Abstract

Background and Aim: Fatal disease of cancer, ranks second after cardiovascular disease, as the most important causes of mortality in many developing countries, including Iran. Breast cancer is the most common disease among women. TPS is one of the tumor markers which has specially been considered due to its exclusive physiological characteristics like its easy measurement in serum of cancer patients.

Materials and Methods: This study has been set up to evaluate the efficiency of this tumor marker in the prognosis, treatment control and follow up of patients with Breast cancer. TPS has been measured with ELISA in 34 persons including 28 healthy people and 6 patients with breast cancer. Sampling has been done in three times depending on treatment methods.

Results: TPS has been measured  in samples which contend of 2 to 3 ml of serum from patients and the health.

Conclusion: The results has revealed that the serum TPS is not only as a measure of prognosis but also would be helpful in follow up and treatment control of the disease.

Conclusion: Serological analysis can be settled in the diagnosis and trend of cancer with production of polyclonal antibody against TPS gene family and planning appropriate pattern.

Abstract

Background And Aim : Nowadays, many inflammatory diseases such as rheumatoid arthritis (RA) and Multiple sclerosis (MS) and many other autoimmune inflammatory diseases has involved the people of different communities and cure for them so far has not been provided. Many waste irreparable these diseases caused by overexertion and chronic inflammatory cells and effects of destructive enzymes. Proteolysis like matrix Mtaloproteinazes (MMPs). Hence in this study was discussed effect of drug 4 - amino pyridine (4-AP) to Progress of inflammation in a experimental model of inflammatory response.

Materials and Methods: In this study were used, 48 Out bred rat race, all female Sprague Dawely about 200-150 gr Weight and ages 6 to 8 weeks to induce inflammatory response experiment By bovine Serum albumin (BSA) + adjuvant complete Freund (CFA). Rats were in six groups of 8 each, including: Normal group, patient group, groups of Treatment-1,Treatment-2 , Prevention-1 and Prevention-2. drug 4-AP at different concentrations (400 and 800 micrograms) to groups of Treatment-1, 2 and Prevention-1, 2 rate (0 / 1 microliter) were injected and 48 to 72 hours after injection dosage reminder (only BSA) studied and were compared severity of inflammation.

Results: 48 Rats that were studied, eight rats (66/16%), no inflammatory signs, histopathology and production of antibody anti-BSA did not show and this same group were normal. 8 rats (66/16%) clear signs of severe inflammation showed that were our patient group.32 rats (66/66%), showed less severe signs of inflammation than patients that were the same groups of Treatment-1, 2 and Prevention-1, 2 that they had been received different concentrations of 4-AP.

Discussion and Conclusion: The difference in the severity of inflammation and diffuse infiltration of inflammatory cells in Evaluation clinical, radiology, histopathology and rate of antibody anti- BSA shows that the drug 4-AP reduces the severity of inflammation in the Groups receiving medication than normal and the patient group (p <0.05). Although statistical analysis showed 4-AP Has no effect on production rate of MMPs. The groups of rats had received high concentration the drug (Prevention2 and Treatment2) than groups that low concentration of the drug (Prevention-1 and Treatment-1) showed lower signs of inflammatory.

Abstract

Background and Aim: Tuberculosis is an infectious disease with worldwide distribution. Based on WHO report one third of people are affected with mycobacterium tuberculosis. Today, the traditional bacteriological methods are only available using diagnostic methods for detection of tuberculosis. Innovation of a simple and efficient method for the diagnosis of tuberculosis is one of the most important aims of the world program against tuberculosis. The methods which are based on indicating of specific antibody response against mycobacterial antigens are potentially valuable for substituting traditional methods. These methods have attracted the researchers' attention for some reasons. Firstly the strong antibody response against mycobacterial antigens is detected only during active tuberculosis. Secondly the serologic methods are simple rapid, cheap and noninvasive. Thirdly, these methods in contrast to cutaneous PPD test are able to differentiate between an asymptomatic contamination with mycobacterium and active disease. In this study, a secretary glycoprotein antigen of mycobacteriurn tuberculosis, named 38 kD antigen, was used for a screening test for diagnosis of tuberculosis by immunodot method, since in previous researches, this antigen was considered as a special specific one in serological tests and showed much specificity.

