Abstract
Background and Aim: Patients' needs should be acknowledged properly in order to enable hospitals to take responsibility for their expenses. In this study, patients' satisfaction is assessed in various fields which are key elements of hospitals assessment program.
Materials and Methods: In this cross-sectional study, 120 consecutive patients which are randomly selected from Hospitals affiliated to Tehran University of Medical Sciences (TUMS) were enrolled. A questionnaire was completed for their demographic, occupational, insurance status and their satisfaction of clinical services, nutrition, hospital environment and hospital facilities.
Results: The age range of participants were11 to 80 years old 61.9% were females and 38.1% were males. Overall, 78.1% were satisfied in respect to hospitals general status while there were significant differences considering satisfaction in hospital environment and facilities, clinical services and nutrition among recruited hospitals.
Discussion and Conclusion: A strong correlation between the index of hospital environment and patients' satisfaction signifies further enhancements of hospital environment which could potentially increase patients' satisfaction. Also, the relationship between patients well being and satisfaction indicates health care staff role should be emphasized for patients' appreciation. Consequently, hospital administration should focus on current unsatisfied sections to improve health care quality.

Abstract
Background and Aim: Quality of printed papers in 4 four medical journals affiliated to one of the universities of medical sciences in Iran was assessed based on methodological and statistical methods.
Materials and Methods: In this cross sectional study conducted in 2010, out of all articles published between the years 2002 to 2008 in one of the universities of medical sciences for affiliated journals that were equal to 1568, a sample of 162 articles were selected with Multi-stage stratified sampling method . For data collection, standard questionnaire was used to develop an information form. One sample t-test and Kruskal-Wallis test were used to compare scores from different parts of article.
Results: Cross sectional studies with 61.7% (100 papers) formed most of studies. Median of authors and number of references for each article were 3 and 19, respectively and 10% of articles had more than 35 references. Material and methods with 3.53 points from 4 points (88.2%) earned the mighest score which was of high quality and conclusion achieved the lowest score. Also p-value has been reported correctly in 102 articles (81.6%). In 95 articles (58.6%) results were not compare with the similar internal articles whereas only in 21 articles (13%) comparison was not made with similar external researches.
Conclusion: The present study showed that the overall quality of the medical articles is not desirable. Hence, employing experts in research methods and statistics in various medical studies can improve the quality of articles in this branch of science.

Abstract
Background and Aim: Organizations have to establish modern management systems to progress and to obtain maturity. One of the techniques which started in Europe in the last two decades and is considered in management area of country during recent years is the model of organizational promotion (EFQM). The purpose of this study was Performance Evaluation of Rajaei Hospital Based on «EFQM» Organizational Excellence Model.
Materials and Methods: This cross-sectional study was conducted in year 2010 in Qazvin Rajai Hospital. Data collection tool, was EFQM organization Excellence Model questionnaire which was completed by all the managers. Validity and reliability of questionnaire was calculated. Results were analyzed using descriptive statistics.
Results: Results indicated that this hospital has acquired 99.183 scores out of 500 in enablers factor(88.36 percent of optimum score). Also has gained 183.71 scores out of 500 (36/75 percent of optimum score). Overall, this hospital has obtained 366.7 scores out of 1000 as the total score in the model for performance evaluation in year 2010.
Conclusion: Findings indicated that based on Excellence model criteria, this hospital is in a moderate level. Applying this model for Iranian hospitals which often meet serious problems in this area is informative and practical. Strong systematic view, paying attention to organizational process-based management and being result-oriented are advantageous of using this model.

