Abstract

Background and Aim: Acute promyelocytic leukemia (APL) is associated with the t(1517) ,fusing promyelocytic leukemia (PML) and retinoic acid receptor-a (RARa) genes. This disease is uniquely sensitive to treatment with all-trans retinoic acid (ATRA) and highly responsive to conventional chemotherapy. The t(1117)(q23q21) abnormality associated with a PLZF-RARa rearrangement is the commonest of the alternative translocations accounting for less than 1% APL. Blasts from PLZF-RARa cases have been found to be resistant to the differentiating effects of retinoids. In this study we aimed to determine the PLZF-RARa and fusion genes in patients with APL morphology who referred to Hematology-Oncology and BMT research center, Tehran Shariaty Hospital in 2006.

Materials & Methods: Peripheral blood and/or bone marrow samples were taken from 200 patients with APL morphology and 200 patients with other subtypes of AML. The mono-nuclear cells were enriched by centrifugation over a ficoll-isopaque gradient. RNA extracted by Trizol or TriReagent and then reverse transcribed to cDNA using random primers. PCR performed using specific primers for each fusion. PCR products electerophoresed on a 2% agarose gel containing 0.05% ethidiume bromide.

Results: In 2 (1%) patients with APL-morphology the RT-PCR analysis showed PLZF-RARa fusion transcripts.

Conclusion: It can be concluded that RT-PCR is a rapid and sensitive method for detection of abnormal fusion genes in leukemia and allows the definition of a correct strategy for treatment and subsequent minimal residual disease monitoring

Abstract

Background and Aim : Myeloproliferative neoplasms are clonal and heterogeneous disorders of hematopoietic stem cells lead to increase of one or more cell lines in the blood. Recently, the acquired mutation JAK2 V617F has been described in the majority of patients with myeloproliferative neoplasms (MPNs).This mutation is characterized by a G to T transverse at nucleotide 1849 in exon 12 of the JAK2 gene, located on the chromosome 9p, leading to a substitution of valine to phenylalanine at amino acid position 617 in the JAK2 protein. The aim of this study was to assess the prevalence of JAK2 mutation in MPN patients.

Materials and Methods: In this experimental study we evaluated JAK2 mutation in 58 patients with MPNs by simple randomized sampling. The mutation was detected by ARMS-PCR in patients.

Results: The JAK2 V617F mutation was detected in 86.6% (26/30) of patients with polycythemia Vera, 46.6% (7/15) of patients with essential thrombocythemia and 61.5% (8/13) of patients with idiopathic myelofibrosis. Polycythemia Vera patients carrying the mutation displayed a higher levels of WBC (p=0.03) and 61.5% (16/26) of these patients were females. The differences in other groups were not significant. The mutation was confirmed by sequencing.

Conclusions: Our Results show similarity with other studies. Thus, ARMS-PCR can be applied as differential diagnosis test for detection of JAK2 mutation in suspected patients with MPNs.

Abstract

Background and Aim: APL is a Prevalent leukemia that Approximately included 5-10% of patients with acute myeloblastic leukemia. ATRA and recently arsenic is used for treatment. ATRA leadsto resistance to treatment and arsenic is toxic in high doses.AZT induce cell death in different ways. The purpose of this study was Assessment of effect of AZT, a telomerase inhibitor, on NB4 cell line (APL cell line) to reduce toxic effect of high dose arsenic.

Materials and Methods: In this study, viability and metabolic activityof NB4 cells, treated by different concentrations of AZT(50,100,200 µM), was assessed by trypan blue dye method and MTT assay respectively.

Results: Treated cells with AZT=50,100,200µM showed decreased viability, both in dose-dependent and time-dependent through trypan blue dye method and decreased cell metabolic activity by MTT assay.

Discussion and Conclusion: Considering that AZT is able to induce apoptosis and decrease cell activity, it seems AZT is a suitable drug for inhibiting the growth of tumor cells.

Abstract

Background and Aim: The balance between ROS generation and antioxidant activity is critical to the pathogenesis of oxidative stress related disorders. In this study the prooxidant - antioxidant balance and its correlation with lipid profile and uric acid was determined to evaluate the PAB as a prognostic factor for CAD.

Materials and Methods: Seventy - two patients and sixty eight healthy volunteers were enrolled in this study. The values of PAB were determined by using standard solutions and ELISA method.
Triglyceride, total cholesterol, LDL- cholesterol, HDL - cholesterol and uric acid were measured by enzymatic method.

Results: The PAB values of CAD patients and control group were 70.01±3.36 (HK unit) and 66.40 ± 2.84 (HK unit) respectively. There was no significant difference between PAB values among the two groups (P= 0.41). There was no significant difference between uric acid levels among the two groups (P= 0.46). There was a significant correlation between the uric acid values among patients and healthy volunteers and PAB values (P <0.01). There was a significant correlation between the TG, values among patients and healthy volunteers and PAB values (P<0.05).

