Abstract

Introduction: Needlestick (NSI) is a known serious treat and occupational exposure among health care workers especially nurses. Since nurses are the biggest part of healthcare workers, and considerable percent of NSI are preventable, we decided to determine frequency of NSI and some related occupational factors among a group of them.

Methods and Materials: This cross-sectional project was conducted among all the nurses (340 nurses with at least one year work experience) in Babol and Amirkola educational and private hospitals (in north of Iran). The data were gathered by means of a tailor-made data collection sheet including personal information and some occupational effective factors, during autumn and winter. The data were analyzed with descriptive statistical indexes and Chi-square test.

Results: Our result showed that the relative frequency of NSI among the nurses group (the mean age of nurses was 33.8±7.7 years old and the mean work experience was 10.3±8 years) was 59.7 percent. Chi-square test result indicated that there were significant differences between NSI in different groups of some personal and occupational factors including age, years of experience, training condition, and needle recapping(p<0.05).

Conclusion: According to positive effects of training and negative effects of incorrect working habits, repetition and completion of occupational education is necessary.

Abstract

Introduction: Low back pain (LBP) is a common and costly health problem. It has high prevalence in nursing job and caused shortage of nursing staff. This study was conducted to determine the prevalence of LBP, knowledge level of LBP risk factors and assess relationship between LBP prevalence and nurses’ knowledge level of LBP risk factors among nurses of Shiraz University of Medical Sciences (SUMS).

Material and Method: In this cross-sectional study, 118 randomly selected registered nurses participated from SUMS hospitals with at least one year of job tenure. In order to assess nurses’ knowledge of LBP risk factors, a self-administered survey questionnaire consisted of four parts was used as data collecting tool. Statistical analyses were performed using SPSS (version 11.5). Duncan, T-test and ANOVA tests were used for data analysis.

Result: LBP prevalence rate was found to be 79.7% (94 nurses) during the previous year from which 12 nurses were male (63.2%) and 82 nurses were female (82.8%). Nurses’ knowledge level of LBP risk factors in different hospitals was not significantly different. The Results showed that nearly half of the nurses had poor knowledge about LBP risk factors and the remaining had good knowledge level. The main source of information about LBP risk factors among nurses were related to their university education.

Conclusion: A high prevalence of the LBP was found among nurses working at SUMS hospital. Knowledge level of nurses about LBP risk factors needs improvement. In addition to university education, which is the main source of information of nurses about LBP risk factors, on-the-job training seems essential in this field.

Abstract

Introduction: Provision of high quality nursing care for patients, has made patient safety culture as an important issue for improving the quality of health care in the country. This study aimed to determine the relationship between nurses’ job satisfaction and patient safety culture in hospitals of Rasht city, Iran.

Material and Method: This research is a descriptive-analytical, cross-sectional, and field data collection study. Nurses working in public and private hospitals in Rasht City comprised the study population, of whom 322 subjects were selected randomly as the study sample. The Wakefield questionnaire and the Agency for Healthcare Research and Quality questionnaire were employed to investigate nurses’ job satisfaction and patient safety culture, respectively. The Pearson correlation coefficient was used for statistical hypothesis testing, employing SPSS software version 19.

Result: The results showed that according to the respondents’ viewpoints, scores of job satisfaction (Mean(SD):3.59±0.68) and patient safety culture (Mean(SD):54/0±31/3) Rasht city hospitals were at the average level. Furthermore, there was a significant direct relationship between nurses’ job satisfaction and patient safety culture with the correlation coefficient of 0.643 at the 0.01 level of significance.

Conclusion: According to the findings, it is necessary to improve study nurses’ job satisfaction and patient safety culture. Moreover, according to statistical correlation between research variables, increasing nurses’ job satisfaction results in improvement of patient safety culture.

Abstract

Introduction: Human error is considered as a crucial challenge in occupational settings. Health care system is amongst occupational environments with high rate of human errors. Numerous preceding studies noted that more than 2/3 of medical errors are preventable. Accordingly, different methods are suggested to evaluate human errors, especially in nuclear industries. The aim of this study was to evaluate the application and accuracy of HEART technique in medical health system.

Material and Method:  This qualitative study was conducted in surgical intensive care units of a hospital in Shiraz city. All nurses recorded errors were categorized regarding the given tasks and then all tasks were ranked based on the number of errors. The probability of nurses’ tasks error was estimated through AHP-HEART method and the resultant ranking was compared with the recorded errors. Additionally, the prioritization of contributing factors to errors, determined by AHP and AHP-HEART methods, was compared employing Pearson statistical test.