Abstract
Background and Aim: Hospital internal evaluation is considered as one of the most important steps in strategic planning. Therefore, besides the recognition of the weaknesses and limitations of the organization, especially affecting issues on the performance, the responsibility of the organization will be defined. This study was performed for the assessment of internal environment of the Gachsaran's Shahid Rajai Hospital based on Weisbord Organizational Diagnosis model.
Materials and Methods: In a cross-sectional study, 123 hospital staff were selected using random sampling formula. Data was collected using Weisbord Questionnaire which consisted of 35 questions in seven areas. Data analysis was conducted using SPSS statistical software and the t-test.
Results: Among six variables studied, variable communication with 3/15 ± 1/25 mean and standard deviation has the best situation and reward mechanism with the mean and standard deviation of 4/59 ± 1/32 was the worst. There was a meaningful difference among all variables except for the leadership variable.
Conclusion: Based on the findings of this study, Gachsaran's Shahid Rajai hospital has strength in the fields of goals, organizational structure, coordination, and internal organizational communication

Abstract
Background and Aim: According to the objectives of the information systems and to avoid duplication and help to improve the quality of care and reduce costs, HIS ongoing evaluation should be conducted to achieve these goals. This study has evaluated hospital information systems in selected hospitals with the use of "integrated hospital information system evaluation criteria-2011".
Materials and Methods: This study was a descriptive-sectional research. The study population consisted of information systems in Shohada, Rsool Akram, Khatamolanbia, Imam Khomeyni and Milad hospitals. The data collection tool was a checklist of HIS indicators. Checklist was completed with direct observation and interviews with HIS users in selected hospitals. Data was analyzed by SPSS statistical software, and results were presented in statistical tables and charts.
Results: In the studied hospitals most of the subtypes of organizational and server components of the hospital information system has been set up and used. However, pharmacy, decision support, medical services, communication services and telemedicine information systems, are not yet fully set up in the studied hospitals.
Conclusion: Most subtypes of organizational and server components, currently exist in all fields in the studied hospitals.

Abstract
Background and Aim: Collaboration within and between healthcare teams facilitates effective healthcare provision. Fundamental strategies in effective healthcare services focus on collaboration and teamwork. This study was an attempt to assess reliability and validity of Team STEPPS Teamwork Attitudes Questionnaire (T-TAQ) in Iranian context.
Materials and Methods: This cross-sectional survey was conducted in Iran in 2012. Delphi method was applied and the questionnaire was submitted to 11 experts in 2 rounds to determine its face and content validity. Besides, it was translated and back translated to determine external validity. To estimate the reliability of the instrument, test-retest method was used. A sample of 67 medical and nursing senior students was stratified from Tehran University of Medical Sciences (TUMS). T-TAQ is a questionnaire having 30 questions in 5 themes. The obtained data were analyzed by SPSS13.
Results: The content, face and external validity of the tool were approved by Iranian experts. Meanwhile, applying Cronbach's alpha, total reliability was estimated to be 0.80 moreover, the ICC turned out to be 0.8.
Conclusion : Since the validity and reliability of this instrument are confirmed in an Iranian context, it can be used to measure attitude regarding teamwork in this context.

Abstract
Background and Aim: In order to develop teaching competencies and prepare PhD candidates for future roles as faculty members of medical schools, the present study conducted to determine PhD candidates` educational needs and their skills concerning teaching competencies.
Materials and Methods: The present study was a descriptive- analytical and cross-sectional study that was conducted in year 2011. The Study population was PhD candidates who studied in Health School of Tehran University of Medical Sciences. In the present study a “Teaching Competencies Assessment” questionnaire was used that included 2 part s the demographic information and 16- items of educational competencies and needs. The validity and reliability of th e questionnaire was approved by alpha Cronbach’s coefficient (educational need 94% & educational skill 87%). Data analysis was conducted using SPSS.
Results: The result of the present study showed that teaching skills of PhD candidates were at “familiar without implementation capability” level. The lowest candidate s` skill was “Student Assessment” field. The candidates had educational needs in all 16 areas of teaching skill fields (3.85:5). “Lecture Presentation” (4.1:5), “logical structure of Presentation” (4.02:5) and “Motivating methods” (4.01:5) fields were the highest educational needs of PhD candidates.
Conclusion: The results of the present study confirmed the need for systematic planning in order to develop teaching competencies and prepare PhD candidates for teaching role in future.

