Abstract
Background and Aim: The benefits of a unique computerized medical records and communication equipments is utilized to collect, storage, process, extract and relate to patient care information and management information. If this utilization is in accordance with the needs of managers and staff of medical records, many of the nation's health system problems will be dissolved. Information systems in health supply system by compiling, analyzing and publishing data provide utilization of information improve the health of the society. The purpose of this study is to investigate the process of clinical information systems in general hospitals of Tehran University of Medical Sciences based on available clinical information system in these hospitals.
Materials and Methods: This descriptive study is an application study that conducted in order to determine the process of clinical information system in general hospitals of Tehran University of Medical Sciences based on available clinical information in these hospitals. The required data have been collected by a valid and reliable questionnaire. Descriptive statistics were used to analyze data.
Results: With regard to the current condition and the importance of survey of the process of clinical information system in the studied hospitals, the authors provided the desired checklist in general hospitals with studying the related system in America, Australia and England countries, on the basis of country requirements in the form of clinical information system processes. We studied clinical information system processes in departments of admission, filling and retrieving, statistical and coding and also soft wares in this department.
Discussion and Conclusion: It is necessary to create a coordinated and integrated clinical information system with suitable process and empower experts in the field of proper use of data with regard to the position of information system in each organization and its fundamental role in the adoption of accurate policies. We found that relative frequency of clinical information system processes is more than 60% in this department and clinical information system elements was observed 75% in all the departments except admission.

Abstract
Background and Aim: Hospital's traditional charter that mostly emphasized productivity has been replaced by a charter that stresses social and environmental issues. On this basis, to achieve success, managers should regard both the inner and the outer environments of hospitals. This study aims at measuring hospitals' social responsibility.
Materials and Methods: The present descriptive-analytic study was carried out through a cross-sectional method in 2010. The study population consisted of hospital managers of the city of Isfahan (n=21). The data collection instrument was a researcher-made questionnaire. The instrument's content validity was determined by the experts' views and its reliability calculated through Cronbach's α was found to be 0.95. The collected data were analyzed using SPSS software (version 16) and the hypotheses were tested by t-test, ANOVA and Eta coefficients. The maximum mean score was 100.
Results: The mean score of hospitals' social responsibility was computed to be 68.6. The mean scores of leadership and inner processes, market place, community policies, environmental policies, and workplace policies were found to be 72.2, 70, 67.8, 67.4 and 66.2, respectively. There was no significant relationship between hospital's social responsibility and type of hospital's possession and activity (pvalue > 0.05).
Conclusion: The social responsibility of hospitals was assessed as moderate. To improve hospital's social responsibility, it is suggested that more attention be paid to workplace and environmental policie

Abstract
Background and Aim: Over the past decade, the number of private centers providing health care services in Iran has had a continuous growth and the highly competitive market in private hospitals industry has caused increasing pressure on them to provide services with a higher quality. The purpose of this study was to assess the quality of services being provided in private hospitals of Tehran as judged by patients.
Materials and Methods: This cross-sectional study was conducted in 2010 in Tehran. The study sample comprised 983 patients, randomly selected from 8 private hospitals. The survey instrument was the standard SERVQUAL questionnaire. Wilcoxon, t-test, and Kruskal-Wallis tests were used for statistical analyses.
Results: The total mean scores of patients' expectation and perception were 4.92 and 4.02, respectively. The highest figures were related to the tangible factors dimension and the lowest ones to the empathy dimension. The differences between the perception and expectation for all the 22 items and the 5 dimensions were found to be statistically significant(p< 0.001).
Conclusion: SERVQUAL is valid and reliable in hospital settings as a standard tool for the assessment of quality. The results of the present study have revealed the areas that need to be improved as far as the patients are concerned. Measuring the opinion of patients can help facilitate the provision of hospital services, and improve the quality of such services and patients' satisfaction.

