Abstract

Background and Aim:  Hospital as an economic entity, should utilize economic analysis for the optimal use of resources and production facilities. The aim of this study was to estimate the production function of Ahvaz University of Medical Sciences during the period 2006-2014.
Materials and Methods: This study was a descriptive cross-sectional analysis. The Data were collected during a period of 9 years in 7 hospitals in Ahvaz. The dependent variable in this study was the number of hospital admissions as the production level and the explanatory variables, including nurses, physicians, staffs, and the number of beds. To determine and specify the form of subordinated Cobb - Douglas production function was used. Data analysis and econometric model was estimated using STATA11. 
Results: Research findings showed 0.28 elasticity for the physicians, elasticity for  nurses, number of beds , outpatient reception and other staffs were 0.10, 0.95, 0.30 and -0.11 respectively. In other words, the most production in these hospitals was dependent on the number of beds. It was an economic activity that consumed a lot of capital. The sum of the coefficients of the production function was 0.88 and the return to the scale was downward.
Conclusion: The findings of this study showed that hospital beds have great importance in increasing production. Therefore, the rate of active bed occupancy in these hospitals is high and has the largest role in the production of hospitals or the number of inpatient discharge.

Abstract

Background and Aim: This study aimed to propose a consistent architecture to design integrated and flexible information systems for the Vice-Chancellor for Research and Technology of Ahvaz Jundishapur University of Medical Sciences (AJUMS). 
Materials and Methods: This applied research employed an integrated design based on business system planning (BSP) and James Martin's model for the design of information systems. The statistical population of this study included 27 managers and experts of AJUMS Vice-Chancellorship department for Research and Technology. Data collection was carried out using structured interview, observation, and document analysis. A contrast matrix was then used for data analysis.
Results: In total, 53 processes and 60 data classes were identified. Using a multi-layered architectural model (presentation, process, data and infrastructure), the results were presented in the form of a proposed architecture. In the proposed architecture, 12 subsystems and the relationships among them were identified; such subsystems as library, research activities, sabbatical leave, conference, fellowship, publication, innovation and technology, laboratory information, theses, budget and planning, social subsystem and knowledge-management networks. The results also showed that the level of coverage of the processes, data classes, and objectives by existing information systems were 53%, 50%, and 55%, respectively, and increased to full coverage in proposed or optimal modes.
Conclusion: By identifying the required information systems, it is possible to accurately plan and successfully use these systems in this study. The results of the present study can be used to implement the architecture of information systems of Research and Technology Departments of other universities of medical sciences (UMSs) in Iran, which can have a significant effect on saving cost and time.

Abstract

Background and Aim: The growing health care needs of students actuated world Health Organization to develop School-based health projects in the last decades. The most important plan that has received worldwide attention and in Iran for nearly one decade is Health Promoting School (HPS) program which being implemented with collaboration of Ministries of Health and Education. Present qualitative study was conducted to evaluate this program implementation in Ahvaz education organization (District 1).
Materials and Methods: In this qualitative study data was collected by semi-structured interview. The study population consisted of school health experts affiliated with Ahvaz West Health Center and health deputy, health experts affiliated with education organization (District 1 and school headmasters and expert staffs in HPS (22 persons). The sampling was purposeful and the interviews continued until saturation of data. Content analysis was used for analysis of the data.
Results: Data analysis showed that most components of the HPS program in schools belong to education organization were not implemented properly. The major barriers were categorized into 5 and 47 main and subsidiary themes respectively. Poor stewardship and authority, policymaking and legislation, weaknesses in motivation, attitude and awareness of managers and experts, inadequacy in resources and infrastructure, ineptitude in program implementation and finally inappropriate evaluation system was specified as the main themes. According to results, some of the most important reasons for failure in HPS program consisted of, lack of functions definiteness between health deputy in ministry of health and education organization, job dissatisfaction in school health experts, not being a priority behalf education organization, insufficient delegation of authorized budget, lack of interpectoral cooperation between trustee organizations and indifference about program evaluation.
Conclusion: The most important challenge in the HPS program is the implementation barriers that make the establishment and implementation of this program almost ineffective. The serious and committed interaction between the ministry of Health and Education and between their respective provincial and county districts can provide the foundation for the implementation of this valuable program. A review of the organizational structure and executive framework of the program is also necessary.
