Abstract

Background and Aim: In the recent years the number of science research health journals has increased in Iran. These journals should be based on the standards and criteria required in international indexing database. The aim of this study was to determine the adaptation rate of structural requirements on the Iranian medical journals with the criteria of indexing based on Scopus indexing database.     
Materials and Methods: This study was a descriptive-applied study done using bibliometric methods. The statistical population was all of the science research journals in health ministry on 2014. The data collection tool was the inventory control extracted from Scopus database. The data was collected and analyzed by SPSS20 software and descriptive statistics methods.
Results: The results showed that more than half of the biomedical journals were not based on the international editing statements and also, participation of the members of international editing committee was very low. The mean of international editing committee members was 83.3% and all of them were in Iran besides, participation of non Iranian authors was very low in these journals, with a participation rate of only 5.79%.
Conclusion: The Iranian medical journals did not follow the expected criteria in Scopus indexing database in a satisfaction level. Adaptation to the expected criteria can facilitate the entering and indexing procedures for these health journals in Scopus indexing database.

Abstract

Background and Aim: Hospitals should be able to satisfy their clients’ needs in less time and with best quality possible. Length of Stay (LOS) in hospitals is considered as a key performance indicator, especially in emergency departments. In this study, the analysis of LOS in emergency departments was performed so that some of the factors affecting patients’ length of stay could be found. 
Materials and Methods: This descriptive-analytical and cross-sectional study was performed in the emergency departments of two teaching hospitals of Tehran University of Medical Sciences (TUMS). Seventy-two patient samples were included. The patients’ length of stay in emergency ward was measured from the entrance time until the time they left the emergency department. Data on both descriptive (mean and standard deviation) and analytical statistics (one-way ANOVA, independent sample t-test and linear regression) were analyzed with SPSS software.
Results: Patients’ average LOS in emergency departments was estimated at around 3 hours and 13 minutes (SD=1.52). Such variables as patients’ marital status, days of the week (holidays or regular days) and admission time (day or night) were determined as the factors affecting LOS. 
Conclusion: The time spent to give services in hospital emergency rooms is at an appropriate level and items such as marital status, holidays of a week, and time of the day can be referred to as factors affecting patients’ length of stay. With regard to these findings, health managers can rely on the results for the effective planning and management of hospital emergency departments.

Abstract

Background and Aim: Iron deficiency anemia is one of the most common nutritional problems of women in reproductive age, which has a major impact on fetal growth, low birthweight and premature delivery. This study aimed to assess the prevalence of anemia and related factors in pregnant women having referred to health centers of Mazandaran province.
Materials and Methods: In this descriptive-analytical study, 866 pregnant women were selected through stratified cluster sampling method from 19 cities, affiliated to Mazandaran University of Medical Sciences. Data was collected by using the registration form and information obtained through maternal family documents and a questionnaire. SPSS software version 18 was used and the data were analysed by descriptive and analytical statistics such as mean comparison test, chi-square and logistic regression. 
Results: The prevalence of anemia in Mazandaran was 13.4% and it was higher among pregnant women in urban areas than those in rural areas. Logistic regression showed that mother’s age, number of pregnancies, prenatal care, unwanted pregnancy, consumption of red meat, using iron tablets, tea drinking, and mother’s education level had a significant effect on anemia.
Conclusion: Since iron deficiency anemia is a preventable problem and pregnant women are among the vulnerable groups, considering care programs before and during pregnancy, having appropriate dietary patterns, consumption of iron supplements, and having a program to prevent unwanted pregnancies may lead to reducing the risk of catching a disease.

Abstract

Background and Aim: Indexing a country's scientific journals in international citation databases can lead to the promotion of its academic status and its participation in global knowledge production. This study aimed to assess the conformity of Iranian English-language medical journals’ citations with the objective criteria of journals in the selected databases.
Materials and Methods: This was a descriptive, evaluative research. The study population included 52 Iranian English-language medical journals that are not indexed in Web of Science, PubMed and Scopus databases and 465 articles in these issues, and about 11235 citations to the articles. The data have been collected by a checklist. For data analysis, Excel software was used.
Results: Some 88.5 percent of journals had a style for writing references; a style that has been observed by most of them. The rate of self-citation was low. Only 2.9 percent of citations were done in the year this study was performed. Journals with 87.2 percent had received the highest number of citations. The old citation time and the imbalance in the types of cited resources were the most important reasons of the weakness of the studied journals and of their not being indexed in the mentioned databases. 
Conclusion: Providing valid styles for writing references and controlling the citations accurately should be put on the agenda of journals. The time of references must be newer, and there should be a relative balance in the types of referenced sources.