Abstract
Background and Aim: Work engagement is defined as a positive, fulfilling, work-related state of mind that is characterized by vigor, dedication, and absorption. Since the quality of care delivery to patients depends on nurse's practices, the objective of this study was to determine work engagement & its effective factors among nurses of general hospitals of Shiraz University of Medical Sciences.
Materials and Methods: This cross sectional study was performed among 273 in-patient nurses of Namazi and Faghihi hospitals using 2 stages of sampling. Our questionnaire was validated by experts opinions and Cronbach's alpha index (86%). Descriptive statistics and correlation tests were used for the analysis of data by SPSS.
Results: According to the results obtained, mean work engagement was 58.95 ± 10.51 and there was a significant correlation between work engagement and organizational - supervisor support, organizational justice, job characteristics, reward & recognition, organizational citizenship behavior, organizational commitment, job satisfaction, turnover( p<0.05). The relationship between work engagement and reward was the strongest(r=0.512).
Conclusion: The results imply the importance of work engagement and its related factors. Nurses are one of the biggest health system groups, and their burnout disadvantages hospitals and community. Thus, work engagement should be noticed in all policy-making related to nurses.

Abstract
Background and Aim: In many countries, hospitals have triple ownership (governmental or public, for-profit, and not-for-profit). Numerous studies have been performed in order to investigate the differences among these hospitals in accordance to expenditures, presentation of uncompensated therapeutic services, number of outgoing patients and other measurement devices of hospital performance. The aim of this research was to investigate the role of ownership on total Factors Productivity changes in Qazvin`s selected hospitals.
Materials and Methods: Descriptive-analytic study was conducted in three periods of time from 2005 to 2007 in 8 selected hospitals of Qazvin. Inclusion criteria included the active beds and the value of intermediate consumptions in hospitals.The exclusion criteria included the total number of inpatient and surgical operations. Data collection tool, was researcher-designed checklist. Data were analyzed using DEAP 2/1 software and ANOVA, HSD and TUKEY tests.
Results: There was a significant difference between the mean productivity of three hospital groups due to the existence of various ownership(p=%012), Also a significant difference was observed between the mean of technological efficiency of the three groups of hospitals(p=%006), No significant differences was found between the mean of technical efficiency of the three groups of hospitals(p=%23).
Conclusion: Cause of the variation in total factor productivity in private hospitals, is due to their suitable production technology which is always improved. But in respect to their efficiency promotion and optimization of production and products, there was no distinguishing difference with other hospitals.

Abstract
Background and Aim: Self-reliance is a psychological belief that is formed in an individual during growth and is not changed easily and quickly.
The present study aims to evaluate the relationship between nursing managers' self-reliance and patients' satisfaction in hospitals affiliated to Tehran University of Medical Sciences(TUMS).
Materials and Methods: In this cross-sectional study, 400 individuals(200 nursing managers and 200 patients) were interviewed in various clinical wards of TUMS hospitals. Nursing managers' self-reliance and patients' satisfaction were evaluated using a structured questionnaire.
The data were entered into Stata 11 software and analyzed with non-parametric ANOVA and Spearman's correlation coefficient tests. The significance level was determined to be p<0.05.
Results: Of the nursing managers, 58(29%) were male and 142(71%) were female. The mean and SD of their self-reliance were 134.9 and 19.8, respectively. The patients, however, comprised 81(40.5%) males and 118(59.5%) females. The mean and the standard deviation of patients' satisfaction were 57 and 18.2, respectively the only individual factors affecting their level of satisfaction were educational level(p<0.005) and insurance status(p<0.0001). Besides, a multivariate analysis showed that there was a significant negative association between managers' self-esteem and patients' satisfaction(P=0.04).
Conclusion: The findings revealed that female managers had less self-esteem than male managers. Moreover, it showed that managers' higher self-esteem would cause patients' lower satisfaction. Furthermore, patients with higher education and/or with health insurance were more satisfied than others.