Conclusion: This study showed oxidative stress could be used as a significant risk predictor in the coronary artery disease patient

Abstract

Background and Aim: Acute promyelocytic leukemia (APL) is a distinct subtype of acute myeloid leukemia. APL is characterized by a balanced reciprocal translocation between chromosomes 15 and 17, t(517)). Important therapeutic strategies for this disease are ATRA and Arsenic trioxide. To eliminate tumor cells with arsenic, high dose of arsenic is needed. But high dose is toxic for normal tissue. The purpose of this study is Assessment of effect of low dose of arsenic trioxide in combination with AZT on NB4 cell line (APL cell line) to reduce toxic effect of high dose arsenic.

Materials and Methods: In this study after NB4 cell line culture and proliferation, the cells treated with low dose of arsenic trioxide(0.5µM) in combination with different doses of AZT(50, 100, 200 µM) and then viability and metabolic activity was assessed by try pan blue and MTT assay respectively.

Results: Low dose of arsenic (0.5µm) alone and in combination with dose of 50µm of AZT has little effect on viability and metabolic activity but in combination with higher dose of AZT has significant effect on viability and metabolic activity and both viability and metabolic activity significantly reduced.

Conclusion: Different apoptosis- induced mechanisms cause apoptosis by arsenic and AZT. Since some of these mechanisms between AZT and arsenic are similar, so maybe these similar mechanisms cause synergic effect and significant reduction of viability and metabolic activity in combination of these two drugs.

Abstract

Background and Aim: Internal Evaluation is a process of providing evaluative-descriptive information based on the value and significance of educational objectives, operations and results to direct decisions, responses, information and research processes.

Materials and Methods: In this research, studying the opinions of faculty members, students, and graduates, the educational system of Medical Laboratory Sciences Department at Tehran University of Medical Sciences (TUMS) was evaluated in the year 2010. The aim the study was to remodel and adjust the educational system with the society's needs and to promote the educational quality. The employed method was a cross-sectional, descriptive one and consisted of 10 procedural steps in eight sections dealing with the evaluated factors such as faculty members and students, and the satisfaction expressed by students.

Results: The results of the study showed that the average of the eight investigated sections was 70.6% for the Department of Medical Laboratory Sciences, which can be interpreted as desirable.

Conclusion: Internal evaluation can be considered as one of the best methods of improving the quality of education in the faculty of Allied Health Sciences School

Abstract

Background and Aim: Human Giardisis infection is caused by the flagellate protozoa. Giardia lamblia, which lives in the small intestine, causing damage and may also cause gastrointestinal symptoms. This parasitic disease has a worldwide distribution and prevalence varies from 5% to 30%. The role of both humeral and cellular immune response in the host defense against parasites has proven. Since an immune response is directly affected by cytokine, study of cytokines changes in patients with giardiosis is of particular importance.

Materials and Methods: In this study the serum levels of IL-2 - 4 - 6 to 17 and 23 in patients and healthy subjects were measured and compared. Fasting blood samples were taken from the volunteers, ELISA was performed to measure cytokines.

Results: In this study, the amount of IL-2 and IL-6 in infected patients was significantly more than the controls, but IL-4 levels were significantly lower in infected individuals (P=0.0001). Also for the first time in people with giardiasis IL-17 and IL-23 was measured and there was a significant aifference in these cytokines between the infected patients healthy controls(respectively P=0.044 and P=0.03)

Conclusion: Host defense response against parasitic infections, is directly and affected by cytokines. Increased interleukin 2, 6, 17 and 23 in patients with Giardiosis was probably caused by immune response and local intestinal inflammation.

Abstract

Background and Aim: Achievement-based evaluation is an important part of the educational planning process, in which learners are screened besides, it helps teachers in their self-evaluation. The purpose of present study is to survey the methods through which laboratory sciences students were evaluated by their professors at Tehran University of Medical Sciences(TUMS) in the academic year 2009-2010.

Materials and Methods: In this cross-sectional study, the population consisted of the faculty members who teach laboratory sciences courses. Data collection instrument was a researcher-made questionnaire. The collected data were analyzed to describe all evaluation methods using tables and necessary distribution frequencies. Then, the results were compared with those of Medical Planning Council resolutions.

Results: The results show that mid-term exams were used for the evaluation of students in 35% of all theoretical and practical courses. Besides, of the evaluation methods used, multiple-choice questions occupied the first rank(70%) among the methods normally employed.

Conclusion: According to the results, routine evaluation of undergraduate students of laboratory sciences at the Paramedical Faculty of Tehran University of Medical sciences not only conformed to the ratifications of the Supreme Council of Medical Planning, but also was done more extensively and with a higher degree of consistency in the academic year 2009-2010.