Results: Based on the results, there was a concordance in the rate of nurses’ error determined by HEART method and the recorded errors. However, no significant correlation was between errors contributing factors determined by AHP and AHP-HEART methods.

Conclusion: This study suggested that although HEART technique was successful to rank the tasks considering the magnitude of error probability, but the coefficients of error producing conditions should be customized for nurses’ tasks in order to provide appropriate control measures.

Abstract

Introduction: Quality of Work Life (QWL) is a new form of job satisfaction which involves ones feelings about all aspects of their job. The aim of this study was to assess the psychometric properties of the Persian version of WRQoL-2 questionnaire for use in the nursing community.

Material and Method: In this study, backward translation method was used in order to verify the linguistics stability. Then, the cross-sectional study was conducted among 270 nurses working in a hospital, affiliated to Tehran University of Medical Sciences. Reliability and construct validity was investigated using test-retest and factor analysis, respectively. In addition, discriminant validity was assessed by comparing QWL mean scores of nurses satisfied with their salary and those who were dissatisfied.

Result: Regarding the reliability, it was observed that the repeatability coefficient were at an acceptable level for all dimensions of the WRQoL-2questionnaire. According to factor analysis, six components were identified with some questions in different groups compared to the original questionnaire. Considering discriminant validity, all components, except Stress at Work, obtained P-value less than 0.05.

Conclusion: Based on the acceptable level of reliability and validity of the WRQoL-2 questionnaire, use the questionnaire to assess the quality of work life of nurses is recommended.

Abstract

Introduction: Nursing is one of professions that employees are at risk of fatigue and work related musculoskeletal disorders, because of high physical workload and high job stress. Connecting serum set to serum solution is one of the repetitive tasks for nurses in their working times and it may cause pain and discomfort in their hand. The aim of this study was to design and ergonomic evaluation of a tool for connecting serum set to serum solution.

Material and Method: This experimental- interventional study conducted on 12 nurses (6 men and 6 women) in 3 phases. First phase was assessment of manual connecting of serum set to the bag by nurses, second phase was design and manufacturing of serum set connector and third was ergonomic assessment of the manufactured tool. In first and third phases, amount of perceived exertion by nurses was assessed using the Burg scale (CR10), and electromyography assessment for hand muscles activity was conducted during connecting serum set. Data of first and third phases compared with statistical tests.

Result: Mean score of perceived effort during manual connection of serum set was 5±1/2 (of 10) and while mechanical connection with designed tool was 2.3±0/49. There was significant difference between activity in 5 muscles (Flexor Digit Comonis, Flexor Carpi Radial, Biceps, Triceps and Deltoid) in manually and mechanically connection mode (P-value<0.05).

Conclusion: Use of serum set connector reduced the perceived effort and activity of hand and wrist muscles. This device can be used as an ergonomic tool for nurses to easing the inserting the serum set to serum solution

Abstract

Introduction: Hospital acquired infections recognize as an important risk factor in health care. In this regard health care and hand hygiene is the first step in controlling the infections. This study, carried out with the aim of comparing the effect of hand washings in the usual way with disinfectants without water in microbial contamination of the hospitals hand nurses.

Material and Method: Accordingly, samples were taken from hand of 30 nurses in the wards of surgery, pediatrics, emergency, CCU, ICU and cardiology in 4 steps before and after washing with soap and washing with liquid hand rub. Samples were cultured in EMB and Blood Agar medium, then identified by gram staining, catagase and coagugas tests and growth on manitol salt agar.

Result: The findings showed that 30 samples were not grown on EMB medium، but 30 samples of Blood agar, before and after washing with soap and  liquid hand rub, were grown and applying hand rub alcohol liquid is most effective than washing with soap. Identification of bacteria in manitol salt agar medium and coagulas test revealed %80 were coagulas negative staphylococcus and %20 coagulas positive staphylococcus.

Conclusion: In this study washing with soap dose not effect on hand hygiene of nurses but disinfectant with liquid alcohol shows significant correlation in reducing microbial load on personnels’ hand that is revealed alcohol was more effective in reducing pollution.

Abstract

Introduction: Job stress is one of the occupational hazards in the modern era. The National Institute for Occupational Safety and Health introduced the nursing job as a top 40 stressful jobs. In this job, multiple training techniques are considered to reduce stress and stress factors. This study was conducted to response the question “Is the use of health education programs can be effective in reducing job stress?”

