Abstract

Background and Aim: Motivation and satisfaction of manpower are the most important variables in the field of organizational behavior and because of the strong relationship between these two variables with the job and organization productivity, it’s necessary to look job characteristics as increasing factor of employee satisfaction and performance in redesigning jobs. The objective of this study was determining motivating potential score (MPS) of the jobs and survey the relation between MPS and job satisfaction in staff employees of educational and research deputy of TUMS (Tehran University of Medical Science).

Materials and Methods: This was a cross-sectional and descriptive-analytical study. The research context was to staff employee of education and research deputy of TUMS. Sampling frame of the study was simple random sampling. Data collection method was a questionnaire. Reliability of the questionnaire was ascertained with pilot study with and confirm with Cranach's Alpha. T-test and Pearson correlation were the analytical methods.

Results: The results showed the jobs of the study population have skill variety, task identity, task significance and potential motivation but haven't job authority and feedback. The findings showed there was significant relation between motivating potential power and its dimensions with the job satisfaction ( P<0.05 ).

Conclusion : The job characteristic have great impact on the job satisfaction so this model can be used for redesigning the jobs to increase job satisfaction.

Abstract

Background and Aim: Infertility is a major problem of modern medicine as it affects almost 20% of reproductive-aged couples. The cause of this problem is attributed to the male partner in nearly 40%-50% of these cases. The role of herpes simplex virus (HSV) in male infertility has been investigated using the sensitive methods. The aim of this study was to determine the prevalence of HSV-1, 2 DNA in the semen, blood and urine of idiopathic infertile men and its association with altered semen parameters.
Materials and Methods: A total of 150 semen, blood and urine samples from infertile men were collected in the Shariati hospital, Tehran (2012-2014). Sample analysis and diagnostic PCR using specific primers was performed for detection of HSV-1, 2 DNA in the specimens.
Results: Analysis showed six groups of infertile men, including HSV-1, 2 positive and negative groups in semen, blood and urine samples. HSV-1, 2 DNA was detected in 38 (18.66%) semens, (6.66%) 10 in blood and 2 (1.33%) in urine. Only HSV-1, 2 positive samples of semen had abnormal semen parameters.
Conclusion: Using a powerful molecular method, we detected a high prevalence of HSV-1, 2 DNA in the semen of asymptomatic infertile patients. Although HSV-1, 2 infections were not associated with motility and morphology defects of the sperms, it was related with decreased sperm count in the semen fluid. In addition there was not a significant role for detection of HSV-1, 2 DNA in blood and urine samples of infertile men and abnormal semen parameters.

Abstract

Background and Aim: Creativity and innovation are considered among the most important factors that help an organization survive. Therefore, it seems necessary to examine the status of this important issue in healthcare organizations such as hospitals. The present study was conducted to determine the relationship between organizational innovation and creativity among the staff of teaching hospitals of Tehran University of Medical Sciences (TUMS).  
Materials and Methods: This descriptive-analytical and cross-sectional study was conducted on 285 staff members of TUMS selected teaching hospitals in 2015. To collect the required data, demographic data form, organizational creativity 
questionnaire and Martin Patchen’s innovation questionnaire were used. Data were analyzed in SPSS software version 21, using descriptive statistics, t-test, ANOVA and Pearson correlation analysis at a less-than-0.05 significance level. 
Results: The organizational creativity rate for the studied staff showed an average of 2.99±0.80, assessed at middle and lower middle levels and the organizational innovation rate showed an average of 19.37±3.71, assessed as middle and upper middle. A statistically significant relationship was observed between organizational creativity and organizational innovation (p=0.02, r=0.20).
Conclusion: The results suggest that an increase in creativity leads to an increase in innovation. Therefore, to survive and maintain a competitive advantage to provide more desirable treatment services at a high quality, hospital managers are required to use their best effort and expenditure so that they can enhance innovation within their organizations.

Abstract

Background and Aim: Health insurance organizations play an important role in increasing people’s access to health services and protecting them financially against catastrophic health costs. Iran Health Insurance Organization (IHIO) is one of the largest public health insurance organizations in Iran, which faces many challenges. The purpose of this research is the strategic analysis of health financing performance of IHIO.
Materials and Methods: This qualitative research was conducted using interpretive phenomenology method through using semi-structured interviews with 25 managers and employees of IHIO. In addition, relevant documents and archival data of IHIO were collected and analyzed. Thematic analysis method was used to analyze the data. 
Results: Overall, 19 strengths, 24 weaknesses, 14 opportunities, 37 threats and 43 solutions were identified for the health financing system of IHIO. Increasing the coverage of health services, correcting the information databases of the insured and electronic prescribing were the most important strengths, and inappropriate pooling of financial resources, incomplete risk pooling, high administrative costs, and inefficient control were the most important weaknesses of IHIO. The most important opportunities for IHIO include the government’s support for universal health coverage and emphasis on primary health care, legal support for consolidating health insurance funds and improving the health technology assessment system in the country. The main threats to IHIO include political and economic unstability, low health insurance premiums, decisions without scientific support and insufficient enforcement of laws. Finally, solutions such as modernizing the tax system, increasing the health literacy of the community, reducing bureaucracy, increasing transparency and accountability, and reforming the monitoring and evaluation system were identified to strengthen the performance of the financing system of IHIO.
Conclusion: Iran’s health insurance organization is facing numerous structural, contexual and process challenges that have reduced its productivity. Decrease in revenues, increase in costs and decrease in efficiency have caused problems in the financing performance of this organization. Recognizing the weaknesses and challenges of financing performance and applying corrective interventions is the first step in strengthening the sustainability of health financing of IHIO.