Abstract

Background and Aim: Conditions, advantages and limitations of professional ethics have not been discussed. The present study aimed to determine the framework, dimensions and requirements of professional ethics in selected countries in order to review their experiences.
Materials and Methods: The present study was comparatively conducted in 2017. Considering inclusion criteria in designing study including purposeful and specified activity in terms of professional ethics, five countries of United Kingdom, Malaysia, Japan, Turkey and United States of America achieved the conditions to enter the study. For data analysis, comparative tables were used which included dimensional comparison obtained from professional ethics development study in the selected countries. 
Results: Medical and nursing associations and headquarters units in Ministries of Health, prepare appropriate utilization of professional ethics principles. Using temporary or permanent and regular committees in line units, also includes other characteristics of attention to the professional ethics concept in the selected countries. In Malaysia and Japan, professional ethics is taught in different courses and majors. 
Conclusion: Applying professional ethics concept requires information transparency and factors such as education, organizational structure set up and their establishments and institutionalizing must should be discussed more and to be localized on the basis of the countries situations.

Abstract

Background and Aim: In today’s world, antibiotic resistance is inevitable. This has been the case since the discovery of antibiotics. The aim of this research is to study serotyping and multiple antibiotic resistance pattern of Shigella sonnei isolated from diarrheal stool of patients hospitalized in Children’s Medical Center in Tehran.
Materials and Methods: In this descriptive study, 600  diarrheal  stool specimens were obtained from patients hospitalized  in Children’s Medical Center in Tehran over a period of twelve months. The stool samples were collected in Cary-Blair transport medium and transferred to the laboratory. The identification was carried out according to the standard cultivation method, and the antibiotic susceptibility test was performed by Kerry Bauer disk method according to with CLSI procedure.
Results: Out of 600 samples, only 18 (3%) were found to be contaminated with Shigella sonnei. The results of  antibiotic resistance patterns of these isolates showed that they were resistant to tetracycline; streptomycin, clindamycin and cortimoxazol. Furthermore, 66.67% of isolates had multiple resistance to tetracycline, cortimoxazol, streptomycin, ticarcillin and clindamycin antibiotics.
Conclusion: The results of this study showed that multiple resistance of Shigella sonnei to tested antibiotics is increasing. This  is alarming; necessary steps should be taken to prevent such a phenomenon.

Abstract

Background and Aim: Thalassemia is a chronic disease which is extremely expensive, complex and debilitating. The management skill of thalassemia patients should be enhanced to minimize the risk of disease complications. The main purpose of this study was to develop personal electronic health records for thalassemia major patients.                                             
Materials and Methods: This is a developmental applied study which was conducted to develop a personal electronic health record for thalassemia major. First, a questionnaire was prepared to determine the data elements and was filled by Hematology and Oncology professionals in the country (110 persons). Then, based on the results of needs analysis, the system was designed using PHP programming language and MySQL database and was evaluated by 50 thalassemia patients who referred to the Thalassemia Clinic of Bu Ali Sina Hospital of Mazandaran University of Medical of Sciences during the second half of the month of Aban. Finally, a standard questionnaire of usability and user satisfaction assessment was distributed among them.   
Results: Usability evaluation of the system showed that patients evaluated the system at a good level with a mean rating of 7.91 (out of 9 points). 
Conclusion: The web-based systems can be used to help thalassemia patients to control injection and reduce the complications of the disease and to promote health.

