Abstract
Background: While excellent organ quality and ideal transplant conditions eliminate many of the known factors that compromise initial graft function (IGF), slow graft function (SGF), still occurs after living donor kidney transplantation (LDKT). The aim of our current study is determination SGF frequency and its risk factors in LDKT
Methods: In this prospective study, between April 2004 and March 2006, data were collected on 340 LDKT, in Baghiyattallah Hospital, Tehran. Recipients were analyzed in two groups based on initial graft function (IGF): Creatinine <3 mg/dl 5 day after transplantation, SGF: Creatinine ≥ 3 mg/dl 5 day after transplantation with out dialysis in the first week. Donors' and recipients' characteristics and recipient lab. data were compared in two groups by chi-square, Mann-whitney & independent samples T-test.
Results: The incidence of SGF was 22 (6.2%) and IGF 318 (89.8%), Recipients' BMI in IGF were 22.1±3.9 and in SGF were 25.3±3.8 (P=0.001 95% Cl 1.097-1.401 OR= 1.24). SGF relative frequency in female donors is more than male donors. A multivariate analysis model confirms this significant difference. (P=0.044 95% Cl 1.028-7.971 OR= 2.862). SGF relative frequency in PRA (Panel Reactive Antibody) positive recipients are more than negative ones. A multivariate analysis model confirms this significant difference. (P=0.007 95%Cl 1.755-35.280 OR= 7.849). Recipients' age and donors' BMI are significant in univariate analysis (P=0.002 & P=0.029 respectively) but multivariate analysis model dose not confirm those significance. Serum ca & P & PTH levels don't have significant difference between IGF & SGF. Using calcium channels blockers have not a protective effect.
Conclusions: We conclude that negative PRA and lower recipient BMI have protective effects on SGF. Recipients with female donors have higher chance to develop SGF. We recommend recipients reduce their BMI before transplantation. The male donors are preferred to female ones.

Abstract
Osteoporosis is a condition characterized with reduced bone density and destruction of the bone structure. The ideal treatment aims to reduce the risk of fracture while improving the density and structure of the bone. Parathormone (PTH) and its main analogue triparatide (rhPTH [1-34]) is a new class of anabolic medications which accelerates the healing process in the fractured bone through improving bone formation and therefore is used for treating severe osteoporosis. The present review article was designed to report the history, different types, anabolic and catabolic effects, complications, indications and contraindications of the hormone.

Abstract
Background: Conservative treatment needs to be tried prior to surgical treatment of knee osteoarthritis. This study was designed to evaluate the short-term effects of dextrose prolotherapy on pain relief and functional improvement in knee osteoarthritis in comparison with intra-articular hyaluronic acid injections.
Methods: In this double blind clinical trial, 100 patients, aged 40-70 years, with complaints of knee pain lasting >3 months were recruited in Akhtar hospital during the years 2010 to 2011. The patients met the criteria proposed by the American College of Rheumatology (ACR) for knee osteoarthritis. 50 patients in hyaluronic acid group received five 2 ml injections of hyaluronic acid (Synocrom Forte® 1%) weekly and 50 patients in dextrose prolotherapy group received three 2 ml bimonthly injections of 25% dextrose. The patients were evaluated before and after treatment in terms of pain and functionality using the Knee injury and Osteoarthritis Outcome Score (KOOS) self-questionnaire. The patients were followed up for 12 weeks and were examined 12 weeks after the injections by an observer unaware of group assignments. The data were recorded for statistical analysis.
Results: The mean age of the patients was 60.68.2 years. No significant differences were found between the two groups with respect to pre- and post-treatment KOOS scores. The scores showed significant improvements in all items following treatment in both groups (P<0.001).
Conclusion: It seems that intra-articular injections of 25% dextrose prolotherapy could be as effective as hyaluronic acid injections for the treatment of knee pain due to OA.

