Abstract
Background and Aim: Given the biomechanical differences between over ground and treadmill walking, it is expected that the contraction of muscles should be also different, so exercise training in these scenarios may have various effects on trunk muscles. The aim of this study was to compare amplitude and duration of activation of trunk muscles during over ground and treadmill walking to prescribe a suitable exercise mode considering the special circumstances of each person.
Materials and Methods: Nineteen healthy 20-40 years old male with sedentary life style were selected via simple sampling. Surface electromyography of rectus abdominis, external oblique, longissimus and multifidus muscles were recorded.
Results: In each gait cycle, there was not statistically significant differences on duration of trunk muscles activity between treadmill and over ground walking. The mean amplitude of these muscles was greater on treadmill in comparison with over ground and this difference was statistically significant for rectus abdominis (P=0.005), longissimus (P=0.018) and multifidus (P=0.044) muscles.
Conclusion: Stabilizing role of trunk muscles during walking causes them to be active throughout the entire gait cycle. The increased muscle amplitude on treadmill might show that more motor units may be recruited during the contraction, which is helpful in prescribing the appropriate type of exercise especially to patients who have trunk muscles weakness.

Abstract
Background and Aim: Intonation has important roles on increase in intelligibility and conveyance correct meaning especially in conversational speech. Recognition of intonation correlates with three acoustic parameters: fundamental frequency, intensity and duration. The purpose of the present study is to investigate the acoustic correlation of intonation and intelligibility of speech in children with cochlear implant (CI) and compare with normal hearing (NH) children.
Materials and Methods: This study has been carried out on 25 cochlear implant children and 25 children with normal hearing in primary school age ranges.Ten pictures were shown to children and statement and question sentences were elicited from them. To investigation of intelligibility, produced sentences were perceptually judgment by seven speech therapists and were scored by five point rating value. Acoustic correlationwere determined using Praat software.
Results: The results showed that there were significantly difference between two groups in each of acoustic correlations (P<0.05). Intelligibility scores of CI children were significantly lower than ones of NH children (P<0.05). There were significantly correlation between intonation and intelligibility scores, also between intelligibility and duration of implant use (P<0.05).
Conclusion: According to the findings of the present study, cochlear implant children's performance in using of acoustic correlation of intonation is lower than normal hearing peers that results in inappropriately intonation. Inappropriately intonation can be the cause of low intelligibility in cochlear implant children. Thus, intervention of intonation should be considered in treatment program of cochlear implant children.

Abstract
Background and Aim: Although motor difficulties are not considered as a diagnostic criterion in Diagnostic and Statistical Manual of Mental Disorders: However its association with Attention Deficit Hyperactivity Disorder (ADHD) is commonly reported. CO-OP is a problem-solving approach developed to help children with Developmental Coordination Disorder and addresses their motor-based problems. In this approach, therapist supports the children to use their cognitive strategies in a process of guided discovery to solve occupational performance problems. The objectives of CO-OP are to improve the motor performance of children with ADHD by helping them to discover various procedures to remove barriers and enhance supports by facilitating activities and participation. This study was designed to examine the influence of CO-OP on motor-based occupational performance of children with ADHD.
Materials and Methods: A single case experimental design (multiple baseline) was used to examine the influence of a 12 weeks intervention of CO-OP (twice a week) on Six children with ADHD. Each child selected three different goals to practice during intervention. Outcome measures included the Canadian Occupational Performance Measure (COPM), and the Bruininks-Oseretsky Test of Motor Proficiency and Performance (BOTMP). At first and prior to the intervention, the COPM was administered to the children and parents separately to obtain a rating of performance and satisfaction of the child in selective goals. The CO-OP intervention protocol was based on Polatajko and colleagues' work in 2001. During intervention, children were taught the global problem solving strategies helping them to discover and perform domain-specific strategies. Therapist and family also supported children to achieve the performance of selected goals.
Results: The results of this study demonstrated improvements in both goals and motor performance in the participants due to the intervention. These results support the use of the CO-OP with children with ADHD.
Conclusion: Further research into the application of CO-OP with children with ADHD is warranted based on preliminary positive findings regarding the efficacy of this intervention to address motor-based performance difficulties.

