Abstract

Background and Aim: Systematized Nomenclature of Medicine systems are the important supportive for electronic health record in registration and retrieval of data. Systematized Nomenclature of Medicine - Clinical Terms (SNOMED CT) is the most comprehensive language and then the consistency of exchanged data across health care providers and finally the high effectiveness of health care.

Materials and Methods : This research is a descriptive - comparative study and has been done in certain time section. Ideally , it have been used the descriptive - comparative method for studying of selective countries.

Results : Findings of this study show that American and England countries have accepted the SNOMED CT with application in all clinical activities and Australia country is evaluating this system and is using older versions of SNOMED only in pathologic applications now. Study shows that SNOMED CT structure is complex and based of concept (clinical concept) and contains two fundamental parts : 1) content of SNOMED CT core (concepts , descriptions and relations) that are always constant 2) content of out of SNOMED CT core (mappings or relation with systems and local extensions) that according to health and management needs and native terms of country are different. Such a structure executes in different software applications.

Conclusion : SNOMED CT structure in America and England countries has been compared and with attention to similarities (content of SNOMED CT core) and differences (content of out of SNOMED CT core : cross mappings and local extensions) , a basic SNOMED CT structure has been provided for Iran. Because of SNOMED CT in Australia is under development, researcher has studied its evolution and execution process in this country and has provided an appropriate structural pattern for Iran.

Abstract

Background and Aim: General Store department is responsible for on time preparing commodities for hospitals departments. It's a place for saving materials and resources. Safety in store department and its materials has importance due to preserving high quality and continuous services.
General Store department safety should be audited by assessment System to reduce waste materials and human resources such as building, commodities and patient and personnelُ s lives and in case of facing with a problem, try to solve it. The aim of this study was determination of safety standards in hospital general stores in TUMS.

Materials and Methods: This research was conducted as a cross-sectional study safety and usage of management principles was advised and measured in hospitals of TUMS sampling was nُ t performed.

Results: Safety rate in research population was more than 85/42 percent. Score in safety aspects like as physical spaces, preventing robbery, blazing materials appropriate storing, commodities arrangement, utilizing suitable tools was more than 80%. Protection against fire and storing rote able materials estimated 79% and 77%.
Safety rate in several aspects was assessed in the study hospitals.

Discussion and Conclusion: Although, hospital store rooms assessed as safe but they should try to develop fire protection systems and store rote able materials preservation appropriately.

Abstract

Background and Aim: Occupational diseases are certain types of diseases that have been caused by special jobs and subject to special circumstances. There are several factors that can intensify these diseases. Various countries have indicated that they have played an effective role in decreasing exposures and its economic costs and attracting optimal resources through establishment of a comprehensive information system for occupational diseases with a appropriate structure. Health improvement is considered as the first aim of the health system which requires a comprehensive, complete and integrated information system on occupational disease.

Materials and Methods: This is a descriptive Comparative study which was conducted in 2008 during which information systems in countries such as US, Finland, France and Iran have been studied based on their objectives and structures. Main instrument of this research was articles, books, magazines, valid internet sites in English language. Then, the gathered data were classified and analyzed.

Results: All the three selected countries had an electronic information system and database in the field of occupational disease. In Finland, data of occupational diseases as well as their damages are kept in separate databases while US lacks a comprehensive system. There are countless systems in these countries in this field. In France, supervision on laboratories is a priority. In Iran, there is no certain comprehensive system for registering diseases.

Discussion and Conclusion: All of these countries have the same objectives. Structure of occupational diseases information system, in Finland, is more comprehensive. Iran lacks such a system, so it is recommended to make use of the experiences of other countries and establish electronic comprehensive disease system in this country.

Abstract

Background and Aim: Electronic learning system is a new educational process which designed upon basis of computerized technology, multimedia and processors. It has several potentials and contexts.
To do comparative study and utilizing of successful electronic learning in developed countries could help us to develop this new process in Iran's Universities of  Medical Sciences.

Materials and Methods: This research is developed descriptive comparative study in 2007 - 2008 by using of developed countries universities websites information and 62 references and keywords such as Electronic Educations,Medical Sciences Courses,Commutation Technology, Comparative Study and University in the field of electronic medical sciences. Then, the findings will compare with each developed countries universities and Iran's Universities of Medical Sciences.

Results: The results showed that Manchester and Dekin University have much experience in developing of electronic learning. The Students should be assessed by attendance, online and blended in developed universities.
Degree based courses are only present in South - Africa University, meanwhile single courses and training are present in other universities. Electronic health sciences courses and training are developed much more in compressive with others.