Materials and Methods: In this study the results of immunodot tests on serum of 32 patients with pulmonary tuberculosis and positive sputum smear compared with 32 healthy people. Then, the results were compared with similar tests on protein was mixture of culture filtrate.

Results: Assay by using 38 kD antigen, resulted in 29 positive response among 32 patients, whereas by using protein mixture of culture filtrate, the test resulted in positive response in all patients (sensitivities are 90.6%, and 100% respectively). On the other hand, using 38 kD antigen resulted no positive response in assay with serum from healthy ones, but by using protein mixture from culture filtrate resulted in 8 positive response in healthy people (specificities 100% and 75% respectively).

Discussion and Conclusion: Results of this study showed the efficacy of using 38 kD antigen in rapid diagnosis of tuberculosis by immunodot method, because this method has more specificity and does not require sophisticated and expensive equipments. Thus this test may be used in field studies as a simple, precise and efficient test.

Abstract

Background and Aim: Obesity is now considered as one of the main risk factors of certain known diseases such as cardio-vascular diseases, non- insulin-dependent diabetes, and common cancers. Moreover, the increase of white fat tissue is known as a main factor in the obesity process, in terms of physiology and pathology. Therefore, the understanding of adipocytes differentiation processes is crucial.

Materials and Methods: In this study, mesenchymal stem cells (MSCs) were isolated from human bone marrow by ficol-gradient, and then, their surface markers were confirmed by flow cytometry. Osteoblastic and adipocytes differentiation were done by dexamethasone protocol and confirmed by staining. Then qualitative and quantitative expressions of PPARgamma (PPAR-γ) gene as an important transcription factor in the differentiation of fat were studied by RT-PCR and REAL TIME PCR before and after differentiation into adipocytes. For statistical analysis, paired t-test was performed, using pfaffl and graph pad software.

Results: PPAR-gamma gene expression showed a significant increase after differentiation of human bone marrow mesenchymal stem cells into adipocytes (p<0/05).

Conclusion: According to the results, the PPAR-γ gene acts as one of the important factors in the differentiation of MSCs into adipocytes. In brief, the inhibition of this gene's expression to prevent obesity is suggested as an idea for treatment in the future.

Abstract

Background and Aim: Chronic lymphocytic leukemia (CLL) is a lymphoprolifrative disorder, which is very heterogeneous in prognosis. Therefore, the analysis of the prognostic factors should be very helpful in diagnosing patients with a poor prognosis. The aim of this study was to determine the relationship between circulating Endothelial cells number with VWF levels along with the other hematological findings in CLL.

Materials and Methods: Peripheral blood samples were obtained from 30 CLL patients admitted to the hematology department of Firozgar hospital and 30 healthy subjects. The levels of CEC were measured by the presence of CD34 and VWF markers, and absence of CD45 marker, using flow cytometry. VWF levels was evaluated by ELISA.

Results: The CEC levels were significantly higher in blood of the CLL patients (0.64%), when compared to the controls (0.12%, P=0.002). The levels of plasma VWF were higher among high grade patients, when compared to controls and patients in lower grades. There was a negative correlation between CEC levels, and Hb concentration in the patients group (r=-0.47, P=0.01).

Conclusion: Although the levels of both CEC count and plasma VWF in patient with high grade CLL were increased when compared to patients with low grade, there was no significant correlation between these two parameters.

