Abstract

Background and Aim: Since managers' efficiency depends on their communication skills suitable for their employees, this study was conducted to determine such skills among Shiraz University of Medical Sciences (SUMS) managers.

Materials and Methods: In this cross-sectional study, 298 SUMS employees working at different departments were selected through simple random sampling technique. The data-collection device was a 21-item questionnaire having 3 parts: verbal skill, effective listening, and feedback communication skills. The data were analyzed using SPSS software. Statistical tests employed included Pearson correlation, T-Test and ANOVA, and the level of significance was determined at 0.05.

Results: The respondents' mean age was 32.7 years. Most of them were female, had a bachelor's degree, and worked in logistics department. According to their judgment, their managers had good verbal and effective listening skills, but were average in their feedback. The highest score for communication skills was reported for the students' department and the lowest score for the research department. Older and more experienced employees evaluated their manager's feedback skill (p= 0.001) at a lower level. The statistical test indicated a significant relationship between feedback and level of education (p= 0.001).

Conclusion: It seems that the intermediate level of effective listening and feedback skills are due to the managers' lack of sufficient scientific and practical attention to the issue of communication. Therefore, it is recommended that educational workshops be held for managers to make them familiar with the importance of effective communication skills.

Abstract

Background and Aim: Obesity is now considered as one of the main risk factors of certain known diseases such as cardio-vascular diseases, non- insulin-dependent diabetes, and common cancers. Moreover, the increase of white fat tissue is known as a main factor in the obesity process, in terms of physiology and pathology. Therefore, the understanding of adipocytes differentiation processes is crucial.

Materials and Methods: In this study, mesenchymal stem cells (MSCs) were isolated from human bone marrow by ficol-gradient, and then, their surface markers were confirmed by flow cytometry. Osteoblastic and adipocytes differentiation were done by dexamethasone protocol and confirmed by staining. Then qualitative and quantitative expressions of PPARgamma (PPAR-γ) gene as an important transcription factor in the differentiation of fat were studied by RT-PCR and REAL TIME PCR before and after differentiation into adipocytes. For statistical analysis, paired t-test was performed, using pfaffl and graph pad software.

Results: PPAR-gamma gene expression showed a significant increase after differentiation of human bone marrow mesenchymal stem cells into adipocytes (p<0/05).

Conclusion: According to the results, the PPAR-γ gene acts as one of the important factors in the differentiation of MSCs into adipocytes. In brief, the inhibition of this gene's expression to prevent obesity is suggested as an idea for treatment in the future.

Abstract

Background and Aim: Cancer disease is one of the main problems of Iranian health system. It is after Cardiovascular diseases and accidents, the third leading cause of death in Iran. In many countries, differences in socio-economic status have been linked with the incidence of disease, death and in general, health inequalities. The aim of this study is to determine the socioeconomic factors associated with the incidence of leukemia in Iran.
Materials and Methods: The present descriptive-analytical study was done with panel data modeling, including information related to 30 provinces of Iran from 2004 to 2009. Socioeconomic data were collected from provincial statistical yearbooks and data on age-standardized incidence rate (ASIR) of leukemia cancer per 100,000 populations were obtained from published reports by Iran Cancer Registry. 
Results: The results showed that the leukemia incidence in men and in women during the period under review has been upward. The highest and lowest incidence of leukemia was in Yazd and Sistan provinces, respectively. Direct relationship between unemployment rate, urbanization ratio, and human development index with cancer incidence rate was evident in this study.
Conclusion: The increase of leukemia cancer in Iran has been confirmed by the current study. Leukemia cancer was significantly higher among provinces with higher socioeconomic status. This should be considered for planning support.

Abstract

Background and Aim: Cosmetic surgery rhinoplasty, has dramatically been increased during the last two decades. This study aims to explore its effects on the quality of life of individuals gone under the surgery.

Materials and Methods: This was a descriptive and analytical study which was performed as a cross-sectional study. From all plastic surgery clinics in Teheran, 25 clinics were selected randomly as the site of study. In the next step 110 patients undergone rhinoplasty were selected randomly from these clinics. Collected data was analyzed by Spss17.

Results: Findings revealed that, the mean score of quality of life before and after surgery was statistically significant. While mean score of general benefits increased the score of social support and physical health was decreased after the surgery. The most motivating factors for surgery were external factors. In addition there was a positive correlation between quality of life and the time of interview. Also the mean cost of surgery was increased.

Conclusion: Overall quality of life among study population was decreased. It could be because of unnecessary surgeries, medical errors, and etc. The reduction for social support may be resulted from unacceptable consequences of rhinoplasty particularly in terms of appearance. Physical health of respondents was negatively affected by rhinoplastiy surgery. Malfunctions of upper respiratory system after rhinoplastiy surgery is known as the main reason for it. Correlations between the times of surgery and interview suggest a long term assessment of impacts of such surgeries.

Abstract

Background and Aim: The issue of organization fit is an important factor for attracting expert individuals within the organization and get their loyalty and commitment. Therefore, this study aimed to investigate and determine the relationship between person–organization fit with organizational commitment of managers of Alborz University of Medical Sciences.
Materials and Methods: This cross-sectional study was done on all managers who employed in Alborz University of Medical Sciences. Data collection conducted using a questionnaire which had two parts: person – organization fit and organizational commitment. The validity and reliability of the questionnaire was confirmed and about 180 questionnaires had been collected. And for analyzing data, Mann-Whitney and Kruskal-Wallis methods were used. 
Results: The average score of person – organization fit was 5.16 with 0.28 standard deviation and the average score of organizational commitment was 4.91 with 0.44 standard deviation. The results showed statistically significant relationship between person – organization fit and organizational commitment (r=0.63, p=0.000).
Conclusion: Managers of Alborz University of Medical Sciences had average fit with their organization and high organizational commitment. So it is suggested that managers should be oriented about the goals and values of the organization. Moreover, policy makers and leaders of this university should be aware about the characteristics and values of the manegers. In addition, they must  pay more attention to the issue of person–organization fit.

