Abstract

 Background and Aim: X-Ray departments are among the most essential and expensive parts of any healthcare organization. Meanwhile, the proper application of hospital information systems (HIS) leads to the quality enhancement of health services and to the cost reduction of medical departments. The purpose of this study is to evaluate the knowledge of radiology department staffs in hospitals affiliated to Hamadan University of Medical Sciences (HUMS) about HIS applications.

 Materials and Methods: In this descriptive research (2012), the knowledge of radiology department staffs in HUMS hospitals was studied using a two-part questionnaire including a demographic part and standard items to evaluate the HIS knowledge of radiology department staffs. Finally, the obtained data were analyzed statistically by SPSS (version 17).

 Results: The results showed that 3.8% of staff members had a weak awareness, 65.4% an average awareness, and 30.8% a good awareness about HIS applications. Perfect awareness was not observed in this study.

 Conclusion: According to the results of this study, running related and up-to-date courses for staff members working in radiology departments is recommended.

Abstract

 Background and Aim: The lack of paying attention to overlap between two databases cause purchase of duplicate resources waste of funds. This study examined overlap of articles in the field of cardiovascular which was indexed in Scopus and Web of science databases during years 2001 to 2010.

 Materials and Methods: In this study, at first, sub-headings in cardiovascular field from medical thesaurus (Mesh 2012) was extracted, then each keyword in the title of the articles in both databases were searched and articles retrieved. Then bibliographic information of these articles entered into Endnote X3 software and afterwards was arranged based on alphabetically title. As a result common articles between the two databases and unique articles to each database was determined. Using SPSS version 0.1.0.8 the amount of relative and traditional overlaps was calculated between the two databases.

 Results: Totally 45,680 articles were retrieved. There were 27,979 common titles, 14,875 articles belonged only to Scopus and 2826 articles belonged to Web of Science. Finally traditional overlap was 61/5%. Overlaps of Web of Science with Scopus 65/28% and Scopus with Web of Science were 65/28% and 90/82% repectively.

 Conclusion: The amount of relative overlap between the two databases as well as relative overlaps from Scopus with Web of Science is more than the relative overlaps from Web of science with Scopus. It means many articles that have been indexed in Scopus don’t exist in the Web of science.

Abstract

Background and Aim: Nurses are major end-users of hospital information systems (HIS). Therefore their views about strengths and weaknesses of the system are important, and, if addressed, could improve the software system and, in turn, the hospital performance. The purpose of this study was to assess the nurses' perspective about the impact of HIS in Farabi Hospital on nursing process in terms of precision, accuracy and speed .

 Materials and Methods: This was a cross-sectional study . Validity of the adopted questionnaire was approved by a panel of experts in the field. Its reliability was confirmed by Cronbakh-α coefficient test.

 Results: Sixty one percent of responding nurses were somehow satisfied with the implemented HIS, while a remaining 30.5 percent had a complete satisfaction. Also, nurses comments on HIS showed, in general, it enhanced the effectiveness of their related processes.

 Conclusion : According to the nurses, HIS has improved the speed, precision and accuracy of the processes. However, at the same time references was made to some shortcomings in the system, such as low-speed and lack of envision for some important processes, such as appropriate forms for documentation.

Abstract

 Background and Aim: Personal Information Management (PIM) is a novel approach comprising three interactive parts of information resources, dimensions, and tools. The main aim of this study was to determine the degree of Tehran University of Medical Sciences (TUMS) management and information science students’ familiarity with PIM process.

 Materials and Methods: The present work is a qualitative-applied study and the population includes 145 TUMS management and medical information students in 2012. The subjects were randomly selected and the data were finally obtained from 73 persons through semi-structured interviews. The data collection tool was a questionnaire with 10 open-ended questions classified based on the conceptual framework  of interviews. The questions were about how to acquire, organize, maintain, and retrieve information. To analyse the interviews, the qualititative data analysis software Nvivo 8 was employed.  

 Results: According to the obtained results, 51% (37 persons) of interviewed persons have applied methods and tools in order to organize and maintain information and have had exclusive methods to retrieve information. The most frequent worries were the information storage 49% (35 persons) and acquiring information 29% (21 persons).

 Conclusion: It is noteworthy to say that the PIM approach is not very popular among university students, and many of them acquire, maintain, and organize their personal information inappropriately in spite of their need for the method. Therefore, educating the students and promoting their performance can result in the improvement of main PIM-related problems.