Abstract

Background and Aim: Scientometric studies are one of the most effective methods of scientific evaluation in databases. The aim of this study was to investigate Breast cancer in Iran from 2000-2016.
Materials and Methods: This study has an applied approach and was conducted using scientometric indices. The st tistical population was the indexed articles related to Breast cancer between the years 2000 and 2016 by Iranian researchers at the Science Web site.
Results: During the period 2000-2016, researchers have published 2198 articles on Breast cancer that indicate a steady and steady increase in the number of studies conducted in this area. The results of the study showed that Qaderi is the most prolific researcher in the field of Breast cancer in terms of the number of articles in Iran, Ebrahimi and Montazeri are in the second and third positions respectively. The highest H-index belongs to Montazeri, Qaderi and Abraham, respectively. Researchers in the field of Breast cancer have collaborated with researchers from 65 countries and more with the United States and Canada. The most co-operation has been between researchers in Tehran and Tabriz. The analysis of the keywords used in Breast cancer research in the form of supragloss showed that Iran, Apoptosis and Polymorphism were the most frequent keywords in the studied works.
Conclusion: The upward trend in Breast cancer research indicates the growing importance of this area in Iran. Given the global growth of Breast cancer research and the importance of international research participation, Iranianresearchers should more and more engage in scientific collaboration with their counterparts abroad.

Abstract

Background and Aim: In recent years, tuberculosis is a major cause of illness and mortality due to infectious diseases. In fact, one of the reasons for increasing the incidence of tuberculosis is that it is often associated with HIV infection. The present study aimed to design and establish a mobile-based self-care system for controlling tuberculosis.
Materials and Methods: The data required in this research for evaluating the information and designing the application were collected in two main steps: first, the data required in this research for information needs assessment and application design were collected by searching in library resources. A questionnaire was applied by physicians to validate and determine the significance of these elements.  Afterwards, based on information elements and the self-care, application was designed in the Android Studio environment.
Results: Information elements and functional capabilities required by the program were determined. Web-based services as a system interface, not only provided the communication between the patient and the care provider, also allowed for patient monitoring and disease control. Then evaluation of the performance and performance of the program was done by doctors and software experts. The findings showed user satisfaction with the application.
Conclusion: Monitoring of treatment electronically allows for less costly monitoring of the patient. In this study, we present a prototype of the relationship between self-care application and portal and its potential benefits, which will allow future studies to improve and add new capabilities.

Abstract

Backgrounds and Aim: One of the effective strategies for targeting chemotherapy in the treatment of cancer is the use of lipid nano-carriers. In this study, an optimal formulation of niosomal drug containing doxorubicin has been developed to better fight cancer cells.
Material and Methods: Niosomal vesicles were prepared using phosphatidylcholine (22%), span60 (52/5%), cholesterol (22/5%) and DSPE-PEG2000 (5%) by thin-film method. Doxorubicin were loaded into the niosomes using inactive loading method. Their physico-chemical features were assayed using Zeta-Sizer, FTIR and SEM, and drug release amount was calculated at 37° C and 44° C. At the end, the toxicity of the nano drug carrier system was measured on the KG-1 cell line of the bone marrow cancer by MTT method.
Results: Niosomal vesicles containing Doxorubicin showed the size of 160.37±65.2 nm, 94.18% drug encapsulation efficiency  -58.11± 1.24 mV of zeta potential and polydispersity index (PDI) of 0.234±0.02 The prepared niosomal system presented drug controlled release and FTIR investigation showed almost no interaction between nano-carrier containing drug and the drug itself. As well, morphological examination of nano-carriers using SEM microscopy revealed that they have spherical structures. Also, cellular studies showed that drug toxicity was higher in encapsulated conditions compared to non-encapsulated conditions.
Conclusion:The results of this study, meanwhile confirming the proper physicochemical characteristics of the system and being Slow-release system indicate that this nano-carrier anionic increases the toxicity of the drug for the KG-1 cell line of the bone marrow, thus, this niosomal nano-carrier can be a suitable carrier for drug delivery to cancer cells.