Material and Method: Electronic search was done using key words in Persian and English database (Medical Sciences, Iran Medex), Scientific Information Database (SID), Bio Med Central, Pub Med, Science Direct) and were found 3,412 paper totally. Articles examined based on defined criteria and 9 qualified articles were discussed systematically.

Result: Among the reviewed studies, educational interventions in 7 studies were without, and 2 studies were with the use of health education models. The results of studies indicated significant effect of educational interventions on reducing job stress and using health education models in interventions had a greater effect on reducing job stress.

Conclusion: The results highlights the importance of educational interventions based on health education models to reduce occupational stress among nurses.

Abstract

Introduction: Impairment of alertness, attention and performance associated with sleepiness and fatigue in nurses occur in night and long-term shifts that in the end of night shift reach to the maximum level can lead to traffic accidents when they returning home. The purpose of this study was to determine the effect of night shift on psychomotor abilities of driving in nurses after shiftwork.
 

Material and Method: A cohort study was carried out on 23 night shift and 24 day shift female nurses aged 20 to 40 at Sina Hospital in Tehran city, using the Vienna Test System (VTS). The concentration and selective attention, reaction time, pheriperal perception, and coordination before and after night and day shifts were measured. A multiple linear regression model and Backward stepwise selection method was used for analyses.
 

Result: In the concentration and selective attention test, sum hits  (p = 0.038) and in the visual perception test , divided attention (p =0.006) and visual field (p =0.019), and in the  reaction time test the mean motor time  (p =0.034) showed a significant adverse relationship with working in night shift, but the visomotor coordination variables did not show any significant correlation.
 

Conclusion: The results showed that the concentration and selective attention, peripheral perception, and reaction time of psychomotor ability of driving were significantly adversely impaired in nurses after night shift. These results in evidence of the mechanism of increasing traffic accidents after night shift among nurses added to the previous studies in this subject.

Abstract

Introduction: One of the most common psychiatric disorders in nurses is job stress. Regarding the inconsistency of the studies conducted in nurses working in Iran, the present structured review and meta- analysis study was conducted to determine the prevalence of job stress in nurses working in Iranian hospitals.
Materials and Methods: The present study is a systematic review and meta-analysis conducted during 2001-2017. Articles related to job stress were obtained by searching SID, MEDLINE (PubMed), Science direct, and Google Scholar databases, and data were analyzed using Comprehensive Meta-Analysis software.
Results: Having investigated 17 studies, the overall prevalence of Job stress in nurses working in Iranian hospitals was obtained 60% (48.1% - 70.7%: 95% confidence), meta-regression test was conducted to investigate the heterogeneity of meta-analysis results. It was reported that increasing sample size decreases the prevalence of Job stress (P <0.05).
Conclusion: Regarding the high prevalence of job stress among nurses working in Iranian hospitals, health policymakers have to take effective measures to remove and reduce the stress causes in nurses to reduce the prevalence of stress in nurses and enhance work efficiency.

Abstract

Introduction: Nursing Known as a stressful and overwhelming profession. Due to the stressful nature of the nursing job, nurses experience burnout more than any other occupation, which may affect the quality of health care. Emotional intelligence is one of the important predictors of psychological well-being and it plays an important role as a distinguishing factor for organizational performance and achievement of job success. The purpose of this study was to determine the relationship between emotional intelligence and nurses' personal and occupational characteristics.
 
Material and method:  In this cross-sectional analytical study, 135 nursing staff working in Gonabad hospitals was selected through a census sampling method. Written informed consent of nurses and working in hospitals of Gonabad city were the inclusion criteria and Failure to complete the questionnaires or the questionnaire being confounded or withdrawal from the study was considered as the exclusion criteria. They were evaluated by Shirring Emotional Intelligence Questionnaire and Demographic Information Form. The research data were analyzed using SPSS software version 16, Chi-square test, one-way ANOVA and Pearson correlation coefficient. The significance level was less than 0.05.
Results: In this study72.6% of participants were women. The majority of nurses were married (81.5%) and 87.4% of nursing staff had high emotional intelligence. In this study, there was no significant relationship between individual characteristics and emotional intelligence, but there was a significant association between job characteristics, job shift (P = 0.03) and employment status (P = 0.03) with total emotional intelligence. The results showed that there was a significant relationship between social consciousness with education level (p = 0.019) and employment status (p = 0.001) and there was a significant relationship between social skills with work shift (p = 0.003) and nurses' marital status (p = 0.007)
Conclusion: According to the findings, considering the appropriate job shifts for nurses and formalizing them to create a strong organizational commitment to take benefit of nurses with higher emotional intelligence and this leads to improved performance of nurses.