Abstract
Background and Aim: Every educational system, whether micro or macro, needs its training programs to be studied and educationally evaluated. This study aims to assess training and internship programs in hospitals based on surveying the students of Tehran University of Medical Sciences (TUMS).
Materials and Methods: This is a cross-sectional study with survey descriptive design. The data collection tool was a questionnaire with 14 five-point Likert style items. The TUMS Educational Development Center (EDC) surveyed all 185 students of training and internship programs, using a questionnaire with the Cronbach’s alpha coefficient of 0.81 in 2011, in order to adjust the education with the society’s needs and to improve its quality.
Results: The mean assessment scores of training and internship programs were 3.32%, 2.98%, 3.38%, and 3.29% for applied training, mental skills, communication skills, and practical skills courses, respectively.
Conclusion: The students’ satisfaction rate regarding the quality of educational courses of training and internship programs was 50.2%. Educational authorities have made necessary attempts to implement these programs and use potential resources for realizing the objectives and achieving the educational quality however, they should make further attempts in this regard.

Abstract
Background and Aim: Thermodynamic indicators are considered in scientometrics studies to survey research performance as the Exergy that is most important ones. Therefore, this article is aimed to introduce, review, definition, application and importance of Exergy.
Materials and Methods: This study is a review article that determined by using related literatures with thermodynamic indicators of scientometrics by library research, and simple and advanced searches of English papers on valid websites. 30 English papers were retrieved and 24 articles related to this study were selected but among Persian literatures, no related papers were found.
Results: Exergy is a thermodynamic measure which reperesenting a maximum useful work that a system can perform in order to create balance. This indicator is applicable in bibliometrics and as an indicator of research performance for ranking of research institutions.
Conclusion: According to the role of exergy in assessing the research performance of individuals and organizations, and its advantage over H-index, due to consideration of all the articles and all the citations, this indicator can offer new reperesentation of rankings.

Abstract
Background and Aim: To improve the implementation of clinical governance (CG) in Iran's hospitals, awareness of various aspects of its implementation and assessment is important. The aim of this study was to find out the obstacles and challenges of clinical governance implementation and assessment in Qazvin teaching hospitals. Materials and Methods: Semi-structured interviews with 17 senior managers, clinical staff and clinical governance experts were conducted in six hospitals of Qazvin University of Medical Sciences (QUMS). To analyze the collected data, framework analysis was used. Results: The challenges and obstacles of CG implementation in Qazvin teaching hospitals were explained using three themes of (1) challenges of CG implementation, (2) improvement in seven dimensions of CG, and (3) challenges of CG implementation assessment. Conclusion: The results of this study show that CG implementation needs to address three issues: improving clinical staff awareness about CG, changing organizational culture so as to make it more receptive to CG, and creating higher levels of cooperation among physicians, managers, patients, specialists, and professionals. Abstract
Background and Aim: Given the increasing growth of new technologies in health care and their rising cost, the necessity of concentration on assessing new technologies and their potential role in quality of health care is more crucial. The aim of this study was to investigate the role of health technology assessment (HTA) in improving healthcare quality and offer the best evidence for decision makers.
Materials and Methods: The study was based on systematic review of papers in health technology assessment via searching in databases such as Pubmed, Cochrane and other HTA related databases. The search has covered the period between years 2000 to 2013. Studies analyzed by narrative synthesis method.
Results: Related studies show that assessing new technologies has potential role in quality improvement of healthcare. HTA can boost the competition among companies which this in turn increases the quality of technologies and ultimately enhances the quality of health services and stakeholders satisfaction.
Conclusion: HTA may increase quality of healthcare services through facilitating a move towards evidence-based decision-making, providing safe health care and increasing patients’ trust and satisfaction to clinical procedures and team members.

