مرور سيستماتيک روشهای مختلف کوچکسازی بيمارستان و پيامدهای آن در جهان و ارايهی روش مناسب برای ايران |

زمينه و هدف: بيمارستانها مهمترين سازمان بهداشتی و درمانی و مصرف-کنندهی بخش اعظم بودجهی نظام سلامت هستند. راهکارهايی همچون کوچکسازی از جمله استراتژيهای بکار رفته در ارائهی خدمات مطلوب در بيمارستان به شمار میروند. پژوهش حاضر با هدف جستجوی سيستماتيک روشهای مختلف کوچکسازی بيمارستان و پيامدهای آن به منظور انتخاب روش مناسب برای کشور انجام شده است. مواد و روشها: در اين پژوهش، جستجوی سيستماتيک در database های Medlib , SID ,Pub Med, Science Direct و ابر جستجوگر Google Scholar با استفاده از کليدواژههای Downsizing, Hospital Downsizing, Hospital Rightsizing, Hospital Restructuring, Staff Downsizing, Hospital Merger, Hospital Reorganization و معادلهای فارسی صورت پذيرفت. 379 مقالهی بدست آمده بصورت مرحله به مرحله مطالعه و پالايش شده و در نهايت 25 مقاله مرتبط با اهداف مطالعه انتخاب و ارائه گرديد. نتايج: روشهای کوچکسازی شناسايی شده شامل کاهش تعداد کارکنان و کاهش تعداد تخت، برون سپاری ، ادغام واحدهای بيمارستانی و روش ترکيبی (ترکيبی از روشهای پيشين) بود. مهمترين پيامدهای استفاده از اين روشها مربوط به هزينه، درآمد، ويزيتها و جراحیهای سرپايی، مدت اقامت و ميزان پذيرش در بيمارستانها، نرخ مرگ و مير، رضايت ذی نفعان، بارکاری ، سلامت روانی و ميزان غيبت کارکنان بود. نتيجهگيری: از نتايج مطالعات چنين بر میآيد که روشهای مختلف کوچک-سازی در راستای ارائهی خدمت مطلوب و کاهش هزينهها در بيمارستان مورد استفاده قرار میگيرد. از ميان روشهای مختلف، ادغام و برونسپاری با توجه به شرايط فعلی کشور ما برای استفاده در بخش بيمارستانی پيشنهاد میشود. |

Conducting A Systematic Review on Hospital Downsizing Methods and its Global Consequences and Choosing Suitable Method for Iran |

Abstract Background: Hospitals are the most important health organization and consume major part of health system`s budgets. Downsizing is one of the strategies used for providing suitable services in hospitals. This study was conducted to do a systematic search for hospital downsizing methods and theirs consequences in order to choose suitable method for Iran. Materials & Methods: Related Articles were identified by systematic search on databases such as Medlib, SID, Pub Med, Science Direct and Google Scholar meta search engine using keywords like Downsizing, Hospital Downsizing, Hospital Rightsizing, Hospital Restructuring, Staff Downsizing, Hospital Merger, Hospital Reorganization and Persian equivalents. 379 articles were extracted and refined step by step. Finally, 25 articles were selected according to study`s goals. Results: The identified downsizing methods included reducing the number of employees and hospital beds, outsourcing, merging hospital wards and combination of fore mentioned methods. The most important consequences of using these methods were: cost, revenue, outpatient visits and surgery, length of stay and hospital admission rates, mortality rate, stakeholder satisfaction, workload of employees mental health and absenteeism . Conclusion: study results indicated that different methods of downsizing are used for providing suitable services and decreasing costs in hospitals. Using Merger and outsourcing are suggested among different methods in hospital wards according to current situation of our country. |