Abstract
Background and Aim: The effect of rocker soles on improvement of foot plantar pressure distribution is well documented. Rockers are commonly prescribed for diabetic patients with neuropathy in order to offloading the particular area of the foot sole thereby decreasing the chance of the foot ulcers. On the other hand, these patients may experience balance problems because of the somatosensory defects due to peripheral neuropathy. There is an evidence of postural destabilizing effect of rocker soles in healthy adults. The purpose of the current study was to investigate the postural responses to the rocker sole in diabetic patients with neuropathy.
Materials and Methods: Seventeen diabetic patients with neuropathy participated in this study. Canvas shoes were modified by addition of toe only rocker soles made of EVA (Ethyl Vinyl Acetate). The motor control test was conducted by the use of Neurocom system to evaluate postural stability with and without rocker sole. The Center of force displacement, response latency and response strength were measured.
Results: No significant difference was observed between the toe only rocker sole and without it in center of force displacement and response latency of the patients (P› 0.05). The results also indicated a significant increase in the response strength of patients by the rocker shoe, only in medium perturbations (P‹ 0.05).
Conclusion: The results of this study demonstrated that the postural stability of diabetic patients with neuropathy in response to the toe only rocker shoe is the same as the shoe without any rocker profiles. However, rocker shoes increase the need for generating active force during the automatic postural response of these patients which may indicate a training effect on diabetic patients with neuropathy.

Abstract
Background and Aim: Cerebral Palsy (CP) children are the most clients referred to occupational therapy settings. Hand function is important in Activities of Daily living .There is a hand dysfunction in most cerebral palsy children. Orthosis and splints are commonly used to improve the position, range of motion, quality of movement and function of a person's arm or hand. The aim of this study was to investigate the effects of static cock up c-bar splint on strength,spasticity, range of motion, and dominant hand function in spastic diplegic children with 8 to 12 years old.
Materials and Methods: This interventional study is in a before-after design. Thirteen spastic diplegic cerebral palsy children with 8 to 12 years old were selected through students who studied in physical disabled schools in Tehran city and had the inclusion criteria. Patients used a static cock up c-bar splint (wrist in 10 degrees of extension and thumb in palmar abduction and opposition) for two months, 2 hours daily and 4-6 hours at night. In this study the Jebsen Taylor test was used to evaluate hand function, the Modified Ashworth Scale was used to assess spasticity of thumb, wrist and elbow, ROM of thumb, wrist and elbow were tested with goniometer and power grip was evaluated with dynamometer.
Results : Results showed a significant improvement in hand function (p<0.001), in wrist`s spasticity (p<0.001), elbow`s spasticity(p<0.008), palmar abduction of thumb spasticity (p<0.002), wrist`s ROM (p<0.001), Palmar abduction of thumb ROM(p<0.001) and power grip(p<0.001). The data did not show significant improvement on elbow`s joint ROM.
Conclusion: Information from present research shows that using static cock up c-bar splint for 2 months, 2 hours a day and 4 to 6 hours nightly, can be an effective method to improve power grip, hand function, thumb, wrist and elbow spasticity and range of motion of thumb and wrist of spastic diplegic CP children.