Abstract

Background and Aim: Purchasing hospital equipment is one of the most important decisions made by managers and health professionals. Gray systems theory is an approach to deal with issues under uncertainty conditions where decision makers are facing a small number of unspecified data. The purpose of this paper is to apply the gray theory for the purchase of anesthesia machine .

Materials and Methods: This study was a survey in which mathematical analysis was used. Data collection devices included documents, interviews and questionnaires. In order to solve multiple criteria decision making in uncertain conditions, a model based on gray system theory was used. Data analysis was performed using Excel software.

Results: The findings show that among anesthetic equipment bought by public and private hospitals, brand A is superior to other brands. However, the gray possibility degree for the other brands is significantly different from brand A.

Conclusion: In addition to helping to purchase anesthetic equipment for hospitals, the results of this study are also useful for companies manufacturing medical equipment to evaluate their situation in terms of the studied criteria and take appropriate measures to develop their strengths and improve their weaknesses.

Abstract

Background and Aim: The Household financial contribution is one of the most important issues in health system. This study aims to determine and present some indices of h ousehold financial contribution in health system in Iran.

Materials and Methods: In this cross sectional- descriptive study, the methods were introduced by World Health Organization. Fair Financing Contribution Index (FFCI), catastrophic health expenditure and impoverishment in Iran were calculated from a national representative data derived from Iranian Statistics Center, Household Budget Survey, and Consumption Expenditures, at 2011. The indicators have been calculated on the basis of the total cost analysis, health and capacity to pay Iranians households.

Results: Household financial contribution in the health system for households living in rural areas is more than urban areas. In this study, more than 3 percent of the households were facing catastrophic health expenditure . Also 1.5 percent of the households have been impoverished. Fair Financing Contribution Index in rural and urban areas was 59 and 65 percent respectively.

Conclusion: Calculation of the indicators related to households financial contribution in Iranian health system need to predict and implement appropriate plans, as well as monitoring and assessment of health system programs in order to increase the equity in the household financial contribution and effectiveness of Utilization of resources.

Abstract

Background and Aim: Identification of possible animal sources of infection is applied for preventing the outbreak and progressive epidemics of infection. The aim of present study is isolation of pathogenic and opportunistic fungi from the skin of ownership dogs with emphasize on transmission risk of fungal zoonoses in rural areas of Meshkin Shahr, Ardebil province of Iran. Materials and Methods: This cross-sectional and descriptive study was performed in laboratory of Medical Mycology, School of Public Health, Tehran University of Medical Sciences from April 2011 to November 2012. Hundred and thirty dogs were studied. Collected scales and hair samples were examined by direct smear and SC, SCC and DTM culture with the aim of full coverage of whole fungal growth such as saprophytic and pathogenic fungi for 1-3 weeks. Results: From 130 examined samples, Alternaria 41 (31.5%) as the most frequent, Chaetomium 1 (0.9%) and Nattrassia 1 (0.9%) as the least frequent saprophytic agents, were isolated from the samples. Conclusion: The dogs in these areas are not the source of infection for dermatophytosis and had no role in the epidemiology of the disease

Abstract

Background and Aim: Nearly a decade has passed since the use of computers to record patients' medical records in Iran. Despite the advantages of computer systems, healthcare providers are reluctant to use them therefore, parts of computerized records will be incomplete, which lead to information gaps. The purpose of the present study is to investigate the completion of electronic reports and its reasons from employees' viewpoint.

Materials and Methods: In this descriptive study, 2499998 electronic reports were evaluated using a checklist that examined types of identity and clinical data meanwhile, staff’s attitudes were investigated by a researcher-made questionnaire. The validity of the questionnaire was confirmed by experts and its reliability was estimated through test-retest method.

Results: The results showed that 100% of some clinical and identity data were not recorded. Some 58.7% of staff members reported the absence of clear regulations and 54.7% reported inappropriate electronic forms as the reasons. Some 24.1% of staff members suggested the codification of clear guidelines and 15.6% offered surveillance programs as solutions to completing electronic reports.

Conclusion: The staff reported human and organizational factors as the most important elements influencing the quality of electronic reports. It seems that the following are among the solutions which can remove many defects of electronic reports: investment in education, management support for the codification of relevant policies, health information technology professionals' participation in designing electronic forms, and the use of high technologies to record data for busy employees.

Abstract

Background and Aim: In a severely inflationary environment with unemployment, social problems increase and, therefore, high health care expenses are imposed on the society.

Materials and Methods: This was a correlational study. Panel data model and EVeiws 6 software were used in this study. The data were collected from the World Bank website. The required pre-tests, including F-Limer and Hauseman tests, were given to check the appropriateness of data and to determine a appropriate model. Following the pre-tests, the results were estimated in the form of a regression model.

