main aspects including cultural problems (51.88%), structural issue (57.50%), insurance payment system (16.04%), information system and communication channels (41.34%) the economic challenges (50.35%). The information system and communication channels had high effect on economic aspects (p<0.001).
Conclusion: It seems necessary that health policy makers and managers pay more attention to foreign medical tourists and faced challenges correction in order to change Tabriz city to a medical tourism center of excellence and improving regional economic development.

Abstract
Background and purpose: One of the main challenges of human resource management in hospitals is the proper distribution of nurses, which is necessary to improve the efficiency and quality of health services. In this review, this rate is compared in selected countries and Iran in order to provide the necessary evidence for managers and policymakers in human resource management
Materials and Methods: This study was conducted as a field review in 2019 with the aim of comparing the standards of nurse distribution to beds/wards in selected studies. The search was conducted with related keywords in "Scopus", "Web of Science", "PubMed" databases without a time limit.
Results: The distribution of nursing per bed was reported in Australia, England, Northern Ireland, Wales, Israel, America, California, Pennsylvania and New Jersey. In Iran, the rate of nurses to intensive beds was slightly higher than the average and the ratio of nurses to ordinary and emergency beds was lower than the average of selected countries average.
Conclusion: Nurses' distribution was generally lower than the average of other countries, and this ratio was lower in some sectors and higher than the average in some sectors. Recruitment and redistribution of the appropriate nurse in different sectors by benchmarking successful countries are suggested.