Abstract
Background and Aim: Impairment of noun retrieval is common among aphasic patients, but some of previous studies represent differences between noun retrieval ability of various aphasia syndromes. Current study investigates and compares the picture naming ability of 4 patients with fluent and non-fluent aphasia.
Materials and Methods: Present study investigated the naming ability of 2 fluent and 2 non-fluent aphasic patients with oral picture naming test. This test contains line drawing of 109 nouns. Patients were asked to name each picture with a single word. Responses were classified according to Philadelphia Naming Test scoring system as correct and semantic, formal, mixed, non-word, unrelated and others error (no response, description/circumlocution, miscellaneous error). Then, the data were investigated in terms of descriptive statistics and analyzed by Mann-Whitney U test.
Results: The score of non-fluent aphasics was more than fluent ones. The percentage of semantic errors in fluent group and no responses among non-fluent patients were more than the other types. Despite of personal differences, all error types were found in responses of patients. The difference between score and error types in fluent and non-fluent patients was not significant (p .05).
Conclusion: This scoring system can define a variety of word retrieval errors. The fluent aphasics have more noun retrieval problems than non-fluent patients. Although, there are differences between two groups, but error type in picture naming is not a precise factor for distinguish between various aphasia syndromes and there is similarity in impaired underling mechanisms and naming behavior of them.

Background and Aim: In studies of the spinal circuits, change of Renshaw cell activity is measured
by H´ response. For recording H' response, new electrical stimulation device was designed and built
and the first recorded H' response in Iran was done. The purpose of this study was to investigate the
intersession and intra-session reliability of peak amplitude of H´ response and its related intensity
detected of new device. Materials and Methods: Soleus muscles of dominant extremity in 6 subjects were tested in 3 sessions
and 2 times in each session that intra-session interval was 20 minutes and intersession interval was 1
week. Results: Peak amplitude and its related intensity exhibited intra-session and intersession reliability
0.92, 0.98, 0.86 and 0.9 respectively. Standard Error of Measurement was 0.37 and 0.47 for intrasession
and 0.44 and 1.06 for inter session respectively. Conclusion: Peak amplitude and its related intensity exhibited very high intra-session and intersession
reliability.

Abstract
Background and Aim: Children's participation in leisure and recreational activities is associated with their well-being. The Purpose of this study was to compare of participation between children with cerebral palsy and typically developing peers 8-14 years old.
Materials and Methods: In this cross sectional study, 30 children with cerebral palsy (mean age: 11 years and 1 month) from special education school and 30 typically developing peers (11 years and 6 months) from typical schools through convenient nonprobability sampling were selected. Participants were evaluated with the Children's Assessment of Participation and Enjoyment (CAPE).The data were analyzed by spss17.
Results: There were significant differences between children with cerebral palsy and typically developing peers (p= 0.000) in diversity scale. The intensity of participation in peer group was more than children with cerebral palsy )p=0.000). No significant differences were found between the groups in the enjoyment. There were no significant main effects of gender, also significant interaction effect between group type and gender for CAPE variables.
Conclusion: The participation of peer group in most activities was more than children with cerebral palsy. Physical disability can affect participation of children in everyday activities and socialization. Knowledge about participation of cerebral palsy children can help to health care professional to establish interventions that fit with children's preferences and can promote their health and participation.

Abstract
Background and Aim: Neck pain is one of the most common musculoskeletal disorders in industrial society. There are complex changes in motor control of cervical spine in neck pain patients. The aim of this study was to compare the latency of activation of neck muscles during upper limb movement between neck pain patients and healthy subjects.
Materials and Methods: Eighteen chronic non-specific neck pain patients and eighteen healthy subjects who were matched in age, weight and height participated in this study. Subjects were standing on the ground and performed flexion and abduction movement of the right arm until at least 90 degree in five trials and consequently surface electromyography of neck muscles was recorded. Activation time of neck muscles was determined in comparison with the activation time of deltoid muscle (as an event) and then compared between two groups.
Results: Findings of this study showed that in patients with chronic neck pain and healthy subjects during upper limb flexion and abduction movements, the superficial neck muscles were activated after deltoid activation. As well as in the neck pain patients compared to the healthy subjects superficial neck muscles onset was significantly appeared with delayed (P<0.05).
Conclusion: In patients with neck pain compared to healthy controls, the neck muscles were significantly delayed in responses to the internal perturbations of fast upper limb movements. It may be to increase the risk of injuries of neck region during fast upper limb movements.

