Abstract

Backgrounds and Aim: Self-care behaviors are an important aspect of Heart Failure (HF), management, Educating self-care behaviors to HF patients have to be a part of routine management of HF in hospitals and health care Centers. The aim of this study was to determine the impact of an educational intervention on self-care behaviors and its perceived benefits and barriers in patients with HF in Tehran.

Materials and Methods: This is was an interventional study. A Simple sampling was conducted on 180 outpatients (90 intervention group and 90 control group) with HF referring to Shaheed Rajaee Heart Hospital in Tehran to participate in this study. Before and 2 months after implementing the educational program through group discussion, face to face interview, educational movies and pamphlets, a self-prepared questionnaire was completed by the participants. Validity and reliability of the scales were approved. SPSS software was used for the purpose of data entry, manipulation, and analysis.

Results: After implementing the educational program, patients' self-care (74.5%) and perceived benefits (19.35%) were significantly increased (P<0.0001) and their perceived barriers (27.76%) were significantly decreased (P<0.0001). After intervention there was no significant difference in these variables in control group.

Discussion and Conclusion: In order to design effective educational programs, healthcare professionals and nurses should better understand self-care behaviors in HF patients and their determinants and develop educational interventions. Promoting perceived benefits and barriers of these behaviors are priorities of the program. Healthcare professionals and nurses applying such educational programs can have a more effective role in promoting self-care behaviors and consequently quality of life of HF patients.

Abstract

Background and Aim: Teeth-mouth diseases include a cost between 10-5% of the total of cost of health care and have the damaging effects on childhood and in adult. Transtheoretical Model shows change of behavior dynamically. The purpose of this study was to determine Stages of change of inter-dental cleaning behavior based on Transtheoretical Model in male and female students in Yazd, Iran.

Results: In this study 54.8% female and 45.2% males participated with mean age of 17.35 ±.55 years. %49.6(59student) of students were in pre contemplation stage. Other participants were 5.3 % (19) in contemplation, 24.1 % (87) in preparation, 6.8% (31) in action and 12.5% (5) in maintenance stage. There was a positive significant relationship among stages of change with self-efficacy (r=0.33) and perceived benefits (r=0.19). Also a negative significant relation between stages of change and perceived barriers (r= -0.19) was oberved. Constructs of the model were statically significant predictors of stages of change and accounted for 0.15% of the variation that self efficacy was most important predictors (β=0.212).

Materials and Methods: This cross-sectional study was performed on pre-university 361 students in 8 schools cluster sampling. The instrument for collecting the information were questionnaires consisting of demographic characteristics, self-efficacy questions, decisional balance (perceived advantages and perceived obstacles) and stages of change of interdental cheaning behavior. Reliability and validity of questionnaires was confirmed. The data collected by interview and analyzed by SPSS.

Discussion and Conclusion : In this study, most of students were in pre contemplation stage. They will be probably exposed to dental caries. Not only Trans-theoretical Model can be a model to determine strategies to each stage of change of teeth-mouth self - care Behavior but can also be used in educational interventions.

Abstract

  Background and Aim: Gender in performing self-care behaviors is a non- modifiable factor. Despite the belief that women are better than men in self-care, there is little evidence to challenge this view. The purpose of this study was to determine the status of self-care behaviors in men and women with heart failure.

  Materials and Methods: In this analytical cross-sectional study, 239 patients with heart failure were assessed. Data were collected by Self-Care Heart Failure Index, Cardiac Depression scale, and Mini Mental status Examination questionnaires . Data were analyzed using SPSS version 19 , descriptive and inferential statistics (Chi-Square, Mann-Whitney, Kruskal-Wallis , t-test and Logistic regression ) .

  Results: Mean score of self-care behaviors in men and women was 44/60±12/05 and 39/71 ± 9/88 from 100 total score, respectively. In the regression model, gender was not a significant predictor of self-care in any dimensions but, there was a significant relationship between sex and self-care confidence (OR=0/52, CI=0/27-1/03), which indicated that women as compared to men have less chance of self-care behaviors due to their status of self-confidence.

  Conclusion : In attention to the point that women had worse self-care behaviors in all dimensions, it is suggested more attention be paid in identifying patients at risk of poor self-care behaviors while planning care and treatment for them, since they are more vulnerable and have more emotional effects of disease and gender should be regarded as a predictive factor.