Results: The results indicated that F-test and t-statistic were significant, and that R2 statistic was high. In the three models, R2 was 99%, 99% and 98%, respectively. Moreover, it can be said that employment is positively related with life expectancy, but negatively related with infant mortality and total fertility rate. Also, inflation is negatively related with life expectancy, but positively related with infant mortality, and total fertility rate. Finally, Gross Domestic Product (GDP) is negatively related with total fertility rate and life expectancy, but has no meaningful relationship with infant mortality (p = 0.31).

Conclusion: Employment, inflation and GDP are the variables that have the greatest impact on health indicators. however, life expectancy is more sensitive to explanatory variables.

Abstract

Background and Aim: Onychomycosis or nail fungus infection has an increasing prevalence with many effects on patients’ social life and mental health dermatophytes, yeasts and non-dermatophyte molds are among the best known agents of fungal infections of nails. The purpose of this study was to determine the prevalence of non-dermatophyte molds using morphological (direct examination and culture) and molecular (PCR) methods in patients referring to Medical Sciences Mycology Laboratory in Tehran, Iran.

Materials and Methods: In this study, samples were taken from 170 patients. For direct microscopic examination (DME), 15% KOH solution was used for the culture of samples, Sabouraud dextrose agar media (S) was applied together with chloramphenicol (SC) and chloramphenicol and cycloheximide (SCC). Meanwhile, differential tests were done for mycological diagnosis (slide culture), and 28SrDNA amplification and sequencing were performed for suspect or unknown samples.

Results: Of the 170 patients, 74 cases (43.5%) had onychomycosis, of which 53 cases (71.62%) were female and 21 cases (28.38%) were male. Also, of the 74 cases of onychomycosis, 40 cases (54.05%) were reported candidiasis, 21 cases (28.37%) non-dermatophyte molds, and 12 cases (16.21 %) dermatophytes.

Conclusion: The prevalence of onychomycosis in this study was 43.5% and the application of polymerase chain reaction (PCR) technique in cases of false positive, false negative and long-term culture was valuable meanwhile, given that all the samples that had positive results in DME with negative cultures were positive in molecular tests, this study reveals the power of molecular techniques compared with culture method.

Abstract

Background and Aim: Lung cancer is the most common cancer among men around the world. The aim of this study was to determine the expression levels of CK19 mRNA marker in the affected patients’ peripheral blood.

Materials and Methods: Thirty patients with lung cancer (NSCLC type) were compared with 30 healthy controls. After taking peripheral blood samples and extracting total RNA, cDNA was synthesized and examined by Real-time RT-PCR technique. Then, the CEA antigen was measured by ELISA.

Results: The CK19mRNA was positive in 7 out of 30 lung cancer patients. Hence, its sensitivity was determined to be 23.3 %. The serum level of CEA antigen was positive in 11 out of 30 lung cancer patients. The mean serum level of CEA antigen markers was higher in patients than in controls; the difference was statistically significant (P< 0.001). The sensitivity of this test was determined to be 36.6 %.

Conclusion: This study showed that the sensitivity of CK19 mRNA in peripheral blood was low, but it had a high specificity for the diagnosis of primary lung cancer. Also, it was found that CEA antigen could be the specific marker for the early detection of lung cancer in patients’ peripheral blood.

Abstract

Background and Aim: Nurses’ main tasks include the provision of healthcare services at the highest levels of quality and quantity standards, and based on the findings of scientific research. The aim of the present study is to identify and determine the barriers of evidence-based practice among nurses of Qazvin hospitals, in Iran, at 2013.

Materials and Methods: The present cross-sectional research was a descriptive- analytic study conducted among 260 nurses practicing in six hospitals of Qazvin in 2013. The barriers scale questionnaire developed by Funk, et al. was used to examine the four dimensions of evidence-based practice (EBP) barriers including adopter, organization, innovation and communication characteristics. To analyze the data, descriptive and analytic statistics were used.

Results: The mean score of EBP barriers among subjects was 3.07 out of 4. Among the barriers, "organization" and "adopter" dimensions had the highest and lowest mean scores, respectively. There was no significant statistical relationship between gender and EBP performance, But a significant relationship was observed between organizational dimension with age, employment type and work experience.

Conclusion: Since “organization” was found to be the main  barrier to  the use of research in nursing performance, policy-makers and administrators should pay attention to the use of research in performance, the establishment of a research culture in organization, the creation of appropriate infrastructures, and the allocation of sufficient time to the nursing personnel to have access to evidences.