Abstract
Background and Aim: Based on theories and researches, two variables effective in the continuous improvement of services are trust in and identification with hospital goals. The present study aims to explore the relationship between such variables among nursing staff members in private hospitals of Shiraz.
Materials and Methods: This research is a cross-sectional correlation study. Some 340 nurses(58 males and 282 females) were selected using the stratified sampling technique.
Three questionnaires for the assessment of identification with hospital goals, measurement of continuous improvement, and determination of trust in hospital were used. The reliability of data collection devices turned out to be 0.70, 0.70 and 0.79, respectively. Besides, the construct validity was confirmed through factor analysis. Data were analyzed inferentially using Pearson's correlation test and hierarchical regression analysis.
Results: The findings show that the relationship between identification with hospital goals and trust and nurses' effort to improve continuously is positive and significant. Moreover, the results of hierarchical regression analysis show that after controlling gender, age, experience and education, first identification with hospital goals and then trust in hospital play a significant role in the effort to improve continuously.
Conclusion: Nurses' effort to improve continuously is mostly influenced by identification with hospital goals rather than trust in hospital.

Abstract
Background and Aim: Focus on organizational health could ensure the future success of any organization. Considering the fast changing conditions of hospitals and the necessity of effectiveness, employees are required who can show voluntary behavior towards the organization and customers. Therefore, this study mainly aims to survey the relationship between organizational health and organizational citizenship behavior in selected hospitals of Tehran University of Medical Sciences(TUMS).
Materials and Methods: This cross-sectional, descriptive-analytic study was conducted in 2010. The population of the study comprised the entire staff of TUMS selected hospitals. In total, 312 employees were selected. The researchers gathered their data using questionnaires of organizational health and organizational citizenship behavior. For data analysis, both descriptive statistics(percentage, mean, standard deviation and variance) and inferential statistics(Pearson correlation and regression) were applied. The data were analyzed using SPSS18 software.
Results: The correlation between the organizational health and five dimensions of organizational citizenship behavior(Conscientiousness, Altruism, Helping Behavior, Civil Virtue, Sportsmanship, and Courtesy) was significant(p = 0/0001).
Conclusion: Given this significant correlation between organizational health and organizational citizenship behavior, organizations can make improvements in employees' behavior. This can be achieved by creating harmony among members and through continuous improvement and growth.

Abstract
Background and Aim: Clinical laboratories, due to their succeptibility to potential risks, are priorities in all activities concerning safety issues in hospitals.
Total Quality Management (TQM) is a model considered by WHO for clinical laboratories safety development. This study aimed to assess how safety standards were followed at the clinical laboratories of hospitals affiliated with Tehran University of Medical Sciences (TUMS) based on TQM safety regulations.
Materials and Methods: This descriptive study was conducted on a research population including TUMS hospitals clinical laboratories in 2012. For data collection, census was applied and sampling was not considered. The data gathering instrument was a checklist measuring seven safety components based on TQM. The validity of the instrument was determined through content validation, and Cronbach alpha was calculated to be 0.82. For the analysis of the results, descriptive statistics including frequency percentage of scores was employed.
Results: The safety of research population turned out to be suitable in terms of four factors: designing, existence of general safety items, conducting standard safety activities, and safe disposal of laboratory wastes. Besides, implementation of safety programs and increase in employees’ knowledge to cope with accidents were relatively acceptable. The emergency evacuation program, however, was estimated as poor.
Conclusion: The studied laboratories enjoy relatively safe conditions, but imperfect implementation of safety protocols can endanger safety conditions. Therefore, it is necessary to take the required correcting measures to ensure full safety in clinical labs.

Abstract
Background and Aim: Development of information and communication technology has led to enormous changes in different areas. Electronic medical records system is valuable to access patient data in hospitals. This study aimed to investigate and compare the educational hospitals of Uemia University of Medical Sciences in case of technical, organizational and legal to establish the system.
Materials and Methods: The study was a descriptive cross-sectional study. The study population consisted of 98 senior and central managers. In this study population census was used and the entire population were considered as the sample. A questionnaire was used for data collection, which included two sections in order to determine the level of research community awareness and to analyze the standards related requirements for the implementation of the system. Validity and reliability were assessed and the data was analyzed by SPSS.
Results: Sample awareness in 5 hospitals of this study was moderate. In terms of requirements, there was a significant difference between the means of Electronic Medical Records in terms of three variables between hospitals "D" and the rest of the hospitals. And no significant difference was seen among other hospitals.
Conclusion : Three hospitals, "a", "c" and "b", among five studied hospitals are in preparation for the deployment of electronic medical records. Other two hospitals were not prepared. However, the implementation of electronic medical records, increases health care quality, patient safety and patient care and also decreases health costs. So it is suggested that hospitals do necessary efforts to establish EMR.