Abstract

Background and Aim: Preeclampsia is one of the most prominent cases of pregnancy related diseases that threatens health at global level, especially in developing countries. In Iran, with 14% of outbreak, it is the second most common cause of maternal mortality. The main goal of this study was to identify the information requirements of the Android-based preeclampsia self-Management application.
Materials & Methods: This was a descriptive study that was done in 2018 in Amir_Almomenin Hospital affiliated to Semnan University of Medical Sciences in two stages of reviewing the sources and the need for data elements. In the review phase, after studying the articles and study, the data requirements and factors which affecting the prevalence of preeclampsia were identified and a survey of qualified physicians was done by designing a researcher-made questionnaire.
Results: This research results indicate that 63.9% of the respondents assigned to the elements mentioned in the demographic findings. 75.9% of them identified health information elements as very important. Also, 77.85% of the research community considered the elements in the lifestyle sector to be of the highest importance. All participants recognized that reminder in the program was necessary. Approximately 33.33% of them reconsidered sport education to be at the lowest level, while 45.24% rated it as being of the highest importance.
Conclusion: The information requirements of this program were determined in 6 groups of health history, educational tips, lifestyle, alarms, referral, and reporting. These programs can help pregnant mothers with preeclampsia to control their disease to minimize complications by observing proper nutrition and principles of treatment.

Abstract

Background and Aim: The online social networks as new and widespread sources of information have been able to facilitate the accessibility of people to health information. The aim of this study was to determine the use of online social networks and their role in sharing health information among pregnant women.
Materials and Methods: This descriptive-analytical study was conducted in Kerman in the winter of 2017. The study participants were the pregnant women referring to 21 gynecologists’ private offices in Kerman city. Data were collected by a researcher-made questionnaire. The validity of this questionnaire was confirmed by 4 medical informatics and health information management specialists, and its reliability (71%) was estimated with Cronbach's alpha. Data were analyzed by SPSS using descriptive statistics and analytical tests.
Results: In general, 89% of pregnant women used online social networks and more than 80% of them used it daily. Moreover, more than 57% used these networks for the related information about health and pregnancy period care, and almost 68% of pregnant women considered these networks useful.
Conclusion: This study showed the widespread use of online social networks among pregnant women in Kerman city. If accurate and trustworthy information bits were shared in such networks, they could provide an appropriate platform to improve the awareness, knowledge and health literacy of pregnant woman, and facilitate the exchange of information and experience among them.

Abstract

Background and Aim: Pelvic floor disorders are a common disease which, with varying degrees of severity, disrupts daily life and also decreases the quality of life of the individual. This disease can be cured by lifestyle modifications and self-care education; on the other hand, smartphones have become a good platform for health services and therapeutic interventions. The aim of this study was to identify information and educational components for the self-care application of women with pelvic floor disorders.
Materials and Methods: This descriptive study was performed in Yas Hospital affiliated to Tehran University of Medical Sciences. According to Morgan table, 30 gynecologists who were randomly selected were included in the study. The data gathering tool was a researcher-made questionnaire which its validity and reliability were confirmed (α=0.85) and its content was provided by reviewing the scientific sources.
Results: The results of the analyzes showed that from the experts’ point of view the interventional section including reminders, daily activities diaries and targeting with an average score of 4.14 has been of great importance. Since then, the applied and self-care section with an average score of 4.364is ranked second in priority.
Conclusion: According to a survey by experts who play a major role in the management of the disease. The essential components of this application were identified in three main sections consist of: electronic records, the interventional section and the practical and self-care and their subsections.

Abstract

Background and Aim: Given the increasing research, the purpose of this study was to explain the effectiveness of this training and its effective factors.
Materials and Methods: The present study is a mixed and explanatory project. In the first step to obtain the effectiveness of self-care education through quantitative meta-analysis and secondly to examine its effective factors the qualitative method of the case study was used. Statistical population of the first stage includes all relevant internal research and secondly, it included all cardiologists in Isfahan province. The data gathering tool is firstly a researcher-made checklist and for the second stage, the semi-structured interview method was used. To analyze the first stage data Comprehensive statistical meta-analysis software CMA Version II and for the second step, coding methods were used.
Results: The results showed that self-care education interventions were highly effective in cardiac patients(ES=1.616, P<0.05) In other words, the average effectiveness of self-care education in (experimental groups) 94% were more effective than control groups. On the other hand, the results of the second stage showed Factors affecting effectiveness include seven factors: education, personal control, physical activity, nutrition, emotion control, optimism, and continuous follow-up.
Conclusion: Heart disease self-care based on the above mentioned factors, as the most effective factor in controlling and improving heart disease this will lead to a longer life expectancy and a better quality of life for patients with heart disease.

