Abstract

 Background and Aim: Evidence Based Medicine (EBM) is the explicit use of current best evidence in making decisions about the care of individual patients. Hospital information system (HIS) can act as a bridge between medical data and medical knowledge through merging of patient's data, individual clinical knowledge and external evidences .The aim of this research was to determine the Capability to establish EBM by HIS.

 Materials and Methods: This descriptive cross-sectional study was carried out on HISs of 30 hospitals from March to October 2011. Data were collected using a researcher- constructed checklist including applicant’s background information as well as information based on research objectives. Validity of the checklist were assessed by the qualified specialists and then the data were analyzed using descriptive statistics and SPSS software.

 Results: HISs lacked the essential components for providing access to CDSS, Reference databases and internet-based health information in 19, 16 and 20 hospitals were 63.3%, 53.3% and 66.7%, respectively. Twenty-two hospitals (70%) had more than two-thirds of the essential components to access clinical and administrative data repositories 23 hospitals (76.7%) had at least one essential component to access contextual and case specific information.

 Conclusion: The Capability of HIS is better in order to place EBM in having access to the clinical and administrator data repositories while it needs more attention in other areas.

Abstract

 Background and Aim: Probiotics are useful bacteria which, after being consumed, leave useful effects on human health. The present study was conducted to examine the antimicrobial effect of Lactobacillus paracasei on pathogenic bacteria, especially on Staphylococcus aureus in the field of food microbiology.

 Materials and Methods: In this study, the effect of Lactobacillus paracasei isolated from yoghurt (1×108 cfu/ml) on the growth of Staphylococcus aureus (1×105 cfu/ml) was assessed at two different temperatures of 25 °C and 35°C in TSB. Then, samples from cocultures and control were pour plated in Baird Parker agar and MRS agar to compute the number of S. aureus and Lactobacillus paracasei at 0, 24, 48, 72 hours after incubation. Then, enterotoxin production was evaluated by ELIZA rida screen kit. The data were investigated by ANOVA and K-square (chi-square) tests.

 Results: It was found that compared to controls, Lactobacillus paracasei inhibited the growth of Staphylococcus aureus and reduced the number of S. aureus cells about 3 logs in 25°C and 2 logs in 35°C. The production of A, C, and E enterotoxins was inhibited by S. aureus at a temperature of 25°C according to ELIZA rida screen test. The figure was 35°C for enterotoxins E and A.

 Conclusion: Probiotics can be used as a new approach for the biocontrol of S. aureus without using antibiotics.

Abstract

Background and Aim: Strategic planning has an important role in achieving competitive advantage for nonprofit organizations. However, some strategic plans fail. The present study aimed to investigate challenges of strategic planning in Welfare Organization.
Materials and Methods: A qualitative approach was used for this case study. In-depth semi-structured interviews were conducted with 12 senior and middle managers who were members of the strategic planning committee. Content analysis method was used for data analysis, and 7 themes and 16 sub-themes were identified from the data.
Results: Lack of justification of the strategic planning, the strategic planning model, inappropriate composition of the strategic planning team, the structural and managerial changes of the Welfare Organization, time limitation, insufficient training, and budget constraints were the main challenges of strategic planning in the Welfare Organization. 
Conclusion: If the strategic planning challenges are not managed well, more problems are expected to occure during the implementation of the strategic plan. Senior managers’ commitment, strong management and leadership, participative corporate culture, education and training, staff empowerment, and provision of required resources will facilitate strategic planning implementation in organizations.

