Abstract

Background and Aim: Diagnosis is an implement of organizational development. The main objective of this study was to diagnosis the TUMS by using Six Box Model of the Weisbord to identify in which areas of this model Purposes, Structure, Leadership, Rewards, Relationships, Helpful Mechanisms and Attitude toward Change is more vulnerable.
Materials and Methods: The research was descriptive and the statistical population included five hospital staff subset of TUMS. Out of 280 questionnaires we received 220 completed ones. For data gathering we used standard questionnaire of the Weisbord which was estimated as both reliable and valid. By the use of Cronbach alpha coefficient, reliability coefficients was obtained equal to 0.85. Also, questionnaire was confirmed by 5 faculty members in terms of nominal and content validity. The data were analyzed by SPSS software.
Results: All the variables except rewarding and Attitude toward Change had significant changes p<0/05 thus the H0 for Purposes, Structure, Leadership, Relationships and Helpful Mechanisms were rejected. These variables stay in acceptable condition. However there was a significant difference between Relationships and rewarding; considering their T-Value it could be concluded that Relationships situation was desirable (T=3/62) but rewarding situation was undesirable (T=5/70).
Conclusion: The findings conclude that TUMS, currently the most appropriate situation was in communication and the most inappropriate situation was rewarding. Also other aspects are between these two conditions. Also, rewarding area needs planning to increase employee satisfaction.

Abstract

Background & Aim: Planning in medical tourism is important. This study was conducted to assess factors, elements and development plans of medical tourism and situation study and analysis of Tehran University of Medical Sciences (TUMS).
Materials and Methods: This mixed method study was conducted in 2017. In systematic review, published studies searched between years 2006 to 2016. In the second phase, medical tourism data of TUMS explored as retrospective cross sectional and expert viewpoint analysis in medical tourism concept. 
Results: 39 published articles were assessed. According to results, admissions and total payment of foreign patients in all hospitals of TUMS in 2016 were 21788 and around 18.5 billion Tomans respectively.
Qualitative results showed that, there were not transparence program and suitable physical area and TUMS needs to comprehensive planning, supportive infrastructure of foreign patients, correction of transportation system and improve marketing.
Conclusion: Despite the history of TUMS hospitals in attraction of foreign patients and analyzing it, there were many limitations to achieve good situation. Although assessing patients viewpoints can provide a platform to solve problems and limitations according to priorities.

Abstract

Background and Aim: Job stress is one of the reasons of low motivation and leaving one's job, and its reduction can have positive consequences. According to studies, staff's development and their positive self concept toward workplace can reduce job stress. The aim of this research is to study the effect of human resource development on job stress with regard to the mediator role of self concept in Shiraz University of Medical Sciences(SUMS).
Materials and Methods: This is a correlational descriptive study. The research community included 584 staff members, of whom 234 were selected using Morgan's table in 2018. To collect data, 3 standard questionnaires were used, and their content validity and reliability were confirmed. For data analysis, structural equation modeling and Amos and SPSS softwares were used.
Results: The findings showed that human resource development had a reverse effect on staff job stress(path coefficient -0.778). Moreover, human resource development had a direct effect on job stress(path coefficient 0.843), but staff self concept did not have any effect on job stress(path coefficient 0.166). Since the paths of human resource development-self concept and self concept-job stress were not meaningful, human resource development did not have any effect on job stress by way of the mediator variable of self concept in confidence level 0.95.
Conclusion: According to the results of this study, conducting the staff's continuous development to promote their skills and abilities for declining job stress is emphasized.

Abstract

Background and Aim: Preeclampsia is one of the most serious cases of high-risk pregnancies that endanger women’s health worldwide, especially in developing countries. Preeclampsia is a specific pregnancy syndrome with a prevalence of about 7-14%, which is one of the three leading causes of death in pregnant women. Preeclampsia is the second most common cause of maternal mortality in Iran and accounts for 14% of maternal mortality. The present study was conducted to design, create and evaluate mobile-based preeclampsia self-care application.
Materials and Methods: This study was conducted in four stages to assess the needs of information elements, design, create and evaluate preeclampsia self-care application. In needs assessment step, 42 specialists, assistants and personnel related to the subject working in the Amir Al-Momenin (AS) Educational, Research and Treatment Center affiliated to Semnan University of Medical Sciences participated. The program was then initially evaluated by 7 physicians, and finally the suggestions provided by users in the design of the program were applied and the final version of the program was completed. The application was designed in the Android Studio environment and then its usability was evaluated using the opinions of 20 mothers and the QUIS tool.
Results: The information elements and functional capabilities required by the program were determined. In addition, the program established communication between the patient and the provider, also created the possibility of care management and control of the disease process. The performance of the program was evaluated by physicians and experts and then evaluated by pregnant mothers in terms of usability. The findings showed that users were satisfied with the application.
Conclusion: The use of mobile-based applications is a useful way to increase knowledge and promote the health of pregnant mothers and facilitate their access to medical information and acquire the necessary skills in their disease. This program helps pregnant mothers with preeclampsia to control their disease by observing proper nutrition and treatment principles to minimize the complications of their disease.

Abstract

Background and Aim: The healthcare sector plays an important role in the development of communities by ensuring the health of individuals in each community and can have many social and economic effects on the lives of individuals in the community. The purpose of this study was to determine the development of the country’s provinces based on health facilities and the level of access to these facilities, to find solutions to reduce differences between different parts of the country.
Materials and Methods: This study was an applied study with a descriptive-analytical approach. In this study, 31 provinces of the country have been studied in terms of 10 indicators. Data and information of this study were extracted from the national statistical yearbook and ranked using Center-weighted index method by using Excel software. Then, using taxonomy method, the degree of development of the provinces has been determined. 
Results: The results of the study show that South Khorasan province with a centrality index of 45,692 is in the first place, Yazd province with a centrality index of 43,933 is in the second place and Ilam province with a centrality index of 40,668 is in the third place. East Azerbaijan province with a centrality index of 27,375 is ranked 29th, Qom province with a centrality index of 25,417 is ranked 30th and Hormozgan province with a centrality index of 24,286 is ranked 31st. The degree of development of the provinces is between 0.507 to 0.98, which in terms of development of Yazd province is in the first rank and Hormozgan province is in the last rank.
Conclusion: The results show that there is a deep gap between the provinces in terms of development and access to health indicators; Therefore, it needs more attention and adopt the necessary policies and plans to reduce this gap.