Abstract

Background and Aim: The patient is the main customer for hospital services. Patient satisfaction can reflect if the services are performed correctly. The aim of this study was to determine the relationship between the degree of client tribute plan and inpatients and outpatients satisfaction in Shiraz hospitals.
Materials and Methods: This descriptive-analytic and cross sectional study was done among 211 inpatients and 1,175 outpatients in 8 Shiraz hospitals. The instruments for data collection were two client tribute plan and patient satisfaction questionnaires. Data collected by interviews and observation. Data were entered into SPSS. Descriptive statistics and Pearson correlation test were used for data analysis.
Results: Forty five point four percent of patients and 44.1 percent of outpatients assessed the hospital service satisfaction as weak. The mean score of inpatient and outpatient satisfaction was 60.53±8.9 and 54.22±12.6 respectively. There were significant relationship between type of admission and satisfaction (p=0.001). Mean score of client tribute plan was 182.44±11.8 that placed in the appropriate level. There were no significant relationships between client tribute plan and inpatients and outpatients satisfaction (p=0.143).
Conclusion: Although client tribute plans perform properly, patient satisfaction is low. In this regard, it is recommended that review client tribute plan clients in healthcare systems and adopt its contents with the needs of patients.

Abstract

Background and Aim: To fulfill its mission, the health system needs reform in various sectors including supervisory areas. That is because the quality of supervision can have various consequences such as increase or decrease of innovative self-efficacy through various factors. Therefore, the aim of this study was to explain the mediator role of perception of organizational justice in the relationship between abusive supervision and innovative self-efficacy.
Materials and Methods: This is a correlational descriptive-analytical study. The sample included 225 employees of Shiraz University of Medical Sciences, who were selected through random sampling. The research instruments were questionnaires of abusive supervision, innovative self-efficacy and perception of organizational justice, which were distributed among the sample after calculating their reliability and validity. 
Results: The results showed that abusive supervision had a significant negative impact on the perception of organizational justice and employee’s innovative self-efficacy. Also, the perception of distributive justice has a significant positive impact on innovative self-efficacy and a mediation role in the relationship between abusive supervision and innovative self-efficacy.
Conclusion: In order to enjoy innovation, health-oriented organizations should reduce abusive supervision in managers’ and supervisors’ behavior; therefore, with the increase of employees’ feeling of innovative self-efficacy, they can lead to the realization of the mission of these organizations.

Abstract

Background and Aim: In recent years, the family physician plan has been implemented as a main strategy of health system in Iran. Therefore, the necessity to reform organizational structure based on experiences of other countries is felt more than before. The aim of this study was to explore required structures of Family Physician Program to achieve service quality dimensions in Primary Health care through analyzing country experiences.
Materials and Methods: This study was a systematic review. All relevant databases were searched using appropriate search strategies and keywords (Family Physicians, Primary Health Hare, Quality of Care). To evaluate the quality of selected papers, CASP tool was applied by 4 experts, and their choices were discussed to reach a final decision.
Results: In order to achieve the quality of services in the field of family medicine, based on the findings of this study, eight important executive structures must be considered. These structures are: Organizational and managerial structures, including health system governance, Support  mechanisms and referral system, Systematic communication platform, Electronic health services, Service delivery processes,Insurance structure, Supervisory and control structure including financial control mechanisms, competitive control and quantitative control, Payment structure, quantitative and qualitative development structure of service providers, Quality structure that includes the definition and evaluation of quality and accountability mechanisms as well as incentive mechanisms for service quality, Support structure including insurance support, classified support for specific groups, and finally, the cultural structure in the two areas of culture building of the referral system and strengthening the position of family physicians.
Conclusion: Quality improvement in primary health care requires attention to executive structures. Use of executive experiences of other countries will be useful in achievement of quality health care in family physician system.

Abstract

Background and Aim: Many health policies are not implemented or incompletely implemented after approval. This issue significantly reduces the popularity and responsiveness of the top managers of the health system and leads to service disruption in the target community. Therefore, this study was conducted to explain the existing challenges in the implementation of health policies.
Materials and Methods: A qualitative study was conducted in 2021. The participants in the study were 20 scientific and executive experts. The data was collected using an interview guide and the in-depth individual interview method and was analyzed using the content analysis method using thematic analysis with Graneheim and Lundman's approach.
Results: The challenges include the policies formulation (lack of evidence-informed decision-making, lack of attention to the details of the implementation, the change of the initial text of the policy, conflict of interests, unclarity the executives, the lack of proper integration between the political and technical components of the policies, the weak participation of the main stakeholders, the lack of the same understanding of policies concept between policy makers and executives and lack of budget forecasting), challenges of policies implementation (uncertainty about roles of executive organizations, lack of executive attachment, conflict of interest,  lack of project management, change of formulated policies with change of executives, little belief among executives, lack of executive guarantee, lack of operational plan) and challenges of policies evaluation (Inadequate supervision and the absence of an informational and supervisory dashboard).
Conclusion: Using effective solutions to solve the challenges extracted in the stages of formulation, implementation and evaluation of health policies can lead to achieving the goals of the formulated policies and improving the health of the society.