Discussion and Conclusion: The studied universities are different in history of construction, student assessment type, homework presence type, registration procedures, electronic medical education type with each other.
Iran's Universities of Medical Sciences, electronic education have several deficiencies in comparison with developed countries universities. We suggest appropriate technological and cultural infrastructure and use's skill improvement to develop this process in our country's universities of Medical sciences.

Abstract

Background and Aim: Accreditation is one of the evaluating health care organization tools especially in hospitals and is also one of the priorities of Ministry of Health in Iran. The aim of this study was to examine the impact of accreditation on health service delivery in hospitals from perspective views of experts.

Materials and Methods: This qualitative study was conducted by semi-structured interviews and opinions of 12 experts at Isfahan University of Medical Science in the field of hospital accreditation in 2012. After transcription of each interview, content analyses was used to minimize and structuring qualitative data.

Results: According to this study, 10 main theme and 72 sub- themes were identified. Main themes included: Necessity, implementations priorities and mechanisms of Accreditation effect, accreditation impact on service quality and organizational performance, patient satisfaction, commitment and job satisfaction of staff, reducing factors the impact of accreditation, confounder factors the effect of accreditation and the executive proposals in order to implement accreditation program in Iran.

Conclusion : Accreditation could be properly implemented through setting Conditions, selecting the appropriate accreditation model, justify stakeholders about the necessity of accreditation, monitoring, establishment of appropriate information systems, information transparency and changing the general attitude of the organization. It would take positive effects to achieve hospital goals and improve the quality of services.

Abstract

Background and Aim: Medical Laboratory Sciences is one of the important fields in Medicine that is closely associated with other disciplines in this group. This study aims to evaluate laboratory sciences curriculum in Iran and several countries worldwide.

Materials and Methods: In this research, data collection is based on documents and curriculum goals of countries. The data collected was analyzed using qualitative methods.

Results: Comparative study of curriculum courses of bachelor of Laboratory Medical sciences was done and compared with the number of basic and specific units in Iran and other countries.

The curriculum in Iran with Turkey and Australia is remarkably consistent. Internship units in hospitals have lower returns in compared to other universities in the world.

Conclusion: Laboratory Science curriculum in Iran is matched with developing countries. However, more attention to training units and projects to improve the quality of teaching laboratory sciences could be more effective.

Abstract

Background and Aim: Health communicators with volunteer activities meet many people’s educational and informational needs in different aspects of health without additional costs. Despite the importance of communicators in health programs, the level of their cooperation or connection with our health centers has been reduced in recent years. So, this study was performed with the aim of affective factors on recruitment and retention of health communicators.
Materials and Methods: This study was an applicable study with the use of phenomenological qualitative methods. Data gathering tool was focus group discussions that were done in year 2014. A total of 39 health communicators, coaches, and experts of health communicators plan were selected using purposive sampling in Kerman city health center. Data achieved by focus group interviews were analyzed using framework analysis.
Results: Three main concepts were extracted as the results of the analysis of the qualitative data. Factors related to the health care system weaknesses of the communicator's plan, weakness of human factors (coaches and communicators), and factors related to society were the three main concepts of recruiting and retaining the health communicators.
Conclusion: Setting up the supportive and supervisory regulation by the Ministry of Health and Medical Education in conjunction with this program, allocating the appropriate budget to this program, documenting achievements and appreciating the better volunteers and their coaches in the country, and planning for education of volunteers according to the health needs of the society are the solutions proposed in this study.

Abstract

Background and Aim: Strategic planning has an important role in achieving competitive advantage for nonprofit organizations. However, some strategic plans fail. The present study aimed to investigate challenges of strategic planning in Welfare Organization.
Materials and Methods: A qualitative approach was used for this case study. In-depth semi-structured interviews were conducted with 12 senior and middle managers who were members of the strategic planning committee. Content analysis method was used for data analysis, and 7 themes and 16 sub-themes were identified from the data.
Results: Lack of justification of the strategic planning, the strategic planning model, inappropriate composition of the strategic planning team, the structural and managerial changes of the Welfare Organization, time limitation, insufficient training, and budget constraints were the main challenges of strategic planning in the Welfare Organization. 
Conclusion: If the strategic planning challenges are not managed well, more problems are expected to occure during the implementation of the strategic plan. Senior managers’ commitment, strong management and leadership, participative corporate culture, education and training, staff empowerment, and provision of required resources will facilitate strategic planning implementation in organizations.

