Abstract

Background and Aim: Lung cancer is the most common cancer among men around the world. The aim of this study was to determine the expression levels of CK19 mRNA marker in the affected patients’ peripheral blood.

Materials and Methods: Thirty patients with lung cancer (NSCLC type) were compared with 30 healthy controls. After taking peripheral blood samples and extracting total RNA, cDNA was synthesized and examined by Real-time RT-PCR technique. Then, the CEA antigen was measured by ELISA.

Results: The CK19mRNA was positive in 7 out of 30 lung cancer patients. Hence, its sensitivity was determined to be 23.3 %. The serum level of CEA antigen was positive in 11 out of 30 lung cancer patients. The mean serum level of CEA antigen markers was higher in patients than in controls; the difference was statistically significant (P< 0.001). The sensitivity of this test was determined to be 36.6 %.

Conclusion: This study showed that the sensitivity of CK19 mRNA in peripheral blood was low, but it had a high specificity for the diagnosis of primary lung cancer. Also, it was found that CEA antigen could be the specific marker for the early detection of lung cancer in patients’ peripheral blood.

Abstract

Background and Aim: Learning is essential in healthcare environments, where knowledge and skills are quickly outdated due to continuous advances in medical science. Organizational learning is a dynamic process that enables learning organizations to be campatible with change in good time. The aim of this study was to determine the status of organizational learning in an Iranian healthcare organization in Tehran, and to assess the extent to which this organization could be considered as a learning organization.

Materials and Methods: In this cross-sectional and descriptive study, 200 nurses were selected in an Iranian healthcare organization. Dimension Learning Organizational Questionnaire (DLOQ) was used to collect data to be analyzed using descriptive statistics methods by frequency, percentage, mean, and standard deviation.

Results: The mean overall score for organizational learning was 3.36±0.69. Among the various dimension of organizational learning, continuous learning had the highest average (3.44±0.39), and the empowerment had lowest amount (2.72±0.06).

Conclusion: The findings from this study provide useful information for these organization's managers regarding the areas where there is a need for improvement in OL and to make it a more LO.

Abstract

Background and Aim: Job rotation is one of the management policies in human resource management. If these policies are done with proper planning, they can impress job satisfaction and job burnout of the employees. Present survey has been conducted in order to find relationship between job rotation with job satisfaction and job burnout.

Materials and Methods: A descriptive analytical methodology was used in 2013. The statistical community included all personnel with job rotation at the deputy of development in Tehran University of Medical Sciences (n=200). A researcher-constructed questionnaire was used to collect data. The data was analyzed by SPSS.

Results: The mean job rotation and job burnout among men were more than women. A direct and significant relationship exists between job rotation and job burnout among the women. Job rotation and job burnout were inversely and significantly correlated.

Conclusion: Job rotation can have influence on increasing employees’ incentives and satisfactions and reduce Job burnout. Thus, the managers need to create an environment which contributes to knowledge transactions within an organizational framework together with promotion of encouraging their mentalities and with designing a good incentive system for teamwork, through the movement of workers in different occupations.

Abstract

Background and Aim: Patient safety is a critical element of health care quality, and is a priority of health system that pursue to improve the quality of health services. This study aims to determine the attitude the nurses about patient safety in hospitals of Tehran University of Medical Sciences.

Materials and Methods: Nurses employed in 7 emergency and intensive care ward specialty and subspecialty hospital affiliated with Tehran University of Medical Sciences was invited to anonymously complete safety attitudes survey. Reliability of the questionnaires was evaluated by calculating Chronbach’s alpha. ANOVA were used to compare the score means between the wards. Spearman correlation test was used to analyze the correlation between the safety dimensions and the outcome variables.

Results: The results showed that the highest score was dedicated to stress recognition, and the lowest score was related to management perception and working conditions. No significant difference was between the perceptions of emergency department nurses and intensive care nurses with various aspects of safety culture in hospitals.

Conclusion: Patient safety status described as acceptable in emergency and intensive care wards. There is a need to understanding of the atmosphere and the culture of safety by hospital executives deeply.

Abstract

Background and Aim: Timely diagnosis and treatment of abnormal thyroid function can reduce the mortality associated with this disease. However, lack of timely diagnosis will have irreversible complications for the patient. Using data mining techniques, the aim of this study is to determine the status of the thyroid gland in terms of normality, hyperthyroidism or hypothyroidism.
Materials and Methods: Using supervised and unsupervised methods after data preprocessing, predictive modeling was performed to classify thyroid disease. This is an analytical study and its dataset contains 215 independent records based on 5 continuous features retrieved from the UCI machine learning data reference.
Results: In supervised method, multilayer perception(MLP), learning vector quantization(LVQ), and fuzzy neural network(FNN) were used; and in unsupervised method, fuzzy clustering was employed. Besides, these precision figures(0.055, 0.274, 0.012 and 1.031) were obtained by root mean square error(RMSE) method, respectively.
Conclusion: Reducing the diagnosis error of thyroid disease was one of the goals of researchers. Using data mining techniques can help reduce this error. In this study, thyroid disease was diagnosed by different pattern recognition methods. The results show that the fuzzy neural network(FNN) has the least error rate and the highest accuracy.