Abstract

Introduction: Considering the accreditation of international standards of hospitals and the necessity to improve the safety and quality of patients’ care, this study aimed at evaluating reliability among nurses using predictive analysis of cognitive errors and human event analysis techniques.
Material and Methods: The analysis of nurses̓ tasks was done by HTA method. Then, the types of errors and their causes were identified by TRACER method. In the next step, the error probability of each task was calculated by ATHEANA method. In order to calculate the probability of total event, the probability of human error was imported to probabilistic risk assessment.
Results: Factors affecting performance of the nurses were included: the complexity of the work, high workload, nurse’s experience, work environment design, fatigue, anxiety, shortage of the workforce, insufficient time period for doing job, sleep disturbance, and poor lighting and noise pollution. According to the instruction of ATHEANA method, the error probability for each base event was considered 0.001. Given that there are 15 base events, the probability of human error in the heart attack event was calculated 0.015.
Conclusion: The finding of this study was indicated the need for providing required nursing workforce, reducing overtime, scientific planning for nurses’ work shifts and giving practical training and stress management methods in the emergency conditions.

Abstract

Introduction: Recognizing the personality traits of employees and examining the role of these traits in motivating people’s safety can greatly lead to the formation of safe behavior of employees. Considering the significant importance of reducing adverse events and developing safe behaviors, the present study aimed to investigate the role of nurses’ personality traits in their safety behaviors with the mediating role of safety motivation during 2019-2020. 
Material and Methods: This is a descriptive-analytical study conducted among nurses working in educational hospitals affiliated by Qazvin University of Medical Sciences, from which 435 individuals were selected by multi-stage proportional sampling method. Data were collected using McCray and Costa Five-Factor Personality Questionnaire, Saleh Motivation Questionnaire (2010) and Safe Behavior Questionnaire (2016) and analyzed by SPSS software version 26 and AMOS software version 23.
Results: The path coefficients between nurses’ personality traits and their safety motivation (0.557) and safety motivation with safe behavior of individuals (0.267) were statistically significant (p<0.05). Furthermore the path coefficient between nurses’ personality traits and safe behavior (0.573) was statistically significant (p<0.05).
Conclusion: Results affirmed that safety motivation in nurses had a significant effect on their safety behavior. Therefore, it is recommended to improve safety motivation through establishing an incentive systems to appreciate the employees who had an appropriate and safe behavior in the workplace.

Abstract

Introduction: Occupational fatigue is one of the harmful factors in many work environments, including health centers, which can have adverse effects on the health and safety of staff. This study was designed and conducted to determine the relationship between occupational fatigue and elements of the systems engineering model for patient safety in nurses.
Material and Methods: This descriptive correlational study was conducted with the participation of 457 nurses of Shahid Beheshti University of Medical Sciences in 2018. Dimensions of fatigue were assessed by a Multidimensional Fatigue Inventory (MFI) and Systems Engineering Initiative for Patient Safety (SEIPS) model’s elements using the SEIPS model’s questionnaire. Validity was examined using the Lawshe method; calculating Content Validity Ratio (CVR) and Content Validity Index (CVI) was approved through the confirmation of experts. Reliability was assessed using the intraclass correlation coefficient (ICC) and Cronbach’s alpha. Data analysis was performed using SPSS version 21
Results: The findings of this study indicated that the highest score of fatigue was related to the general fatigue dimension with an average of 12.86 and SD of 3.23, and the lowest score was related to the reduction of the motivation dimension with an average of 9.11 and SD of 3.66. In this study, no significant relationship was observed between demographic characteristics and fatigue dimensions, but a significant relationship was observed between the dimensions of fatigue with the element of organization, task, technology/tools, and physical environment.
Conclusion: The results of this study showed that fatigue in nurses was moderate, and the factors of the work system play a greater role in the occurrence of fatigue than demographic factors. Therefore, planning to improve the work system can help reduce fatigue in nurses.