Abstract
Background and Aim: Tele pathology is one of the medical subdivisions that has opened a new approach in the telepathology, e specially to organize consultations. In this research, feasibility of Telepathology implementation in teaching hospitals of Tehran University of Medical Science was studied.
Materials and Methods: This study was a cross-sectional and descriptive study. The study population was included 8 hospitals directors and administrator, 20 pathologists, and 8 informatics staffs, in four teaching hospitals of Tehran University of Medical Sciences. A researcher constructed questionnaire was used for data collection . The validity of the questionnaire was confirmed by expert panel and using by Test – retest method confirmed its reliability. The data was collected and analyzed by SPSS software to prepare descriptive findings.
Results: The R esults showed that 65.6% of hospitals had hardware facilities . Procedures based on legal issues related to information security and privacy was 95.71%, while t here was no guideline for telemedicine and telepathology.
Conclusion: I t could be concluded that in line with considrating the importance and benefits of telepathology, it is necessary to provide software requirements and hardware infrastructure. It should be noted that available properties also must be improved in terms of implementation of telepathology. Also, rules to support patients’ and staff’s rights should be developed for better implementation of such new technologies

Abstract
Background and Aim: Based on theories and researches, three variables effective in the organizational citizenship behavior are e thical values, job satisfaction and psychological empowerment. The present study aims to explore the relationship between these variables and the organizational citizenship behavior (OCB) among the staff members of private hospitals in Shiraz.
Materials and Methods: This is a cross-sectional correlational study. Some 330 staff members (147 males and 183 females) were selected using stratified sampling technique. The data collection devices consisted of five standardized questionnaires, the reliability of which turned out to be 0.85, 0.93, 0.70, 0.89, 0.90 and 0.70 Besides, the construct validity was confirmed through factor analysis. The data were analyzed inferentially using Pearson’s correlation test, stepwise regression analysis and SEM.
Results: The findings revealed that there was a significant positive correlation between ethical principles, psychological empowerment and job satisfaction with organizational citizenship behavior (p<0/001). Moreover, the findings of stepwise regression analysis showed that ethical climate played a significant role in predicting OCB.
Conclusion: The staff members’ effort to enhance OCB is mostly influenced by psychological empowerment in private hospital.

Abstract
Background and Aim: The function of any technology in a society depends on the culture of that society. Organizational culture leads to a common understanding and agreement on institutional practices. Meanwhile, it is a powerful barrier to change, especially technological change. This study aims to determine the effect of indicators of organizational culture on the unified theory of acceptance and use of technology in hospitals of Rasht, Gilan Province.
Materials and Methods: This is a cross-sectional, descriptive-analytic study . From April to July 2013, 235 questionnaires were completed by health, administrative, financial and educational personnel of Razi, Poursina, and Heshmat Hospitals. For data analysis, such tests as compound reliability test, root mean square of variance, bootstrap test, t-test, and hierarchical regression were applied using Lisrel, PLS and SPSS softwares.
Results: Among the components of organizational culture (performance expectancy, effort expectancy, subjective norm, and facilitating conditions), there was a significant relationship only between "performance expectancy and facilitating conditions" and "behavioral inclination and using computers" (P<0.0001). Gender played no role in the relationship between cultural components and the use of computers (P>0.05).
Conclusion: The results of the study indicate that there is a relationship between organizational culture and application of information technology, and that a unified organizational culture can facilitate and speed up the acceptance and application of information technology.

