Abstract

 Background and Aim: The applicability of any technology to enter a certain field is determined by defining the advantages and disadvantages of the system in that field. The aim of this study is to show the advantages and limitations of using speech recognition systems in health care and providing practical solutions to improve the acceptability of the system in that field.

 Materials and Methods: This is a descriptive research with a review method that employs library resources and online databases such as Proquest, Pubmed, Science Direct, Ovid and Scientific Information databases using key words like speech recognition software, health care, benefits, barriers, and solutions.

 Results: Speech recognition system has many advantages like increased accuracy of medical documentation, and reduced documentation time. It is a tool for data entry into electronic health records. However, there are several limitations in applying the system in Iran, such as the lack of definition of database system and the high cost of hardware and software.

 Conclusion: Considering the study results in relation to the benefits and limitations of systems in healthcare area, solutions such as production of a national integrated database for the exchange of health information, improving database to increase the accuracy of word recognition, and training the users of the system can reduce the limitations of the system to some extent. Also, in the country’s movement towards the implementation of electronic health records and the users’ need to enter data into the computer, the software is a good alternative to keyboard and mouse input.

Abstract

Background and Aim: Acute lymphoblastic leukemia patients show differences in serum levels and toxicity associated with methotrexate after its treatment. Pharmacogenetics is an important determining factor for these differences. In this study, the effect of +452 C / T and -401C / T polymorphisms of GGH gene on serum levels and toxicity associated with methotrexate was studied. The aim of this study was to evaluate the effect of +452C / T and -401C / T polymorphisms of GGH gene on methotrexate level in serum and its associated toxicity in patients with acute lymphoblastic leukemia. Furthermore, the frequency of the above polymorphisms was investigated for the first time in Iran.

Materials and Methods: The prevalence of these polymorphisms was assessed in 83 Iranian patients with ALL using PCR and RFLP. The relationship between the polymorphism and serum methotrexate levels and its toxicity was estimated by calculating the Odds Ratio.

Results: No correlation was found between +452CT polymorphism and serum levels of methotrexate and methotrexate-related toxicity. -401CT polymorphism was found to be correlated with methotrexate-related toxicity leading to thrombocytopenia (95% CI= 0.009-0.019, odds ratio=0.265) and leukopenia (95% CI = 0.021-0.042, odds ratio = 2.182) in consolidation phase of the treatment.

Conclusion: Evaluation of patients for methotrexate-related polymorphism of GGH gene may be useful in selecting the appropriate dose of methotrexate and reducing the toxic side effects associated with its administration.

Abstract

Background and Aim: Cystic fibrosis is a common hereditary and autosomal disorder. One of the factors in cystic fibrosis is Burkholderia cepacia which can be transmitted through the sharing of admitted patients with hospitalized patients. Purpose of this study, was isolation and identification of Burkholderia cepacia from respiratory secretions from Masih Daneshvari Hospital cystic fibrosis patient. Materials and Methods: In this study, during 8 months from 2011 till 2012, 100 cases of pulmonary secretions were collected in Masih Daneshvari hospital patients. The specimens were cultured on both MacConkey agar and Burkholderia cepacia selective agar (BCSA) medium. After an incubation of 48 hours at 35°C, suspected colonies were isolated and by using of phenotypic and biochemical tests like TSI agar, simmons citrate medium, indol and motility, MRVP, oxidation - fermentation (OF), arginine- ornithine - lysine decarboxylase test, Dnase test, and polymyxin resistance were identified. Results: Using BCSA medium and biochemical tests led to isolation and identification of five Burkholderia cepacia strains from 100 respiratory samples. However, using Mac Conkey no Burkholderia cepacia was detected. Conclusion: Our results clearly indicate the use of BCSA medium as a specific medium for isolation of Burkholderia cepacia in cystic fibrosis patients. So, presence of 5% Burkholderia cepacia in cystic fibrosis infections suggests detecting this bacterium, in a specific and sensitive culture medium.

Abstract

Background and Aim: Technical videos and images are of great importance in learning different topics of medical sciences. This study is conducted to determine the effect of videos and images in learning from students’ point of view and also their problems in accessing them.
Materials and Methods: This is a survey study. Data were collected by a self-made questionnaire and the population included postgraduate students of Qazvin University of Medical Sciences in the first semester of 94-95.
Results: More than half of the students stated that images and videos have an average or more-than-average effect on their learning achievements. There is no significant relationship between students’ fields of study and their feeling the need for specialized images and videos. To get advice, they refer more to their professors or classmates than to librarians. A large number of responding students had not heard even the names of specialized image-and-video databases or they had never used them before. Low-speed internet and filtering of sites are the most important factors in students’ failure to access their required images and videos while students themselves have ranked ‘unfamiliarity with specialized databases’ as the fourth important factor.
Conclusion: The students know the importance of images and videos in learning achievements, but since they don’t know the professional databases and don’t have an appropriate interaction with librarians, they resort to general search-engines and, as a result, they face with blocked sites and are deprived from what they really want.
