بررسی ديدگاه پرستاران در مورد موانع مديريتی و اخلاقی گزارش خطاهای کاری پرستاری |

با وجود اين‌که خطا در عرصه‌ی ارائه‌ی خدمات سلامت، پديده‌ای آسيب‌رسان و در بعضی مواقع غير قابل جبران می‌باشد، هيچ‌يک از افراد ارائه‌کننده‌ی خدمات بهداشتی از ارتکاب خطا مبرا نيستند. از آنجايی که هدف غايی حرفه‌ی پرستاری تأمين بهبودی و سلامت بشريت است، ضرورت گزارش دادن خطاهای کاری جهت تحقق اهداف پرستاری مشخص می‌شود. اما موانع متعددی بر سر گزارش‌دهی خطاهای پرستاری وجود دارد که بايد به شناسايی و رفع آن‌ها پرداخت. لذا اين مطالعه با هدف بررسی موانع مديريتی و اخلاقی گزارش خطای کاری پرستاران، از ديدگاه آن‌ها انجام گرفت.در اين مطالعه‌ی توصيفی- تحليلی 201 پرستار شاغل در بيمارستان‌های آموزشی دانشگاه علوم پزشکی شهر سبزوار با روش نمونه‌گيری در دسترس شرکت داشتند. ابزار گردآوری اطلاعات، پرسشنامه‌ای دو قسمتی شامل مشخصات فردی و پرسشنامه‌ی پژوهشگر ساخته موانع مديريتی و اخلاقی به ترتيب با 13 و 27 سؤال بود. شاخص اعتبار محتوا و پايايی ابزار با روش همسانی درونی (80=α) و آزمون مجدد (84/0= r) اندازه‌گيری شد. تجزيه و تحليل داد‌ه‌ها با استفاده از SPSS نسخه‌ی 18 و با آزمون‌های آماری توصيفی (فراوانی) و استنباطی (اسپيرمن) انجام شد. نتايج نشان داد که مهم‌ترين مانع مديريتی گزارش خطای پرستاری از ديدگاه آنان، به خطر افتادن موقعيت شغلی با 3/46 درصد و مهم‌ترين مانع اخلاقی، عدم حمايت پزشک از پرستار مرتکب اشتباه با 2/55 درصد بود. احساس ناامنی در حيطه شغلی و ارتباطی به‌ويژه در ارتباط با پزشکان، می‌تواند تهديدی برای گزارش خطاها و آسيب‌پذيری بيش‌تر سيستم مراقبتی از بيمار شود. لذا بايد تدابير مديريتی و اخلاقی مناسبی در اين راستا اتخاذ شود. |

Although errors in health services are harmful and the consequences might be irreversible in some situations, none of the health care providers are free of errors. Since health provision is the main goal of the nursing care, it is important to report professional errors in order to reach this goal. However, there are many obstacles in reporting nursing errors that should be identified and solved. The present study tried to evaluate the nurses’ point of view about managerial and moral obstacles in expressing nursing errors.Two hundred and one nurses of the educational hospitals of Sabzevar University of Medical Sciences were enrolled in this descriptive analytic study. The students were selected by convenience sampling method and the study data was collected using a questionnaire that consisted of a demographic part and a researcher made part with 13 and 27 questions related to managerial and moral obstacles respectively. Content validity index and reliability were measured by internal consistency (α = 80) and test-retest (r = 0.84). Data analysis was done by SPSS 18 software.The results of this study showed that the main reported managerial obstacle in reporting nursing errors was endangerment of academic position (43.3%), and the main moral obstacle was losing physicians' support (55.2%). Thus this problem might be solved by designing appropriate plans from both managerial and moral points of view. |