Abstract

Background and Aim: To provide effective care, health care providers need timely and appropriate information. Electronic records provide quick access and easy management of data. The aim of this study was to develop electronic health records for patients with hydatidiform mole and evaluation of completeness of medical records
Materials and Methods: This applied study was conducted in 2017. After verifying the minimum data set required for the system, data were extracted from patient records using a checklist and entered into SQL server. SQL server 2012 and Visual Studio 2013 to design electronic records and SPSS 20 for data analysis was used. Extent of data completion in patient records was also assesed.
Results: Data on the completion of paper records indicated that in 100% of cases, “address” item was filled in. The less completed data was related to carotene deficiency (%1.1). Our findings also showed that the eight most important items like age of first menstruation, first gestational age, interval between pregnancies, number of sexual partners, menstruation between pregnancies, contraceptive methods, social habits and radiotherapy, were not completed in all records.
Conclusion: Many of the important minimum data set for hydatidiform mole disease were either not completed or completed in limited numbers in paper records. By developing such health records, we can ensure better prevention and treatment, and regular follow-up for the patients and help them to save their time and costs.

Abstract

Background and Aim: Ontologies facilitate data integration, exchange, searching and querying. Open Biomedical Ontologies (OBO) Foundry is a solution for creating reference ontologies. In this foundry, the design of ontologies is based on established principles which allow for their interactions as a single system. The purpose of this study is to determine the main features of ontologies developed based on foundry principles.
Materials and Methods: This descriptive cross-sectional study (2017) retrieved data about the ontology components, subject, language and tool from the OBO foundry website, and OntoBee (the default linked data server for the foundry ontologies). The frequency of classes, instances, Object Properties, DataType Properties and Annotation Properties are classified by the use of descriptive statistics.
Results: The main subjects of 151 ontologies were Anatomy, Health and Phenotype. Most languages and tools were, respectively, OBO and OBO-edit versions used for the development of ontologies. Five percent of ontologies contained more than 78977 classes, 150 object relations, 39 DataType relations, 110 annotation relations, and 356 instances.
Conclusion: Increasing the number of ontologies developed based on OBO foundry principles as well as using OBO language and OBO-Edit is indicative of the fact that ontology developers have accepted the principles and tools.

Abstract

Background and Aim: The purpose of this study was to analyze the co-occurrence of the terms of medical and laboratory equipment patents in the United States Patent and Trademark Office between 1984 and 2014.
Materials and Methods: This research was an applied study using scientometrics and co-word analyses. The statistical population of the present study included all patents of medical and laboratory equipment registered in the United States Patent and Trademark Office database between 1984 and 2014. As a result, a total of 13424 patents were retrieved.
Results: The results revealed that in terms of frequency, the keyword "Menstrual Fluid" and in terms of co-occurrence, two keywords (Menstrual Fluid and Magnetic Resonance Image Apparatus) were the most frequent ones in medical and laboratory equipment studies. The results of hierarchical clustering with "Ward's method" led to the formation of eight clusters in this area including the following: General Equipment, Rehabilitation Equipment, Dental Equipment, Therapeutic Equipment, Emergency Equipment, Laboratory Equipment, Diagnostic Equipment, and Medical Consumables.
Conclusion: The analysis of the co-occurrence of words revealed the scientific structure of medical and laboratory equipment well. Accordingly, the scientific issues were extracted and the relationship between them was discovered. The maps of co-word analysis showed several changes, sustainability of concepts, and terms related to this field of science.

Abstract

Background and Aim: Web 2.0 technology has various usages in libraries all over the world. According to studies, however, it seems that this technology is rarely used in Iranian academic libraries. Therefore, the present study aims to determine the level of familiarity with and use of Web 2.0 tools among librarians working at Iran, Tehran, and Shahid Beheshti Universities of Medical Sciences.
Materials and Methods: This is a survey study. The statistical sample included 91 librarians working at the libraries of Iran, Tehran, and Shahid Beheshti Medical Universities.The data were collected through a questionnaire that was graded in the Likert 5 range (very low to very high) and analyzed by descriptive and analytical (one-way ANOVA) statistics using SPSS software.
Results: The level of awareness of Web 2.0 tools among librarians working at the studied libraries was relatively satisfactory with an average score of 3.13. However, the level of their use for personal activities (with an average score of 2.89) as well as for professional activities (with an average score of 2.78) was relatively low.
Conclusion: Due to the unsatisfactory level of librarians' familiarity with and use of Web 2.0 tools, holding training courses as well as encouraging medical librarians to acquire more knowledge and to use these technologies in library services are recommended.