Abstract

Background and Aim: The presence of a competent and capable human resource management is one of the preconditions for the success of the organization. Factors such as the emphasis on the need to establish knowledge-based companies, the activities of third-generation universities and technology, and the challenges of financing the health sector, the scientific and technological attractiveness of developed countries and the consequent withdrawal of active and specialized forces from the field of health, importance. Attention to the organization of human capital is more prominent and due to the fact that the modern development process of the organization is based on the paradigm of innovation, the interest in ways to guide the human resource management of the entrepreneur is increasing. Therefore, the purpose of this study is to predict the human resource management model based on a comprehensive organizational entrepreneurship system in the third generation of medical universities.
Materials and Methods: In the present study, with exploratory interviews, the proposed model for human resource management based on organizational 
entrepreneurship was predicted and analyzed using content analysis method. The research population consists of basic and middle managers in the field of health, from which 20 people were selected as a sample by non-probability judgment sampling and snowball sampling.
Results: The results showed that in the field of organizational entrepreneurship (at all organizational levels in the field of health, including queuing and headquarters) 4 main and effective categories on soft and hard skills of human resource management can be classified. The three categories, including structural, behavioral and environmental branches in all government agencies and even in the field of industry have relatively similar concepts, codes and essentially functions. However, regarding the fourth category, namely excellence in the field of health, especially in medical universities, health centers and affiliated hospitals, it is quite noticeable.
Concolusion: Organizational strategies and decisions at the macro level have an undisputed and effective role in laying the groundwork for organizational 
entrepreneurship and, ultimately, strengthening processes and feedback from human resource management at the level of health organizations.

Abstract

Background and Aim: Excessive production of free radicals and the accumulation of oxidative damages play an important role in accelerating the aging process. However, one of the ways to fight against aging and related diseases is through physical activity. The aim of the present study was to investigate the effect of 12 weeks of resistance training on oxidative stress indicators (8-iso-prostaglandin-F2α: 8-iso PGF2α) and oxidative DNA damage (8-hydroxydeoxyguanosine: 8-OHdG) in elderly men.
Materials and Methods: In this quasi-experimental study, 30 elderly men were selected voluntarily and purposefully as a statistical sample and were randomly divided into experimental (n=15) and control (n=15) groups. Participants of the experimental group performed resistance exercises three sessions per week for 12 weeks. At the beginning and end of the study, serum levels of 8-iso PGF2α and 8-OHdG were measured by ELISA method. Correlated and independent t-tests were used to compare intragroup and intergroup means, respectively.
Results: Serum values of 8-iso PGF2α (P=0.007) and 8-OHdG (P=0.013) decreased significantly after 12 weeks of resistance training. Also, there was a significant difference between the experimental and control groups in serum values of 8-iso PGF2α (P=0.009) and 8-OHdG (P=0.02).
Conclusion: It seems that 12 weeks of resistance training reduces the values of oxidative stress (8-iso PGF2α) and oxidative DNA damage (8-OHdG) indicators in older men. Therefore, regular resistance training can play an important role in preventing injuries caused by oxidative stress, especially in old age.

Abstract

Background and Aim: All people living in a society are not equally at risk of COVID-19 infection and hospitalization. The current study is aimed to determine the factors related to COVID-19 infection and hospitalization among participants of Ravansar Non-Communicable Disease (RaNCD) cohort study. 
Materials and Methods: The present study was conducted between 28th February, 2020 to 27th November, 2021. Data from the RaNCD study, morbidity and hospitalizations cases (extracted from the medical care monitoring system, SAMA system and the handwritten list of the health department) and positive serological results were used. For the case-cohort study, 626 confirmed and/or probable cases and 2107 randomly selected participants as sub-cohort were recruited. For the case-control study, 188 COVID-19 hospitalized patients from RaNCD (as cases) and 632 positive outpatients identified as controls. Data were analyzed using Cox and logistic regression, respectively. 
Results: In case-cohort study, the hazard ratio of COVID-19 among people living in rural areas, in smokers and those in middle socioeconomic status was, 0.37 (0.24, 0.58), 1.50 (1.07, 2.11), as compared to the reference groups. While the hazard ratio of people aged 46-64 and those ≥65 years old, as compared to the reference group within the first year, was 3.47 (1.76, 6.86) and 6.45 (2.80, 14.85), the corresponding value after one year was 2.23 (1.13, 4.40) and 3.52 (1.53, 8.10), respectively. The hazard ratio among those with at least one comorbidity was 2.45 (1.53, 3.92) in the first year of study and after one year decreased to 1.82 (1.14, 2.92). In addition, moderate and vigorous physical activity decreased the hazard ratio. In case-control study the odds ratio of hospitalization increased by 0.03% (1.03(1.01, 1.05)) with an increase of one year in age and in people with at least three comorbidities was 2.43 (1.24, 4.73) times of those without comorbidity.     
Conclusion: Overall, one year after starting the study the decrease in hazard ratio of COVID-19 was significant. Increase in age and having comorbidities are factors that increased the odds ratio of hospitalization. Such people can be considered for the next waves of COVID-19 prioritized for health care and booster vaccination.