Abstract
Background and Aim: More recently, attention has turned to the role of proximal structures in function of lower limb. Decreased lumbo-pelvic (or core) stability has been suggested to contribute to the etiology of lower extremity sport injuries. This is largely due to the closed chain nature of athletic activities. Groin pain is a common complaint for athletes participating in sports such as soccer. Adductor-related groin pain was the most common primary entity in soccer players. Therefore, the aim of this study was to compare of core stability between male soccer players with and without hip adductors strain. Various studies have been done for treatment and prevention of recurrent of this lesion and the most important thing in these studies was to keep the integrity of the core stability. Materials and Methods: Fifty professional soccer players (25 healthy and 25 with adductors strain. mean age: 21.32±2.73) was studied. Core stability was compared among them by 5 endurance and functional tests. Endurance of back extensors, trunk flexors and lateral flexors were tested by Modified Biering-Sorensen and Mc.Gilltests.In current research, the Double Leg Lowering test is applied to measure the function of the oblique abdominals muscles and the Single Leg Squat test is applied to measure the function of hip abductor and external rotators muscles. Results: Endurance of back extensors, flexors and lateral flexors were significantly lower in the injured soccer players in comparison with the healthy players. Significant decrease in the function of oblique abdominal muscles, hip abductor and external rotator muscles were founded in the injured players (p<0.05). Conclusion: Decrease of endurance and function in the trunk muscles is probably an important risk factor for hip adductors strain injuries in the soccer players. Increasing the endurance of the core muscles can be reduced the occurrence of hip addactors strain.

Abstract
Background and Aim: Diabetes has world wide prevalence and it is estimated that up to ten years later, more than 300 million people would suffer from it. The disease has serious secondary complications. Physical activity can decrease the complications by altering the blood profiles. Patients with diabetes are unwilling to do physical exercise. Whole Body Vibration (WBV) might be a proper substitution for exercise. The effects of WBV on patients with diabetes have not been studied enough, so the aim of this study was to evaluate the effects of WBV on the blood profiles in patients with diabetes type 2.
Materials and Methods: Twenty patients with diabetes were assigned using matched assignment either into control (n=10) or intervention (n=10) group. Intervention group received the WBV with frequency of 30 Hz and amplitude of 2.5 mm for six weeks twice a week. The control group did not receive any intervention during this period. Blood profiles were measured in both groups at the baseline and the end of the study.
Results: FBS and HbA1C decreased in intervention group in comparison with the control group but the differences were not statistically significant (P=0.44). There were not significant statistical differences in Cholesterol, HDL, LDL and TG before and after the treatment between the two groups.
Conclusion: The application of WBV with the above parameters might reduce FBS and HbA1C in patients with diabetes type 2. Increase in the amount of Glucose Transporter protein 4 (GLUT-4) is one of the contributing factors that these changes can be ascribed to.

Abstract
Background and Aim: The translation, cultural adaptation and validation of the Persian version of the American shoulder and elbow surgeons (ASES) were carried out in accordance with published guidelines.
Materials and Methods: The study was conducted on 100 athletes (25.96±5.81 years), which suffered from different shoulder disorders. When the translation and cultural adaptation of the original questionnaire was carried out on the standardized method, the participants were asked to complete a questionnaire booklet including the DASH, the Short Form General Health Survey (SF-36) and ASES questionnaires. In addition, 30 randomly selected patients were asked to complete the ASES questionnaire 48 hours later for the second time. After data collection the Statistical analysis was performed by using SPSS software. Results: The Cronbach's alpha was used to indicate internal consistency. Cronbach's alpha was 0.91 for ASES. Test-retest reliability was quantified by use of the intraclass correlation coefficient. It was 0.91 for ASES questionnaire. The Pearson Correlation Coefficient was used for the questionnaire validation study. The DASH questionnaire showed high correlation of ICC=0.78 with ASES. The ASES showed moderate correlation of 0.48 to 0.62 with those of the various scales of the SF-36.
Conclusion: The Iranian ASES showed good reliability and validity and can be used for shoulder-specific patient self-assessment.