Abstract

Introduction: Health workers, especially nurses, are facing a high risk of contracting the COVID-19 and consequent mental disorders such as stress, anxiety and depression. We aimed to study the relationship between anxiety, stress and the protective behavior of nurses during the Covid-19 pandemic.
Material and Methods: This cross-sectional study was conducted on nurses working in COVID -19 referral hospitals in Qazvin, Iran, in 2020. Out of 645 nurses working in two hospitals, 260 of them were selected randomly. Three questionnaires including demographic and contextual information, COVID-19 anxiety, stress and precautionary behaviors against COVID-19 were used. Descriptive statistical methods, Pearson correlation, t-test and one-way analysis of variance and binary logistic regression analysis were applied at 95% confidence interval.
Results: The prevalence of anxiety symptoms in nurses was 32.08%, the mean score of protective behavior was 18.45+5.66 and the maximum score was 22.06+4.7. The negative relationship between anxiety and nurses precautionary behavior) β=-0.36, P<0.05) and the significant effect of COVID-19 stress on nurses’ behavior) β=-0.22, P<0.05) were confirmed. Furthermore, adding stress as an interfering factor, affirmed the mediating role of stress in the relationship between anxiety and nurses’ precautionary behavior (β=-0.18, P<0.05). Nurses under 35 years of age were more likely (OR=1.62, P=0.004) to follow the protective rules; those with 6 to 10 years of hospital experience were respectively 32 times and 37 times more probable to use personnel protective equipment (PPE) (OR=1.32, P=0.002) .
Conclusion: The probability of effective use of PPE and observance of protective guidelines among nurses with severe and very severe anxiety levels was less than their counterparts. While in those with controlled level of stress level such obedience was much more probable. Therefore, all health service providers, need to develop supportive programs to emphasize on the promotion of employees’ mental health.

Abstract

Introduction: Nurses are under the influence of stressful factors and high work pressure, which increases nursing errors. One factor affecting nurses’ behavior and performance is the mental workload and cognitive failures. This research has investigated the mental workload and cognitive failures in two groups of nurses with and without unsafe behaviors during the outbreak of Covid-19.
Material and Methods: This case-control study was conducted on 312 nurses during the severe period of the Covid-19 outbreak through the public hospitals in Guilan province. Sampling was performed by selecting 104 nurses in the case group (with unsafe behavior) and 208 nurses in the control group (with safe behavior) using the full count method. The data collection tool is a three-part questionnaire including demographic information, the mental workload (NASA-TLX) questionnaire, and the occupational cognitive failures questionnaire. The incidence of needle stick as a consequence of unsafe behavior was used as a criterion.
Results: The results have shown that the workload in this research was 63.96±18.26, and the total score of cognitive failures was 59.1±13.9. There was no statistically significant difference between the two groups of nurses with and without unsafe behaviors (P>0.05). Still, there was a significant correlation between the total mental workload score in all areas and the total score of cognitive failures in the group of nurses with and without unsafe behaviors (p=0.001, r=0.225).
Conclusion: According to the results, the perceived mental workload is high for the nursing job. Despite of positive correlation between mental workload and cognitive failures, it can be mentioned that there are two-way interactions through their dimensions. Therefore, the nursing job requires adopting optimal engineering and management strategies to reduce the workload.

Abstract


Introduction: Compared to other hospital departments, emergency department (ED) nurses experience higher levels of occupational fatigue. However, despite the importance of fatigue in EDs, limited studies have investigated the issue. Therefore, the present study aimed to identify factors affecting ED nurses’ fatigue with a systemic approach and provide a comprehensive model of fatigue.
Material and Methods: The present study was the first phase of a mixed-methods study with an exploratory sequential design that was conducted qualitatively. Data was collected using a semi-structured interview technique among ED nurses in TUMS hospitals in Tehran. The data was analyzed with a systematic approach and qualitative content analysis method. To check the credibility and trustworthiness of the study results, Goba and Lincoln criteria, including transferability, peer review, and member check, were used.
Results: Extensive factors were identified in the four levels of nurses’ work system, including individual factors, work unit, organization, and extra-hospital factors that could facilitate or prevent nurse fatigue. Specifically, the categories that were mentioned the most by the nurses were aggression and conflict with visitors, ED workload, understaffing, crowding and noise, obligatory overtime, acknowledgement, visitor’s expectations, reporting (paperwork), and clients’ distrust of nurses. Finally, based on the qualitative results, a conceptual model of factors affecting occupational fatigue of ED nurses in Tehran was presented.
Conclusion: Traditionally, fatigue management programs in hospitals only consider issues related to work shifts and sleep hygiene. However, the results of the present study revealed other fatiguing factors in the work system of ED nurses. These findings can help health system decision makers and HFE experts in designing effective fatigue risk management systems in Iranian hospitals, especially in emergency departments.
