عنوان فارسی علل پرداخت‌های غيررسمی در بخش سلامت از ديدگاه پزشکان، سياست‌گذاران و بيماران: مطالعه‌ی کيفی
چکیده فارسی مقاله در سال‌های اخير بحث رابطه‌ی مالی پزشک و بيمار و مسائلی هم‌چون پرداخت‌های غيررسمی به‌عنوان کاری غيراخلاقی ولی جاری در بخش سلامت اکثر کشورهای جهان از جمله جمهوری اسلامی ايران مطرح شده است که تهديدی برای اعتبار حرفه‌ی پزشکی محسوب می‌شود. اين پرداخت‌ها در نقاط مختلف جهان می‌تواند علت‌های خاص خود را داشته باشد. هدف اين پژوهش بررسی علل و چگونگی پرداخت‌های غيررسمی در بيمارستان‌های شهر کرمان در سال 1390 بود. اين مطالعه با استفاده از روش پژوهش کيفی و از طريق مصاحبه‌های نيمه ساختارمند انجام شده است. مصاحبه‌های هدفمند با استفاده از يک نمونه‌ی هدفمند متشکل از 45 نفر از بيماران، پزشکان و سياست‌گذاران در شهرستان کرمان در ايران در سال 1390 انجام شد. اين مطالعه با مجوز کميته‌ی اخلاق پزشکی دانشگاه و تکميل فرم رضايت از مصاحبه‌شوندگان انجام شد. در اين مطالعه در خصوص دلايل پرداخت‌های غيررسمی از مصاحبه شوندگان سؤالاتی پرسيده شد و داده‌ها با استفاده از تحليل محتوا مورد آناليز قرار گرفت. يافته‌های پژوهش نشان داد که دلايل متعددی برای پرداخت‌های غيررسمی وجود دارد که شامل عوامل فرهنگی، عوامل مرتبط با کيفيت و عوامل قانونی است. دلايلی نيز برای دريافت‌های غيررسمی توسط پزشکان کشف شد که شامل عوامل مربوط به تعرفه‌ها، عوامل ساختاری، عوامل اخلاقی و هم‌چنين نشان دادن مهارت و شايستگی ارائه‌کنندگان خدمات است. مطالعه‌ی حاضر منتج به ارائه‌ی دلايلی برای پرداخت‌های غيررسمی شد و اکثر دلايلی که کشف شد، شباهت زيادی با نتايج مطالعات ساير کشورهای جهان دارد؛ به اين ترتيب که نشان داد که بودجه‌ی ناکافی نظام سلامت و پرداخت‌های رسمی ناکافی به پزشکان، از مهم‌ترين عوامل مرتبط با عرضه هستند که منجر به پرداخت‌های غيررسمی می‌شوند. اما با توجه به اين‌که مطالعات کيفی معمولاً دلايل احتمالی را کشف می‌کنند، بنابراين، برای بيان قطعی علل به مطالعات بيش‌تر نياز است.

عنوان انگلیسی Causes of informal payments in health sector: a qualitative study of viewpoints of physicians, policy makers and patients
چکیده انگلیسی مقاله In recent years, the financial relationship between the physician and the patient and some issues such as informal payments for health care have arisen as an unethical but common problem in many countries, including the Islamic Republic of Iran. Such issues are a threat to the professional reputation of physicians, and can have their own causes in different parts of the world. This study attempts to assess the causes of informal payments and the manners in which they are done in the hospitals of the Kerman Province in Iran in 2012. This study was carried out using qualitative research methods, and semi-structured interviews. Structured interviews were conducted on a purposeful sample of 45 participants including patients, providers and policy makers in the Kerman province in Iran in 2012. This study was authorized by the ethics committee of Kerman University of Medical Sciences, and the consent form was completed by all participants. In this study the participants were asked questions regarding reasons for informal payments, and data were analyzed using content analysis. There are several reasons for making informal payments, which include cultural, legal and quality factors. A number of reasons for asking informal payments by providers were discovered, including those related to tariffs, structural factors and ethical factors as well as to demonstrate the skill and competence of service providers. Most of the reasons discovered for informal payments in Iran are similar to other countries in the world. They showed that inadequate funding of the health systems and inadequate formal payments to providers are the most important supply-side factors leading to informal payments. Given that qualitative studies usually cover potential reasons only, further studies are needed to investigate the matter more extensively.
