Abstract

Background and Aim: Analysis of efficiency in hospitals, as the most expensive operating unit of the health system, is very important. This costly unit has several departments that one of them is medical records. This study is trying to analyze the efficiency of medical records department for the enhancement of using medical records facilities.

Materials and Methods: In this study, technical, scale and managerial efficiency of medical records department has been studied by data envelopment analysis method, and assuming the scale efficiency variable and the input axis. Information used in this study was the input variables (number of technician personnel and lower and Master of Science personnel and higher), output variables (number of emergency and outpatient and inpatient medical records and the number of filing unit visitors to respond) during 2007-2009.

Results: The average technical efficiency of medical records departments is 84 percent, that indicates there is at least 16 percent capacity for an increase in performance. Average of management and scale Performance in units during the 3 years of study are 95 and 89 percent respectively. Most input surplus is relevant to technician and lower personnel. The average performance of these units in general hospitals is 80 percent and in specialized activities is 92 percent.

Discussion and Conclusion: Relationship between studied indicators in this study with performance medical records departments showed that using such as employee personnel with undergraduate degrees and higher and educated in medical records filed, the use of managers with enough experience in this department and more use of computer systems for unit activities can fill 16 percent capacity of performance improvement in these units.

Abstract

 Background and Aim: Managers are always looking for better ways to improve productivity of staff. The aim of this study was to introduce a model based on work and time measurement for Staff Management based on performance in hospital.

 Materials and Methods: This descriptive - practical study was conducted in ten steps for a medical documents expert’s in a general hospital. The main activities of employees were determined via interview and direct observation. Then expected time, tolerances, final coefficient and work units for each activity were calculated. Finally, the total working units were calculated. 

 Results: Implementation of introduced model for a medical documents expert’s in a general hospital showed that the total work units which the staff was obliged to do in one month was 918 units and the remaining (1160-918) was the extra units.

 Conclusion: Using appropriate models for the performance management of hospital staff workloads, can help to improve the hospital productivity and staff satisfaction.

Abstract

Background and Aim: The Household financial contribution is one of the most important issues in health system. This study aims to determine and present some indices of h ousehold financial contribution in health system in Iran.

Materials and Methods: In this cross sectional- descriptive study, the methods were introduced by World Health Organization. Fair Financing Contribution Index (FFCI), catastrophic health expenditure and impoverishment in Iran were calculated from a national representative data derived from Iranian Statistics Center, Household Budget Survey, and Consumption Expenditures, at 2011. The indicators have been calculated on the basis of the total cost analysis, health and capacity to pay Iranians households.

Results: Household financial contribution in the health system for households living in rural areas is more than urban areas. In this study, more than 3 percent of the households were facing catastrophic health expenditure . Also 1.5 percent of the households have been impoverished. Fair Financing Contribution Index in rural and urban areas was 59 and 65 percent respectively.

Conclusion: Calculation of the indicators related to households financial contribution in Iranian health system need to predict and implement appropriate plans, as well as monitoring and assessment of health system programs in order to increase the equity in the household financial contribution and effectiveness of Utilization of resources.

Abstract

Background and Aim: Considering the scope of tasks and the role of teaching hospitals in the promotion of population health, it is important to consider their responsiveness as one of the three goals of health system. The aim of this study was to evaluate the level and relative importance of responsiveness domains in teaching hospitals from the viewpoint of households living in selected areas of Mashhad.
Materials and Methods: In this cross-sectional study, a valid questionnaire designed by World Health Organization (WHO) was used. From among the households of two selected areas of Mashhad, 561 families that had the background of referring to teaching hospitals were selected via multi-stage sampling. Based on WHO pattern, descriptive and inferential statistics were used for data analysis.
Results: About 50 percent of the respondents expressed the level of responsiveness as good and very good. Information confidentiality domain and the quality of amenities got the highest and lowest ranks from the respondents’ viewpoint, respectively. The highest and lowest importance belonged to the domains of quality care facilities and family and social support, respectively. 
Conclusion: Considering the relatively low responsiveness of different domains and also the existing gap between families’ priorities and performance of teaching hospitals, it seems necessary that policymakers pay more attention to patients’ priorities such as the quality of care facilities and the creation of appropriate educational content related to medical students. Also, periodic evaluation of responsiveness can be useful in promoting the responsiveness of teaching hospitals.
