Abstract

Background: Study of the attitudes of health information management experts in health systems could be regarded in order to maximize utilization of information. The research reported here aimed to study of attitudes of medical records departments in Iran about mechanisms of development of health care system information technology.

Material and method: A cross- sectional research was developed by a questionnaire to study the atti­tudes of medical records departments of 17 universities by mail in Iran. 49 of them completed the questionnaire and returned it to us. Therefore the response rate was 70 percent. The data were saved by SPSS software and analyzed by statistical method.

Results: Faculty members of universities of medical sciences believe that the must hindrances of de­velopment of electronic health records in Iran are low budget and lack of standards.

Conclusion: Utilization of health information management experts attitudes could be assured the im­provement of health care information technology.

Abstract

 Background and Aim:To assess of human force and population's programming at high level, is very important that we know rates and characteristics of population. Growth of population has direct relationship with fertility rate of women in community. To control of population and arrive to index of N.R.R.(Net reproduction rate) equal to one ,each family must be has one or tow children .Thus ,in families that they have more over tow children, we must be search about factors affected and attempt to reduce of population's growth , until arrive to a stationary population.

Materials and Methods: The cross-sectional study upon 500 women that are married and have 45 years old and more in Bushehr. Tow-stage random sampling was used in this study. First, Bushehr divided into 23 hypothesis blocks of 1000 households and thus, 20 points from each of blocks were selected by chance. Also, from each of 20 pointes, 25 families were selected by chance. In this study, a questionnaire was used for collecting of data, that questioners were asked of chief of households. The data was analyzed with SPSS package version 13. To find relationship between variables together and with response variable , was used kendall's tau test and chi-square for trend test. The multiple-logistic regression was used to find factors affected on parity progression ratios.

Results: In this study 181 persons(36.2%) of women had illiterate, 229 persons(45.8%) had primary education and 90 persons(18%) of them had high and more education .The age of the first marriage of 456 persons(91.2 %)  of women were under 25 years old and the age of the first pregnancy of 196(39.2 %) of them were under 20 years old. Probability of arrive to the first child in Bushehr was equal 0.98,and probability of arrive to second, thirst and fourth children was equal 0.92,0.81 and 0.72 respectively. This study shows that the factor like the age of the first marriage of women has a negative effect on PPR (parity progression ratio) from zero to one child, and the factors like the women's education and having a dead child have a positive effect on it. The results indicate that the factor like having a dead child has increasing effect on PPR from one to tow children, and the factors like the women's education, the age of the first pregnancy and marriage have a decreasing effect on it. In this research it is observed that having a dead child has a positive effect on PPR from tow to three children, and the factors like the age of the first pregnancy and marriage, education of women and their husbands have a negative effect on PPR from tow to three children. Also it is observed that sexually and having a dead child has increasing effect on PPR from three to four children, and the factors like, the women's education, job and education of husband , and the age of the first pregnancy of  women have decreasing effect on PPR from three to four children.

Conclusion: The results indicate that probability of arrive to third and fourth children are high. Thus it is show the controlling of family planning in Bushehr dose not good carry out within families belong lately decades.

Abstract

Background and Aim: The Emergency Medical services(EMS) is a community-based health management system that has coordinated with the entire health care system and the employees are the most important asset of it. The purpose of this study was to identify the stressful factors that cause dissatisfaction of employee in the pre-hospital EMS in Kerman.

Materials and Methods: This study was a qualitative study and sampling was targeted. Acception criteria for the study was more than three years of experience as a technician operating in urban and road sites in the EMS. In this study, semi-structured interviews were conducted with 15 personnels and data was analyzed using "frame structured analysis" was used.

Results: This study identified four main axis and fifteen sub axis. The main issues related to the tensions and dissatisfaction include the employees personal problems, organizational problems, poor coordination, and problems with the community.

Conclusion: Considering Psychotherapy for staff and the creation of job rotation between the bases are the constructive suggestions for improving employee performance and improve the system of EMS is Kerman.