Abstract

Background and Aim: Today, OSL (Optically Stimulated Luminescence) dosimetry system based on optical stimulation, includes systems that can meet many requirements for radiation protection in the field of medicine and space.  The Calcium Sulfide (CaS) is one of the earth's alkaline sulfide materials which can be used as a detector in this system. In this research, some of the dosimetry properties of CaS doped with  Cerium, (Ce), and Samarium, (Sm) elements were investigated as OSL dosimeter sensor.
Materials and Methods: First, attenuation of x-rays through the sensors were analyzed and the absorbed dose rate was evaluated using MCNP code. After calcium sulfide tablets were fabricated and their concentration optimized, the maximum waiting time prior to readout was obtained. In addition, the repeatability and linear response of the detector were determined as a function of CaS concentration.
 Results: Eight minutes after radiation exposure of detectors, the detection output signals became stable. This stability was monitored for at least 30 minutes after irradiation.  The repeatability in measurements was observed within the dose ranges of 100 to 860 mGy. The dosimeter response was observed linear over this dose ranges.
Conclusion: According to the above-observed results and statistical evaluations, one can conclude that the CaS:Ce,Sm crystal  is a proper sensor for OSL dosimeter systems in medicine and space studies.

Abstract

Background & Aim: Physicians as human capital and resources are one of the main components of health production. The imbalance of physician supply and demand affects the health and economics. Therefore, this study aimed to estimate and forecast the supply and demand of physician working in Iranian medical universities.
Materials and Methods: This a descriptive-analytical and applied study was conducted at national level for Iran during 1991-2017. The statistical population was physicians working in Iranian medical universities. ARIMA method was used to estimate and forecast physician supply and Vector Error Correction Models was used for physician demand. The data is annual time series that was extracted from the statistical yearbooks of the Statistical Center of Iran and the World Bank database. Eviews 10 software was used to estimate the models.
Results: The results showed that physician demand in Iran was affected by Gross Domestic Product, age structure and hospital beds, and according to the forecast of supply and demand of physicians, we will be faced to the physician shortage in the years 2018-2030.
Conclusion: In the coming years, Iran is facing with physician shortage. Therefore, it is recommended to adopt policies to increase physician capacity in medical universities and to increase strong incentives to retain physicians and prevent their migration.

Abstract

Background and Aim: All people living in a society are not equally at risk of COVID-19 infection and hospitalization. The current study is aimed to determine the factors related to COVID-19 infection and hospitalization among participants of Ravansar Non-Communicable Disease (RaNCD) cohort study. 
Materials and Methods: The present study was conducted between 28th February, 2020 to 27th November, 2021. Data from the RaNCD study, morbidity and hospitalizations cases (extracted from the medical care monitoring system, SAMA system and the handwritten list of the health department) and positive serological results were used. For the case-cohort study, 626 confirmed and/or probable cases and 2107 randomly selected participants as sub-cohort were recruited. For the case-control study, 188 COVID-19 hospitalized patients from RaNCD (as cases) and 632 positive outpatients identified as controls. Data were analyzed using Cox and logistic regression, respectively. 
Results: In case-cohort study, the hazard ratio of COVID-19 among people living in rural areas, in smokers and those in middle socioeconomic status was, 0.37 (0.24, 0.58), 1.50 (1.07, 2.11), as compared to the reference groups. While the hazard ratio of people aged 46-64 and those ≥65 years old, as compared to the reference group within the first year, was 3.47 (1.76, 6.86) and 6.45 (2.80, 14.85), the corresponding value after one year was 2.23 (1.13, 4.40) and 3.52 (1.53, 8.10), respectively. The hazard ratio among those with at least one comorbidity was 2.45 (1.53, 3.92) in the first year of study and after one year decreased to 1.82 (1.14, 2.92). In addition, moderate and vigorous physical activity decreased the hazard ratio. In case-control study the odds ratio of hospitalization increased by 0.03% (1.03(1.01, 1.05)) with an increase of one year in age and in people with at least three comorbidities was 2.43 (1.24, 4.73) times of those without comorbidity.     
Conclusion: Overall, one year after starting the study the decrease in hazard ratio of COVID-19 was significant. Increase in age and having comorbidities are factors that increased the odds ratio of hospitalization. Such people can be considered for the next waves of COVID-19 prioritized for health care and booster vaccination.