Abstract

Background and Aim: Oxidative interactions such as the formation of oxygen, peroxy radicals and LDL-cholesterol oxidation are involved in the development of atherosclerosis process This study aims to examine the relationship between serum bilirubin levels and the incidence of coronary artery disease.
Materials and Methods: Eighty-five patients and ninety-two healthy volunteers were enrolled in this study. Total and direct bilirubin levels were measured using diazo method. Besides, triglycerides and total cholesterol were determined by enzymatic method, HDL-Cholesterol by polyanionic method, and LDL-Cholesterol by direct method. For statistical analysis of data, SPSS 17 was applied. For qualitative variables, Chi-square and for quantitative variables, t-student tests were used. The significance level was set at P=0.05.
Results: Direct, indirect and total bilirubin levels were 0.213, 0.375, 0.588 mg/dl for control group and 0.228, 0.365, 0.593 mg/dl for patient group, respectively. No significant difference was observed between the mean values for direct, indirect and total bilirubin in the two groups. Furthermore, there was no significant difference between triglycerides and total cholesterol level figures in the two groups. However, there was a significant difference between HDL-Cholesterol levels (P=0.001), smoking (P=0.031), family history (P=0.006), and mean blood pressure (P<0.001) of the two groups.
Conclusion: The results of this study indicate that measurement of bilirubin as a marker for predicting coronary artery disease may be important. In the end, it should be mentioned that the findings of this study are consistent with some previous studies, but incompatible with others in this area.

Abstract

Background and Aim: Medical Laboratory Sciences is one of the important fields in Medicine that is closely associated with other disciplines in this group. This study aims to evaluate laboratory sciences curriculum in Iran and several countries worldwide.

Materials and Methods: In this research, data collection is based on documents and curriculum goals of countries. The data collected was analyzed using qualitative methods.

Results: Comparative study of curriculum courses of bachelor of Laboratory Medical sciences was done and compared with the number of basic and specific units in Iran and other countries.

The curriculum in Iran with Turkey and Australia is remarkably consistent. Internship units in hospitals have lower returns in compared to other universities in the world.

Conclusion: Laboratory Science curriculum in Iran is matched with developing countries. However, more attention to training units and projects to improve the quality of teaching laboratory sciences could be more effective.

Abstract

Background and Aim: Giardiasis is the most common pathogenic gastrointestinal parasite worldwide and transmitted by ingesting parasite cyst, through water and food and person-to-person contact. Therefore in food handlers and child care workers it is of high importance. In this study we surveyed the job of individuals with giardiasis.
Materials and Methods: This study was a pilot descriptive-analytic study. Hundred and nine stool samples were collected from individuals referred to medical centers affiliated to Tehran University of Medical Sciences for health certificate. 
Samples were examined with wet mount smear and formol-ether concentration methods. Also a questionnaire was given to each individual.
Results: Sixty nine samples were infected with Giardia lamblia. among positive samples for Giardia lamblia 25 (36.23%) were related to food handlers and occupations which are most  frequently associated with food, one (1.44%) nurse, one (1.44%) managing director of the nursery, two (2.89%) housekeepers  and one servant (1.44%).
Conclusion: Person-to-person Giardia lamblia transmission is often associated with poor hygiene and sanitation, most commonly by food handlers.
Also, day- care workers have a higher risk of infecting children attending day-care centers. Therefore particular attention should be paid to health of this workers as well as three times stool examination.

Abstract

Background and Aim: Mobile-based programs have been developed as tools to help both patients and physicians in various fields especially in dermatology. Therefore, the main objective of this study was to review the features and contents of dermatology applications.
Materials and Methods: The methodology was comparative and descriptive. Applications in the field of dermatology were evaluated and compared through this research. Inclusion criteria included applications that have been downloaded more than 100 times in Google Play and App Store and applications designed in diagnosis in various fields of dermatology such as treatment, management, remote consulting, and self-care areas. Exclusion criteria included those developed before 2010 and those related to the non-dermatology areas. Besides, different features were considered for comparison based on literature review and expert consultation. Next, the recognized applications were reviewed and compared based on determined categories.
Results: Based on criteria, a total of 33 applications were identified through searching. Of these, 33.3% of Apps were in the field of education, and 24.2% were in self-care. Regarding disease, applications were categorized into nine different domains. Of these, 61% of applications were covering different types of skin diseases. Also, in examining the frequency of the features of the evaluated programs, providing recommendations and suggestions with 57.57% and educational contents and the possibility of uploading images with 51.51%, respectively, had the highest frequency among the features of the programs.
Conclusion: Analysis showed that education and self-care domains have a high rank among others. It indicates that developing such applications could facilitate patient education and self-management by himself or caregivers. However, this area needs more attention and the using health information technology capabilities to make applications smarter in this area.