Abstract

Background and Aim: Diagnosis of preeclampsia has an essential role in applying appropriate treatment plan for the patients. The aim of this study was to design an expert system in order to diagnos preeclampsia in order to assist the clinicians.

Materials and Methods: This was a cross-sectional study which resulted in developing a new system. The study population consisted of all patients admitted to three Maternity hospitals affliated to Tehran University of Medical Sciences (TUMS). Sample size included 215 medical records which were randomly selected. The results obtained were compared with the diagnosis from experts by kappa test using SPSS software.

Results: First of all, input parameters fuzzificated and entered into inference engine. Outputs were categorized in two groups as patients and healthy, with the final diagnosis and clinical explanation. The results obtained from system evaluation showed that accuracy, specificity and sensitivity of the system were 98.2 percent, 100 percent and 96.4 percent respectively.

Conclusion: Based on evaluation results, it could be concluded that fuzzy logic is an efficient method for designing of expert systems in the field of obstetrics and gynecology. Also, due to the similarity of the logic used in the proposed system with workflow and medical decision making, it will be accepted by the physicians.

Abstract

Background and Aim: Caring about workers is important due to protect human capital and economic growth. Understanding health priorities is one of the main challenges of health management. The aim of this research was to priorities health promotion behaviors of female workers.

Materials and Methods: This study was conducted in 2012 and had a triangulation design with sequential qualitative-quantitative method based on "integrated model of planned behavior and self-efficacy". In the qualitative phase of the study, a content analysis approach was done  to develop the questionnaire, based on the model and psychometric properties were assessed. Then, the educational intervention was conducted in an experimental study with randomized sampling in 70 female workers (35 intervention group, 35 controls). The pretest and posttest was compared.

Results: Content analysis demonstrated 6 main themes, including the main constructs of the research model: attitude, subjective norm, perceived behavioural control, self-efficacy, intention and behaviour in 4 domain (nutrition, exercise, cervical and breast cancer screening, cope with stress). Scale level content validity index was 0.93. Confirmatory factor analysis showed CFI=0.97, GFI=0.95, IFI=0.96, NNFI=0.98, NFI=0.97, RMSEA=0.05. Concurrent validity versus the Health-promoting lifestyle profile II showed r=0.60. Cronbach alpha was 0.75–0.93 across the subscales. Test–retest reliability revealed no significant differences.After educational intervention there was a significant increase in attitude, subjective norm, perceived behavioral control, self-efficacy, intention and behavior of female workers in intervention group. Also, effect of intervention program was mostly in self-efficacy construct.

Conclusion: With regards to working conditions economic and socio-cultural barriers in female workers, health policy making is essential in this group to promote health

Abstract

Background and Aim: The first step in reducing patient dose can be administered at the radiological examinations are taken. Since physicians request radiological tests, this study examined physicians' knowledge of the risks of radiological examinations.

Materials and Methods: This descriptive, analytical and cross-sectional study was conducted in 160 general practitioners and specialists' physicians. Based on the estimated dose posterior-anterior hand X-ray, and put it as a single exposure, medical information of physicians about the patient dose in diagnostic radiology tests were examined. The validity of the questionnaire was confirmed by expert panel, and its reliability was 0.80 by Alpha-Cronbach technique.

Results: Most of the general practitioners and specialists did not estimate correctly the radiation dose received by patients in the posterior-anterior hand X-ray, and majority of them underestimated the radiation dose of other radiological examination. The results showed that 48% of General Physician and 5/53% of specialist physicians were familiar with the unit dose.

Conclusion: Training of physicians in the field of radiology patients received doses can be an effective measure for reducing doses to patients.

Abstract

Background and Aim: Today, regarding the role of accurate final information, and decision-making, designing and implementation of a sound costing system that can detect, identify and calculate the exact cost of the delivered services is of great importance. The aim of this study is to estimate the final cost of student education in the School of Allied Medicine, Tehran University of Medical Sciences, through Activity-Based Costing method in 2013.

Materials and Methods: This research was a descriptive and applied study. The study population included School of Allied Medicine. The consumed costs in financial year 2012-2013 was investigated with costs classification, and finalized costs by activity based costing method. After identification of calculated costs and changed to allocation costs, The accumulated costs according to shared basic principle was given to all of delivered services. Finally, the finalized costs for student education based on the number and kind of courses collected and calculated by Excel software 2010.

Results: The results indicated that the final cost for student education in PhD-level was 118,312,679 Rials, which was the highest amount, and was 76,714,824 Rials in MSc. level, and the lowest amount was related to BSc. student as 42,312,537 Rials.