Abstract

Background and Aim: Chronic myeloid leukemia(CML) is a clonal myeloproliferative disease, characterized by BCR/ABL translocation. Using tyrosine kinase inhibitors such as Imatinib, treatment for this disease has progressed remarkably. However, resistance to tyrosine kinase inhibitor is a major obstacle. Signal transducer and activator of transcription 3(STAT3) is an important transcription factor in proliferation and survival of several cancers. The aim of this study was to determine the expression of STAT3 and its role in drug resistant CML patients treated with Imatinib.
Materials and Methods: Peripheral blood was collected from 71 CML patients in different phases of the disease and 10 healthy individuals. After extracting RNA and synthesizing cDNA, expression of STAT3 gene was measured using Real-Time PCR technique. The expression of STAT3 was normalized to ABL control gene. Then expression levels were compared with the control group.
Results: The results showed that expression of STAT3 in the diagnostic stage was significantly higher than healthy individuals(p=0.0001). STAT3 expression was not significantly different from MMR and the control group. STAT3 expression was significantly higher in non-mutated and mutated ABL kinase domain Imatinib resistant patients as compared to patitents in MMR stage (p=0.0014 & p=0.003). This difference was not significant between the two resistant groups. Blastic phase patients had no significant difference in the expression of STAT3 with the control group.
Conclusion: Considering the results of this study and the role of STAT3 in cell proliferation and survival, the targeting of STAT3 seems to be an effective option in the treatment of resistant patients.

Abstract

Background and Aim: The aim of this study was to evaluate the health literacy of patients referring to specialist physicians' offices in the city of Kermanshah.
Materials and Methods: This is a descriptive analytical cross-sectional study. The population of the study (patients and their companions) included those referring to specialist physicians' offices. In 2017, 380 people were randomly selected and were studied during three months. The data collection tool was Health Literacy for Iranian Adults Assessment Questionnaire. Its validity was confirmed by specialists and its reliability turned out to be 0.89 by Cronbach's alpha. Data were analyzed by statistical tests (Spearman's correlation, Mann–Whitney U test and Kruskal-Wallis test) using SPSS software.
Results: The findings of the study showed that the health literacy of patients and their relatives, referring to physicians' offices in the city of Kermanshah, was relatively favorable, i.e., more than average. The three sources of "the Internet", "questions asked from doctors and medical staff" and "radio and television" are the priorities of information or health literacy. The results showed that people's health literacy was different in terms of age, sex, education, and occupation.
Conclusion: The results of this study revealed that often those people who have low and limited health literacy are among vulnerable community groups (having old ages, low educational levels, and low income). So, considering the health information needs and health literacy of vulnerable groups and designing simple and targeted education using the appropriate media are recommended.

Abstract

Background and Aim: Transmission of pathogenic bacteria from animals to humans is possible directly or through the consumption of meat and milk or their products. The aim of this study was to identify and diagnose Enterohemorrhagic Escherichia coli (E. coli) by molecular method in cows' milk in Boroujerd city.
Materials and Methods: In a descriptive cross-sectional study, 150 milk samples were sampled from dairy farms in Boroujerd and its suburbs in four months from the beginning of November 2016 until the end of February 2017. After enrichment, culturing and biochemical tests on EMB agar and IMVIC differential tests, and doing linear culture on Sorbitol McConkey Agar medium to identify negative sorbitol isolates and confirm them by serological testing and eaeA gene identification, milk samples were analyzed by PCR test.
Results: Out of 31 isolates of Escherichia coli species, 6 were isolated as negative sorbitol (19.4%). Of these six isolates, five (16.1%) were identified as negative beta-galactosidase (MUG-) on chrome agar medium. In serological test, all 5 isolates were confirmed by O157: H7 antiserum antibody; besides, in molecular analysis, they had eaeA gene.
Conclusion: The outbreak of 16.1% of enterohemorrhagic E. coli in milk can be of great importance as one of the factors causing diarrhea in the community. Therefore, the outbreaks of consumption of this foodstuff in areas of the country that traditionally still put raw milk in food basket can provide valuable results for the prevention of diarrheal diseases.