Abstract

Background and Aim: The efficiency and effectiveness of any organization depend greatly on proper and effective management and use of human resources. Due to the importance of activities and the motivation of personnel fighting against diseases, as well as the high share of personnel costs in total health expenditures, the use of appropriate methods to determine their workload is important.
Materials and Methods: The present research is a descriptive and applied study, conducted in two quantitative and qualitative dimensions. The research environment was the urban-rural health centers of Savadkuh health network. During the process of calculating the workload, the steps were: identifying activities, correcting and approving activities, determining the percentage of acceptable unemployment, calculating standard time, determining the number of repetitions, calculating work units, determining the workload, and determining the required manpower.
Results: Activities in the field of fighting against diseases were identified and organized in the form of 77 activities. By taking the frequency of each activity into account, the total time of these activities in one month was equal to 10273 work units. The main activities related to vaccine injection and registration of vaccinated specifications in relevant offices and forms included 137 and 115 work units, respectively.
Conclusion: Determining the workload of different health care fields by using this study method creates the potential to estimate the number of manpower required to give all predicted services, and has the potential to reduce the demand for surplus labor and increase employees’ tendency to give more services, if used in current structures.

Abstract

Background and Aim: The growing health care needs of students actuated world Health Organization to develop School-based health projects in the last decades. The most important plan that has received worldwide attention and in Iran for nearly one decade is Health Promoting School (HPS) program which being implemented with collaboration of Ministries of Health and Education. Present qualitative study was conducted to evaluate this program implementation in Ahvaz education organization (District 1).
Materials and Methods: In this qualitative study data was collected by semi-structured interview. The study population consisted of school health experts affiliated with Ahvaz West Health Center and health deputy, health experts affiliated with education organization (District 1 and school headmasters and expert staffs in HPS (22 persons). The sampling was purposeful and the interviews continued until saturation of data. Content analysis was used for analysis of the data.
Results: Data analysis showed that most components of the HPS program in schools belong to education organization were not implemented properly. The major barriers were categorized into 5 and 47 main and subsidiary themes respectively. Poor stewardship and authority, policymaking and legislation, weaknesses in motivation, attitude and awareness of managers and experts, inadequacy in resources and infrastructure, ineptitude in program implementation and finally inappropriate evaluation system was specified as the main themes. According to results, some of the most important reasons for failure in HPS program consisted of, lack of functions definiteness between health deputy in ministry of health and education organization, job dissatisfaction in school health experts, not being a priority behalf education organization, insufficient delegation of authorized budget, lack of interpectoral cooperation between trustee organizations and indifference about program evaluation.
Conclusion: The most important challenge in the HPS program is the implementation barriers that make the establishment and implementation of this program almost ineffective. The serious and committed interaction between the ministry of Health and Education and between their respective provincial and county districts can provide the foundation for the implementation of this valuable program. A review of the organizational structure and executive framework of the program is also necessary.

Abstract

Background and Aim: Effective monitoring and evaluation of human resources in countries require an agreed set of indicators and measurement tools. Due to the existence of different payment mechanisms, this study aimed to identify performance evaluation indicators in hospitals affiliated with the Tehran University of Medical Sciences.
Materials and Methods: A qualitative phenomenological study was conducted in 2022. Data were collected using semi-structured interviews, purposive sampling, and participation of 23 members of the Senior executives and financial team of hospitals affiliated to Tehran University of Medical Sciences and analyzed by content analysis method using MAXQDA-10 software.
Results: Twenty-three experts participated in this research, 78.2% were men and 21.7% were women, and a total of 526 final codes were identified and according to the balanced scorecard management tool in 4 main themes of finance, customers, internal processes, and growth and Development. Also 8 sub-themes of resource efficiency with 86%, medical tourism with 52%, professional ethics with 65%, satisfaction with 78%, accreditation with 48%, diagnosis and treatment with 78%, education with 56% and The research was categorized with 78%.
Conclusion: The provision of optimal medical services requires managers to be aware of the performance of subordinate personnel, which makes it possible to choose the appropriate criteria, develop performance evaluation systems and conduct correct evaluation, informing Doctors regarding the indicators considered for more accurate evaluation are among the suggestions that can be a solution to the existing problems. Another important point considering the limited available resources is the role of doctors in the field of health tourism to which attention will lead to an increase in financial resources.
