Abstract
Background: Twin pregnancy with a fetus and a diploid complete mole is not rare, but, the cases of twin pregnancy with a partial mole are rare. Nowadays, the prevalence of these cases has been increased due to the high rate of assisted reproductive techniques in reproductive medicine. The importance of twin pregnancy with a fetus and a diploid complete mole is mainly due to systemic complications such as hypertension and maternal hemorrhage and the possibility of trophoblastic tumor following delivery. Different studies have reported some results about similar cases, but limited case reports are presented in our country. The aim of this study is to report a case with incomplete molar pregnancy concomitant with a live fetus.
Case presentation: A 21 yr old woman (G2 ab1) referred to emergency department of Ghaem University Hospital in Mashhad. She complained of scant vaginal bleeding and spotting and a sonography report of a combined molar and normal pregnancy with 15 weeks gestational aged. During hospitalization, the pregnancy complicated with hypertension and proteinuria. Termination of pregnancy was planned at 17th weeks of gestation due to severe preeclampsia. After evacuation of uterus, during follow up visits, -hCG titer raised. Metastasis evaluation was negative. Pathology reports showed patial mole. Then, three doses of methotrexate (50 mg/m2 intra muscular) was administered and finally, according to the monthly follow up, -hCG level was undetectable.
Conclusion: The rate of pregnancy complications such as hypertension, hyperthyroidism, and obstetrics hemorrhage and also the risk of Gestational Trophoblastic Neoplasm (GTN) are increasing in incomplete molar pregnancy. Therefore, early diagnosis and timely treatment of molar pregnancy is very important to reduce maternal morbidity and mortality.

Abstract
Background: The occlusion of the artery and vein grafts are currently a major problem in coronary bypass surgery. Degradation of collagen and elastin, the most abundant extracellular matrix proteins in the vessel wall by matrix metalloproteinase (MMPs), leads to a rearrangement of the extracellular matrix and vascular wall structure. The present study aimed to compare the histological and biochemical characteristics of arteries and veins which could have a role in the failure of the graft.
Methods: This study was a cross-sectional study of 80 patients conducted at Heart Hospitals in Isfahan, Iran, between July 2012 and November 2013. Samples were collected from the remains of vessels used in bypass surgery of 11 male nondiabetic patients. The histologic, collagen elastin ratio and MMPs levels of the vessels were investigated. MMPs were determined using the Gelatin Zymography method. For elastin and collagen content measurement, the sample was digested by cyanogen bromide and hydrochloric acid and then hydroxyproline was measured with a spectrophotometer.
Results: The amount of active and inactive MMP-2 and MMP-9 of the left internal mammary artery (Lima) was similar to aorta, but the amount of MMP-2 and MMP-9 in the radial artery and saphenous vein were significantly higher than aorta. Elastin to collagen ratio in Lima (1.92±1.15) was similar to the aorta (3.4±1.66), but this proportion in saphenous vein (1.07±0.47) and radial artery (1.14±0.39) was significantly lower in the aorta (P≤0.05). Most patients had atherosclerotic plaque in radial while there was atherosclerotic plaque in Lima of only one case.
Conclusion: The presence of atherosclerotic plaques in radial and thickening of the intimal layer of the saphenous vein in the majority of patients and decrease of collagen to elastin ratio and the high level of matrix metalloproteinase enzymes in the radial and saphenous vein can induce early pathological conditions, and remodeling of the vessels involved. So the results of this study confirm that Left Internal Mammary Artery (LIMA) is the most suitable candidate for bypass surgery.