Abstract

Background and Aim: Medical diagnostic laboratories are among the most important centers in the treatment cycle of patients. Today, the conscious choice of such laboratories is one of the challenges that patients face in the treatment process. This study was conducted with the aim of improving the knowledge of software users in the field of laboratory sciences and also facilitating the conscious and intelligent selection of the laboratory required by users. 
Materials and Methods: This is a descriptive-developmental research with an applied approach. The steps consisted of library studies, questionnaire-based needs assessment, collection of knowledge and identity data, design through drawing UML diagrams, implementation using Java programming language, and software evaluation.  
Results: A comprehensive system of laboratory information and experiments can be performed in all laboratories in Tehran, based on factors such as location access, types of laboratories and types of tests, a system was designed that allows users to access the most appropriate laboratory centers with high speed and less mobility, sufficient information, and in accordance with their needs. The evaluation was done using a researcher-made questionnaire whose validity and reliability were confirmed. The target population consisted of eleven specialists and forty ordinary users. According to the Likert criterion, the results obtained from the answers of all participants in the study to the questions of the questionnaire were higher than 4.05.  
Conclusion: The software showed that the factors that had priority in the need assessment significantly increased user satisfaction and also provided ease of use of laboratory services in accordance with users' needs.

Abstract

Background and Aim: Transgenic species are the ones whose genomes are genetically modified. The transgenic field is one of the areas that has a high importance and position in the world. Therefore, the aim of the present research is to draw and analyze the co-authorship network of researchers in transgenic subject area.
Materials and Methods: The type of this research is descriptive and was carried out using scientometric techniques such as co-authorship network and social network analysis indices. In this study, 23,456 articles by transgenic researchers indexed in the Web of Science database during the period 2010-2019 were retrieved. VOSviewer and UCINET software were used to draw the co-authorship map and analyze the network indicators.
Results: The scientific cooperation network of transgenic researchers was studied and analyzed using macro and micro indicators of the social network. The status of macro indicators was not appropriate and the network was poorly cohesive. So that, the network density was 0.027, the clustering coefficient was 0.834, the diameter was 15 and the average distance was 4.155. In terms of micro-indicators, the status of researchers in the network was determined in such a way that David Ayares had the most cooperation with other members and also Nam-Hai Chua played the most important role in communicating with people from different clusters of the network, finally, Yan Zhang had the shortest distance with other members of the network.
Conclusion: Based on the findings, it can be concluded that the co-authorship network of transgenic researchers has low cohesion and information is transmitted among members at a low speed. With respect to this, status of different researchers in this study was determined, the results of this study can be used to guide future collaborations, and encourage universities and scientific institutes to develop their interactions with each other and further strengthen collaborations. It should also be noted that according to the findings of the study in this field, Iranian researchers were not identified as key individuals in this network, which requires research on the status and position of Iranian researchers in the field of transgenics.

Abstract

Background and Aim: Diabetes is a metabolic disorder characterized by an elevated blood glucose level, resulting from impairments in insulin action, insulin secretion, or both; which causes abnormalities in the metabolism of carbohydrates, protein, and lipids. Chronic hyperglycemia is associated with long-term damage, dysfunction, and failure of various organs. Adropin and irisin are newly described proteins that can be an essential component in the pathophysiological pathways of diabetes mellitus. The current study was designed to evaluate Irisin and Adropin biochemical markers in patients with type 2 diabetes mellitus and their correlation with risk factors.
Materials and Methods:  A case control study, that included 90 patients of type 2 diabetes mellitus and 90 healthy individuals, who matched for both age and sex with patients. Fasting blood sugar (FBS), HbA1c, serum insulin, total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), and triglycerides (TG), irisin and adropin were measured at the chemistry laboratory of AL-Faihaa teaching Hospital by standard methods.
Results: Serum irisin (8.154±1.642 vs. 14.06±3.916 ng/ml) and adropin (25.39±8.897 vs. 59.43±8.768 pg/ml) levels were significantly lower in the patient group than in the control cases, respectively (P.value<0.0001). Serum adropin levels were significantly and positively correlated with age (r=0.236, P=0.025) and negatively with BMI (r=-0.209, P=0.048). While, serum irisin levels were significantly and negatively correlated with TG (r=-0.248, P=0.018). Based on ROC analysis, the AUC for irisin was 0.937 (95% CI: 0.906-0.969), which showed a sensitivity of 91.1% and a specificity of 80.0% at the cut-off of 9.715 (P<0.0001). In addition, the AUC for adropin was 0.991 (95% CI: 0.980-1.00), which showed a sensitivity of 100.0% and a specificity of 91.1% for this biomarker at a cut-off of 37.945 (P<0.0001).
Conclusion: Our findings showed that the serum levels of irisin and adropin were lower in the patient group than in the control group. Probably, the reduction of adropin and irisin may be used as a biomarker to predict the risk of T2DM, which requires more studies in this regard.