Abstract
Background and Aim: Employee involvement and organizational adaptability are two key factors towards success in almost all organizations. Hospital managers must pay special attention to these two variables and always try to improve them. The present research, therefore, studies the relationship between employee involvement (empowerment, team orientation, and capability development) and organizational adaptability (creating change, customer focus, and organizational learning) in the educational hospitals of Kerman.
Materials and Methods: The population of this quantitative study includes all staff members of Kerman’s educational hospitals, estimated about 1370 individuals. A sample of 302 subjects was systematically selected based on Krejcie and Morgan’s table of sample size. The data collection tool was a questionnaire and the data were analyzed by Pearson correlation coefficient, ANOVA, and Schefe post hoc test.
Results: According to the findings, employee involvement and organizational adaptability -- and their related indices -- were at an acceptable level and were almost equal to those of international organizations. Moreover, there was a positive and significant relationship between these two main variables and their related indices.
Conclusion: The results of this study can help hospital managers to verify the present situation of organizational adaptability and organizational involvement and their relationship. These findings could help hospitals to predict change priorities and select better strategies.

Abstract
Background and Aim: The study of human resources flexibility impact on hospitals performance indicators can be effective on hospitals performance improvements. The main purpose of this research is to investigate the relationship between human resources flexibility and hospitals’ performance indicators of Islamic Azad University.
Materials and Methods: The present research was a descriptive-analytical research that has been induced cross-sectionally in 2013. The statistical sampling has been selected randomly, and class assignment among 108 nurses, and supportive employees in five hospitals of Islamic Azad University. Data collection tools was hospital’s performance indicator forms, and flexibility questionnaire based on Right and Esnel. The process of data analysis has been done using SPSS software and by descriptive statistical indicators and correlation tests.
Results: The studied employees had low flexibility. The total studied hospitals were in undesirable situation from the perspective view of bed occupying and bed rotation ratio. Finally, there were not any meaningful relationship between human resources flexibility and hospitals performance indicators.
Conclusion: According to low flexibility of human resources of Islamic Azad University hospitals, the policies should be made in order to coordinate human resources process with human resources flexibility strategies. Furthermore, human resource for the purpose of creating flexibility and hospital subsidiaries of Islamic Azad University performance improvements could be clearly effective in such hospitals situation promotion.

Abstract
Background and Aim: Nurses’ main tasks include the provision of healthcare services at the highest levels of quality and quantity standards, and based on the findings of scientific research. The aim of the present study is to identify and determine the barriers of evidence-based practice among nurses of Qazvin hospitals, in Iran, at 2013.
Materials and Methods: The present cross-sectional research was a descriptive- analytic study conducted among 260 nurses practicing in six hospitals of Qazvin in 2013. The barriers scale questionnaire developed by Funk, et al. was used to examine the four dimensions of evidence-based practice (EBP) barriers including adopter, organization, innovation and communication characteristics. To analyze the data, descriptive and analytic statistics were used.
Results: The mean score of EBP barriers among subjects was 3.07 out of 4. Among the barriers, "organization" and "adopter" dimensions had the highest and lowest mean scores, respectively. There was no significant statistical relationship between gender and EBP performance, But a significant relationship was observed between organizational dimension with age, employment type and work experience.
Conclusion: Since “organization” was found to be the main barrier to the use of research in nursing performance, policy-makers and administrators should pay attention to the use of research in performance, the establishment of a research culture in organization, the creation of appropriate infrastructures, and the allocation of sufficient time to the nursing personnel to have access to evidences.

Abstract
Background and Aim: Diagnosis of preeclampsia has an essential role in applying appropriate treatment plan for the patients. The aim of this study was to design an expert system in order to diagnos preeclampsia in order to assist the clinicians.
Materials and Methods: This was a cross-sectional study which resulted in developing a new system. The study population consisted of all patients admitted to three Maternity hospitals affliated to Tehran University of Medical Sciences (TUMS). Sample size included 215 medical records which were randomly selected. The results obtained were compared with the diagnosis from experts by kappa test using SPSS software.
Results: First of all, input parameters fuzzificated and entered into inference engine. Outputs were categorized in two groups as patients and healthy, with the final diagnosis and clinical explanation. The results obtained from system evaluation showed that accuracy, specificity and sensitivity of the system were 98.2 percent, 100 percent and 96.4 percent respectively.
Conclusion: Based on evaluation results, it could be concluded that fuzzy logic is an efficient method for designing of expert systems in the field of obstetrics and gynecology. Also, due to the similarity of the logic used in the proposed system with workflow and medical decision making, it will be accepted by the physicians.