Abstract
Background: One of the possible mechanisms of migraine pathophysiology is neuro inflammation in which, according to previous studies, some changes happen in inflammatory factors like interleukins, adhesion molecules or acute phase reactants. Cytokines may have an essential role in the neurovascular inflammation and also in the process of pain especially in migraine patients. On the other hand, one of the mechanisms by which angiotensin converting enzyme inhibitors (ACEIs) work is anti-inflammation. The goal of this study was to evaluate the effect of enalapril as an ACEI drug, on the proinflammatory cytokines (calcitonin gene-related peptide (CGRP), tumor necrosis factor alpha (TNFα)) of migraine patients.
Methods: In this randomized double blind placebo-controlled clinical trial, 40 migraine without aura patients (mean age of 34.42±1.82 years) who were referred to neurology clinics of Isfahan University of Medical Sciences, Iran, between June 2011 and July 2012 were recruited. The patients were randomly divided into two groups. 21 patients were treated with 10 mg enalapril daily for two months as the intervention group and they were compared with matched placebo treated group of 19 patients. Serum samples were collected from all of the subjects and TNFα and CGRP levels were measured by means of ELISA (enzyme-linked immunosorbent assay) kits at the beginning of study (before the intervention) and after 2 months of enalapril or placebo treatment. The serum levels were compared in each group and between the groups. SPSS software, version 16 (SPSS Inc., Chicago, IL, USA) was used for statistical analysis.
Results: Six males (15%) and 34 females (85%) were enrolled in this study. The mean of migraine history among the subjects was 74.40±7.54 months. Patients' TNFα level decreased significantly in the case group after treatment with enalapril (P=0.001) while there was no significant change in control group (P=0.769). There was no significant difference in the CGRP concentrations in the intervention and control groups (P=0.795, 0.708 respectively).
Conclusion: Enalapril may be effective in improvement of inflammatory responses of migraine patients by decreasing the inflammatory factors like TNFα.

Abstract
This review was conducted between December 2018 and March 2019 at Isfahan University of Medical Sciences. A review of various studies revealed what data mining techniques to predict the probability of survival, what risk factors for these predictions, what criteria for evaluating data mining techniques, and finally what data sources for it have been used to predict the survival of breast cancer patients. This review is based on the Prism statement consisting of published studies in the field of predicting the survival of breast cancer patients using data mining techniques from 2005 to 2018 in databases such as Medline, Science Direct, Web of Science, Embase data and Scopus. After searching in these databases, 527 articles were retrieved. After removing duplicates and evaluating the articles, 21 articles were used. The three techniques of logistic regression, decision tree, and support vector machine have been most used in articles. Age, tumor grade, tumor stage, and tumor size are used more than other risk factors. Among the criteria, the accuracy criterion was used in more studies. Most of the studies used the Surveillance, Epidemiology, and End Results Program (SEER) dataset. Typically, in the field of survival probability prediction, data mining techniques in the field of classification are given more attention due to their adaptation to this field. Accordingly, data mining techniques such as decision tree techniques, logistic regression, and support vector machine were used in more studies than other techniques. The use of these techniques can provide a good basis for clinicians to evaluate the effectiveness of different treatments and the impact of each of these methods on patients' longevity and survival. If the output of these techniques is used to provide the data input required by a decision support system, clinicians can provide risk factors related to the patient, the patient's age, and the patient's physical condition when providing services to breast cancer patients. Through the outputs provided by the decision support system, they provided the most optimal decision to choose the best treatment method and consequently increase patient survival.

Abstract
Background: Febrile seizures (FS) are the most common cause of seizure in children. In order to prevent FS and their recurrence, the recognition of the related factors to recurrence of FS is important. According to the high prevalence of FS and iron deficiency anemia (IDA) in children and that IDA is a possible risk factor for FS, as well as controversies in previous studies in this field, in this study, the relationship between iron deficiency anemia and FS in children was investigated.
Methods: This is a case-control study that investigates the relationship between febrile seizures with anemia in 150 patients aged six months to five years with fever who were admitted to Imam Sajjad Yasouj Hospital, from April to August of 2014. In this study, 75 patients with febrile seizures (case group) and 75 febrile patients without seizures (control group) were compared. Cause of fever (urinary infection, gastrointestinal infections, upper respiratory infection, lower respiratory infection and other causes), Information about blood cell count (white blood cell (WBC) and red blood cell (RBC), hemoglobin (HB), mean red blood cell volume (MCV) and serum ferritin level in patients with hemoglobin below 11 were recorded.
Results: The mean age of the study population was 21.4 14 months. 34.7% of the study population were girls and 65.3% were boys. In this study, 10.7% of the cases and 8% of the controls had IDA. Although the prevalence of IDA was higher in the FS group, this difference was not significant (P=0.58). Also, HB and MCV were not significantly different in the two groups (P=0.49, P=0.69). In addition, the mean serum ferritin level in the FS group with anemia and the FS group with anemia did not show a statistically significant difference (P=0.94).
Conclusion: According to the result of this study, IDA is not a risk factor for seizures in febrile children.