Abstract
Background and Aim: An assessment framework is necessary in order to achieve the goals of health system in any country. The current study was done with the aim of measuring the effects of the aspects of performances on realization of health system goals.
Materials and Methods: This study was a cross-sectional, descriptive-analytic which was carried out during the years 2014-2015. Four hundred and nineteen experts among all experts in the area of health by snowball and cluster sampling participated in the study. Collecting data was performed through the questionnaire of Health System Perfomance Assessment. The questionnaire’s validity was confirmed using content validity method and its reliability was confirmed using Cronbach's alpha coefficient. Data was analyzed by means of Pearson correlation coefficient test and Regression using SPSS 22 software.
Results: The results of Pearson correlation coefficient test showed that the two component functions on equitable access to health services, the functions of protection against financial risks and actions on targets were equal to 0/827, 0/655 and 0/870. Respectively the results of the test showed 87% of the variance explained the aims of the functional components. Thus, we conclude that the functions of the financial risk for changes in standard deviation units and functions on equitable access to health services was as much as 0/655, and 0/827 predictable.
Conclusion: Since the aspects of performances influence the realization of health system goals, investigating the effects of performances on the health system goals can help policymakers and managers to improve the performance through identifying weaknesses and strengths, review of interventions and healthcare reform, and the management of the health system.

Abstract
Background and Aim: The starting point for any research is the problem-solving and Research Needs Assessment is needed to identify the underlying issues. This study was conducted to determine the research priorities of the Deputy of Health of Tehran University of Medical Sciences.
Materials and Methods: The present study was an applied study that was designed and implemented in five stages; 1. holding a workshop to teach nominal group technique; 2. Carrying out interviews and the primary extraction of research priorities; 3. Conducting the first round of nominating sessions; 4. holding the second round of nominal group meetings, 5. Finalizing research prioritization titles. Data were analyzed by Excel and Spss version 19 software.
Result: This study was conducted with the participation of 38 people. At first, 1039 minutes of interview, 258 original research titles were extracted. Initial research titles were scored during the 4 sessions. At the end, 75% of the participants rated the 21 study points as 7-9, which were considered as research priorities. The titles "Investigating the root causes of maternal death" was the most priority and "the assessment of the effectiveness of the screening of hypothyroidism" was the least priority.
Conclusion: According to findings, there is consensus among stakeholders on research priorities that require investment in generating knowledge related to health goals. Managing the research resources of the organization towards implementing these priorities will lead to a rational allocation of resources for the production of knowledge and applied products, and ultimately to improve the health of the population.

Abstract
Background and Aim: Diabetes Type II is a chronic metabolic disorder rising its prevalence worldwide. Self-care is the most important management strategy to control the disorder and its adverse effects. The aim of this study was to design and validate an assessment tool to determine the level of self-care of patients affected by Diabetes type II.
Materials and Methods: This study was a cross-sectional study. To conduct this study, based on reviewing the texts and reviewing the existing questionnaires, the proposed items were prepared and by eliminating and integrating similar items into a questionnaire in 4 areas related to diet, blood sugar monitoring, Physical activity, drug use was designed with 15 questions. Thirty patients with type 2 diabetes referred to the endocrinology clinic completed a questionnaire. The reliability of the questionnaire was assessed using Cronbach’s alpha and the validity of the questionnaire was assessed by content validity (CVR). Data were analyzed using SPSS software.
Results: The results showed that the mean and standard deviation of the age of the studied units was 52.4±12.51 years, of which 50% were female and the other 50% were male. 56% of them had type 2 diabetes for less than 5 years. Also, people with higher education had relatively better metabolic control in diabetes management and patients ‘answer to question 6 had the highest mean, which shows patients’ attention to blood sugar control. To determine the reliability of the questionnaire, Cronbach’s alpha coefficient for all questions was 0.773, with the omission of question 9, it was increased to 0.796. Only two validity questions were 0.66 and 0.16 which were excluded from the test. Pearson correlation coefficient was calculated for each question.
Conclusion: The results of this study showed that the questionnaire has the necessary validity and reliability. With this tool, appropriate advice can be provided to patients with type 2 diabetes in the field of self-care, including diet, medication, physical activity and blood sugar control to prevent the progression of the disease and its complications.