Abstract

Background and Aim: Asthma is a common and chronic disease of respiratory tracts. The best way to treat Asthma is to control it. Experts of this field suggest the continues monitoring on Asthma symptoms and adjustment of self-care plan with offering the preventive treatment program to have desired control over Asthma. Presenting these plans by the physician is set based on the control level in which the patient is. Therefore, successful recognition and classification of the disease control level can play an important role in presenting the treatment program to the patient and improves the self-care and strengthens the early interventions to alleviate the Asthma symptoms.  
Materials and Methods: Based on this objective, we collected the data of 96 Asthma patients within a 9-month period from a specialized hospital for pulmonary diseases in Tehran. Then we classified the Asthma control level by fuzzy clustering and different types of data mining method within a multivariate dataset with the multi-class response variable.
Results: Our best model resulting from the balancing operations and feature selection on data have yielded the accuracy of 88%.
Conclusion: Our proposed model can be applied in electronic Asthma self-care systems to support the decision in real time and personalized warnings on the possible deterioration of Asthma control. Such tools can centralize the Asthma treatment from the current reactive care models into a preventive approach in which the physician’s decisions and therapeutic actions are resulting from the personal patterns of chronic Asthma control and prevention of acute Asthma.

Abstract

Background and Aim: Many risk factors can cause biliary system diseases. Hence, this category of diseases is amongst the most common ones. Active patient cooperation is very important in disease management, self-care, and clinical outcomes improvement. A mobile phone application has a high potential in supporting the patients’ self-management. Therefore, this study was conducted to recognize and define data elements to develop a self-care application for biliary patients.
Materials and Methods: The current descriptive study was conducted in 2 stages, resource investigation, and data elements’ need assessment. In the first stage, scientific articles available in databases were used for defining required data elements to develop the application for biliary patients, and a checklist of data elements was prepared. In the second stage, a questionnaire was made based on the checklist. Content and face validity were accepted by the research team and the reliability was calculated 87.2%, using the Cronbach’s alpha test. The mentioned questionnaire was given to Gastroenterologists at Imam Khomeini Hospital complex, and the elected data elements were recognized.
Results: In this application, data elements were categorized into seven sections, including demographic and clinical information, data related to the biliary system diseases, postoperative lifestyle information of the biliary patients, reminders, disease management, and informing. Sixty point five percent of the responders gave the highest importance to data elements in the demographic and clinical data section. Data elements related to patients’ education were considered highly important by 54.2% of the responders. Forty three point eight percent gave the highest importance to data elements in interventional applications’ sections, and only 4.2% gave the least importance to this section.
Conclusion: Based on the identified data elements, a self-care application was designed and developed and can be used as a supplement to specialized care for biliary patients.

Abstract

Background and Aim: Despite the continuous progress in medicine, COPD (Chronic Obstructive Pulmonary Disease) is still a progressive, incurable and chronic respiratory condition that limits the patients’ functions in various dimensions, and significantly reduce their quality of life. In this way, self-care of patients and the use of related tools have a significant effect in disease control and treatment. The purpose of this research was design and development of an android-based application for COPD.
Materials and Methods: This research was a descriptive developmental type with a practical approach. Initially, based on the study of library resources, guidelines, and the examination of the medical records of COPD inpatients in Firouzabadi Hospital, a questionnaire was designed to identify the information requirements, data items and features of the application. Then it was reviewed and finalized by a sample of 10 (randomized and convenience sampling) internal and lung specialist doctors in Firouzabadi and Hazrat Rasool hospitals affiliated to Iran University of Medical Sciences. The data was analyzed using descriptive statistics, and then scenario tables and UML diagrams were illustrated to show the overall flow of the application. The application was designed and developed using the Java programming language in the Android Studio 2021 platform. After installing the application on the mobile phones of ten COPD patients of the internal and pulmonary clinic of Firouzabadi Shahre Rey Hospital, and using it for a week, the opinions of the patients about the usability of the application were collected through the QUIS questionnaire, and analyzed. 
Results: Application sections were determined following experts’ survey, personal information items, clinical information, disease management, reporting, and training points, which were provided to patients after design for use and evaluation. At the end of the research, the results of the evaluation of the usability and satisfaction level of the application showed that from the patients’ point of view, the application is at a good level with an average score of 1.8 (out of 10 points).
Conclusion: The developed self-care application can be used to increase awareness, help to manage the disease, increase the level of quality of life, and reduce the complications and disease burden for patients with COPD.