Abstract
Background: Febrile seizures are the most common cause of seizures in children under 5 years of age. Central nervous system infections are an important differential diagnosis of febrile seizures. Apart from infections, CSF characteristics are affected by many factors including the cause, type and duration of seizures. This study was designed to determine the cerebrospinal fluid (CSF) indices, including white blood cells (WBC), polymorphonuclear cells (PMN) count, and levels of protein and glucose in the CSF of children with fever-induced seizures.
Methods: The present study is a descriptive study that evaluated the results of CSF analysis in 56 children with febrile seizures admitted to Imam Sajjad Hospital in Yasouj from March 2012 to 2013. In the present study, CSF analysis was performed in children with a diagnosis of febrile seizures. In order to collect information from a pre-designed form that includes demographic information (age and gender), degree of fever, and information related to CSF indices including WBC, PMN and levels of protein and glucose in CSF, which have been extracted and recorded, statistical methods are used in this study to express descriptive statistics such as mean, standard deviation, number, percentage, etc.
Results: 56 children with a diagnosis of convulsive fever with a mean age of 21.4±14 months were evaluated. 37.5% (21 individuals) of the study population were females and 62.5% (53 individuals) were males. Among them, 33 individuals had low-grade fever and 23 individuals had high-grade fever. In 10.7%, the number of WBCs was more than 5. The amount of protein obtained was in the normal range, between 20 and 50 mg/dl, in 57.1% of cases. However, a significant percentage (39.3%) of CSF liquid protein was below 20 mg/dl. CSF fluid glucose levels were within the normal range at 91.1%.
Conclusion: Based on the results of this study, febrile seizures may be associated with low protein levels in the CSF. However, due to the limited sample size in our study, similar studies with higher sample sizes are recommended.

Abstract
Background: Coronaviruses were discovered in 1960. Large-sized living organisms from the Coronaviridae family, with single-stranded RNA of animal origin. Coronaviruses in humans can cause mild respiratory illness or severe respiratory illness. In 2020, the World Health Organization declared COVID-19 a global pandemic. The aim of this study is to use the Jaccard similarity coefficient to determine the similarity of COVID-19 behavior patterns in different seasons of the year.
Methods: This study used machine learning systems and similarity metrics to determine the behavior pattern of COVID-19 in different seasons of the year. The location of research was the Mousa ibn Ja'far Hospital in Mashhad, and the time was from May 2020 to August 2021. The symptoms of affected patients were compared with the compiled dataset, and the similarity of patients was prepared in a similarity matrix, and the Jaccard correlation coefficient was calculated on the data. Finally, the analysis of strains from the beginning of emergence to the latest strain was examined. The performance indicators of the algorithm in the Jaccard similarity method showed a recall metric with a value of 0.94, a precision metric with a value of 1, an F1 score with a value of 0.86, and remove accuracy metric with a value of 0.76. The most important factors in the investigation include white blood cells, platelets, RT-PCR, CT SCAN, shortness of breath, fever, SPO2, and respiratory rate.
Results: The transmission of the COVID-19 virus depends on several factors, including human interaction. The evidence of the collected data shows that people with COVID-19 have low lymphocyte count and it is very consistent with the results of recent studies. Due to the lack of a dataset, a comparative study was conducted and a dataset was collected.
Conclusion: This study, leveraging machine learning algorithms, identified a clear seasonal correlation in the spread of COVID-19. Considering geographical and seasonal variations among patients, distinct symptoms were observed in each season corresponding to the prevalent strain during that period.
