Abstract

Association of EBV with the tumor cells of HD has been proven by a variety of the methods, using serologic and immunohistochemical techniques and in the recent years with molecular biologic techniques which can detect EBV genome in tumor biopsies. In this regard we prompted to perform a case control study on 25 childhood HD cases with respected to their antibodies gainst EBNA and EBV-IgM antibodies in Imam Khomeini Hospital in Tehran. In our study the ratio of positive titers was significantly higher among HD patients compared with age and sex-matched healthy controls.

Abstract

A 4.5 years old boy with left postero superior lateral neck mass admitted to Shahid Rahnemoon hospital in Yazd. The mass had been noticed 2 years ago and there was no improvement after three times antibiotic therapy. On physical examination there were two masses on the neck. One of them was approximately 3×4 cm and the other was 2×2 cm. On of the masses was located in the left posterior auricular area and the other one in anterosuperior border of SCM. Both masses were mobile, non adhesive, with slight tenderness and without inflammatory signs (ie, redness, warmness etc). In surgery, the parapharyngeal deep neck space was filled by the yellowish-white color tumor with it's «worm-like» projections, extended into the jugular foramen superiorly, lateral oropharyngeal wall medially and supra clavicular area inferiorly. Total resection of tumor was done. The patient was discharged after 5 days with good general conditions. Pathologic report was «Plexiform neurofibroma».

Abstract

Respiratory syncytial virus (RSV) causes recurrent upper and lower Respiratory tract infections (especially pneumonia and bronchiolitis). Detection of the infection with respect to its morbidity seems to be a nessecity. In this research nasopharyngeal secretions of 145 patients with respiratory symptoms from Imam Khomeini, Markaz Tebbi and Baharami hospitals were analyzed with direct immunofluoresence (DIF) test using monoclonal antibodies. The purpose was to determine the frequency of RSV infections with respect to age, sex, geographical considerations and clinical symptoms and signs. Finally 56 patients in our study were positive in DIF test and RSV is the causative agent for 38.6 percent of all respiratory tract infections. Beside the propensity to RSV infection was significantly greater in patients with bronchitis [OR=2.36 (0.99-5.67)] Bronchitis was the most frequent disorder in our study group

Abstract

Background: In most children with Acute Lymphoblastic Leukemia (ALL) and Non Hodgkin’s Lymphoma (NHL) who have received chemotherapy with and without radi-otherapy, some late effects due to treatment may occur such as endocrinopathies. Methods: We evaluated growth criteria (including short stature, obesity) and thyroid test function in 50 children with ALL (n= 25) and NHL (n= 25) 3-17 year-old in remis-sion period who randomly received chemotherapy with (n= 25) or without (n= 25) radi-ation such as our treatment groups. The values for height, weight and BMI in less than 5th or more than 95 th percentile considers abnormal. Results: Six (12%) patients were in less than 5th percentile height (short stature). Two patients (4.0%) had over-weight and 48 (96%) were in normal range of BMI. Six (12%) patients were in less than 5th and 3 (6%) were in more than 95 th weight percentile. There was no significant difference between two different treatment groups for TSH (P= 0.662 (but there was a significant difference between these groups in case of T4 (P= 0.049(. Mean and SD for T4 in patients with chemotherapy alone was less than in whom received chemotherapy plus radiotherapy. There was no significant difference between ALL and NHL groups for TSH, T4 (P= 0.567, 0.528 respectively). Two boys with ALL without history of radiation had hypothyroidism that had based on their la-boratory data. Conclusion: Regarding to effects of thyroid dysfunction on short stature and obesity in adolescent with ALL and NHL, we suggest to have more attention about growth, thy-roid test to avoid late side effect of malignancy treatment.

Abstract

Background: Standard treatment of Gestational Trophoblastic Disease (GTD) is chemotherapy. Single-agent chemotherapy regime including Methotrexate (MTX) or Actinomycin. Single-agent is widely used in treatment of persistent trophoblastic disease. We reported an uncommon toxicity of low-dose single-agent methotrexate in a patient. Case Presentation: A 20-year-old woman, primary gravid after two months missed period and spotting with diagnosis of incomplete abortion with uterine size equivalent of ten weeks pregnancy (8-10 cm) underwent evacuation curettage. In serial follow-up, based on rise of beta-hCG titer and absence of metastatic disease, it was categorized as low-risk persistent trophoblastic disease. She was referred to gynecology oncology center of Ghaem Hospital, Mashhad University of Medical Sciences in May 2014. Because of rise of beta-hCG titer, after complete metastatic work-up and lack of disease in other sites, persistent disease was diagnosed and candidate for chemotherapy (single agent low-dose). The patient received first course of therapy with MTX (50 mg/m², intra muscular). Unfortunately, after two days of treatment she developed uncommon severe toxicity, fever, severe nausea and vomiting, tachycardia, and generalized weakness. Also, we found hematologic abnormality (WBC: -14000-15000 µI, platelet- 540 µI and sever neutropenia), and abnormal rising in liver function test (SGOT, SGPT) (three to four times) and renal function test (BUN and Creatinine) (two times). In addition, she had disseminated erosive lesion in all of body especially in face. Due to the fatal side effects of chemotherapy, she was admitted to intensive care unit (ICU). Fortunately, after two to three weeks, she was improved by conservative management. After few weeks beta-hCG titer was in normal limit. However she had normal serial beta-hCG in one year of follow-up. Conclusion: It is important to emphasis unpredictable side effects of chemotherapy with low-dose methotrexate.

Abstract

Background: Neck mass is one of the most common clinical findings in all age groups. Differential diagnosis of masses includes a range of pathologies from congenital to infectious or neoplasm. Understanding the risk factors of neck masses can help us to assess them properly. The purpose of this study is to obtain the epidemiological and clinical features of different neck masses in Kerman, Iran.
Methods: This research was a descriptive, retrospective and cross-sectional study. 120 Patients that have been biopsied from neck masses during March 2014 to March 2018  in the otorhinolaryngology department of Shafa hospital of Kerman (referral center of otorhinolaryngology disease of southeastern Iran), were the participants of the study. Data regarding age, sex, history of addiction, smoking and definite pathologic diagnosis were collected and analyzed with SPSS 20th version.
Results: Of 120 cases, 57.5 % were male and 42.5% were female. In the malignant masses group, the male to female ratio was 4.1 to 1. The average age of patients was 39.44 years old. From them, 20.83 percent had a history of smoking but 79.17 percent had never experienced smoking. In addition, 77.5% of the patients have not experienced opium consumption and 22.5% were addicted to opium regularly. There was a significant relationship between smoking and opium consumption and the incidence of malignant cervical masses (P<0.05). In the pathological study, inflammatory and infectious masses with 48.33 percent were in the head of pathology; malignant neoplasm with 25 percent, benign neoplasms with a prevalence of 12.5 percent and congenital masses with a prevalence of 9.7 percent were in the next ranks.
Conclusion: In this study, we investigated the epidemiological and clinical features of neck masses in Kerman. It is obvious that smoking is an important risk factor for neck mass malignancies. Also, the present study revealed that the incidence of malignant tumors increased with age.