Conclusion: Paying attention to activity-based costing system with complete flexibility reveals the drawbacks of the traditional programs used by universities. The reason for higher costs in higher levels is due to the fewer number of students, particular facilities in these level and their own special expenses

Abstract

Background and Aim: The aim of this study was structural monitoring of the articles in knowledge and information science field in five Iranian journals. Today, the publication of papers constitutes one of the written scope among scientific broad cost productions.
Materials and Methods: The research method was analytical-survey. The articles were analyzed by the variables such as author distribution by sex, field of study, scientific notation, types of articles by original papers or translated papers, individual or group articles and their publication dates, and by statistical methods used in the articles in two periods from 2002-2006 and 2007-2011.
Results: The results showed that most of the articles were written as original. International cooperation between authors was minimal. Using qualitative research methods among the articles were low. Majority of the authors had masters degree. There was a significant relationship between the authors’ grades, number of authors and journals, and used statistical methods.
Conclusion: The written articles in the field of knowledge and information, followed by special patterns. The journals policies in publication of articles process is the most important issue. Also, lack of international cooperation between Iranian with foreign authors, type and format of the articles were similar according to mentioned variations.

Abstract

Background and Aim: Hospital is a complex ecosystem in terms of diversity of services, clients, personnel, equipment, technologies, data and information which are generated. Since the ultimate goal of a hospital is to improve quality of care along with reducing cost, using intellectual tool such as dashboard can create strategic value.

Materials and Methods: This review article was performed based on a literature review and internet search through such scientific databases as PubMed, EBSCO host research, Proquest, Emerald, Web of science and search engine such as Google, Google Scholar. In this study, the articles about intellectual capitals as well as the application of dashboard in healthcare between year 2009 and 2013 were reviewed.

Conclusion: Application of dashboard in hospital can create strategic values such as respond to environmental changes rapidly, identify new market opportunities, become a learning organization by discovering new patterns and relationship among data, achieve best practices by analyzing patterns of treatment and results and achieve the highest efficiency.

Abstract

Background and Aim: Giardiasis is the most common pathogenic gastrointestinal parasite worldwide and transmitted by ingesting parasite cyst, through water and food and person-to-person contact. Therefore in food handlers and child care workers it is of high importance. In this study we surveyed the job of individuals with giardiasis.
Materials and Methods: This study was a pilot descriptive-analytic study. Hundred and nine stool samples were collected from individuals referred to medical centers affiliated to Tehran University of Medical Sciences for health certificate. 
Samples were examined with wet mount smear and formol-ether concentration methods. Also a questionnaire was given to each individual.
Results: Sixty nine samples were infected with Giardia lamblia. among positive samples for Giardia lamblia 25 (36.23%) were related to food handlers and occupations which are most  frequently associated with food, one (1.44%) nurse, one (1.44%) managing director of the nursery, two (2.89%) housekeepers  and one servant (1.44%).
Conclusion: Person-to-person Giardia lamblia transmission is often associated with poor hygiene and sanitation, most commonly by food handlers.
Also, day- care workers have a higher risk of infecting children attending day-care centers. Therefore particular attention should be paid to health of this workers as well as three times stool examination.

Abstract

Background and Aim: Electronic Medical Record system collects and stores laboratory data, digital images and electronic versions. It plays a major role in reducing medical errors and duplication and health care providers immediate access to patient medical records. This study aimed to examine the viewpoints of Hormozgan University of Medical Sciences' administrators on the role of electronic Medical records system.  
Materials and Methods: The study was a descriptive-analytical research to examine the viewpoints of Hormozgan University of Medical Sciences' administrators and physicians on the role of electronic medical records system. The study population consisted of 61 managers, matrons and health information managers of Hormozgan hospitals and 121 faculty physicians and residents. Data collection tool was a researcher-made questionnaire. The validity was determined and verified by content validity method and experts' views. Cronbach's alpha coefficient was used for assessing reliability, which was 83%. Data collected from the questionnaires was analyzed using SPSS software and descriptive statistics and analytical statistics.
Results: Results indicated that "data management" (N=146) with 77.5%, "job objectives and processes" (N=160) with 87.9%, "communication" (N=163) with 89.6%, "data privacy and security" (N=152) with 83.1% had the most important roles in the health system. 
Conclusion: Generally, managers and physicians attitudes about the role of electronic medical records system in the health system was evaluated satisfactorily. According to the advantages of the system and removing major obstacles in its implementation, a new step would be taken in order to promote health.