Abstract

Background and Aim: Transforming government hospitals into autonomous units improved hospitals. This study was performed to evaluate the responsiveness of board of trustees hospital according to the world bank’s organizational reform model (Preker Model) in isfahan city.

Materials and Methods: The study was a qualitative research method. In this research, depth and semi-structured interviews were carried out and purposive sampling was used.

The study population were top managers of Amin and Alzahra hospitals . Sampling was started with the first participant until saturated level of information, participants completed the 8 cases.

Results: According to interviews, the current role and contribution of non-university institutions (e.g. municipalities representatives) in hospital board of trustees is neither bold nor transparent. These hospitals can address minimum changes in their mechanisms of accountability after developing board of trustees.

Conclusion : It is rational for hospitals as they undergo organizational reform and developing board of trustees, to have more rights for decision making. Further more, their accountability mechanism should change accordingly.

Abstract

Background and Aim: Vegetables are one of the major components of food basket and consumption of them is on the rise. On the other hand, diseases and outbreaks caused by this group of foodstuffs have increasing trend. This study was performed to investigate the microbial contamination of vegetables used in south of Tehran.

Materials and Methods: Hundred fresh packaged and non packaged vegetable samples composed of 10 kinds, were examined based on the methods presented in Iran national standards. For each sample, enumeration of enterococcus and total microbial count was carried out by the pour plate technique in KF agar and plate count agar, respectively. Escherichia coli detection was also performed using lauryl sulfate broth, EC broth, peptone water and MacConkey agar mediums. Biochemical tests namely urea hydrolysis, motility, sulfide hydrogen (SH2) production, gas production and sugar fermentation were conducted as conformational tests of Escherichia coli.

Results: All (100%) of mixed leafy vegetables, spinaches, peppers, kuku (Persian food) vegetables, wheat sprouts and mung been sprouts had contamination higher than acceptable limits. In the case of green onions, basils, kuku vegetables, mushrooms and broccolis, respectively 40, 60, 30, 90 and 30 % of the samples were in contravention with acceptable limits, in terms of at least one parameter.

Conclusion: Our results show that consumption of vegetables specially those used as raw, need more attention at washing point and need to use hygienic rules of health department and health care centers, at shopping and consumption steps.

Abstract

Background and Aim: Iron deficiency anemia is one of the most common nutritional problems of women in reproductive age, which has a major impact on fetal growth, low birthweight and premature delivery. This study aimed to assess the prevalence of anemia and related factors in pregnant women having referred to health centers of Mazandaran province.
Materials and Methods: In this descriptive-analytical study, 866 pregnant women were selected through stratified cluster sampling method from 19 cities, affiliated to Mazandaran University of Medical Sciences. Data was collected by using the registration form and information obtained through maternal family documents and a questionnaire. SPSS software version 18 was used and the data were analysed by descriptive and analytical statistics such as mean comparison test, chi-square and logistic regression. 
Results: The prevalence of anemia in Mazandaran was 13.4% and it was higher among pregnant women in urban areas than those in rural areas. Logistic regression showed that mother’s age, number of pregnancies, prenatal care, unwanted pregnancy, consumption of red meat, using iron tablets, tea drinking, and mother’s education level had a significant effect on anemia.
Conclusion: Since iron deficiency anemia is a preventable problem and pregnant women are among the vulnerable groups, considering care programs before and during pregnancy, having appropriate dietary patterns, consumption of iron supplements, and having a program to prevent unwanted pregnancies may lead to reducing the risk of catching a disease.