Abstract
Background and Aim: Inattention of hospital waste management has caused many problems. As a consequence, it has put the environment and human health at serious risk. Medical centers are one of the important health institutions which in recent years, population growth, increasing clients and service populations has led to significant growth in waste production in them. The quality and quantity of medical centers waste is becoming more and more problematic and is considered as a dangerous toxic substance. The purpose of this article is to evaluate, prioritize and explain the management plan for treatment and disposal of medical centers waste.
Materials and Methods: Important criteria in choosing the method of waste disposal of medical centers were extracted from previous studies. Then, the importance of these criteria was determined using the Fuzzy Analytic Hierarchy Process (Fuzzy AHP) and at the end, the best disposal method was selected using the Fuzzy Weighted Aggregated Sum Product Assessment (Fuzzy WASPAS).
Results: Among the main criteria, environmental issues was the first priority with 0.517 weight and the criteria of cost and volume of waste with the points of 0.317 and 0.166 were ranked second and third, respectively. Also, between the sub-criteria, the air pollution was ranked first with 0.256 weight. Finally, among the disposal methods, the sanitary landfill method with 0.317 weight was ranked first and the microwave methods, waste incineration and autoclave with the points of 0.265, 0.253, and 0.173 scored second to fourth respectively.
Conclusion: Considering the obtained results and prioritizing, it is possible to achieve more risk reduction, reduction of environmental pollution and reduction of costs by disposing of waste in health centers. Accordingly, the most appropriate method for disposing of waste in medical centers in the country is the “sanitary landfill” method. Of course, sanitary landfilling of waste should be done in an appropriate and standard way to prevent groundwater pollution and damage to the environment. Solving the problems caused by hospital waste is a complex and multidimensional issue that requires the cooperation and assistance and integration of community facilities (government, private and public institutions).

Abstract
Background and Aim: Operation rooms have several specialty processes, a higher level of technology, complicated treatment protocols, and the need for skillful human recourses, which is one of the highest risk wards in the hospital. Therefore, this study was initiated to identify and evaluate potential errors by using the Failure Mode, Effects, and Criticality Analysis (FMECA) approach to recognize the potential errors in operation rooms of Golestan hospital of Ahvaz.
Materials and Methods: This research was done with a qualitative approach in seven stages and it was based on the FMECA protocol. Data were obtained through direct observation, assessment of documents and interviews with the related staff. In this regard, surgical processes were extracted from the beginig of the surgical planning to discharge patient from the post-anesthesia care unit, after that the potential errors associated with each process were identified. Finally, the risk priority number of each of them was calculated according to the indicators of Occurrence (O), Severity (S), and Detectability (D). Score analysis was performed using descriptive statistics and SPSS software.
Results: In the present study, during the analysis of processes related to surgical care, 17 primary surgery processes and 75 sub processes (from surgical planning to discharg from the post-anesthesia care unit) were identified. Seventy failure modes were identified. Ultimately, after analyzing the failure modes in the risk matrix, among the 70 identified failure modes, two failure modes had unacceptable risk, including no proper cleaning of the operation theatre and marking the surgical site, and there was Seven other failure modes with moderate risk, including unappropraite hand hygiene and environmental and operating room fixed equipment disinfection, central oxygen disconnection, lack of equipments in night shift, delay in delivery of prostheses to the surgical site, transfer of patient who requires intensive care to the ward, were identified. Human and organizational causes contributed the most to the occurrence of potential errors.
Conclusion: Analysis of failure modes showed that the highest probability of error occurs in the processes during surgery and due to human and organizational factors. Identification of 70 potential errors in 17 processes of the Operating Room indicates the integrity of FMECA’s preventive approach in identifying and prioritizing the high-risk areas of the processes, insensitive parts such as the operating room.