Abstract
Background: Hypereosinophilic syndrome is commonly found in various diseases such as allergic diseases, parasitic diseases, malignancies, etc. Fasciolosis may present with different clinical features, and it can make a difficult diagnosis of the disease. Laboratory manifestations of fascioliasis are eosinophilia. The purpose of this report was to introduce a child with hypereosinophilia that her diagnosis was fascioliasis.
Case Presentation: The patient was a 3-year-old girl who was referred for prolonged fever (more than two weeks) and abdominal pain from another medical center, and she was hospitalized. In abdominal and pelvic ultrasound, splenomegaly was seen and in laboratory tests, she had hypereosinophilia. In the flow cytometry of bone marrow aspiration, the only finding was increased eosinophil level. Abdominal and thoracic a computerized tomography (CT) scans showed an increased size of para-aortic lymph nodes. On her examination, lymphadenopathy was present in the inguinal region. Therefore, a biopsy of an inguinal lymph node was performed to rule out lymphoma. Lymph node biopsy was negative for lymphoma. Fasciola serology was performed for the patient, and the stool exam was collected three times (for one day in between) to rule out parasitic disease, including Fasciola, etc. Due to weakly positive serology Fasciola hepatica, triclabendazole was started for the patient (it was given in two doses, 12 hours apart), despite the absence of Fasciola parasitic eggs in her stool. During hospitalization, the patient’s fever was stopped and by starting the use of mentioned drug, eosinophilia was reduced. The patient received a complete improvement in the follow-up.
Conclusion: In patients with hypereosinophilia, parasitic diseases such as fascioliasis should be considered even if the fecal specimen is negative for Fasciola eggs.

Abstract
Background: CNS infection can be focal or generalized. Meningitis, besides other etiologies, can occur after a viral infection or viral vaccine. Although meningitis can occur in at any age but it is more common in children less than five years old. Aseptic meningitis and meningoencephalitis are two of the most common complications of mumps (wild type or vaccine). As a result of their serious complication, we decided to obtain more information about clinical manifestations and laboratory findings after the injection of the Hoshino MMR vaccine in a group of children in an Iranian referral hospital.
Methods: In this cross-sectional study, performed from March 2013 to February 2015 on 73 children with meningochemical symptoms of meningitis, children who have been diagnosed with aseptic meningitis followed by an MMR vaccine in an infectious ward or emergency department of the Children’s Medical Center were enrolled in the study and their information was recorded from their files. A questionnaire was provided for children with diagnostic criteria of aseptic meningitis and their data were collected.

Results: The gender ratio of patients was 46 males to 27 females. After collecting the symptoms of meningitis, the frequency of symptoms in these patients was as below: fever 66%, headache 49.3%, nausea and vomiting 74%, parotid swelling 0%, seizure 21.9% and meningeal symptoms 37%. Their laboratory data showed that 8.2% of patients had normal WBC and 76.7% had Abnormal results in their CSF (Cerebrospinal fluid) analysis. Mumps PCR (Polymerase chain reaction) was positive in 85% of samples.
Conclusion: our study revealed that nausea and vomiting were the most frequent symptom after MMR vaccination in children and fever was in second grade in aseptic meningitis. There was no relation between clinical symptoms together, therefore we should take care of children after MMR vaccination to avoid complications when they become symptomatic.

Abstract
Background: Estimating the stature and identifying unknown bodies is an important challenge for forensic specialists and anthropologists, especially when dismembered and mutilated body parts are found. The purpose of this study was to predict the stature by measuring lower limb parameters and to determine the relationship between stature and thigh length, leg length, foot length and lower limb length in Iranian medical students.
Methods: This study was conducted in the Department of Anatomy, Tehran University of Medical Sciences, Tehran, Iran in 2019 (May-August). The study population consisted of 200 Iranian medical students (100 males and 100 females, aged 17-36 years). Four lower extremity parameters (thigh length, leg length, foot length, and lower limb length) and stature were measured on the left side according to standard measurement methods. All measurements were performed according to ethical standards provided by the Ethics Committee for Human Experimentations. The measurements were done using standard anthropometric instruments. Linear regression analysis was used to investigate the relationship between the stature and the lengths of the four lower extremity parameters. Then, stature prediction was done based on regression equations. The data were analyzed using SPSS version 25. Independent-samples t-test was used to compare the mean values of the anthropometric measurements between males and females. The relationships between quantitative data were evaluated using the Pearson’s correlation coefficient and linear regression was applied to determine the relationship between stature and lower extremity parameters.
Results: The results of this study indicated a strong correlation between stature and lower limb length (r=0.892, P=0.000), foot length (r=0.852, P=0.000) and thigh length (r=0.805, P=0.000) and a moderate correlation between stature and leg length (r=0.505, P=0.000).
Conclusion: The results of this study suggest that lower limb dimensions and regression equations are appropriate for stature estimation in forensic medicine. However in the future, further research should be conducted to analyze the applicability of these equations for stature estimation in other Iranian populations.