Abstract
Background and Aim: Rehabilitation interventions have significant role in reducing the disabilities of Down syndrome. Due to great role of balance in the function of individuals particularly in Activity of Daily Living and the effect of strength on balance, the aim of this study was to investigate the effects of Progressive Resistive Exercise on functional balance in children with Down syndrome. Material and Methods: Fifteen girls children aged between 8 and 12 years, with Down syndrome participated in this study by available sampling method. The subjects participated three times per week in a six-week progressive strengthening program. During intervention, 3 of them excluded. Balance were measured by Berg Balance Scale and right/left hip abductor and flexor, knee extensor and flexor muscles isometric strengths were measured by Nickolas hand-held dynamometer. Paired-sampled T-Test was used for data analysis. Results: The results of this study showed that the mean of functional balance and muscles isometric strengths were significantly increased after intervention (P<0.001). Conclusion: It seems that the results of this study confirm the effect of progressive resistance training on progressing functional balance, and 8-12 years old children with Down syndrome can benefit from these exercises.

Abstract
Background and Aim: This study was performed to compare the stiffness of hamstring and rectus femoris muscles in subjects with anterior knee pain and normal subjects.
Materials and Methods: In this non experimental and case-control study, 20 persons with anterior knee pain were selected simply from convenient sample and 20 healthy persons were selected by matching method. proximal and distal stiffness of hamstring and rectus femoris muscles were measured using an isokinetic device in the passive mode. Also, maximal concentric peak torque during flexion and extension movements of hip and knee, in two groups were compared by independent T-test and correlation analysis.
Results: There was no significant difference between anterior knee pain and proximal compartment of rectus femoris(P=0.320) and hamstring(P=0.125) stiffness, but there was significant difference between anterior knee pain and distal compartment of rectus femoris and hamstring stiffness (P<0.001). Significant relationship was found between anterior knee pain and muscles strength (P<0.001). No correlation was found between strength and stiffness of rectus femoris (P=0.356) and hamstring muscles (P=0.643) in patients group, but in healthy subjects this relationship was found (P<0.001).
Conclusion: In patients with anterior knee pain, hamstring and rectus femoris muscles stiffness increased and muscle strength decreased. Increasing of muscle strength was effective on stiffness in healthy subjects, but there was no relation between them in patients group. Also rectus femoris and hamstring muscles possibly have different proximal and distal neuromuscular compartments.

Abstract
Background and Aim: Multifidus muscle is one of the short and deep posterior Paraspinal muscles, which gives stability to the spine during various activities. Studies have shown that in people with neck pain, weakness and atrophy of neck muscles are major factors in the incidence of neck pain. Measurement of muscle's dimensions by ultrasonography provides an opportunity to be able to objectively assess muscle atrophy or hypertrophy. This study aimed to assess symmetry of cervical multifidus muscle size in females with chronic non-specific neck pain and healthy by ultrasonography apparatus. Materials & Methods: Twenty five women with unilateral chronic nonspecific neck pain and 25 healthy women were participated in this study. All subjects were employed and more than 4 hours working with computers or paying office in a day. Imaging of the cervical multifidus muscles were done bilaterally in the level of the fourth vertebra. Anterior-Posterior Dimension(APD), Lateral Dimension(LD), Cross-Sectional Area (multiplied of two diameters(APD&LD)) and the Shape ratio (Lateral diameter divided by the Anterior-Posterior diameter(LD/APD)) of the muscle were measured and recorded. Results: Multifidus muscle size in patients group was smaller than the healthy group. In patients group, size of multifidus muscle in the painful side was smaller than the opposite side. Asymmetry of muscle size between the two sides, in the patient group was higher than the control group (P < 0.05). The ratio of smaller size to the larger size of the muscle, in the healthy group was higher than the ratio in the patient group (P < 0.05). Conclusion: In patients group, asymmetry of the muscle size between two sides, also smaller size of the muscle in the painful side, showed that muscle atrophy has happened in the involved side.