Abstract

Background and Aim: Human resources are one of the most important assets of any organization and the creation of a safe working environment enhances the quality of services in addition to satisfying this valuable capital. The purpose of this study was to evaluate the safety of diagnostic laboratories of hospitals affiliated to Iran University of Medical Sciences (IUMS).
Materials and Methods: This cross-sectional descriptive study was conducted in 7 IUMS hospitals in 2019. Data collection was performed using a checklist that was validated after being translated (by qualitative face validity method); Then, the safety status of the studied laboratories was classified into four categories: Excellent, good, average and poor. Finally, data were analyzed using descriptive statistics and SPSS software.
Results: In determining the validity of the checklist using qualitative face validity, experts' opinions were applied as changes in the tool. In the descriptive part of the study and among the safety dimensions of diagnostic laboratories, the highest mean score was related to the dimensions of "communication hazards" and "safety information" and the lowest to the dimension of "fire safety".
Conclusion: According to the results of the study, the overall safety status of the diagnostic laboratories of the hospitals under investigation is at a good level; however, the "fire safety" component as one the most important safety components was considered poor, and necessary steps must be taken to improve it.

Abstract

Background and Aim: Implementation of a program to support physicians’ working long in deprived areas is one of the most important programs of the Health System Transformation Plan in response to the challenge of the shortage of expert staff, particularly physicians. Numerous factors affect the persistence of physicians in different regions, especially in deprived ones. This study aims to explain the experiences of physicians in relation to the challenges of working long in deprived areas.
Materials and Methods: The present research is a qualitative study that was conducted in 2020 in Tehran, Iran. To achieve information saturation, 16 physicians and specialists were chosen using purposive sampling method. Then, for data collection, semi-structured interviews were used. Moreover, data analysis was performed using Graneheim and Lundman contractual content analysis method, and data management was done with MAXQDA software version 12. Furthermore, Lincoln and Guba reliability criteria were applied to achieve data accuracy and reliability.
Results: Three female and 13 male physicians with a mean work experience of 45.4±7.8 years and an average work experience in deprived areas of 8±6.3 years participated in the study. Six participants were native to the region and the rest were non-native. Twelve participants in the study were the faculty members of the university. When the data were analyzed, 286 initial codes were extracted. The information was divided into four main categories and eleven subcategories. Welfare, motivation, justice and security were the main categories of this study.
Conclusion: Providing individual and social welfare for physicians, and fair treatment in financial payments and educational justice along with establishing security provide the necessary motivation for physicians to stay in a deprived area. Besides by combining several solutions at the same time, the presence of doctors in deprived areas can be guaranteed.

Abstract

Background and Aim: Treatment of acute postoperative pain is one of the most important measures to improve recovery. Different methods of local and regional anesthesia, and also numerous intravenous drugs have been used.  In this study, the analgesic effect ketorolac, paracetamol and pethidine in the postoperative pain control of outpatient transurethral lithotripsy was evaluated.
Materials and Methods: This clinical trial was performed on 96 patients who were candidates for outpatient transurethral lithotripsy in Mashhad Imam Reza Hospital. In randomized double blinded study, at the end of the operation, 30 mg of intravenous ketorolac to the first group, 1 gram paracetamol in half an hour to the second group and 50 mg of pethidine to the third group were injected. Pain was assessed every 10 minutes to half an hour and then, every half an hour to 4 hours by numerical rating scale (NRS). Finally, the data were analyzed by SPSS v16 software. The relationship between quantitative variables was analyzed by analysis of variance and the qualitative variables was assessed by Chi-square test and P<0.05 was considered significant.
Results: There was no significant difference between three groups in demographic information. The pain intensity in the first 10 minutes after surgery was the same between the groups (P=0.372), but the pain intensity at 20, 30 and 60 minutes after surgery was significantly different in the three groups (P.values ​​0.005, 006, and 0.001 respectively), and was lower in the pethidine group. In other hours after surgery, no significant difference in pain control was observed between the three groups. The need for additional analgesia was higher in the paracetamol group than in the other two groups (P=0.025) in the first hour. Hemodynamic changes and surgical complications such as nausea and vomiting were the same in both groups.
Conclusion: The present study showed that pethidine causes better pain relief than ketorolac and paracetamol in the first hour after surgery, but after 60 minutes there is no significant difference between these three groups.