Abstract

Background and Aim: A great interest for determining the particular mechanisms underlying lipogenesis and adipogenesis has been raised among researchers in order to fight obesity. We aimed to investigate the gene expression of FAS and its role in regulation of lipogenesis and adipogenesis in visceral adipose tissues from obese and normal-weight subjects.
materials and Methods: A total of.participants including 40 obese patients(BMI≥35 kg/m2 according to WHO criteria) and 20 healthy subjects(BMI=18.8-24.9 kg/m2 according to WHO criteria) were recruited from who were referred to Erfan, Loghman Hakim, Sina, and Imam Khomeini hospitals bypass or sleeve gastrectomy surgeries in obese ones and elective surgery in controls. Participants were all woman aged from 20-50 years and postmenopausal subjects were not included in this study. Isolated total RNA from adipose tissue was used to synthesize complementary DNA(cDNA) and quantitative real-time PCR was performed for analyzing the gene expression of FAS, and ACC. Data was normalized to geometric means of GAPDH and β-actin expression levels.
Results: in VAT from obese subjects, gene expression of FAS was higher than in those from controls. We found a positive correlation between genes expression of FAS and ACC with obesity indices.
Conclusion: It appears that obesity is associated with dysregulation of FAS genes involved in lipogenesis and adpogenesis.

Abstract

Background and Aim: One of the essential methods of evaluating academic libraries’ mobile websites is the usability analysis method; websites’ usability means their ease and simplicity of use. This study aims to evaluate the usability of the mobile website of the libraries of top medical universities in Iran. This study aims to evaluate the usability of mobile websites of the libraries of top Iranian universities of medical sciences.
Materials and Methods: This study was done by the descriptive-survey method. The study’s statistical population includes mobile websites of the libraries of the top 50 universities of medical sciences in Iran, which have gained the highest rank in universities’ webometrics ranking in January 2019. The tool used in this study was a checklist. Data analysis was performed by descriptive and analytical statistical methods and SPSS software.
Results: The data analysis showed that in terms of usability of the mobile websites of university libraries, Kerman, Birjand, and Tehran universities of medical sciences were in the first to third ranks with 89.23%, 84.62%, and 81.54%, respectively. Neishabour, Gonabad, and Semnan universities of medical sciences were the last ranks with 40%, 43.08%, and 44.62%., respectively. The evaluation of the research components showed that “system visibility status,” “assisting the user in identifying, detecting and compensating for errors,” and “guidance and documentation” were in the best situation, and components of “error prevention,” “flexibility and system efficiency,” and “User control and freedom” were in their worst condition.
Conclusion: Given the increasing influence and popularity of smart cell phones in the professional life of academics, it is necessary for administrators and designers of mobile websites of academic libraries to consider the applicability and effectiveness of mobile websites of the research community, taking into account the practical findings of the present study.

Abstract

Background and Aim: In recent years, the family physician plan has been implemented as a main strategy of health system in Iran. Therefore, the necessity to reform organizational structure based on experiences of other countries is felt more than before. The aim of this study was to explore required structures of Family Physician Program to achieve service quality dimensions in Primary Health care through analyzing country experiences.
Materials and Methods: This study was a systematic review. All relevant databases were searched using appropriate search strategies and keywords (Family Physicians, Primary Health Hare, Quality of Care). To evaluate the quality of selected papers, CASP tool was applied by 4 experts, and their choices were discussed to reach a final decision.
Results: In order to achieve the quality of services in the field of family medicine, based on the findings of this study, eight important executive structures must be considered. These structures are: Organizational and managerial structures, including health system governance, Support  mechanisms and referral system, Systematic communication platform, Electronic health services, Service delivery processes,Insurance structure, Supervisory and control structure including financial control mechanisms, competitive control and quantitative control, Payment structure, quantitative and qualitative development structure of service providers, Quality structure that includes the definition and evaluation of quality and accountability mechanisms as well as incentive mechanisms for service quality, Support structure including insurance support, classified support for specific groups, and finally, the cultural structure in the two areas of culture building of the referral system and strengthening the position of family physicians.
Conclusion: Quality improvement in primary health care requires attention to executive structures. Use of executive experiences of other countries will be useful in achievement of quality health care in family physician system.