Abstract
Abstract Background and Aim: The human needs speech for making relationship, he achieves speech with linguistic structure including sound, word and more important than other the sentence. In Persian grammar, there are different types of sentences including: imperative, question, active, passive, exclamation. A childe makes sentence with single-word, telegraphic speech and then with composing names and verbs and propositions in his stage of growing and finally he expresses the passive, conditional, compound sentences. Because of no previous study has been done about the age of comprehension passive sentences in Persian language so we decided to run the comprehension passive sentences on normal children. And we got the age of growth about it in normal children then we compared it in hearing impaired children. The aim of this study was to compare for comprehension passive sentences in normal children 3 to8 years old with hearing impaired children 8years old. Materials and Methods: A total of 691 normal children 3 to 8 years old in central Tehran and 18 hearing impaired children in special schools participated in this test. At first the test was conducted on 30 children, and proved its reliability. This test had 15sets of three pictures and child must show one of those three pictures. At first, two sets of three pictures were performed to child as example and practice then the passive sentence was expressed about it Colloquial and child must show it and if he/she couldn’t ,the examiner showed the correct picture and showed next picture as be offered to children should be familiar with the test. The child was described that he/she looked at pictures carefully and showed the correct picture after reviewing all the pictures. In continue, 15 pictures of this test were executes and examiner recorded the results. Results: The highest number of correct responses were for normal children 8years old (Average: 13.93) and other records were in order for children 7 years old (Average: 12.51), 6 years old (Average: 9.37), 5 years old (Average: 8.09), 4 yeas old (Average: 5.31), 3 years old (Average: 3.61) and the lowest record of correct responses was for the hearing impaired children. The result of this study showed that there is a significant relationship among the ability of comprehension passive sentences and age, birth order, parents education (p<0.05) but there is no significant relationship between ability of comprehension passive sentences and their gender (p>0.05) and there is a significant deference between the average of percentage of responses and comprehension passive sentences in hearing impaired children in all the range of ages except 3 years old (p=1.000). It means that hearing impaired children in comprehension passive sentences are similar to children 3 years old. Conclusion: The result of this study showed there is a significant relationship among the ability of comprehension passive sentences and age, birth order, parents education but there is no significant relationship between ability of comprehension passive sentences and their gender. The results of this study showed the ability of comprehension passive sentences in hearing impaired children 8 years old is like normal children 3 years old too. Abstract
Abstract
Background and aims: One of the complications of breast cancer treatment is ipsilateral upper extremity lymphedema. Secondary lymphedema is a chronic condition that leads to loss of function and disorder of Quality of life. The aim of this study was to evaluate the effects of Complex Decongestive Physical Therapy (CDP) on pain severity and quality of life in patients with secondary upper extremity lymphedema after breast cancer treatments.
Material and methods: A total 30 women (age 29-70 yrs.) with secondary lymphedema after breast cancer treatments participated in this study. They were referred by general and cancer surgeons to Physical Therapy clinic based on inclusion and exclusion criteria. They were treated with complex decongestive physical Therapy. This protocol comprises of manual lymphatic drainage (MLD), compression garments, remedial exercise and skin care for 4-weeks, once per day, 5 days per week. Quality of life and severity of pain were evaluated before and after treatment. The quality of life was assessed with QLQ-C30 questionnaire. The severity of pain was also assessed by Visual Analog Scale (VAS).
Results: After 4 weeks treatment severity of the pain are decreased after treatment. The physical and social roles of quality of life (QLQ-C30) were significantly increased (p<0000).
Conclusion: Based on the results of this study, utilization of complex decongestive physical therapy (CDP) In upper extremity lymphedema is a useful method for pain relief and increment of quality of life.