Abstract
Background and Aim: Hospital complaining is an important dimension of clinical governance. This study aimed to determine the impact of clinical governance program on patients’ complaint in Shiraz public hospitals.
Materials and Methods: This applied study was conducted as descriptive-analytical in 2012 in Shiraz. The population studied was 10 public hospitals in Shiraz and the patients complaints were studied during 5 years. Data collected through Iranian Ministry of Health standard checklist of clinical governance. The data entered to SPSS version 21, and analyzed using descriptive statistics and analytical tests such as Chi-square.
Results: Registered complaints had increasing trend during the execution of the program and were 1905. Moreover, 60.5% of the complaints have been made by family members. There was significant relationship between complaints registered with the hospital wards (P=0.000) and gender (P=0.000). Therefore, both genders and in all departments of the hospital complaints of poor quality and inappropriate behavior of the staffs was greater than other complaint.
Conclusion: The number of complaints has increased in recent years. It is proposed to reduce the number of complaints attention to improve communication between physicians and patients, medical ethics, cut non-conventional financial relationaship between physicians and patients, and increased scientific and technical knowledge and skills.

Abstract
Background and Aim: Due to social development, population growth, industrialization of countries and require some work, most of the employees are working with program shift. The purpose of this study was determination of relationship between nurses shift rotation and personality characteristics and coping strategies in selected private hospitals in Tehran, Iran.
Materials and Methods: In this descriptive and cross-sectional study standard shift work questionnaire and Eysenck personality questionnaire was used. After confirming the validity and reliability (Cronbach's alpha 0.73), the questionnaire was distributed among 305 nurses and 6 non-governmental hospitals in Tehran on the cluster random sampling method. Descriptive and analytical methods were used for data analysis using SPSSv.22 software.
Results: 43.6 percent of nurses participating in the study were introverted and 56.4 percent were extroverted. The results showed that there was a meaningful statistical relationship between age and health (p=0.008), gender and physical health (p=0.015), education level and physical health (p=0.014), gender and physical-cognitive anxiety (p=0.006), age and family- socioeconomic status (p=0.001), marital status and family-socioeconomic status (p=0.001), second occupation and family-socioeconomic status (p=0.001).
Conclusion: Planning according to nurses working hours standard and to avoid overtime, especially nurses who have more experience can prevent severe complications of shift work and improve health and the quality of care.

Abstract
Background and Aim: In today's complex work environment, organizations need a strong link between strategy and effective strategy execution. The purpose of this research is to integrate Balanced Scorecard model with Kaizen management to meet this need and increase innovation efficiency in organization and create sustainable competitive advantage.
Materials and Methods: This study was a descriptive study in terms of purpose and in terms of data collection, in three stages. Two researcher-made questionnaires(validity and reliability of 0.852)were used perform this applied research in Shahroud hospitals. The first is identify factors using the Delphi method by 16 experts and The second was to confirm the indices, which were completed randomly by 829 hospital beneficiaries and analyzed by confirmatory factor analysis. Structural-interpretive model was used to determine the effect of indicators on each other. In order to determine the cause and effect relationships between the goals and the two models Quality Function Deployment matrix was used by expert opinion.
Results: This study showed that in order to achieve the goals of learning development perspective in BSC model, one needs to use "mura" in Kaizen model. Also, to achieve internal process goals, the muri dimension must be used, to achieve the financial perspective objectives of the "deletion"(muda), and for the customer perspective goals, the "mura" dimension must be applied.
Conclusion: Integration of BSC and Kaizen models can provide a suitable framework for continuous improvement of performance, because it has been successfully Performance in Shahroud hospitals, this research has shown that it seeks continuous improvement and increased productivity in the hospital.