Abstract

Background and Aim: Patient participation is a widespread concept and is considered as an important issue for improving the quality of patient care and patient safety, which has been considered by health managers. Therefore, this study aimed to investigate the effect of nurses' characteristics on their willingness to patient participation in patient safety in hospitals affiliated to Tehran University of Medical Sciences using PaCT-HCW questionnaire.
Materials & Methods: This was a descriptive-analytic study. Cross-sectional information was gathered on 220 nurses working in teaching and non-teaching hospitals affiliated to Tehran University of  Medical Sciences, year of 2017-2018. For sampling, nurses were randomly selected from eligible wards. To collect information validated Malfait and et al questionnaire by named PaCT-HCW was used. The data were analyzed with SPSS 22 software using descriptive statistics and regression test at P-value less than 0.05.
Results: The nurses' two characteristics including the length of work experience in the hospital (P=0.043, B=0.323), and the length of work experience in the ward (P=0.006, B=0.604) The direct effect and type of hospital (P=0.001, B =-8.182) had a reverse effect on nurses' willingness to patient participation in patient safety
Conclusion: Health managers can consider The length of nurses' experience, as well as the type of hospital and the type of specialized ward, by creating the necessary facilities and developing educational programs for patient participation in patient safety.

Abstract

Background and Aim: One of the reasons that caused healths clients are feeling dissatisfaction with health organizations is expectations that they are creating and not able to meet according to their expectations. Therefore, the present study was conducted with the aim of production a branding model for health services with the of clients’ expectations approach.
Materials and Methods: This compound study was performed in 2021. The statistical population of this study in qualitative phase included 20 academic and institutional experts using snowball sampling and the Delphi technique. For the quantitative phase, 830 people referred to health centers were selected as service recipients. The validity of the questionnaire was confirmed by face, content, and structural validity, and its reliability was confirmed by Cronbach’s alpha of 0.96 Quantitative data were presented by EQS software version 6.1 with confirmatory factor analysis and using structural equations.
Results: The results of factor structure in healthcare branding based on six main themes of competitive position, brand equity, brand accessibility, brand consolidation in the minds of clients and the market, branding strategies, and consumer-brand relationship with 19 sub-themes based on the perspective of health services clients (CFI=0/9, TLI=0/9, SRMR=0/049, RMSEA=0/09) had a good fit and the internal consistency of the items had significant levels (P<0/05).
Conclusion: The results of this study show that 19 sub-main themes confirm the six-factor structure of health services branding and were one of the effective themes in branding from the perspective of clients. Since the branding of health services improves the quantity and quality of services provided in the health system, therefore, it is suggested that by creating innovation in the quantity and quality of services provided, access to health services, creating a competitive advantage and empowering employees and improving communication skills, an effective step can be taken in health services branding planning.

Abstract

Background and Aim: this study aims investigating the effect of the physician's presence on the error of recording surgical codes by surgical assistants and the resulting financial effects in Rasoul Akram hospital.
Materials and Methods: The present study was a descriptive-analytical and prospective study that was performed by collecting data in the hospital and reviewing the documents from Jun to March 2022. Therefore, in order to compare the deductions according to the presence or absence of the doctor, an expert was stationed in the operating room to enter the information in special forms, and thus the types of deductions for each of the mentioned situations were identified. T-test was used for analysis between the two groups (presence and absence of physician) and data were analyzed using Excel and SPSS software.
Results: Three hundred and one records were reviewed during the presence of the physician and 300 cases during the absence of the physician based on Cochran sampling formula. The percent difference between hidden deductions in the presence and absence of the physician was not significant (P-value=0.078). However, the difference between the obvious deductions in the presence and absence of the physicians was significant (P-value=0.024). The difference in obvious deduction costs was significant in the presence or absence of a physician (P-value<0.001). But, the difference in hidden fraction costs in the presence or absence of physicians, was not significant (P-value=0.435).
Conclusion: The presence or absence of a surgeon has an effect on the amount of errors in the registration of surgical codes by the surgeon’s assistant and, as a result, this matter affects the percentage and costs of surgical deductions, especially in the obvious deductions. Therefore, planning for the presence of the surgeon in cases where the error of registering the codes by the surgeon’s assistant leads to obvious deductions, can help to more accurate documentation and, as a result, fewer deductions are accrued.
